Abstract

ObjectivesTo assess clinician attitudes and experiences in Health eHearts, a quality recognition and financial incentive program using health information technology.Study designSurvey of physicians.MethodsA survey was administered to 140 lead clinicians at each participating practice. Survey domains included clinicians' experiences and attitudes toward the selected clinical quality measures focused on cardiovascular care, use of electronic health records (EHRs), technical assistance visits, quality measurement reports, and incentive payments. Responses were compared across groups of practices receiving financial incentives with those in the control (no financial rewards).ResultsSurvey response rate was 74%. The majority of respondents reported receiving and reviewing the quality reports (89%), agreed with the prioritization of measures (89%), and understood the information given in the quality reports (95%). Over half of the respondents had a quality improvement visit (56%), with incentive clinicians more likely to have had a visit compared with the control group (68% vs 43%, P = .01). The incentive group respondents (92%) were more likely to report using clinical decision support system alerts than control group respondents (82%, P = .11).ConclusionsClinicians in both incentive and control groups reported positive experiences with the program. No differences were detected between groups regarding agreement with selected clinical measures or their relevance to the patient population. However, clinicians in the incentive group were more likely to review quarterly performance reports and access quality improvement visits. Incentives may be used to further engage clinicians operating in small independently owned practices to participate in quality improvement activities.

18. Economics and the patient's utility function : an application to assist reproductive techniques

Abstract

This thesis questions the assumption that health outcome is the only relevant argument in the patient's utility function. Following this, consideration is given to deciding which methods to use to establish what attributes there are in the patient's utility function and the relative weights to these attributes. Two methods are assessed and considered appropriate: contingent valuation and conjoint analysis. These methods are applied to assess the benefits from assisted reproductive techniques. An attempt is made to determine both the presence and relative importance of health and non-health attributes in the infertile person's utility function. The results from these empirical studies provide evidence of both the potential importance of factors beyond having a child and the sensitivity and applicability of the tools used to establish the relative importance of possible utility bearing attributes in the infertile person's utility function. The factors identified include: knowing you have done everything possible to have a child, the psychological feeling of disappointment, overall satisfaction with life following involvement with assisted reproductive techniques, the provision of follow-up support, and process type attributes such as continuity of care, attitudes of medical staff, waiting time and cost. Health economists concerned with developing economic instruments to measure utility from assisted reproductive techniques, and health care interventions more generally, should find the results useful. So too should those concerned with policy issues around the provision of assisted reproductive techniques and health care interventions more generally.

Abstract

Well-being equations are often estimated to generate monetary values for non-marketed activities. In such studies, utility is often approximated by either life satisfaction or General Health Questionnaire scores. We estimate and compare monetary valuations of informal care for the first time in the UK employing both measures, using longitudinal data on well-being and informal care provision. The choice of well-being measure has some effect on the estimated parameters and resulting monetary valuations, but any differences are not statistically significant. Further research is needed to confirm the comparability of these measures if researchers are to continue to use them interchangeably.

Abstract

Dataset for a journal contribution exploring the opinions and experiences of medical doctors and academic educationalists from four settings on the inclusion of management, health economics, and research in the medical curriculum in South Africa and Botswana. Interviews with medical doctors and academic educationalists were conducted. The attached files are transcripts of semi-structured interviews that were conducted with academic educationalists and medical doctors from the 4 study sites. In total, 49 interviews were conducted. Four interviews from each site (16 in total) have been uploaded. Data analysis was supported by ATLAS.ti version 9.1.7.0 software. Iterative rounds of coding by the principal investigator were followed by discussions with co-researchers. Categories and sub-themes were refined and defined several times before the final themes were generated and named.
