Background This study aimed to investigate the presence of sleep disturbances in children with Prader-Willi syndrome (PWS) using the Sleep Disturbance Scale for Children (SDSC). Methods The SDSC, which was designed to identify the presence and severity of different sleep disorders, was applied to 50 patients with PWS and 112 controls. Results Patients with PWS achieved worse scores in the sleep-disordered breathing and disorders in initiating and maintaining sleep in the SDSC questionnaire as compared with controls. We also observed that patients with PWS were more prone to having hyperhidrosis. We did not observe significant differences in the presence of other types of sleep disorders (such as hypersomnolence) between the PWS and control groups. Conclusions The results obtained with the SDSC questionnaire showed that children with PWS have more sleep breathing disorders and disorders in initiating and maintaining sleep as compared to controls. Additionally, we demonstrated that patients with PWS associates significantly with the presence of hyperhidrosis during sleep. However, SDSC was not reliable to identify the excessive daytime somnolence in patients with PWS, as previously reported in the literature.

OBJECTIVE: To assess the possible factors that influence sleep quality in adolescents with and without chronic immunosuppressive conditions quarantined during the coronavirus disease 2019 (COVID-19) pandemic. METHODS: This cross-sectional study included 305 adolescents with chronic immunocompromised conditions and 82 healthy adolescents. Online surveys were completed, which included questions on socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and the following validated questionnaires: the Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0), and Pediatric Outcome Data Collection Instrument (PODCI). RESULTS: The median current age [14 (10-18) vs. 15 (10-18) years, p=0.847] and frequency of female sex (62% vs. 58%, p=0.571) were similar in adolescents with chronic conditions compared with healthy adolescents. The frequency of poor sleep quality was similar in both groups (38% vs. 48%, p=0.118). Logistic regression analysis, including both healthy adolescents and adolescents with chronic conditions (n=387), demonstrated that self-reported increase in screen time (odds ratio [OR] 3.0; 95% confidence interval [CI] 1.3-6.8; p=0.008) and intrafamilial violence report (OR 2.1; 95% CI 1.2-3.5; p=0.008) were independently associated with poor sleep quality in these adolescents. However, the PODCI global function score was associated with a lower OR for poor sleep quality (OR 0.97; 95% CI 0.94-0.99; p=0.001). Further logistic regression, including only adolescents with chronic conditions (n=305), demonstrated that self-reported increase in screen time (OR 3.1; 95% CI 1.4-6.8; p=0.006) and intrafamilial violence report (OR 2.0; 95% CI 1.2-3.4; p=0.011) remained independently associated with poor quality of sleep, whereas a lower PODCI global function score was associated with a lower OR for sleep quality (OR 0.96; 95% CI 0.94-0.98; p

BACKGROUND Given the limited therapeutic options for Prader-Willi syndrome (PWS), we conducted an open-label clinical trial to evaluate the effects of transcranial direct current stimulation (tDCS) for hyperphagia, food craving, and aberrant behaviors on this population. METHODS Twelve subjects with PWS (11-35 years old) were included. The subjects underwent 10 daily 20-minute sessions of tDCS in 2 weeks. The anode was positioned over the left dorsolateral prefrontal cortex, and the cathode over the contralateral region. RESULTS We observed amelioration of hyperphagic and food craving symptoms (P < 0.05), as well as amelioration of behavioral symptoms measured with the Aberrant Behavior Checklist (P < 0.05). DISCUSSION To our knowledge, this is the first proof-of-concept trial to report the positive effects of increasing excitability of the left dorsolateral prefrontal cortex, using tDCS, for the behavioral, hyperphagia, and food craving symptoms in PWS, which is a low-cost, well-studied, safe alternative for brain stimulation.

Cardiac death is the second most prevalent cause in Prader-Willi syndrome (PWS). Paediatric patients with PWS often present cardiac autonomic dysfunction during wakefulness, obesity and sleep-disordered breathing. However, the extent of cardiac autonomic modulation during sleep in PWS has not been documented. The objective of this study was to assess alterations in cardiac autonomic modulation of paediatric patients with PWS during different sleep stages. Thirty-nine participants in three groups: 14 PWS, 13 sex and age-matched lean controls (LG) and 12 obese-matched controls (OB). All participants underwent overnight polysomnography, including continuous electrocardiogram recordings. Heart rate variability (HRV) was analysed during representative periods of each sleep stage through time and frequency domains calculated across 5-min periods. Between-within ANOVAs were employed (p .05). The results show that total HRV was lower in PWS than OB and LG during slow-wave sleep (SWS) (standard deviation of all NN intervals [SDNN] ms, p = .006). Parasympathetic modulation assessed by time-domain analysis was lower during SWS in PWS compared to both OB and LG (square root of the mean of the sum of the squares of differences between adjacent NN intervals [RMSSD] ms, p = .004; SDSD, standard deviation of differences between adjacent NN intervals [SDSD] ms, p = .02; number of adjacent NN intervals differing by50 ms [NN50] ms, p = .03; proportion of adjacent NN intervals differing by50 ms [pNN50] ms, p = .01). Sympathovagal balance assessed by frequency-domain analysis was lower during both N2 and SWS than during the rapid eye movement (REM) sleep stage, but not different among groups. In conclusion, this group of paediatric patients with PWS had impaired cardiac autonomic balance due to reduced parasympathetic modulation during SWS. This result could imply an underlying increased cardiovascular risk in PWS even during early age and independent of obesity.

BackgroundGrowth hormone (GH) treatment is currently recommended in Prader-Willi syndrome (PWS) patients.ObjectivesTo evaluate the impact (efficacy and safety) of the use of recombinant human GH (rhGH) as a treatment for PWS.MethodWe performed a systematic review and, where possible, meta-analysis for the following outcomes: growth, body mass index, body composition, cognitive function, quality of life, head circumference, motor development/strength, behaviour and adverse effects. We included all PWS patients, with all types of genetic defects and with or without GH deficiency, who participated in rhGH studies performed in infancy, childhood and adolescence, that were either randomised controlled trials (RCTs) (double-blinded or not) or non-randomised controlled trials (NRCTs) (cohort and before and after studies). The databases used were MEDLINE, Embase and Cochrane Central.ResultsIn 16 RCTs and 20 NRCTs selected, the treated group had an improvement in height (1.67 SD scores (SDS); 1.54 to 1.81); body mass index z-scores (−0.67 SDS; −0.87 to −0.47) and fat mass proportion (−6.5% SDS; −8.46 to −4.54) compared with the control group. Data about cognition could not be aggregated.ConclusionBased on high quality evidence, rhGH treatment favoured an improvement of stature, body composition and body mass index, modifying the disease’s natural history; rhGH treatment may also be implicated in improved cognition and motor development in PWS patients at a young age.Ethics and disseminationThe current review was approved by the ethical committee of our institution. Objective: To describe a case of a male adolescent with symptomatic idiopathic intracranial hypertension (IIH) associated with obesity treated with bariatric surgery. Case description: A 16-year-and-6-month-old severely obese boy [weight: 133.6 kg; height: 1.74 m (Z score: +0.14); BMI: 44.1 kg/m2 (Z score: +4.4)], Tanner pubertal stage 5, presented biparietal, high-intensity, and pulsatile headaches, about five times per week, associated with nocturnal awakenings, and partial improvement with common analgesics, for three months. Ophthalmologic evaluation evidenced bilateral papilledema. Cranial computed tomography revealed no mass or anatomic abnormalities. Lateral hypothalamic area (LHA) local field potentials (LFPs) were recorded in a Prader–Willi patient undergoing deep brain stimulation (DBS) for obesity. During hunger, exposure to food‐related cues induced an increase in beta/low‐gamma activity. In contrast, recordings during satiety were marked by prominent alpha rhythms. Based on these findings, we have delivered alpha‐frequency DBS prior to and during food intake. Despite reporting an early sensation of fullness, the patient continued to crave food. This suggests that the pattern of activity in LHA may indicate hunger/satiety states in humans but attest to the complexity of conducting neuromodulation studies in obesity.

Patients with Prader-Willi syndrome (PWS) present with obesity that is often difficult to treat. Objective To test the safety and study the outcome of DBS in patients with PWS. Design, Setting, and Participants This case series was conducted in the Hospital das Clínicas, University of São Paulo, Brazil. Four patients with genetically confirmed PWS presenting with severe obesity were included. Exposure Deep brain stimulation electrodes were bilaterally implanted in the lateral hypothalamic area. After DBS implantation, the treatment included the following phases: titration (1-2 months), stimulation off (2 months), low-frequency DBS (40 Hz; 1 month), washout (15 days), high-frequency DBS (130 Hz; 1 month), and long-term follow-up (6 months). Main Outcomes and Measures Primary outcome measures were adverse events recorded during stimulation and long-term DBS treatment. Secondary outcomes consisted of changes in anthropometric measures (weight, body mass index [calculated as weight in kilograms divided by height in meters squared], and abdominal and neck circumference), bioimpedanciometry, and calorimetry after 6 months of treatment compared with baseline. The following evaluations and measurements were conducted before and after DBS: clinical, neurological, psychiatric, neuropsychological, anthropometry, calorimetry, blood workup, hormonal levels, and sleep studies. Adverse effects were monitored during all follow-up visits. Results Four patients with PWS were included (2 male and 2 female; ages 18-28 years). Baseline mean (SD) body mass index was 39.6 (11.1). Two patients had previous bariatric surgery, and all presented with psychiatric comorbidity, which was well controlled with the use of medications. At 6 months after long-term DBS, patients had a mean 9.6% increase in weight, 5.8% increase in body mass index, 8.4% increase in abdominal circumference, 4.2% increase in neck circumference, 5.3% increase in the percentage of body fat, and 0% change in calorimetry compared with baseline. Also unchanged were hormonal levels and results of blood workup, sleep studies, and neuropsychological evaluations. Two patients developed stimulation-induced manic symptoms. Discontinuation of DBS controlled this symptom in 1 patient. The other required adjustments in medication dosage. Two infections were documented, 1 associated with skin picking. Conclusions and Relevance Safety of lateral hypothalamic area stimulation was in the range of that demonstrated in patients with similar psychiatric conditions receiving DBS. In the small cohort of patients with PWS treated in our study, DBS was largely ineffective.

Aims: The aim of this study was to verify body composition, cardiorespiratory fitness, and biochemical markers of prepubertal overweight and obese boys to a 16-week futsal training program. Methods: Sixteen boys (age: 7-10 y, body mass index>thanat 95th percentileaccording to Center for Disease Control and Prevention; 35.5±7.4 percent fat) participated to futsal training program. The assessment of body composition was estimated using skinfold thickness, and the following variables were evaluated: total body mass, body mass index, body fat percentage, and lean body mass. Aerobic fitness measurementwas performed by gas exchange analysis in treadmill. In addition, an evaluation of the biochemical profile was conducted: triglycerides, total cholesterol, low density lipoprotein cholesterol, high-density lipoprotein cholesterol, plasma concentrations of glucose, and insulin. The futsal intervention included 60-min sessions performed two times/week. The mean intensity during training was between 57 to 88% of maximal heart rate of the age-predicted. Individual portable heart rate monitor controlled training intensity. RESUMO Objetivo: Realizar uma revisão sobre a Síndrome de Prader-Willi (SPW) com base nas publicações mais recentes e fornecer recomendações ao pediatra geral para diagnóstico precoce e seguimento. Fonte de dados: Artigos publicados nas bases Pubmed e SciELO. A pesquisa não foi limitada a um período e incluiu todos os artigos das bases de dados. Síntese dos dados: A SPW é uma síndrome genética rara, resultante da perda do imprinting gênico expresso no cromossomo paterno 15q11-q13, sendo caracterizada por alterações endocrinológicas, como deficiência de hormônio de crescimento, obesidade, insuficiência adrenal central, hipotireoidismo, hipogonadismo, além de alterações comportamentais e déficit intelectual. Há outras comorbidades associadas, como distúrbios de sono, escoliose, constipação, problemas dentários e alterações de coagulação. O protocolo de seguimento da SPW do Instituto da Criança da Universidade de São Paulo se baseia em quarto pilares principais: dieta, exercício físico, terapia com hormônio de crescimento humano recombinante (rhGH) e manejo comportamental e cognitivo. A dieta deve ser restrita a 900 kcal/dia, de acordo com a Pirâmide Alimentar do Prader-Willi, e o exercício físico deve ser diário, aeróbico e postural. The purpose of this study is to present changes of policies and norms aimed to reduce obesity levels that have been adopted in some Latin American countries. The global increase of the excess weight within the population has been demanding governmental actions aimed at preventing health impacts generated by obesity. Over recent years, many Latin American countries have established a number of regulations aimed at reducing weight in the population using interventions that could effectively prevent childhood obesity, including the taxation of sugar-sweetened beverages (SSBs), increasing physical activity in open spaces, and, especially, front-of-package labeling. Some strategies are part of the Action Plan for Prevention of Child and Adolescence Obesity signed by all countries in Latin America, which currently have among the highest prevalence of childhood obesity in the world. Among them are the implementation of fiscal policies on energy-dense and nutrient-poor foods and taxes on SSBs; improvements in nutrition labeling, highlighting the front-of-package (FOP) labeling to promote the choice of healthier products at the time of purchase; and promotion of an active lifestyle, such as encouraging the use of bicycle paths or physical activity programs at school. The real impact of these prevention strategies implemented in Latin America on the prevalence of obesity is still unknown.

In adolescence and obesity, the pathophysiology of polycystic ovary syndrome (PCOS) is very difficult to distinguish. We aimed to assess the diagnosis of PCOS in the population of obese adolescent.Cross-sectional study.Pediatric Endocrinology Weight and Management Clinic from Children's Institute, São Paulo, Brazil.Forty-nine post-menarcheal obese adolescents with mean age of 14.7 years.Anthropometric assessment and review of medical records were done. Clinical and laboratory hyperandrogenism were quantified using Ferriman-Gallwey index and androgenic dosages, respectively. Ovarian morphology was evaluated through suprapubic ultrasonography.By the 2015 Witchel et al. guideline for PCOS in adolescence, 18.4% were diagnosed as having PCOS. When assessed by Rotterdam, Androgen Excess and PCOS Society, and American National Institute of Health criteria's, 26.4%, 22.4%, and 20.4% had PCOS, respectively. Irregular menstrual cycles were found in 65.3% of patients. Clinical hyperandrogenism was observed in 16.3% of girls and 18.4% had elevated serum androgen values. Suprapubic ultrasonography revealed enlarged ovaries in 18.4% of the adolescents.PCOS in all-comers obese adolescents from a Weight and Management Clinic in a quaternary hospital is more frequent when compared to adult women showed in the literature.

To carry out a review about Prader-Willi Syndrome based on the most recent data about the subject and to give recommendation for the general pediatricians for early diagnoses and follow-up.Scientific articles in the PubMed and SciELO databases. The research was not limited to a specific time period and included all articles in such databases.The Prader-Willi Syndrome (PWS) is a rare genetic disorder resulting from the loss of imprinted gene expression within the paternal chromosome 15q11-q13. PWS is characterized by endocrine abnormalities, such as growth hormone (GH) deficiency, obesity, central adrenal insufficiency, hypothyroidism, hypogonadism and complex behavioral and intellectual difficulties. PWS individuals also may present other comorbidities, such as sleep disorders, scoliosis, constipation, dental issues and coagulation disorders. The follow-up protocol of the Children's Institute at Universidade de São Paulo is based on four main pillars: diet, exercise, recombinant human growth hormone (rhGH) therapy and behavioral and cognitive issues. The diet must include a caloric restriction of 900 kcal/day, according to the Prader-Willi Eating Pyramid and exercise plan is focused on daily aerobic exercises and postural therapy. The rhGH therapy is highly recommended by the international scientific literature and must be started as soon as the diagnostic is made. summary Background & aims: Few studies have evaluated how well ultrasonographic measurements of fat correlate with anthropomorphic measurements and insulin resistance in juveniles. Moreover, the significance of an accumulation of visceral versus subcutaneous fat in obese children and young adults remains controversial. The hypothesis of this study was that ultrasonographic measurements of fat would show better correlations with insulin resistance than with anthropometric measurements. Among the variables associated with insulin resistance, we tried to identify those that had the best correlations with ultrasonographic measurements of fat. Methods: Forty-five adolescent volunteers (age range: 10e17 years) were enrolled in this study. Subjects were classified as obese or eutrophic according to their body mass index z-score for Brazilian children and adolescents. Blood samples and anthropometric measurements (waist circumference, waist-toheight ratio, and conicity index) were obtained from all subjects. All patients underwent an ultrasonic assessment of subcutaneous tissue, pre-peritoneal fat, and intra-abdominal fat. Results: Ultrasonographic measures of abdominal fat were associated with anthropometric measurements, glucose level, insulin level, and the Homeostasis Model Assessment of Insulin Resistance (HOMAIR), with the exception that glucose level was not associated with the maximal pre-peritoneal fat. Body mass index z-score, conicity index, and HOMA-IR remained independently associated with the subjects’ total fat in multivariate analysis. Only minimal subcutaneous fat was independently associated with HOMA-IR. Conclusion: Subcutaneous fat may be more useful than visceral fat as a marker for insulin resistance in juveniles.

OBJETIVO: Estudar a prevalência da síndrome metabólica em adolescentes acompanhados em ambulatório de obesidade. MÉTODOS: Foram avaliados 84 adolescentes com idades entre dez e 19 anos, divididos em dois grupos, de acordo com o escore Z do índice de massa corpórea (Z IMC), sendo um o grupo de adolescentes com sobrepeso (GSP) e o outro, o grupo de adolescentes obesos (GOB). GSP: Z IMC>1 e 2 (2,4±0,6, 32M/26F). Três ou mais dos critérios a seguir foram considerados no diagnóstico da síndrome metabólica e avaliados pelo teste do qui-quadrado entre os grupos: Z IMC>2; triglicérides em jejum >130mg/dL; lipoproteína de alta densidade 100mg/dL ou homeostatic model assessment index (HOMA) >2,5; elevação da pressão arterial acima do percentil 90 ajustada para gênero, estatura e idade. RESULTADOS: A prevalência da síndrome metabólica esteve significantimente elevada nos adolescentes obesos (GOB: 40% versus GSP: 4%, p=0,0008). O grupo GOB mostrou maiores valores referentes a insulinismo (54% versus 19%, p=0,003), HOMA (66% versus 38%, p=0,01) e trigliceridemia (21% versus 4%; p=0,04). CONCLUSÕES: É importante que o pediatra fique atento aos sinais de síndrome metabólica em adolescentes obesos. A detecção precoce pode ser feita por meio de simples parâmetros e permite a adoção de medidas preventivas para o desenvolvimento da doença cardiovascular em adolescentes.OBJECTIVE: Evaluate the prevalence of metabolic syndrome in adolescents followed in an outpatient obesity clinic. METHODS: 84 adolescents (ten to 19 years old) were divided in two groups, one composed by overweight adolescents (OWG) and the other, by obese adolescents (OBG), according to Z scores of the body mass index (Z BMI). OWG: Z BMI>1 and 2 (2.4±0.6; 32M/26F). Three or more criteria were considered to define the presence of metabolic syndrome and tested by chi-square: Z BMI>2; fasting triglycerides >130mg/dL; high-density lipoprotein cholesterol 100mg/dL or homeostatic model assessment index (HOMA) >2.5 and blood pressure above the 90th percentile, adjusted for age, gender and length. RESULTS: The prevalence of metabolic syndrome was significantly elevated in obese adolescents (OBG: 40% versus OWG: 4%, p=0.0008). The major differences between groups were observed regarding the presence in OBG adolescents of hyperinsulinism (54% versus 19%, p=0.003), HOMA (66% versus 38%, p=0.01) and hypertriglyceridemia (21% versus 4%, p=0.04). CONCLUSIONS: It is important to be aware of the early signs of metabolic syndrome in obese adolescents, which can be screened by simple techniques. As the syndrome correlates to chronic illnesses, early detection and adequate prevention by pediatricians is mandatory.

This article represents the fourth case in literature of an association between pseudotumor cerebri (PTC) and leuprolide acetate, the first with precocious puberty and severe visual loss. A 9-year-old girl with precocious puberty was treated with a once-monthly dose of leuprolide acetate (3.75 mg) for three months. In the 4th month of treatment, it was decided to increase the dosage of leuprolide acetate to 11.25 mg in once-quarterly doses. After 1 month, she complained of holocranial headache, transient visual obscuration followed by progressive visual loss. After 6 months, she persisted with holocranial headache and progressive visual loss associated with ocular deviation. Neuro-opthalmological examination revealed severe visual loss and bilateral papilledema. Cerebrospinal fluid (CSF) analysis showed opening pressure of 45 cm H2O. The most likely diagnosis was PTC associated with leuprolide acetate. Treatment was started immediately with oral acetazolamide and leuprolide was discontinued. Acetazolamide was discontinued for having induced metabolic acidosis. A ventriculoperitoneal shunt was performed to control intracranial pressure as an alternative to acetazolamide treatment. The follow-up of 18 months showed CSF pressure of 14 cm H2O, stabilization of visual acuity and resolution of papilledema. There appears to be a causal relationship between the onset of leuprolide acetate with the development of PTC. Children should have a complete ophthalmic evaluation if they report headache or visual disturbances after administration of leuprolide acetate.

Objetivo : Avaliar o efeito da sibutramina na perda de peso de adolescentes obesos.Sujeitos e métodos : Estudo duplo-cego placebo controlado tipo cross over com duração de 13 meses. Foram incluídos no estudo 73 adolescentes obesos de ambos os sexos com idades entre 10 e 18 anos. A avaliação antropométrica foi realizada a cada 40 dias em média. Os exames laboratoriais e de imagem foram realizados antes, no período de wash-out e ao final dos 13 meses.Resultados : A porcentagem de pacientes que perderam 10% do peso inicial no placebo foi de 46% e, no grupo sibutramina, foi de 75%. Quando usaram o placebo, o peso em média se elevou em 1,61 kg, e o IMC reduziu em média 0,24 kg/m2, enquanto com o uso da sibutramina o peso reduziu em média 4,47 kg e o IMC reduziu em média 2,38 kg/m2 com p < 0,001.Conclusões : A sibutramina induziu significantemente mais perda de peso em adolescentes obesos em comparação ao placebo, sem efeitos colaterais significativos. A curva de evolução ponderal foi diferente de acordo com o momento em que a sibutramina foi introduzida. Esse achado indica que o melhor momento de introdução da sibutramina é quando a adesão começa a falhar. Objetivo: Avaliar o efeito da sibutramina na perda de peso de adolescentes obesos e as mudanças ocorridas nos parâmetros da síndrome metabólica com a perda de peso. Pacientes e métodos: O estudo foi duplo cego placebo controlado tipo cross-over com duração de 13 meses. Os pacientes receberam placebo ou sibutramina por 6 meses e vice-versa, nos 6 meses seguintes. Foram incluídos no estudo 73 adolescentes obesos de ambos os sexos com idades entre 10 e 18 anos. Os exames laboratoriais e exames de imagem foram realizados antes, no período de wash-out e ao final dos 13 meses de acompanhamento. Foram dosados: Colesterol total e frações, triglicérides, Leptina, proteína C reativa, transaminases e teste de tolerância à glicose oral. Os exames de imagem realizados foram: ultrassom de abdômen, Ecocardiograma, Eletrocardiograma, idade óssea. Resultados: A porcentagem de pacientes que perderam 10% do peso inicial no placebo foi de 46% e no grupo sibutramina foi de 75%. Quando usaram o placebo, o peso em média se elevou em1,61 kg, DP 4,46 e o IMC reduziu-se em média 0,24 kg/m2 DP 1,57 enquanto que os pacientes que receberam a sibutramina, o peso reduziu-se em média 4,47 kg e DP 5,87 e o IMC reduziu-se em média 2,38 kg/m2 e DP 2,36 com p < 0,001. Apesar da sibutramina reduzir o peso, IMC e medida de circunferência abdominal as mudanças metabólicas laboratoriais não foram observadas no período de tempo avaliado. Conclusão:A sibutramina induziu significantemente mais perda de peso em adolescentes obesos comparado ao placebo, sem efeitos colaterais significativos. The study included 73 obese adolescents of both sexes aged between 10 and 18 years (with BMI > 2DP registered at the outpatient clinic of the Pediatric endocrinology Department of Hospital das Clínicas, São Paulo, Brazil, between 2007 and 2010. Patients were randomized to receive either placebo followed by sibutramine (6 months each) or sibutramine followed by placebo (6 months each). Anthropometric measurements, laboratory tests and imaging studies were performed at baseline, during the wash-out period and at the end of the 13 months follow-up. Total cholesterol, HDL, triglycerides, leptin, C-reactive protein, transaminases and oral glucose tolerance were tested. The following Imaging studies were performed: abdominal ultrasound, echocardiogram, EKG, bone age X-ray. Results: The percentage of patients who lost at least 10% of initial weight while taking the placebo was 46% and while taking sibutramine was 75%. While on placebo, average weight increased by 1.61 kg (SD 4.46). BMI decreased by 0.24 kg/m2 (SD 1.57) as a consequence of growth. While on sibutramine, weight decreased on average by 4.47 kg (SD 5.87) and mean BMI decreased by 2.38 kg/m2 (SD 2.36, p

Obesity is a chronic, progressive and prevalent disorder. Morbid obesity, in particular, is associated with numerous comorbidities and early mortality. In patients with morbid obesity, pharmacological and behavioral approaches often have limited results. Bariatric surgery is quite effective but is associated with operative failures and a non-negligible incidence of side effects. In the last decades, deep brain stimulation (DBS) has been investigated as a neurosurgical modality to treat various neuropsychiatric disorders. In this article we review the rationale for selecting different brain targets, surgical results and future perspectives for the use of DBS in medically refractory obesity.

OBJETIVO: Estudar a prevalência da síndrome metabólica em adolescentes acompanhados em ambulatório de obesidade. MÉTODOS: Foram avaliados 84 adolescentes com idades entre dez e 19 anos, divididos em dois grupos, de acordo com o escore Z do índice de massa corpórea (Z IMC), sendo um o grupo de adolescentes com sobrepeso (GSP) e o outro, o grupo de adolescentes obesos (GOB). GSP: Z IMC>1 e 2 (2,4±0,6, 32M/26F). Três ou mais dos critérios a seguir foram considerados no diagnóstico da síndrome metabólica e avaliados pelo teste do qui-quadrado entre os grupos: Z IMC>2; triglicérides em jejum >130mg/dL; lipoproteína de alta densidade 100mg/dL ou homeostatic model assessment index (HOMA) >2,5; elevação da pressão arterial acima do percentil 90 ajustada para gênero, estatura e idade. RESULTADOS: A prevalência da síndrome metabólica esteve significantimente elevada nos adolescentes obesos (GOB: 40% versus GSP: 4%, p=0,0008). O grupo GOB mostrou maiores valores referentes a insulinismo (54% versus 19%, p=0,003), HOMA (66% versus 38%, p=0,01) e trigliceridemia (21% versus 4%; p=0,04). CONCLUSÕES: É importante que o pediatra fique atento aos sinais de síndrome metabólica em adolescentes obesos. A detecção precoce pode ser feita por meio de simples parâmetros e permite a adoção de medidas preventivas para o desenvolvimento da doença cardiovascular em adolescentes. OBJECTIVE: Evaluate the prevalence of metabolic syndrome in adolescents followed in an outpatient obesity clinic. METHODS: 84 adolescents (ten to 19 years old) were divided in two groups, one composed by overweight adolescents (OWG) and the other, by obese adolescents (OBG), according to Z scores of the body mass index (Z BMI). OWG: Z BMI>1 and 2 (2.4±0.6; 32M/26F). Three or more criteria were considered to define the presence of metabolic syndrome and tested by chi-square: Z BMI>2; fasting triglycerides >130mg/dL; high-density lipoprotein cholesterol 100mg/dL or homeostatic model assessment index (HOMA) >2.5 and blood pressure above the 90th percentile, adjusted for age, gender and length. RESULTS: The prevalence of metabolic syndrome was significantly elevated in obese adolescents (OBG: 40% versus OWG: 4%, p=0.0008). The major differences between groups were observed regarding the presence in OBG adolescents of hyperinsulinism (54% versus 19%, p=0.003), HOMA (66% versus 38%, p=0.01) and hypertriglyceridemia (21% versus 4%, p=0.04). CONCLUSIONS: It is important to be aware of the early signs of metabolic syndrome in obese adolescents, which can be screened by simple techniques. As the syndrome correlates to chronic illnesses, early detection and adequate prevention by pediatricians is mandatory.

Objective: The objective was to conduct clinical and metabolic evaluations of obese adolescents before and after laparoscopic sleeve gastrectomy (LSG) (up to 24 months). Subjects and methods: This was designed as a retrospective, descriptive series of cases study, conducted in Instituto da Criança, São Paulo, Brazil. Analysis of clinical and laboratory data from 22 obese adolescents between 14 and 19 years old submitted to LSG between 2007 and 2014. Patients had BMI > 40 kg/m2 or BMI > 35 kg/m2 with comorbidities. Anthropometric, clinical and laboratory assessments were performed: before surgery, 6, 12, 18, and 24 months after surgery. We assessed weight loss and metabolic changes up to 24 months after LSG. Results: The mean preoperative weight and BMI were 128.5 kg (SD = 23.1) and 46.5 kg/m2 (SD = 74), respectively. There was an average weight loss of 34.5 kg in the first 12 months’ post LSG, corresponding to a 60% excess weight loss (EWL), as well as an average reduction in BMI of 12.3 kg/m2. However, after 24 months, the average EWL was 45%, corresponding to an average weight regain (WR) of 13.3 kg (15%) within two years. LSG improved dyslipidemia in 67.8% of patients, a significant remission of hepatic steatosis 47% and 37.7% systemic arterial hypertension; type 2 diabetes remission was complete. Conclusions: LSG proved to be a safe and effective procedure and seems to be the new hope for the obesity epidemic.
