Nutrients play a fundamental role as regulators of the activity of enzymes involved in liver metabolism. In the general population, the action of nutrients may be affected by gene polymorphisms. Therefore, individualization of a diet for individuals with fatty liver seems to be a fundamental step in nutritional strategies. In this study, we tested the nutrient-induced insulin output ratio (NIOR), which is used to identify the correlation between the variants of genes and insulin resistance. We enrolled 171 patients, Caucasian men (n = 104) and women (n = 67), diagnosed with non-alcoholic fatty liver disease (NAFLD). From the pool of genes sensitive to nutrient content, we selected genes characterized by a strong response to the NIOR. The polymorphisms included Adrenergic receptor (b3AR), Tumor necrosis factor (TNFα), Apolipoprotein C (Apo C III). Uncoupling Protein type I (UCP-1), Peroxisome proliferator activated receptor γ2 (PPAR-2) and Apolipoprotein E (APOEs). We performed three dietary interventions: a diet consistent with the results of genotyping (NIOR (+)); typical dietary recommendations for NAFLD (Cust (+)), and a diet opposite to the genotyping results (NIOR (-) and Cust (-)). We administered the diet for six months. The most beneficial changes were observed among fat-sensitive patients who were treated with the NIOR (+) diet. These changes included improvements in body mass and insulin sensitivity and normalization of blood lipids. In people sensitive to fat, the NIOR seems to be a useful tool for determining specific strategies for the treatment of NAFLD. [ABSTRACT FROM AUTHOR]

Background & Aims: Functional gastrointestinal disorders are prevalent worldwide and alterations of gutbrain axis and intestinal barrier integrity may play a pivotal role in both the pathophysiology and clinical course of these bowel malfunctions. We aimed to assess the prevalence of abdominal pain in a selected adult population of Poland to determine potential environmental factors associated with gastrointestinal complaints. Methods: There were 1479 individuals - 657 women (44.42%) and 822 men (55.58%), aged 24.20±6.08 years. The responders fulfilled an authors' questionnaire based on Rome II and III criteria focused on the abdominal pain prevalence and environmental factors involved in its occurrence. Results: The frequency of abdominal pain was found to be as high as 19.2%. Male gender (n=822) and basic education level (n=151) lowered the risk of abdominal pain occurrence (OR=0.7, p<0.012 and OR=0.5, p<0.021, respectively). Psychological distress, proton pump inhibitors (PPIs) and antibiotics usage were found as risk factors of abdominal pain (OR=2.503, p<0.01; OR=3.308, p<0.01; OR=3.105, p<0.01, respectively). Conclusions: Abdominal pain is prevalent in young adult inhabitants of Poland, especially in women. Intense psychological stress, as well as PPIs and antibiotics usage elevate the risk. Background: High caloric diet and lack of physical activity are considered main causes of NAFLD, and a change in the diet is still the only effective treatment of this disease. However, molecular mechanism of the effectiveness of diet change in treatment of NAFLD is poorly understood. We aimed to assess the involvement of epigenetic mechanisms of gene expression regulation in treatment of NAFLD. Eighteen participants with medium- to high-grade steatosis were recruited and trained to follow the Mediterranean diet modified to include fibre supplements. At three timepoints (baseline, after 30 and 60 days), we evaluated adherence to the diet and measured a number of physiological parameters such as anthropometry, blood and stool biochemistry, liver steatosis and stiffness. We also collected whole blood samples for genome-wide methylation profiling and histone acetylation assessment. Results: The diet change resulted in a decrease in liver steatosis along with statistically significant, but a minor change in BMI and weight of our study participants. The epigenetic profiling of blood cells identified significant genome-wide changes of methylation and acetylation with the former not involving regions directly regulating gene expression. Most importantly, we were able to show that identified blood methylation changes occur also in liver cells of NAFLD patients and the machine learning-based classifier that we build on those methylation changes was able to predict the stage of liver fibrosis with ROC AUC = 0.9834. Conclusion: Methylomes of blood cells from NAFLD patients display a number of changes that are most likely a consequence of unhealthy diet, and the diet change appears to reverse those epigenetic changes. Moreover, the methylation status at CpG sites undergoing diet-related methylation change in blood cells stratifies liver biopsies from NAFLD patients according to fibrosis grade. Athletes are a group characterized by intense and regular physical activity. Systematic exercises may lead to a number of adaptive changes in the organism which influence the skeletal system and the metabolism of some elements. The aim of the study was to determine the influence of physical activity on the level of fluoride excretion in urine. The average urine concentration of fluoride among athletes was 0.54 mg/L and 1.14 mg/L in the control group. It has been proven that physical activity plays an important role in the absorption, metabolism, and excretion of fluoride. These changes might be caused by mechanical factors, the stimulation of the skeletal system, and the adaptation of the human organism to physical exertion. [ABSTRACT FROM AUTHOR]

Introduction: Non-alcoholic fatty liver disease (NAFLD) affects a large part of the human population. One of the major environmental factors associated with the risk of NAFLD is the lack of physical activity. Aim: To compare the level of physical activity and the insulin resistance in NAFLD patients. Material and methods: Thirty patients with NAFLD underwent a six-month dietary intervention based on the principles of classical dietetics. Data about diet and physical activity was based on 72-hour nutrition diaries and International Physical Activity Questionnaire (IPAQ). Standard blood biochemical analyses were carried out before and after diet at the University Hospital Laboratory. Results: The study showed that total physical activity and physical activity in leisure time are negatively correlated with insulin resistance (HOMA-IR) (p < 0.05). Insulin (p < 0.05), body weight (p < 0.05), and waist-hip ratio (WHR) (p < 0.05) were also negatively correlated with physical activity in free time. In addition, we noticed a positive correlation between sitting time and the risk of insulin resistance, in the case of HOMA-IR and insulin concentration (p < 0.05). Conclusions: Dietary intervention and a physical activity plan are important factors in the treatment of non-alcoholic fatty liver disease. Taking regular exercise increases insulin sensitivity and prevents further development of the disease. It seems that diet and physical activity are not the only one risk factors of NAFLD. Our study reveals that the reduction of sitting time has a positive effect on the level of insulin and it reduces insulin resistance in patients with NAFLD. [ABSTRACT FROM AUTHOR]

Background Nonalcoholic fatty liver disease (NAFLD) is a spectrum of liver conditions related to fat infiltration. The role of liver triacylglycerol accumulation in NAFLD is not fully understood. Methods Twenty-four patients, 12 in the first and 12 in the second stage of NAFLD, were prospectively enrolled in this study. Biochemical parameters and eicosanoids (HETE and HODE) were compared between the first and the second stage of hepatic steatosis and the effect of a 6-month dietary intervention on these parameters was evaluated. Eicosanoid profiles were extracted from 0.5 ml of plasma using solid-phase extraction RP-18 SPE columns. The HPLC separations were performed on a 1260 liquid chromatograph. Results Patients with stage I NAFLD had a significantly higher level of HDL cholesterol and a lower level of 5-HETE. Patients with grade II steatosis had higher concentrations of 9-HODE. Following the six-month dietary intervention, hepatic steatosis resolved completely in all patients. This resulted in a significant decrease in the concentrations of all eicosanoids (LX4, 16-HETE, 13-HODE, 9-HODE, 15-HETE, 12-HETE, 5-oxoETE, 5-HETE) and key biochemical parameters (BMI, insulin, HOMA-IR, liver enzymes). Conclusion A significant reduction in the analyzed eicosanoids and a parallel reduction in fatty liver confirmed the usefulness of HETE and HODE in the assessment of NAFLD. [ABSTRACT FROM AUTHOR]

Menopause means a definitive end of the generative ovarian function and mostly occurs in women aged 40-58. It is well-known that the sooner it occurs the more serious consequences may result, even in the case of early menopausal hormone therapy. The most serious short-term consequences of menopause are menopausal symptoms (depression, hot flushes, sleep disorders, loss of libido), while long-term estrogen deficiency leads to osteoporosis, cardiovascular system diseases, depression, neurodegenerative diseases, urogenital area atrophy, skin and sense organ disorders. Therefore, an important task is to select women at risk of premature termination of the ovarian function. An important issue thus appears to be identification of factors influencing the age of the last natural menstrual period. Analysis of heritability results has brought the conclusion that the age of menopause is a multifactorial trait. The proportion of the impact of environmental and genetic factors, however, was not precisely estimated. Among environmental determinants the influence of many factors is being considered, including reproductive factors (related to the reproductive period), such as age of menarche, number of pregnancies and children, socio-economic factors, such as education, economic status, health determinants, such as the use of oral contraceptives or lifestyle-related factors, such as physical activity and the use of stimulants. Among the genetic factors, a genetic polymorphism of genes encoding enzymes of the estrogen biosynthesis pathway and other independent markers scattered throughout the genome (linkage studies and genome-wide association studies - GWAS) is taken into consideration.
