Abstract Background The ORBE II study aimed to describe the characteristics and clinical outcomes of adult patients with severe eosinophilic asthma (SEA) treated with benralizumab in a real-world setting in Spain. Methods ORBE II (NCT04648839) was an observational, retrospective cohort study in adult SEA patients who had been prescribed benralizumab. Demographic and clinical data of 204 SEA patients were collected 12 months prior to benralizumab initiation (baseline) and at follow-up. Exacerbation rate, asthma symptoms, maintenance oral corticosteroid (OCS) use and lung function were evaluated, among other variables. Results A total of 204 SEA patients were evaluated. Mean (standard deviation, SD) age of the study population was 56.4 (12.4) years, 62.3% were women and mean (SD) duration of asthma was 15.1 (12.7) years. Median (Q1–Q3) follow-up duration was 19.5 (14.2–24.2) months. At baseline, 72.6% of the overall population (OP) presented blood eosinophil counts ≥ 300 cells/µL; 36.8% had comorbid chronic rhinosinusitis with nasal polyps (CRSwNP); 84.8% reported at least one severe exacerbation, and 29.1% were OCS-dependent. At 1 year of follow-up, patients receiving benralizumab treatment had a 85.6% mean reduction in exacerbations from baseline, and 81.4% of patients achieved zero exacerbations. We also found a clinically relevant mean (SD) increase in pre-bronchodilator (BD) FEV1 of 331 (413) mL, with 66.7% of patients achieving a pre-BD FEV1 increase ≥ 100 mL, and 46.3% of patients achieving a pre-BD FEV1 ≥ 80% of predicted. Regarding symptom control, 73.8% of the OP obtained an ACT score ≥ 20 points. After 1 year of follow-up, mean reduction in the daily OCS dose was 70.5%, and complete OCS withdrawal was achieved by 52.8% of the OCS-dependent patients. Almost half (43.7%) of the OP on benralizumab met all four criteria for clinical remission. Patients with concomitant CRSwNP obtained similar or enhanced outcomes. Conclusions These data support the real-world benefits of benralizumab in SEA patients, and particularly in those with concomitant CRSwNP. Trial registration NCT04648839.

Resumen: Introducción: La tos crónica (TC) asociada a reflujo es muy frecuente en la práctica clínica, pero su diagnóstico no siempre es sencillo. La determinación de pepsina en saliva es un método novedoso, sencillo y no invasivo para el estudio de reflujo. Objetivo: Analizar rentabilidad diagnóstica de la determinación de pepsina en saliva en paciente con TC. Método: Estudio prospectivo sobre una cohorte consecutiva de pacientes con TC. Los pacientes se diagnostican de tos asociada a reflujo según 3 criterios: 1) presencia de síntomas típicos de reflujo gastroesofágico; 2) Reflux Symptom Index > 13, Reflux Findings Score ≥ 5, y 3) tránsito gastroesofágico, endoscopia digestiva o pHmetría-impedanciometría positiva. A todos los pacientes se les realizaron 2 determinaciones de pepsina en saliva, mediante la aplicación del Peptest®. Resultados: Se incluyó a 99 pacientes, de los cuales 64 (64,6%) fueron diagnosticados de TC asociada a reflujo, 9 (14,1%) como única causa y 55 (85,9%) asociada a otros diagnósticos. Cincuenta y seis tenían un Peptest® positivo, con una sensibilidad de la prueba del 87,5%, una especificidad del 65,7% y los valores predictivo positivo (VPP) y negativo (VPN) del 82 y el 74%, respectivamente. En pacientes sin síntomas digestivos, el VPP de la pepsina fue del 64% y el VPN del 88%. Conclusiones: El reflujo es una causa importante de TC y generalmente forma parte de una etiología multifactorial. La determinación de pepsina en saliva tiene una alta sensibilidad para reflujo, aunque con baja especificidad que a día de hoy limita su uso sistemático. Abstract: Introduction: Chronic cough (CC) associated with reflux is very common in clinical practice, but it is not always easy to diagnose. The determination of pepsin in saliva is a novel, simple and non-invasive method for the study of reflux. Objective: To analyze the diagnostic yield of the determination of pepsin in saliva in a patient with CC. Method: This was a prospective study conducted in a consecutive cohort of patients with CC. Patients were diagnosed with cough associated with reflux according to 3 criteria: 1) Presence of typical symptoms of gastroesophageal reflux disease (GERD), 2) Reflux Symptom Index > 13, Reflux Findings Score ≥ 5 and 3) gastroesophageal transit, gastrointestinal endoscopy, and/or positive impedance pH monitoring. All patients underwent 2 determinations of pepsin in saliva, using the Peptest®. Results: We included 99 patients, of which 64 (64.6%) were diagnosed with CC associated with reflux, 9 (14.1%) as the sole cause and 85.9% (55) associated with other diagnoses. Fifty-six had a positive Peptest®, with a sensitivity of 87.5%, specificity of 65.7%, and positive (PPV) and negative (NPV) predictive values of 82% and 74%, respectively. In patients without digestive symptoms, the PPV of pepsin was 64% and the NPV was 88%. Conclusions: Reflux is an important cause of chronic cough and usually forms part of a multifactorial etiology. The determination of pepsin in saliva has a high sensitivity for reflux, but a low specificity that currently limits its routine use.

Introduction There is currently no universally accepted definition of asthma−COPD overlap (ACO). Objective To compare the prevalence of ACO in patients with asthma or COPD, and to assess their clinical characteristics and the capacity of the different definitions to predict the risk of exacerbation. Method Prospective observational study with a 12-month follow-up in an asthma cohort and a COPD cohort. Four diagnostic criteria were compared: A) the Spanish 2012 consensus; B) the 2016 international consensus; C) the 2017 consensus between the Spanish COPD guidelines (GesEPOC) and GEMA asthma guidelines; and D) the single criterion of ≥300 eosinophils/μL, proposed by GOLD 2019. The risk of exacerbations was evaluated in each group. Results A total of 345 patients were included, 233 (67.5%) with COPD and 112 (32.5%) with asthma, aged 63 ± 14 years, 70.4% men. Fifteen (4.3%) patients met the criteria for ACO according to the criteria described under A above; 30 (8.7%) with the criteria of B; 118 (34.2%) with the criteria of C; and 97 (28.1%), with the D criterion. The ACO-COPD subtype were older, had worse lung function, and an increased risk of exacerbation compared with the ACO-asthma group. Of all the definitions evaluated, those which distinguished a higher risk of exacerbations were the GesEPOC-GEMA consensus and the GOLD proposal. Conclusions The prevalence of ACO varies enormously depending on the diagnostic criteria used. The ACO population is heterogeneous, and the ACO-COPD subtype is very different from the ACO-asthma subtype. The definitions that include eosinophilia identify ACO patients with a greater risk of exacerbation.

Resumen Introduccion En la actualidad no disponemos de una definicion de solapamiento asma-EPOC (ACO) universalmente aceptada. Objetivo Comparar la prevalencia del ACO en pacientes con asma o EPOC, valorar sus caracteristicas clinicas y la capacidad predictiva de riesgo de agudizacion de las diferentes definiciones. Metodo Estudio prospectivo observacional con seguimiento de 12 meses sobre una cohorte de asma y otra con EPOC. Se comparan 4 criterios diagnosticos: A) el consenso espanol del 2012; B) el consenso internacional del 2016; C) el consenso entre la guia espanola de la EPOC (GesEPOC) y la del asma (GEMA) del 2017; y D) el criterio unico de ≥ 300 eosinofilos/μl, propuesto por GOLD 2019. En cada grupo se evaluo el riesgo de agudizaciones. Resultados Se incluyeron 345 pacientes, 233 (67,5%) con EPOC y 112 (32,5%) con asma, con una edad de 63 ± 14 anos; el 70,4% eran hombres. Quince (4,3%) pacientes cumplian criterios de ACO con criterios A; 30 (8,7%) con los B; 118 (34,2%) con los C; y 97 (28,1%) con el criterio D. El subtipo ACO-EPOC presento mas edad, peor funcion pulmonar y mayor riesgo de agudizacion que los ACO-asma. De todas las definiciones evaluadas, las que presentaron mayor riesgo de agudizaciones fueron la del consenso GesEPOC-GEMA y la propuesta de GOLD. Conclusiones La prevalencia de ACO varia enormemente en funcion del criterio diagnostico utilizado. La poblacion de ACO es heterogenea, con un subtipo ACO-EPOC muy diferente al ACO-asma. Las definiciones que incluyen la eosinofilia identifican a los ACO de mas riesgo de agudizacion.

[ES] La motivación que ha llevado a realizar este proyecto es el diseño de un seguidor solar para Valencia con la instalación de módulos solares fotovoltaicos y la producción eficiente de energía eléctrica. Esto lleva implícito la mejora del rendimiento respecto a un sistema fijo convencional. Para el diseño del seguidor solar tendremos en cuenta: • Los conocimientos sobre el comportamiento del sol, para poder optimizar y aprovechar de manera eficiente la radiación recibida. • La robustez del seguidor para prevenir fallos y averías que perjudiquen a la producción ya que la se encuentra expuesto a las inclemencias del tiempo que pueden deteriorar las características de los materiales. • El diseño del producto para conseguir un buen precio en relación a su potencia y así competir con los sistemas fijos.

BACKGROUND: The efficacy and safety of benralizumab, an interleukin-5 receptor alpha-directed cytolytic monoclonal antibody, for the prevention of exacerbations in patients with moderate to very severe chronic obstructive pulmonary disease (COPD) are not known.METHODS: In the GALATHEA and TERRANOVA trials, we enrolled patients with COPD (at a ratio of approximately 2:1 on the basis of eosinophil count [≥220 per cubic millimeter vs. RESULTS: In GALATHEA, the estimates of the annualized exacerbation rate were 1.19 per year (95% confidence interval [CI], 1.04 to 1.36) in the 30-mg benralizumab group, 1.03 per year (95% CI, 0.90 to 1.19) in the 100-mg benralizumab group, and 1.24 per year (95% CI, 1.08 to 1.42) in the placebo group; the rate ratio as compared with placebo was 0.96 for 30 mg of benralizumab (P = 0.65) and 0.83 for 100 mg of benralizumab (P = 0.05). In TERRANOVA, the estimates of the annualized exacerbation rate for 10 mg, 30 mg, and 100 mg of benralizumab and for placebo were 0.99 per year (95% CI, 0.87 to 1.13), 1.21 per year (95% CI, 1.08 to 1.37), 1.09 per year (95% CI, 0.96 to 1.23), and 1.17 per year (95% CI, 1.04 to 1.32), respectively; the corresponding rate ratios were 0.85 (P = 0.06), 1.04 (P = 0.66), and 0.93 (P = 0.40). At 56 weeks, none of the annualized COPD exacerbation rate ratios for any dose of benralizumab as compared with placebo reached significance in either trial. Types and frequencies of adverse events were similar with benralizumab and placebo.CONCLUSIONS: Add-on benralizumab was not associated with a lower annualized rate of COPD exacerbations than placebo among patients with moderate to very severe COPD, a history of frequent moderate or severe exacerbations, and blood eosinophil counts of 220 per cubic millimeter or greater (Funded by AstraZeneca [GALATHEA and TERRANOVA] and Kyowa Hakko Kirin [GALATHEA]; GALATHEA and TERRANOVA ClinicalTrials.gov numbers, NCT02138916 and NCT02155660.).

18. Clinical Characteristics and Risk of Exacerbations Associated With Different Diagnostic Criteria of Asthma-COPD Overlap

Introduction: There is currently no universally accepted definition of asthma-COPD overlap (ACO). Objective: To compare the prevalence of ACO in patients with asthma or COPD, and to assess their clinical characteristics and the capacity of the different definitions to predict the risk of exacerbation. Method: Prospective observational study with a 12-month follow-up in an asthma cohort and a COPD cohort. Four diagnostic criteria were compared: A) the Spanish 2012 consensus; B) the 2016 international consensus; C) the 2017 consensus between the Spanish COPD guidelines (GesEPOC) and GEMA asthma guidelines; and D) the single criterion of >= 300 eosinophils/mu L, proposed by GOLD 2019. The risk of exacerbations was evaluated in each group. Results: A total of 345 patients were included, 233 (67.5%) with COPD and 112 (32.5%) with asthma, aged 63 +/- 14 years, 70.4% men. Fifteen (4.3%) patients met the criteria for ACO according to the criteria described under A above; 30 (8.7%) with the criteria of B; 118 (34.2%) with the criteria of C; and 97 (28.1%), with the D criterion. The ACO-COPD subtype were older, had worse lung function, and an increased risk of exacerbation compared with the ACO-asthma group. Of all the definitions evaluated, those which distinguished a higher risk of exacerbations were the GesEPOC-GEMA consensus and the GOLD proposal. Conclusions: The prevalence of ACO varies enormously depending on the diagnostic criteria used. The ACO population is heterogeneous, and the ACO-COPD subtype is very different from the ACO-asthma subtype. Background It has been documented that anxiety and depression are prevalent in patients with asthma and are associated with greater frequency of exacerbations, increased use of health care resources, and poor asthma control. Objective To examine the association of asthma diagnosis with symptoms of depression/anxiety and asthma control not only at baseline but also over a 6-month period of specialist supervision. Methods We enrolled 3182 patients with moderate to severe asthma. All were evaluated with spirometry, the Asthma Control Test, and the Hospital Anxiety and Depression Scale at baseline and at 6 months. Treatments were decided by specialists according to published guidelines. Results At baseline, 24.2% and 12% of the patients were diagnosed with anxiety and depression, respectively, according to the Hospital Anxiety and Depression Scale. After 6 months, anxiety and depression improved, affecting 15.3% and 8.1% of patients, respectively (P Conclusion Under standardized asthma care and after a specific visit with the specialist, patients present significant improvement in these psychological disorders and exhibit better asthma control and functional parameters.

Objective To assess socio-sanitary expenditure after the addition of omalizumab to standard treatment in the control of severe asthma and to analyse its effectiveness under standard clinical practice. Method Observational retrospective multicentre study conducted in 12 pneumology services in the Valencian Community, Spain. Data from 186 patients were analysed. Results of the year before and after the addition of omalizumab were compared. Effectiveness was calculated based on a 3- point increase in the Asthma Control Test and a reduction in the number of annual exacerbations. Utility was calculated by the number of quality adjusted life years. The economic assessment included both direct and indirect costs and results were expressed in terms of incremental cost-effectiveness and incremental cost-utility ratio. Results Significant improvements were found in lung function, asthma control, quality of life, and quality adjusted life years between the year before and after the introduction of omalizumab. Taking into account direct and indirect costs, the incremental cost-effectiveness for each avoided exacerbation was € 1,789.28 (95% CI: € 1,019.13-3,038.12) and € 4,569.38 (95% CI: 3,442.86-6,075.05) per 3-point increase in the Asthma Control Test score. The incremental cost-utility ratio per quality adjusted life years gained was € 50,239.98 (95% CI: 37,209.88-68,923.84). Conclusions The addition of omalizumab to the treatment regime of patients with severe asthma is effective under standard clinical practice, decreases direct and indirect costs, and provides significant improvements in the health status of patients. Resumen Objetivo Evaluar el gasto sociosanitario tras la incorporación de omalizumab al tratamiento estándar en el control del asma grave, así como analizar su efectividad, en condiciones de práctica clínica. Método Estudio observacional, retrospectivo y multicéntrico realizado en 12 servicios de neumología de la Comunidad Valenciana. Se analizaron datos de 186 pacientes. Se compararon resultados del año previo y los cinco años posteriores a la incorporación de omalizumab. La efectividad se calculó a partir del incremento de tres puntos en el Asthma Control Test y la reducción del número de exacerbaciones anuales. La utilidad se calculó mediante el número de años de vida ajustados por calidad. En la evaluación económica se incluyeron costes directos e indirectos. Los resultados se expresaron en términos de relación coste-efectividad incremental y relación coste-utilidad incremental. Resultados Se detectaron mejoras significativas en la función pulmonar, el control del asma, la calidad de vida y el número de años de vida ajustados por calidad, entre el año anterior y el posterior al inicio de omalizumab. Teniendo en cuenta los costes directos e indirectos, la relación coste-efectividad incremental por exacerbación evitada fue de 1.789,28 € (intervalo de confianza 95%: 1.019,13-3.038,12) y de 4.569,38 € (intervalo de confianza 95%: 3.442,86-6.075,05) por incremento de tres puntos en el Asthma Control Test. La relación coste-utilidad incremental por número de años de vida ajustados por calidad ganada fue de 50.239,98 € (intervalo de confianza 95%: 37.209,88-68.923,84). Benralizumab is an interleukin-5 receptor α-directed cytolytic monoclonal antibody that has been shown to safely reduce exacerbations and improve lung function for patients with asthma. We assessed the long-term safety and efficacy of benralizumab for patients with severe, uncontrolled eosinophilic asthma.We conducted a randomised, double-blind, parallel-group, phase 3 extension study at 447 sites in 24 countries. Eligible patients had to have completed the SIROCCO or CALIMA trials and remained on subcutaneous benralizumab 30 mg every 4 weeks (Q4W) or every 8 weeks (Q8W). Patients who had received placebo in those trials were re-randomised in a 1:1 ratio, using an interactive web-based system, to benralizumab 30 mg either Q4W or Q8W (first three doses 4 weeks apart). Treatment lasted for 56 weeks for adult patients (age ≥18 years) and 108 weeks for adolescent patients (age 12-17 years). The primary endpoint was the safety and tolerability of the two dosing regimens of benralizumab up to 68 weeks for adult patients (including the follow-up visit post-treatment) and up to 56 weeks for adolescent patients. This endpoint was assessed in the full analysis set, which included all patients from the SIROCCO and CALIMA predecessor studies who received at least one dose of study treatment in BORA and did not continue into another trial. This study is registered with ClinicalTrials.gov (NCT02258542).Between Nov 19, 2014, and July 6, 2016, we enrolled 1926 patients, of whom 633 had received benralizumab Q4W and 639 had received benralizumab Q8W in SIROCCO or CALIMA. The remaining 654 patients had received placebo in those trials and were randomly re-assigned in this trial to receive benralizumab Q4W (n=320) or Q8W (n=334). 1576 patients, including 783 who received benralizumab Q4W (265 newly assigned) and 793 who received benralizumab Q8W (281 newly assigned), were included in the full analysis set. The most common adverse events in all groups were viral upper respiratory tract infection (14-16%) and worsening asthma (7-10%). The most common serious adverse events were worsening asthma (3-4%), pneumonia (1% to 1%), and pneumonia caused by bacterial infection (0-1%). The percentages of patients who had any on-treatment adverse event, any serious adverse event, or any adverse event leading to treatment discontinuation during BORA were similar between patients originally assigned benralizumab and those originally assigned placebo and between benralizumab treatment regimens. The percentage of patients who had any adverse event was similar between SIROCCO or CALIMA (71-75%; benralizumab group only) and BORA (65-71%), as was the percentage of patients who had an adverse event that led to treatment discontinuation (2% in SIROCCO and CALIMA vs 2-3% in BORA).The 2 years of safety results validate that observations observed in the first year of benralizumab continued through a second year of treatment. No new consequences of long-term eosinophil depletion occurred, and the incidence of other adverse events, including opportunistic infections, were similar during the second year.AstraZeneca and Kyowa Hakko Kirin.

22. Effectiveness and pharmacoeconomic analysis of the treatment of severe asthma with omalizumab in clinical practice

Objective: To assess socio-sanitary expenditure after the addition of omalizumab to standard treatment in the control of severe asthma and to analyse its effectiveness under standard clinical practice. Method: Observational retrospective multicentre study conducted in 12 pneumology services in the Valencian Community, Spain. Data from 186 patients were analysed. Results of the year before and after the addition of omalizumab were compared. Effectiveness was calculated based on a 3 point increase in the Asthma Control Test and a reduction in the number of annual exacerbations. Utility was calculated by the number of quality adjusted life years. The economic assessment included both direct and indirect costs and results were expressed in terms of incremental cost effectiveness and incremental cost utility ratio. Results: Significant improvements were found in lung function, asthma control, quality of life, and quality adjusted life years between the year before and after the introduction of omalizumab. Taking into account direct and indirect costs, the incremental cost effectiveness for each avoided exacerbation was (sic) 1,789.28 (95% CI: (sic) 1,019.13-3,038.12) and (sic) 4,569.38 (95% CI: 3,442.86-6,075.05) per 3-point increase in the Asthma Control Test score. The incremental cost-utility ratio per quality adjusted life years gained was (sic) 50,239.98 (95% CI: 37,209.88-68,923.84). Conclusions: The addition of omalizumab to the treatment regime of patients with severe asthma is effective under standard clinical practice, decreases direct and indirect costs, and provides significant improvements in the health status of patients.

Resumen La mayor expectativa de vida y el progresivo envejecimiento de nuestra poblacion estan produciendo un cambio en el patron epidemiologico asistencial, con una disminucion de las enfermedades agudas y un pronunciado aumento de las cronicas. Este importante cambio conlleva notables consecuencias sociales, sanitarias y economicas, lo que plantea la necesidad de una reordenacion en la forma de atender a nuestros pacientes. En este contexto, la Estrategia para el abordaje de la cronicidad del Sistema Nacional de Salud propone un cambio de enfoque sustancial para pasar de una medicina de rescate tradicional a una medicina centrada en el paciente y su entorno, una medicina planificada, proactiva, participativa y multidisciplinar. Algunas de las enfermedades cronicas mas prevalentes son de origen respiratorio. En la EPOC, esta aproximacion integral ha demostrado reducir las exacerbaciones, mejorar la calidad de vida e incluso disminuir el coste. Sin embargo, la heterogeneidad de las intervenciones dificulta las conclusiones definitivas. Algo parecido sucede en asma y en otras enfermedades respiratorias. La neumologia puede y debe participar de los nuevos paradigmas de la cronicidad, aportando conocimiento, experiencia, innovacion, investigacion y valor anadido.

Longer life expectancy and the progressive aging of the population are changing the epidemiological pattern of healthcare, with a reduction in the incidence of acute diseases and a marked increase in chronic diseases. This change brings important social, healthcare and economic consequences that call for a reorganization of patient care. In this respect, the Spanish National Health System has developed a Chronicity strategy that proposes a substantial change in focus from traditional rescue medicine to patient- and environment-centered care, with a planned, proactive, participative and multidisciplinary approach. Some of the more common chronic diseases are respiratory. In COPD, this integrated approach has been effective in reducing exacerbations, improving quality of life, and even reducing costs. However, the wide variety of management strategies, not only in COPD but also in asthma and other respiratory diseases, makes it difficult to draw definitive conclusions. Pulmonologists can and must participate in the new chronicity models and contribute their knowledge, experience, innovation, research, and special expertise to the development of these new paradigms.

[ES] Realización de programa de recogida y análisis de datos de proyecciones funcional para toda la planta Body III y mejora de la calidad en puntos de soldadura por resistencia en la estación 6Y.El presente proyecto se inicia con el sistema de adquisición de datos proveniente de las máquinas de soldadura (Bosch6000). Posteriormente se deberá desarrollar un sistema de análisis de dichos datos que permita concluir con varios informes automatizados. Finalmente deberá programarse la máquina de soldadura, cambiando los parámetros de soldado para evitar proyecciones (errores en los puntos de soldadura).

[ES] Realización de programa de recogida y análisis de datos de proyecciones funcional para toda la planta Body III y mejora de la calidad en puntos de soldadura por resistencia en la estación 6Y.El presente proyecto se inicia con el sistema de adquisición de datos proveniente de las máquinas de soldadura (Bosch6000). Posteriormente se deberá desarrollar un sistema de análisis de dichos datos que permita concluir con varios informes automatizados. Finalmente deberá programarse la máquina de soldadura, cambiando los parámetros de soldado para evitar proyecciones (errores en los puntos de soldadura).

[ES] La motivación que ha llevado a realizar este proyecto es el diseño de un seguidor solar para Valencia con la instalación de módulos solares fotovoltaicos y la producción eficiente de energía eléctrica. Esto lleva implícito la mejora del rendimiento respecto a un sistema fijo convencional. Para el diseño del seguidor solar tendremos en cuenta: • Los conocimientos sobre el comportamiento del sol, para poder optimizar y aprovechar de manera eficiente la radiación recibida. • La robustez del seguidor para prevenir fallos y averías que perjudiquen a la producción ya que la se encuentra expuesto a las inclemencias del tiempo que pueden deteriorar las características de los materiales. • El diseño del producto para conseguir un buen precio en relación a su potencia y así competir con los sistemas fijos.

Background Although airway hyperresponsiveness to inhaled acetaldehyde has been documented in Japanese patients with asthma, the response to this bronchoconstrictor agent has never been studied in Caucasians. Objectives The objectives of the study were to determine differences in airway responsiveness to acetaldehyde between asthmatic and healthy subjects, and to examine the relationship between acetaldehyde responsiveness and the variability of peak expiratory flow (PEF). Methods The response to methacholine and acetaldehyde challenges was measured in 81 non-smoking adults (61 asthmatics and 20 normal controls). Subjects recorded PEF morning and evening for 14 days. The response to both bronchoconstrictor agents was measured by the PC20 (provocative concentration required to produce a 20% fall in FEV1). PEF variation was expressed as amplitude percentage mean, and as low percentage best (lowest PEF expressed as a percentage of the best PEF recorded). Results The two types of challenge yielded a similarly high level of sensitivity (100% for methacholine and 92% for acetaldehyde) and specificity (90 and 100%, respectively) to distinguish between asthma and controls. Asthmatic subjects were on average 265-fold less sensitive to acetaldehyde than to methacholine. PC20 acetaldehyde correlated weakly but significantly with both indices of PEF variation (amplitude percentage mean: ρ = − 0.36, P = 0.004; low percentage best: ρ = 0.42, P = 0.001). Conclusions These results indicate that airway hyperresponsiveness to acetaldehyde is a sensitive and specific indicator for separating asthmatic and normal subjects. Airway responsiveness to methacholine or acetaldehyde and PEF variation are not reflecting the same pathophysiological process in the airways.

Se estudian 467 sujetos comprendidos entre los 35 y 65 anos de edad, extraidos de una muestra seleccionada aleatoriamente de la poblacion del Area Sanitaria 5 de la Comunidad Valenciana. A traves de un cuestionario y de una espirografia se obtienen tasas para sintomas respiratorios, tabaquismo, antecedentes pulmonares, profesionales y funcion pulmonar. Se observa una prevalencia para el asma del 2,78% (IC: 2,76-2,79), para la bronquitis cronica del 4,0% (IC del 2,375,77) y para la limitacion al flujo aereo del 6,4% (IC: 4,5-8,3). El habito tabaquico estaba presente en el 33,2%. Se detecta una relacion significativa entre el grado de intensidad de los sintomas y funcion pulmonar. No hay diferencias entre fumadores y no fumadores con respecto al FEV1, % y la relacion entre exposicion laboral y espirografia solo se observa para el FEV1/FVC %.

Figures for respiratory symptoms, smoking, previous lung disease, professional activity and lung function were obtained through questionnaires and spirometry. The prevalence of asthma found was 2.78% (C.I. 2.76-2.79); chronic bronchitis was 4.07% (C.I. 2.37-5.77) and airflow limitation was 6.4% (C.I. 4.5-8.3). Smokers represented 33.2% of the sample. A significant association was found between the level of intensity of symptoms and lung function. There were no differences between smokers and non smokers with respect to FEV1%. An association between workplace exposure and spirometry was found only for FEV1/FVC%.

Pulmonary infection by Pneumocystis carinii in patients with acquired immunodeficiency syndrome (AIDS) can result in different radiological patterns with an ever expanding spectrum. A 40-year-old male, infected with the human immunodeficiency virus (HIV), presented with toxic symptoms and multiple pulmonary cystic lesions in the context of a Pneumocystis carinii pneumonia. The rarity of this radiological presentation is discussed and literature is reviewed. In addition, the possible pathogenetic mechanisms are discussed, and emphasis is made on the need for higher suspicion index in similar presentations in patients at risk of HIV infection.
