2. An interdisciplinary investigation into return to play in professional rugby union following lower limb injury

Abstract

Rugby union is a contact sport with an inherent risk of injury, typically lower limb. The focus of return to play (RTP) following lower limb injury is predominantly at the point of return to sport, with limited assessment of the progression encountered throughout rehabilitation. This thesis used a interdisciplinary approach (using the proposed lower limb functional rehabilitation framework) to examine biomechanical and psychological changes during three phases of RTP (acute, middle, and late), following a lower limb injruy in  professional rugby union players. Study 1 investigated injury rates and trends for a professional rugby union team. Players were found to be at the greatest risk of experiencing non-contact lower limb injuries, with knee injuries accounting for the highest incidence and burden across both contact and non-contact mechanisms. In order to undertake a rigorous methodological assessment of the three phases of RTP, study 2 then examined changes within professional rugby union players unilateral static postural control. Study 3A and 3B, assessed the concurrent validity and test-retest reliability of discrete biomechanical assessments of team-based athletes [bilateral and unilateral drop jump; countermovement jump (CMJ) and unilateral lateral hurdle hop]. Static postural control changed over the trial duration, while validity and reliability of discrete biomechanical measures for assessments of RTP phases was supported. The methodology validated in study 3A and 3B were subsequently used in studies 4-6 to characterise the RTP of rugby union players following lower limb injury. Study 4 assessed changes in static postural control and self-efficacy during the acute phase of RTP, whilst study 5 assessed alterations to bilateral and unilateral vertical jump and self-efficacy during the middle phase of RTP. Study 6 explored the changes to lateral hurdle hop and self-efficacy during the the late phase of RTP. The results suggest improvements across each phase of RTP between the initial to end testing session. However, inconsistent improvements were evident when comparing pre-injury baseline to the end session. In addition, non-significant medium associations between biomechanical assessment and self-efficacy were observed across all phases. The thesis findings suggest that improvements occur in each of the RTP phases for biomechanical and self-efficacy variables, but postural control deficits are present at the end of each phase. The novel findings of the assessment of professional rugby union players' RTP suggest that practitioners/clinicians need to consider both biomechanical and self-efficacy measurement in their RTP assessment, as although both improve through the rehabilitation process, they are independent of each other. The thesis findings were applied to the proposed lower limb functional rehabilitation framework where the assessment of the independent measurements are accounted for.

Abstract

Assessment of player's postural control following a lower limb injury is of interest to sports medicine practitioners due to its fundamental role in daily tasks and sporting activities. The aim was to longitudinally monitor professional rugby union players' postural control during each phase of the rehabilitation program (acute, middle, and late) following a lower limb injury. Seven male rugby union players (height 1.80 [0.02] m; mass 100.3 [11.4] kg; age 24 [4] y) sustained a time loss, noncontact lower limb injury. Static postural control was assessed via sway path (in meters), and dynamic postural control was assessed via vertical postural stability index. Group differences (P <.05) were reported across the acute, middle, and late phase. Smaller magnitudes of sway path were observed for eyes-open sway path, and for the middle and late phase smaller magnitudes of vertical postural stability index (P <.05) at the end session compared with first session. Whereas larger magnitudes of vertical postural stability index were found between baseline and the last session (P <.05). Large interindividual and intraindividual variation was apparent across the 3 phases of rehabilitation. Postural control improvements were identified during rehabilitation. Abstract

Boosting high school graduation rates is a growing concern to educators and policymakers. Researchers stress that, although graduation rates have varied little in recent years, "[w]hat makes current graduation rates alarming is a reality of the new U.S. economy: It is practically impossible for individuals lacking a high school diploma to earn a living or participate meaningfully in civic life." Research shows that students who fail to graduate high school exhibit clear signs of dropping out. Warning signs such as irregular attendance, poor academic performance, behavior problems and grade retention are more precise predictors of whether a student will ultimately drop out than are social and economic indicators such as family income and parental involvement. States have made notable progress collecting longitudinal information that follows individual students over time. Access to such student data has prompted state and school district officials across the country to develop "early warning indicator systems" to efficiently identify students who are at risk of dropping out and provide targeted supports to get them back on track and graduate. This paper provides summaries that highlight the statewide early warning indicator systems in Louisiana, South Carolina, and Alabama and the Philadelphia Public Schools district-wide system. Abstract

This review discusses female‐specific health considerations in injury and illness surveillance and provides rugby‐specific recommendations for future surveillance. Identifying priority injury and illness problems by determining those problems with the highest rates within women's rugby may highlight different priorities than sex comparisons between men's and women's rugby. Whilst sports exposure is the primary risk for health problems in sports injury and illness surveillance, female athletes have health domains that should also be considered. Alongside female athlete health domains, studies investigating rugby injuries and illnesses highlight the need to broaden the health problem definition typically used in rugby injury and illness surveillance. Using a non‐time‐loss health problem definition, recording female‐specific population characteristics, embedding female athlete health domains and having up‐to‐date injury and illness coding systems should be prioritized within surveillance systems to begin to shed light on potential interactions between sports exposure, health domains and, injuries and illnesses. We call for a collaborative approach across women's rugby to facilitate large injury and illness datasets to be generated and enable granular level categorization and analysis, which may be necessary for certain female athlete health domains. Applying these recommendations will ensure injury and illness surveillance systems improve risk identification and better inform injury and illness prevention strategies in women's rugby. My role as a creative researcher in this project is to unravel the multiple strands of knowledge that can be obtained through re-creating historical objects. The interdisciplinary engagement with lace attempts to define and characterise worn, historic cloth objects as stimuli for speculation on personal histories, tools to extend social and cultural knowledge.

Abstract

There are currently a multitude of tests used to assess readiness to return to sport (RTS) following anterior cruciate ligament reconstruction (ACLR). The aim of this study was to establish the extent to which movement strategies transfer between three common assessment tasks to help improve design of athlete testing batteries following ACLR. A cohort of 127 male patients 8–10 months post-ACLR and 45 non-injured controls took part in the study. Three movement tasks were completed (unilateral and bilateral drop jump, and 90° pre-planned cut), while ground reaction forces and three-dimensional kinematics (250 Hz) were recorded. Compared to the bilateral drop jump and cut, the unilateral drop jump had a higher proportion of work done at the ankle (d = 0.29, p < 0.001 and d = −1.87, p < 0.001, respectively), and a lower proportion of work done at the knee during the braking phase of the task (d = 0.447, p < 0.001 and d = 1.56, p < 0.001, respectively). The ACLR group had higher peak hip moments than the non-injured controls, although the proportion of work done at the ankle, knee and hip joints were similar. Movement strategies were moderately and positively related at the ankle (rs = 0.728, p < 0.001), knee (rs = 0.638, p < 0.001) and hip (rs = 0.593, p < 0.001) between the unilateral and bilateral drop jump, but there was no relationship at the ankle (rs = 0.10, p = 0.104), knee (rs = 0.106, p = 0.166) and hip (rs = −0.019, p = 0.808) between the unilateral drop jump and the cut. Clinicians could therefore consider omitting one of the drop jumps from assessment batteries but should include both jumping and cutting tasks.
