Objectives The aim of this AFIRE (Atrial Fibrillation and Ischemic Events With Rivaroxaban in Patients With Stable Coronary Artery Disease) trial subgroup analysis was to examine rivaroxaban monotherapy benefits and their relation to the time between stenting and enrollment among patients after coronary stenting. Background Of 2,215 patients with atrial fibrillation and stable coronary artery disease in the AFIRE trial, rivaroxaban monotherapy was noninferior to rivaroxaban plus antiplatelet therapy (combination therapy) in terms of efficacy and superior for safety endpoints. However, thrombotic risk after antiplatelet therapy cessation remained a concern among 1,444 patients who had undergone coronary stenting >1 year before enrollment. Methods The benefits of rivaroxaban monotherapy in coronary stenting subgroups were assessed for efficacy (a composite of stroke, systemic embolism, myocardial infarction, unstable angina requiring revascularization, or death of any cause), safety (major bleeding defined according to International Society on Thrombosis and Haemostasis criteria), ischemic endpoints, net adverse clinical event, and time between stenting and enrollment. Results Efficacy and safety endpoints for monotherapy were superior to combination therapy, with HRs of 0.70 for efficacy (95% CI: 0.50-0.98; P = 0.036) and 0.55 for safety (95% CI: 0.33-0.92; P = 0.019). For ischemic endpoints, the HR was 0.82 (95% CI: 0.58-1.15; P = 0.240). The HR became smaller with longer time between stenting and enrollment (efficacy, P for interaction = 0.158; safety, P = 0.097). Conclusions In patients with atrial fibrillation after coronary stenting, the benefits of rivaroxaban monotherapy for efficacy and safety endpoints were consistent with those in the whole AFIRE trial population. The benefits became apparent with longer time between stenting and enrollment. In order to prepare microcapsules (MC) using polyvinylacetal diethyl-aminoacetate (AEA) gel, we have studied both the unique properties of AEA gel, in which cold-water solution forms a hard gel with exclusion of water at high temperature, and the microencapsulation technique for clarithromycin (CAM), which has an unpleasant taste (1, 2). In our previous paper (3), we described a new uniform-sized microencapsulation machine with a vibration nozzle with 536 holes 100µ in diameter designed for precise regulation of dissolution of CAM from MC.In order to evaluate the MC thus obtained, we prepared syrups (without water) by adding sweetening agents to MC, and conducted a dissolution test at various pHs using the method described in the Japanese Pharmacopoeia (12th edition), a test for bitter taste and a test to determine bioavailability (BA) in human subjects. At pHs below 4.0,100% of CAM in MC was dissolved from the syrup within 5min. However, there was little dissolution of CAM from syrup at pH6.8. The results of the test for bitter taste showed that the syrup preparations, which were administered to 12 male and female volunteers with 20ml water, masked bitter taste completely during the 60min period following administration of the syrup. The mean area under the serum concentration-time curve (AUCo-24) and the maximum serum concentration (Cmax) for administration of syrup preparations to 6 male volunteers were 5.66 ± 0.73µg·hr/ml and 0.66 ± 0.13µg/ml, respectively. Blood concentration was maintained at a level above the minimum inhibitory concentration for gram-positive bacteria for a period of about 14hr after administration, demonstrating the usefulness of the syrup preparation.

Introduction While effective as chemotherapeutics, anthracyclines can cause cancer therapy-related cardiac dysfunction (CTRCD), which adversely affects the prognosis of patients with malignancies1–5. Despite guideline recommendations6–9, repeated echocardiograms are rarely performed10 with delayed diagnosis of CTRCD leading to unrecoverable cardiac dysfunction11. Recently, artificial intelligence (AI) was shown to be capable of detecting reduced left ventricular ejection fraction (LVEF) solely from electrocardiogram (ECG)12. Furthermore, this model was predictive of a future decrease in LVEF. Therefore, we hypothesized that an AI model detecting reduced LVEF (AI-EF model) could predict CTRCD from ECGs. Purpose To assess whether the AI-EF model could detect patients at a high risk of CTRCD by analyzing ECGs taken immediately prior to the initiation of cardiotoxic chemotherapy. Methods Among patients who received chemotherapy with a regimen including anthracyclines in two institutions between June 1st, 2015 and October 1st, 2020, those who underwent both an ECG and echocardiogram ≤90 days prior to initial treatment were selected. The ECGs were analyzed by the AI-EF model and patients were divided into two groups according to the scores from the model. CTRCD was defined as LVEF Results 1,158 patients were included in this study. 99 of them developed CTRCD during follow-up. The AI-EF model displayed excellent risk stratification of developing CTRCD: while 7.1% in the low AI-EF score group developed CTRCD, 12.9% of the patients in the high AI-EF score group developed CTRCD (hazard ratio (HR), 2.14; 95% confidence interval (CI), 1.43–3.19; log-rank p Conclusion The AI-EF model, by utilizing baseline ECG, could stratify patients according to the risk of CTRCD and robustly augmented CTRCD prediction. Small bowel obstructions are a common general surgery occurrence. In a patient with prior abdominal surgeries, the usual diagnosis is secondary to adhesions. The management is typically conservative, which usually avoids operative intervention. Computed tomography (CT) scans help diagnosticians take a snapshot inside the abdomen; however, CT scans are not perfect and intra-abdominal pathologies can be missed requiring surgery. We present a case of an atypical small bowel obstruction. The initial CT scan showed a transition point in the right lower quadrant, which was managed non-operatively. One week later, the patient then re-presented with hematochezia following an outpatient pill cam procedure. Imaging showed the presence of a new small bowel mass, which was not seen on imaging done 1 week ago or from 10 months prior. He was then taken for a diagnostic laparoscopy, in which a small bowel mass was found, pathology positive for recurrent renal cell carcinoma.

Study question Which single nucleotide variant (SNVs) are associated with aneuploid pregnancy loss? Summary answer We identified a SNV on MEIG1 gene, which are associated with meiosis/spermiogenesis. What is known already Recurrent pregnancy loss (RPL) refers to the loss of two or more pregnancies, with a frequency of 5%. Chromosomal abnormalities in embryos are found in 80% of first trimester miscarriages, 86% of which are aneuploid. Recently, embryonic aneuploidy was found to be the most common cause of RPL, with a frequency of 40-50%. Most trisomy miscarriages are of maternal origin, with errors occurring during meiosis of the oocyte. Chromosome segregation abnormalities in oocytes are thought to be an event associated with increasing maternal age, but in addition, maternal genetic causes are thought to contribute. Study design, size, duration A Genome wide association study (GWAS) was performed on a clinically well characterized cohort of 189 women with RPL whose previous aborted conceptus was ascertained to be an aneuploid embryo. Samples were mainly collected from 2007 to 2018 mainly at Nagoya City University Hospital. For control samples, we used 1157 samples from the population-based prospective cohorts that included fertile women. Participants/materials, setting, methods All patients underwent a systematic examination. Patients with antiphospholipid syndrome, an abnormal chromosome in either partner, or uterine anomaly were excluded. Patients whose previously miscarried POC exhibited triploidy or 45, X were excluded. DNA was isolated from stored EDTA-blood samples and genotyped by Axiom Japonica-array v2659,503 SNVs). For the GWAS, a chi-squared test was applied to a two-by-two contingency table in allele frequency model. Main results and the role of chance The mean (SD) ages and number of previous miscarriages of the patients were 36.8 (4.3) and 3.09 (1.13). GWAS data revealed 5 SNVs with suggestive significance (p Limitations, reasons for caution Since this study was conducted in a single center and had a small sample size, it needs to be replicated in different centers with more subjects and on an international scale. Whole genome imputation analysis will be performed to detect SNVs with more significant associations. Wider implications of the findings Our findings demonstrate that a specific genotype of MEIG1 gene can be a risk factor for aneuploid pregnancy loss. The establishment of clinically applicable maternal germ cell markers could identify groups for whom PGT would be more useful or provide patients with counseling that provides prognostic information about pregnancy. Objective To determine the feasibility of three-dimensional conformal radiation therapy for canine aortic body tumours. Materials and methods Medical records of dogs that had undergone three-dimensional conformal radiation therapy with presumptive diagnosis of aortic body tumour were reviewed for clinical characteristics, treatment modality and outcomes. Results Eight dogs were diagnosed with aortic body tumour and were treated with three-dimensional conformal radiation therapy. One dog had proliferation of a mass in the right atrium during treatment and died of respiratory distress. Another dog did not undergo follow-up CT to evaluate the treatment response due to the increased blood urea nitrogen values. The remaining 6 dogs were included in the case series. Radiotherapy was performed using a median dose per fraction of 7 Gy (3.3-7.14 Gy), a median of seven divided doses (7-15) and a total median dose of 49 Gy (45-50 Gy). The median number of CT scans during the follow-up period was 5 (range: 3-8 times). CT revealed acute side effects in four dogs-grade 1 effects related to the lung (n = 4) and skin (n = 2). Self-limiting or asymptomatic late side effects (grade 1 lung-related effect) were observed in three dogs. After therapy, one dog demonstrated a complete response, another demonstrated a partial response and the disease remained stable in four animals. The median follow-up period was 514.5 (235-1219) days. After three-dimensional conformal radiation therapy, the aortic body tumour reduced gradually over time without regrowth in all these 6 dogs. Clinical significance In this small case series, aortic body tumours responded to three-dimensional conformal radiation therapy. Transient and self-limiting side effects of the treatments were common. Further controlled studies are required to prove the effectiveness and the safety of this intervention.

Beard samples were collected every day from a single person (55 [at start] to 66- [at end] years old) over a 12-year period. Beard hair samples were analyzed by particle induced X-ray emission (PIXE) to determine variations in concentrations of aluminum, chromium, arsenic, mercury and lead over the sampling period. Only mercury showed a gentle upward trend with age, and a significant increase of 20–30% was observed. This fact was consistent with the results of our previous studies on more than 1250 hairs collected from healthy people. It was also found that concentrations of many toxic elements tended to be higher in summer than in winter, except for chromium. In particular, arsenic and mercury concentrations significantly increased in summer. Their concentrations in the winter dropped to less than 50% and 86% of those in the summer, respectively. These seasonal changes can be attributed to changes in the ingestion of season-specific foods, such as marine products. These findings indicate that beard analyses are a powerful tool for evaluating the exposure to toxic-element.

Causes of the extraordinarily high and increasing incidence of snorkeler drownings in Hawai‘i have remained unexplained for years. Defining the mechanisms and factors predisposing to drowning while snorkeling is needed to provide recommendations to substantially mitigate the incidence of this form of preventable death. The mechanisms of drowning are described and insight into the predisposing factors are explored in this study. Methods included measuring snorkel airway resistance characteristics, case reports from the State of Hawai‘i Medical Examiner’s office, and collating information by survey, principally from rescued survivors. This study identified 2 modes of drowning while snorkeling that need further investigation: accidental or inadvertent aspiration, and hypoxia resulting from acute negative pressure pulmonary edema. The incidence of drowning from mechanisms of hypoxia due to rapid onset pulmonary edema is an important focus of the study and a number of potentially significant predisposing factors are presented that need further investigation but provide bases that may become part of updated policies and practices for snorkelers to substantially lower the risk of death. This report is meant for both medical and public health information purposes.

We present a precise measurement of the cross section, proton and $\rm ^3He$ analyzing powers, and spin correlation coefficient $C_{y,y}$ for $p$-$\rm ^3He$ elastic scattering near 65 MeV, and a comparison with rigorous four-nucleon scattering calculations based on realistic nuclear potentials and a model with $\Delta$-isobar excitation. Clear discrepancies are seen in some of the measured observables in the regime around the cross section minimum. Theoretical predictions using scaling relations between the calculated cross section and the $\rm ^3 He$ binding energy are not successful in reproducing the data. Large sensitivity to the $NN$ potentials and rather small $\Delta$-isobar effects in the calculated cross section are noticed as different features from those in the deuteron-proton elastic scattering. The results obtained above indicate that $p$-$\rm ^3He$ scattering at intermediate energies is an excellent tool to explore nuclear interactions not accessible by three-nucleon scattering., Comment: 22 pages, 6 figures

Background Pulmonary artery hypertension (PAH) is a poor prognostic disease. Some causative genes were reported as the PAH-associated genes. However, the pathogenetic variants in PAH-associated genes have not been identified in majority of patients with idiopathic PAH. Purpose Our aim was to investigate the new causative gene variants associated with PAH. Methods We performed whole-exome sequencing in 272 patients with idiopathic/heritable PAH. Structural analysis simulation was performed to define how the candidate gene variant affected the structure of protein. Results We identified the heterozygous substitution change of c.226G>A (p.Gly76Ser, rs146436713) in tumor necrotic factor receptor superfamily 13B gene (TNFRSF13B) (NM_012452.2) in 6 (2.2%) patients with idiopathic/heritable PAH, although the allele frequency of this rare variant is 0% in Integrative Japanese Genome Variation Database (control population database). Two of the six cases were blood relatives, although they did not have the known causative gene variants of PAH. One of these two relatives died of right heart failure despite the combination medical therapy, and her pathological anatomy demonstrated intimal thickening and medial hypertrophy in the pulmonary arteries, formation of plexiform lesions (Heath-Edwards classification grade V). Time-lapse images from structural analysis simulation showed the instability of N-terminal in the protein, which regulates the vascular inflammation, synthesized from TNFRSF13B p.Gly76Ser variant (Figure), suggesting that p.Gly76Ser variant may be involved in the development of PAH via aberrant inflammation in pulmonary vessels. More than 4000 beard samples were collected from a single person every morning over a 12-year period and analyzed using a standard-free method. We detected significant annual variations in the concentrations of magnesium and calcium in the study period, although significant monthly and seasonal variations were not noted. Calcium tends not to be obtained from season-specific foods or drinks, so its variations may be affected by changes in the metabolism of the body. In contrast, however, potassium showed marked long-term variations with significant seasonal differences. As our previous studies confirmed that green vegetables contain large quantities of potassium and that their supply and consumption increase from May to July, the seasonal variations in potassium concentration were attributed to the ingestion of green vegetables. Although magnesium is also contained in green vegetables in large quantities, its principle supply sources are diverse and its concentration did not show any clear seasonal changes.

A total of 4269 beard samples were collected from the same person every day over a 12-year period and analyzed by PIXE using a standard-free method. It was found that the concentrations of copper and zinc showed certain short-term changes but did not show a noticeable long-term trend over the study period, with only iron showing a slightly decreasing tendency with age. All of these elements showed clear yearly variations with a cycle of a few years possibly due to periodic metabolic changes in the subject’s body or long-term changes in eating habits. In contrast, however, selenium showed clear seasonal variations. Its concentration significantly increased in the summer and decreased in the winter, just as was observed with arsenic and mercury. This suggests that most of the subject’s selenium intake was from marine products, whose supply and consumption were increased in the summer. These findings confirmed that beard analyses are useful not only for evaluating essential-element intake but also for estimating the relationship between the body-element concentrations and ingestion of certain foods.

Background The aims of this study were to clarify the changes of patellar tendon length during isometric knee joint extension and the double leg squat position using ultrasonography. Methods The left legs of 17 healthy adults were investigated. Isometric knee extension motion was performed at three positions of knee flexion 30° (knee 30°), knee flexion 60° (knee 60°), knee flexion 90° (knee 90°), and at each limb position, 0% (0% peak torque (PT)), 40% (40% PT), 50% (50% PT), and 60% (60% PT) of the maximum knee joint extension torque were executed at random. Both double leg squat motions were randomly performed in three positions: hip flexion 30°, knee flexion 30°, ankle dorsiflexion 10° (squat 30°); hip joint flexion 60°, knee joint flexion 60°, ankle dorsiflexion 20° (squat 60°); and hip joint flexion 90°, knee joint flexion 90°, ankle dorsiflexion 30° (squat 90°). Ultrasonography was used to measure patellar tendon length. Findings There were no significant changes in patellar tendon length and strain between knee flexion angles of 30°, 60°, and 90° in isometric knee joint extension and the double leg squat limb position. Interpretation The loading rate and limb position do not appear to affect the length and strain of the patellar tendon.

We performed the experiment of p–^{3}3He scattering at 65 MeV by using polarized proton beams and the newly constructed polarized ^{3}3He target. The proton analyzing power A_{y}Ay, the ^{3}3He analyzing power A_{y}^{T}AyT, and the spin correlation coefficient C_{yy}Cyy were measured. In the conference, the experimental data were compared with the rigorous numerical calculations based on various nuclear potentials. Large discrepancies between the experimental data and the calculations were found in the A_{y}^{T}AyT and the {C_{yy}}Cyy at the backward angles.

Along continental margins, various types of methane seeps with a variety of flux rates, spatial extents, and durations occur. Although numerous studies have investigated spatially extensive high-flux-rate seeps, seeps with ephemeral and/or weak flows, known as diffusive seeps, have received less attention. This study investigates ancient diffusive methane seeps in the Shin'etsu sedimentary basin, central Japan, aiming to deduce their spatial extent and their sedimentological, paleontological, and geochemical properties. The seeps were identified as centimeter-scale carbonate concretions in two outcrops of Neogene siltstones in the basin. A comprehensive examination of the spatial distribution of the carbonates, their associated molluscan fossils, petrography, carbon and oxygen isotopic compositions, and lipid-biomarker contents of the concretions revealed that they originated from the anaerobic oxidation of methane (AOM) and formed via a weak, diffusive fluid flow. The small size and irregular or burrow-shaped morphologies of the concretions indicate that methane-bearing fluids diffused through small pore spaces such as burrows. Associated molluscan fossils consist of vesicomyids and other bivalves typical of methane seeps. Fossils of predator species were also identified, but epifaunal seep organisms are notably absent. The carbonate concretions are composed of strongly 13C-depleted micrite (δ13C values as low as −46.0‰ vs. VPDB) with minor 13C-rich void-filling sparry cements. The presence of lipid biomarkers pentamethylicosane (PMI; with δ13C values as low as −119‰) and biphytane and the absence of crocetane suggests that the AOM was performed by an ANME-1-dominated archaeal community. The insights of diffusive methane seepage in the geological record gained from this study can help to better understand depositional processes of seep deposits and variations in fluid flows in both ancient and modern methane-seeping systems.

We evaluated the formation of electric-field domains (EFDs) in three kinds of asymmetric double-quantum-well (ADQW) superlattices (SLs) whose photoluminescence (PL) properties revealed various characteristics affected by EFD formation. In particular, anomalous comb-shaped PL branches were clearly observed. Each PL branch corresponds to one EFD, giving important information on EFD distribution in ADQW-SLs. We determined the strengths of all electric-fields in all the EFDs from their redshifted PL signals and plotted their reverse bias voltage dependence. We clearly identified the EFD distributions in the ADQW-SLs in which the resonant tunneling effect plays an important role to make carrier transport paths. These results demonstrate that the analysis of EFD distributions from PL properties is an effective tool to investigate the carrier transport phenomena in biased SLs.

We investigated the relationships between the element concentrations in blood and hair to clarify the sex-specific differences in elemental movement from blood to hair, as we confirmed the presence of large sex-specific differences in the concentrations of certain elements in hair in our previous study. First, we compared the elemental concentrations in serum/plasma taken from healthy people ([Formula: see text] females, 142 males) and found that there were significant differences (up to 20%) between females and males in levels of phosphorus, sulfur, calcium, bromine, and lead. We then examined the sex-specific differences in the elemental transfer factors from blood to hair by analyzing plasma and hair samples that had been simultaneously collected from the same people (20 females, 21 males). Calcium and selenium showed significant differences in transfer factors from plasma to hair. Particularly, large sex-specific differences in the calcium concentration were explained by the difference in the transfer factors from blood to hair. We also examined the sex-specific differences among teenaged subjects alone ([Formula: see text] females, 9 males; average age was approximately 17 years old) and noted no significant difference in the transfer factors between sexes. Taken together, these findings indicate that the transfer factors themselves are influenced by the excretion of sex hormones.

An austenitic model alloy (316L) and a low alloy steel (A533B) were exposed to constant or fluctuating temperature after electron irradiation to a cumulative damage level of 1 displacement per atom. 316L model alloy was exposed to LWR operating temperature during electron irradiation, and were exposed to a higher temperature at a high heating and cooling rates. The annealing experiment after irradiation to 316L resulted in the change in irradiation-induced microstructure; both the size and the number density of Frank loop and black dots were decreased, while the volume fraction of void was increased. In the case of A533B, the aging experiment after electron irradiation resulted in the shrinkage or the disappearance of black dots and the growth of dislocation loops. It is suggested that during annealing and/or aging at a high temperature the excess vacancies could be provided and flew into each defect feature, resulting in that interstitial type feature could be diminished, while vacancy type increased in volume fraction if exists. Subsoil liquefaction during strong earthquakes is one of the major seismic threats to the human community. Over the past few decades various methodologies have been proposed to assess liquefaction vulnerability; however, recent seismic events in Japan have revealed that current assessment methods are over conservative, which results in unnecessarily higher costs in mitigation measures. To overcome this situation, this study reinterprets the dynamic behaviour of sandy subsoils subject to recent and historical ground motions from Japanese seismic events and is able to demonstrate that aged soils have a higher liquefaction resistance than suggested by current design codes. Laboratory tests were also devised and conducted to study the mechanism of soil ageing at the particle scale level. Grain dislocations were observed to take place over time while boundary stress conditions maintained constant. These dislocations erased big voids, promoted a more stable granular structure and have the potential to give rise to greater liquefaction resistance.

We performed the measurement of the cross section and the proton analyzing power \(A_y\) for p–\(^3\)He elastic scattering with a 65 MeV polarized proton beam at Research Center for Nuclear Physics (RCNP), Osaka University. The proton analyzing power \(A_y\) data are compared with the theoretical calculations based on the nucleon–nucleon potential (INOY04).

Background Advance care planning (ACP) is a crucial end-of-life care practice. However, an ACP educational program for practitioners in an acute care setting has not yet been established. This study aimed to examine the effects of an ACP educational program in acute hospital. Aim: This work is evaluating the effect of ACP education programme on acute hospital practitioners. Design: A mixed-methods, Pre-and post test study to evaluate changing practitioner attitudes post program. The intervention program was three times during the 3 month in 90 minutes per session. As a program evaluation, comments on participant’s questionnaires two questions about an attitude and practice were categorized and evaluated. We used conventional content analysis. Setting/Participants: The final sample of 63 practitioners whose they were working at B acute hospital. Results As a result of analysis of qualitative data, Question1 was categorized into five categories, and Question2 was categorized into seven categories. The results that participants thought important of ACP implementation and talk with patient and family. It was suggested that participants were able to look back on their own way of thinking about death and found the tips of ACP implementation. Conclusion The key message of this study was change sustained of awareness about positive attitude of ACP and EOL after 6 month intervention. These results suggest that the present ACP educational program was effective at improving staff attitudes towards the end of life care of patients.

This chapter explores the philosophical and ideological landscape of the so-called bushidō (way of the warrior) tradition in Japan. It contends that bushidō has often been misunderstood or misrepresented as only a simple code of conduct tied to the historical samurai. Instead, this chapter seeks to reveal the richness of bushidō as a sophisticated and complex field of philosophical inquiry into questions of ethics, justice, being, violence, conflict, and death. Drawing on intellectual and cultural traditions as diverse as Buddhism, Shintō, Confucianism, the Kyoto School, and currents of Western philosophy, bushidō’s full philosophical importance emerges only in the twentieth century. While the political and ideological dangers of aspects of bushidō were clearly manifest in this period, its philosophical potential was only just beginning to be understood.

Random amplified polymorphic DNA (RAPD) markers were used to determine the genetic stability of long-term (more than 10 years) micropropagated shoots of Japanese black pine (Pinus thunbergii Parl.). Thirty-six shoots consisting of three morphotypes (short, medium, and long needles) were randomly chosen from about 4,000 micropropagated shoots regenerated from the explants of a single nematode-resistant mother plant. Out of 126 primers screened, 30 gave 134 clear reproducible bands. A total of 4,824 bands obtained from these studies exhibited no aberration in RAPD banding patterns among the tested shoots. Our results show that regenerants from our plant micropropagation system are genetically stable.

We present a precise measurement of the cross section, proton and 3He analyzing powers, and spin correlation coefficient Cy,y for the p-3He elastic scattering near 65 MeV, and comparison with rigorous four-nucleon scattering calculations based on realistic nuclear potentials and a model with Δ-isobar excitation. Clear discrepancies are seen in some of the measured observables in the regime around the cross section minimum. Theoretical predictions using scaling relations between the calculated cross section and the 3He binding energy are not successful in reproducing the data. Large sensitivity to the NN potentials and rather small Δ-isobar effects in the calculated cross section are noticed as different features from those in the deuteron-proton elastic scattering. The results obtained above indicate that p-3He scattering at intermediate energies is an excellent tool to explore the nuclear interactions not accessible by the three-nucleon scattering.
