Introduction Preeclampsia (PE) causes morbidity and mortality in several women in the productive phase, including definitive sequels. Recognizing the socio-demographic profile of pregnant women may justify the allocation of resources to promote an effective care for this population. Objective To describe the socio-demographic profile of pregnant women diagnosed with preeclampsia. Method Retrospective study of medical records of 98 patients. Place: Hospital Guilherme Alvaro Santos/ Brazil, (from May 2017 to April 2018). Variables analyzed: Previous history of systemic arterial hypertension (SAH) and preeclampsia, first-degree relative with SAH, recreational drug use, cohabitation with the partner, pre-eclampsia, employment status, years of schooling, average number of pregnancies and body mass index (BMI). For all the information in the sample, a description of the data was made through numerical means and descriptive measures of variables with the intention of searching for clues about the behavior of the phenomena studied. The study has ethical approval. Result We identified that 53.1% had previous SAH; 29% reported a history of preeclampsia; 83% had first-degree relative with SAH; 19% used recreational drugs; 32% lived with partner; 38% were unemployed. How many means: the time of schooling was of 10,23 years; that of pregnancies of 2.27 previous; and the BMI of 31.93 (obesity). Discussion These data suggest that it is necessary to create strategies for reproductive planning access for this population because several women were in the third gestation and advised of the risks identified in their own history as relatives with SAH. Avoiding additional complications due to drug use, especially among obese women, hypertensive and with a history of pre-eclampsia and that these guidelines need to be reinforced also by low education. These aspects can be recognized by the anamnesis itself, which can optimize resources and optimize financial resources.

Introduction Current diagnostic criteria for pre-eclampsia include organic dysfunctions. Recognizing predictive signs of complications in the initial clinical evaluation could optimize resources guiding care strategies. Objective To describe the status of pre-eclampsia patients in the initial evaluation by clinical and laboratorial parameters. Methods Retrospective study approved by Ethics Committee. Location: Guilherme Alvaro Hospital, Santos, Sao Paulo/Brazil; (May 2017 - April 2018). Inclusion: 85 patients. with pre-ecampsia, according to NHBPEP (2000) criteria. Analized Variables: Gestational age and severe admission complications, pressure values, symptomatology (visual, headache, epigastralgia) and medication use. For the sample’s information, numerical means and descriptive measures were used. Results The means of gestational age was 31.7 weeks, systolic and diastolic blood pressures 137.51 and 83.34 mmHg respectively and weight 91.67 kg. 18,8% of urine dipsticks tests were negative or trace. The symptomathic evaluation, 11.8% was visuals (scotoma), 21.2% headache and 12.9% epigastric pain. For medication, 23.5% used Acetylsalicylic acid (ASA), 89.4% methyldopa and 28.2% corticosteroid. About 1.2% of the patients presented placental abruption, 1.2% eclampsia and 5.9% thrombocytopenia as severe complications at admission. No fetal or maternal death occurred in the current analyzed population. Discussion Early clinical evaluation may demonstrate the potential risk of prematurity associated with pre-eclampsia, once the mean of gestacional week was 31 weeks. A considerable fraction of the analyzed women presented obesity and serious clinicals expressions of pre-eclampsia, although approximately 20% was not recognized by the initial proteinuria test screening. An expressive amount of the patients used ASA, without a characterization about the use. Most patients used at least one hypotensive agent, which could be an exaggerated expectation for these drugs. Although just a few patients presented serious complications these risks should not be neglected, mainly the thrombocytopenia. We also observed that some clinical-laboratory data can guide actions since the admission of the pregnant woman.

Introduction The maternity mortality rate in the Santos lowlands region, priority region of Sao Paulo/Brazil, is higher when compared to regions in the state of Sao Paulo. The hypertensive disorders are implicated in approximately 20% of death. Severe preeclampsia (PE) is recognized as the frequent diagnosis associated with maternal and fetal ominous outcomes. The recognition of potential risk support antenatal intervention which could anticipate the installation of catastrophic results. Identifying the epidemiological profile of women who may present with this evolution can guide public health policies and actions of the professionals involved in caring for these women avoiding preformed concepts and contributing to the recognition of local reality situation. Objectives Identify the epidemiological profile of women affected by severe PE. Methods A descriptive study involving 46 pregnant women admitted to specialized hospital for high-risk pregnancy with the diagnosis of severe PE according to the criteria of NHBPEP (2000), as: blood pressure ⩾160/110 mmHg, proteinuria 2.0 g/24 h or +2 dipstick, serum creatinine >1.2 mg/dl (new onset), platelets 3 , microangiopathic hemolysis (increased lactate dehydrogenase), elevated alanine aminotransferase or aspartate aminotransferase, persistent headaches or other cerebral or visual disturbance, persistent epigastric pain or eclampsia in the period from January/2008 to November/2010. Results The average age of the pregnant women was 25.5 years (±6.5), 11 women were multiparous and nulliparous and 35 (76.1%), the average gestational age at admission was 34 4/7, developing to eclampsia and/or HELLP syndrome were 07 women (15.2%), admission often occurred before the installation of labor in (93.5%) had a previous diagnosis of chronic hypertension (30.4%) and in all cases magnesium sulfate administration with an average length of 32 h (±16.6 h). Conclusion The occurrence of severe PE was more common in nulliparous women at the age of 25 years, admitted in prematurity and not in preterm labor. The outcome of seizure and/or HELLP has a considerable impact. The association of chronic hypertension with severe PE is more than 1/4 of the cases. These data are equivalent to other areas of Brazil and may contribute to emphasize the practice of prenatal acting that can recognize and pre-comment possible adverse developments and attitudes aimed at intercepting the process.

Introduction Maternal death is the death of a woman during pregnancy within 42 days after delivery. It is one of the most serious violations of human rights of women, because it is a preventable tragedy in 92% of cases and occurs mainly in developing countries. Brazil, a country of continental dimensions has the challenge of reducing maternal deaths; one of the priority regions is Santos, a city in the state of Sao Paulo. It is felt that efforts are made to the adoption of care that can minimize risks during complicated pregnancy with hypertensive disorders, but not always this service is offered proportionally to postpartum. Perhaps the erroneous concept that the delivery is capable of quickly balancing the harm of maternal hypertensive disorders is implicated in reducing postpartum surveillance, exposing this group of women to undesirable outcomes. Objectives Analyze the percentage of postpartum maternal deaths until 42 days after delivery for hypertensive disorders in Santos in the period of 1999–2009. Methods Data were provided by the Ministry of Health of Brazil in www.datasus.gov.br (accessed February 2012). The number of maternal deaths was divided into maternal death in pregnancy, delivery and abortion and maternal death in puerperium up to 42, considering all causes of maternal death and identifying the proportion of deaths due to hypertensive syndromes. Results According to the general causes of death, more women die during pregnancy, delivery and abortion (69.7%) than postpartum (30.3%) and hypertensive disorders account for 20% of these outcomes. Analyzing the proportion of deaths due to hypertensive disorders in each period, we observed that during pregnancy the disorder accounts for 16.5% of all causes, however, the percentage reaches 30.3% in the puerperium. Conclusion The increase of death in postpartum due to hypertensive syndromes, regarding all general causes, shows the necessary action to be done by the professional team to offer continuity care in and out of hospital in this period, considering that the challenges persist even after delivery.

Introduction Know the additional risks associated with hypertensive disorders in pregnancy, such as the use of licit and illicit substances, may contribute to the adoption of strategies that minimize adverse outcomes. Objective Estabilish the prevelence of the use of harmful substances such as alcohol, tobacco and ilicit drugs in pregnant women in relation to healthy pregnant women. Methodology A transversal study has been conducted in the prenatal and maternity of the Hospital Guilherme Alvaro from April to June 2016. 61 patients were selected, 34 from the study group and 27 from de control group. Among the patients in the study group, are included pregnant or postpartum women with hypertensive disorders, according NHBPEP 2000. In the control group, are included only pregnant or postpartum women without previous comorbidity, which gave birth to one healthy live newborn, vaginal delivery. It was applied two questionnaires, ASSIST, screening test of involvement with alcohol, tobacco and illicit substances, developed by WHO, useful for health professionals to identify the use and dependence on psychoactive substances, evaluated through a score, the answer ranges from 0 to 4 points, and the sum can range from 0 to 20, it is considered casual use 0 to 3, 4 suggestive of abuse 4–15 and suggestive of addiction 16–20. And another that evaluates epidemiological woman information, current and obstetric pregnancy as well as age, color, living city, number of consultations prenatal, site of consultations, among others. The data were passed to the Excel program and analyzed quantitatively, to compare Study and Control groups on the ASSIST score related to each psychoactive drug, we used the Student’s T Test for unrelated samples. It was considered significant samples where p Results The obtained results, shown below, do not allow to say that the groups differ, although the sample size and the low frequency of drug use in the groups studied compromise the accuracy of information. In percentage, 80.32% of all pregnant women have proved any drugs. Among the groups, the Study 70.83% have used drugs and in the control group 81.48% have used drugs. Psychoactive drugs most used by pregnant women are: alcohol 77,0% and tobacco 42,6%. Although we saw that the Hypertensive group made greater use of cocaine, amphetamine and opioids than those of the Control group. The other comparisons were not significant and the larger sample can clear the results.

Introduction Possibly the nitric oxide production is decreased in patients with preeclampsia. Therefore this molecule could be used as a potential marker. Objective Identify differences in nitric oxide serum levels between postpartum women with and without preeclampsia and non pregnant women. Methods Cross-sectional study developed any Hospital Guilherme Alvaro Santos/Brazil and the University Hospital Antonio Pedro, Niteroi/Brazil, between (December/2015 and May/2016). Were collected 106 blood samples in the mediate postpartum (32 study group, 42 control group and 42 non pregnant women). The study group was composed by pregnants with preeclampsia according to criteria of NHBPEP (2000) and gave birth to a single newborn. Control group: spontaneous vaginal delivery, term newborn, without hypertension and/or any other comorbidities. The group of non-pregnant women was included healthy volunteers, between 20 and 40 years old, without any contagious infectious, genetic or metabolic disease. Exclusion criteria: collagen disease, smoking, twin pregnancy, fetal malformations and fetal death. Initially it was measured serum value of nitric oxide in these three groups. Then it was analyzed the levels of this metabolic only in women who have had preeclampsia to correlate to the following variables: maternal age, gestational age, body mass index (BMI), systolic blood pressure, diastolic blood pressure, neck, arm and abdominal circumferences, newborn weight, first and fifth minute Apgar score, somatic Capurro, adequacy weight for gestation age, newborn ICU admission and association with diabetes mellitus and arterial hypertension. This research has ethical approval. The statistical analysis was performed using Fisher’s exact test to the comparisons between two groups the Man–Whitney test. The significance level is p Results Comparison of nitric oxide results concentration in the preeclampsia group – average (*p Dosing nitric oxide serum levels in women with preeclampsia (n = 32) the value was 30,15 μ M, without preeclampsia (n = 42) 30,03 μ M and non pregnant women (n = 32) was 17,23 μ M. We could identify a significant statistical difference between the postpartum women compared to the non-pregnant, but not among themselves. Comparing variables and serum levels of nitric oxide from women with preeclampsia, we observed that the value is directly proportional to the age of the patient, with statistical significance. Other variables such as body mass index, gestational age, systolic blood pressure, diastolic blood pressure, cervical, brachial and abdominal circumferences, newborn weight, first and fifth minute Apgar and somatic Capurro did not present statistical significance. Conclusion We identified a significant difference in serum levels of nitric oxide between groups of patients with and without preeclampsia and non-pregnant women, but it was not found when we considered only the post partum women with and without hypertension. In preeclampsia group nitric oxide levels were significantly higher in those with advanced age. But there was no significant difference in maternal variables: body mass index, blood pressure, measurements of body circumferences and association with comorbidities. Neither in the neonatal variables: gestational age at delivery, Apgar score, care unit intensive admission and birth weight.

Control group: 20 pregnant women with single and live fetus, > 38 weeks, without comorbidities. Exclusion criteria for study group: twin pregnancy, fetal anomalies and gestational age under 37 weeks. Control group exclusion criteria: twin pregnancy, gestational age under 38 weeks, cesarean/ forceps/ induced vaginal delivery and comorbidities. Maternal variables: age, gestational age at delivery, previous pregnancy history, body mass index (BMI), comorbidities, length of hospital stay, intensive care unit admission, previous preeclampsia and delivery route. Neonatal variables: birth weight, adequacy weight for gestational age, Apgar score, meconium fluid at delivery, intensive care unit admission, acute respiratory distress syndrome, cardiopulmonary arrest, jaundice, oxygen therapy and intrahospital mortality. Women diagnosed with preeclampsia appears to be older, have higher body mass index, longer hospital stay (including intensive care unit) and have previous preeclampsia more often than those without this comorbidity. There is a lack of significance in comparing neonatal outcomes between the systematic delivery approach at 37 weeks and the control groups.

Objectives Compare epidemiological characteristics, maternal and neonatal outcomes between preeclampsia and normal pregnancies with spontaneous vaginal delivery after 38 weeks. Methods Retrospective study with 124 women, through medical records review, was developed in Guilherme Alvaro Hospital- Santos/Sao Paulo/Brazil (July/2015–May/2016). Study group: 71 women with preeclampsia according to NHBPEP- National High Blood Pressure Education Program (2000) criteria, with single and live fetus, regardless of gestational age. Control group: 53 pregnant women with single and live fetus, after 38 weeks of gestation, without comorbidities. Exclusion criteria for study group: twin pregnancy and fetal anomalies. Exclusion criteria for control group: twin pregnancy, gestational age 38 weeks, cesarean/forceps/induced vaginal delivery, fetal anomalies and presence of comorbidities. Maternal variables: age, gestational age at admission, previous pregnancy history, body mass index and length of hospital stay. Neonatal variables: weight and adequacy weight for gestational age, height, Apgar score, intensive care unit admission, length of hospital stay and respiratory and non-respiratory complications. The results show that newborns from the study group present lower weight and smaller height at birth, longer hospital stay (including in intensive care unit) and acute respiratory distress syndrome development.

Introduction Currently, two factors are considered important in the pathophysiology of preeclampsia: lipid peroxidation and exacerbated inflammatory response. The reduction in the clinical expression of preeclampsia 12 weeks postpartum motivates the search to recognize the behavior of lipoprotein and inflammatory substrates. Objective Evaluate the influence of lipoprotein and inflammatory markers in women with preeclampsia during the mediate and late postpartum. Methods Cohort study developed at the Guilherme Alvaro Hospital- Santos/Brazil. Were collected blood samples of 107 patients (48 case group and 59 control group) from January/2015 to May/2016 in the mediate postpartum. After that, the same women returned to the second sample, 12 weeks postpartum. It was a total of 31 patients (17 case group and 14 control group). Lipoprotein substrates and selected inflammatory possibly related to preeclampsia were analyzed. Group case: women with preeclampsia according to the criteria of NHBPEP – National High Blood Pressure Education Program (2000). Control group: patients after vaginal delivery with term newborn without hypertension and/or other comorbidities. Exclusion criteria: collagen disease, smoking, twin pregnancy, previous diagnosis of diabetes mellitus and fetal malformations. Variables analyzed: total cholesterol, LDL (low-density lipoprotein) fraction, triglycerides, fasting glucose, insulin, HOMA-IR, C-reactive protein (CRP), total protein, albumin, globulin, uric acid, glutamic oxaloacetic transaminase (GOT), glutamic pyruvic transaminase (GPT), gamma-glutamyl transferase (GGT). For the descriptive measures of each variable of interest we calculate in the two evaluations. Then to study the behavior of the variables of interest in the chosen group, in the two assessments, we used the model of multivariate analysis and repeated measures. Download : Download high-res image (187KB) Download : Download full-size image Conclusion The glycidic and inflammation effectors such as blood glucose, insulin, HOMA -IR, C-reactive protein and uric acid showed uniform and significant changes between the groups comparing mediate and late postpartum. Lipoproteins indicated an upward trend in women with preeclampsia, while total protein, albumin and globulin fractions seem to be reduced in this group. Comparing the mediate and late postpartum period we identified a significant difference between the serum levels of lipotoxicity and inflammatory markers.

Objectives Evaluate epidemiological characteristics and compare maternal and neonatal outcomes of mild and severe preeclampsia in pregnancies that results in delivery after 37 weeks of gestation. This first group had no need of mother ICU admission, their BMI is higher and was the group with more previous preeclampsia. The severe preeclampsia group has a significant level of arterial hypertension, stayed longer in the hospital and their newborn had presented more general complications. Data shows more favorable maternal and neonatal outcomes in the control group.

Introduction Preeclampsia (PE) affects 5–8% of pregnant women, it is a multi-systemic damage which obesity is a predisposing factor and may be recognized by anthropometric measurements. The increased adiposity results in tissue necrosis and release of fatty acids, promoting metabolic changes known as lipotoxicity that causes endothelial damage. This immune disorder causes: vascular reactivity, vasospasm, alterations in capillary permeability and coagulation system. Resulting in anatomical/functional damage such as brain, cardio-pulmonary, renal, hepatic, hematological and utero-placental. This research project is part of a line of research that will assess the long-term values of the selected indicators in the remote postpartum period, thus seeking to observe the behavior of these effectors in the mother's body. Objective To evaluate possible laboratory and anthropometric markers in selected mothers with PE in the immediate postpartum compared to women without this complication. Methods Case-control study developed at the Hospital Guilherme Alvaro, Santos/Brazil. Blood samples and anthropometric data from 15 mothers were collected in the period from January to May/2015 in the immediate postpartum and analyzed selected laboratory and nutritional markers, possibly related to PE. Case group: women with PE according to the NHBPEP (2000). Control group: women with vaginal birth and newborn full-term, without hypertension and other comorbidities. Variables analyzed: maternal age, gestational age at birth, body mass index (BMI), neck, arm and abdominal circumferences, total cholesterol, HDL (high density lipoprotein), LDL (low-density lipoprotein), VLDL (very low- density lipoprotein), triglycerides, fasting glucose and insulin levels to calculate HOMA-IR index, C-reactive protein. Exclusion terms: collagen disease, smoking, twin pregnancy and fetal malformations. Results The total of 15 blood samples (09 – study group and 06 – control group). Conclusion This study showed that potential markers lipotoxicity and anthropometric data can relate to the PE, possibly indicating probable risk factors, contributing to the understanding of certain clinical expressions and possibly correlate with prognostic factors. More studies are needed to expand the preliminary information of the present study.

Introduction Calcium supplementation in pregnant with low calcium intake as part of the care for the prevention of preeclampsia. The monitoring of daily calcium intake is inappropriate. Objective Investigate the dietary calcium intake in pregnant with higher risk of preeclampsia. Methods The study was performed involving pregnant women with chronic arterial hypertension (CAH), Diabetes Melittus (DM) and/or obesity (Ob) seen for their prenatal care at the Guilherme Alvaro Hospital, Santos, Brazil. Calcium intake was estimated through Food Frequency Questionnaire (FFQ) proposed by Fisberg et al. (2005) adapted. The Body Mass Index (BMI), calculated as kg/m 2 , was classified according Atalah et al. (1997). Data were compared using Mann–Whitney U or Kruskal–Wallis test (post hoc Dunn), p Results We studied 55 pregnant, mean ± standard deviation age of 32,7 ± 6,0 years, gestational age of 26,5 ± 7,6 and BMI=35,2 ± 5,8kg/m 2 that characterizes 63,4% the sample as obesity pattern and 29,3% as overweight. Mean daily calcium intake from food records was low (672,2 ± 352,3mg/day), 90,9% had lower average consumption that the WHO recommendation.The relationship between risk factors of preeclampsia (CAH, DM or CAH/DM) or BMI classification and calcium intake not differed. It was found that milk was the most common (60%) among pregnant women with adequate dietary calcium intake (9,1%; 1383,5 ± 117,5mg/day). Calcium supplementation with or without vitamin D was reported by 12,7% ( n =7). The calcium intake from the group calcium supplementation (1000mg elemental calcium/day) said use was higher (median 1691,4mg/day; p Conclusion In this study it was highly inadequate dietary calcium consumption. Calcium supplementation was critical to the adequacy of calcium intake in this group of pregnant women, it is could contributes with reduced preeclampsia development.

Objectives Analyze the epidemiology of affected women with cases of hypertensive disorders (SH), severe maternal morbidity (SMM) and near-miss (NM). Methods Prospective longitudinal/descriptive study (June/2013-May/2014-Guilherme Alvaro Hospital/Santos-Brazil), included women with at least one criteria recommended by the authors (Santos criteria) derived from the definition of MMG and NM (admitted to ICU) of the World Health Organization (2009), Waterstone et al. (2001), Mantel et al. (1998), seeking to homogenize/facilitate this definition according to clinical/laboratory/ management, using hospital records and interviews with women, describing the epidemiology/outcomes/economic status and quality of prenatal care. Results We identified 54 women (46 MMG, NM 07 and 01 maternal death from sepsis). Reasons (/1000 births):MMG 33.63 and NM 5.11. White women (48.8%), above 35 years old (38.8%), 70% overweight/obese (82.5% associated with SH), and were often poor, incomplete secondary education, 41.5% multiparous, with previous preeclampsia (PE), 29.5% eclampsia and 8% fetal death , when prenatal consultation 57.64% were less than six with difficult access (35%). 88.8% SH: 36 chronic hypertension and superimposed PE (63.8%) and one with eclampsia, 22% severe PE, mainly contributing to NM, 54.1% and were not counseled on signs/symptoms of severity. The prevalent mode of delivery was cesarean section (79.54%), prematurity (69%), 03 cases of hysterectomy and 07 stillbirths. Objectives Comparison between two electronic methods of antepartum fetal assessment in hypertensive pregnant women. Methods Comparison of conventional cardiotocography tracing and transabdominal fetal electrocardiography with the use of the fetal monitor AN24 (Monica Healthcare), in women with gestational age ⩾34 weeks, single fetus, with hypertensive disorders according to the criteria of NHBPEP-2000 at Guilherme Alvaro Hospital – Santos/SP – Brazil (Dec/2013–Jun/2014). The cardiotocography recorded the fetal heart rate by a Doppler transducer, dependent on correct identification of fetal positioning, the AN24 detected electrophysiological signals using electrodes arranged on the maternal abdomen. The tests were performed according to standard recommendation; variables evaluated: gestational age, number of pregnancies, parity, baseline, variability, body mass index (BMI), obesity, preservation of mobility adhesion, successful recording, Preparation time ( 20″), facility of captation/stability of signal in the group with BMI > 30, agreement in diagnostic classification (ACOG Practice Bulletin 106, 2009). Results Of the 10 tracings evaluated: 03 pregnant women with pre-eclampsia, 07 with chronic hypertensive (06 developed superimposed pre-eclampsia). The AN24 monitor preserved the mobility and made it easier to obtain the registration in women with BMI > 30 with better adherence in this group once it dismissed the additional manipulation of the abdomen to obtain registration. Objectives Identify maternal and perinatal outcome of vaginal parturition in pregnant women with Chronic Arterial Hypertension (CAH). Methods A transversal and retrospective study was performed involving all pregnant women who were attended at Hospital Guilherme Alvaro’s Prenatal Service for Women with Hypertension or Nephropathies in Santos–SP/Brazil in the period (January 2012–December 2013) and that had obstetric resolution by vaginal delivery, totaling 31 pregnant women. A descriptive and exploratory analysis of the collected data through frequency tables and graphics was performed. Results The most frequent expression of CAH found was the mild form (diastolic arterial blood pressure 90–100 mmHg) with a 93.55% share of all cases studied. Objectives Analyze epidemiological data and the relation with potential markers of lipotoxicity in pregnancies with and without preeclampsia in a reference hospital. Methods A case-control study, 110 blood samples were collected from (August 2011–2013) in the immediate postpartum and biological potential markers of lipotoxicity analyzed which can be related to preeclampsia. Inclusion for the study group followed the NHBPEP – 2000: systolic blood pressure (⩾140 mmHg) and/or diastolic blood pressure (>−90 mmHg) two times, associated to proteinuria/24 h ⩾2 g/dL or tape reagent ⩾1 cross. Inclusion for the control group: healthy women, vaginal birth, newborn term. Exclusion criteria for all postpartum women were: comorbidities such as collagen diseases, diabetes, smoking, gemelarity and fetal malformations. Data analyzed: maternal age, body mass index (BMI) [weight/(height)[2], high urine protein, HDL (high density lipoprotein), VLDL (very low-density lipoprotein), triglycerides, mode of delivery, maternal clinical outcome, gestational age, birth weight, Apgar newborn score at one and five minutes and admission to the neonatal Intensive Care Unity (ICU). Objectives Identify maternal and perinatal outcome of vaginal childbirth in pregnant women with pre-eclampsia (PE). Methods Retrospective study involving 524 pregnant women with single and live fetus who gave birth at the Hospital Guilherme Alvaro – Santos/SP (January, 2012 to December, 2012) presenting at gestational age ⩾ 37 weeks. Selected: 12 for study group (pregnant with preeclampsia NHBPEP-2000) and 12 for the control group (normal pregnant women who delivered vaginally after the study group). Several maternal variables were analyzed: obstetric history, method of early labor, administration of magnesium sulfate and/or antihypertensive, complications during labor and delivery, postpartum, days of hospitalization and ICU admission. Also, perinatal variables: birth weight of the newborn, meconium or not amniotic fluid at delivery, Apgar score, therapeutic care, with respiratory complications, length of stay in hospital and ICU admission. To compare the groups in terms of numerical variables, we applied the Student t test for unrelated samples and, regarding categorical, we used the Fisher exact test. OBJETIVO: comparar, macro e microscopicamente, cicatrizes uterinas pós-cesáreas, nas quais foram feitas suturas com pontos separados, contínuos e contínuos ancorados. MÉTODOS: utilizamos três coelhas prenhes, realizando parto cesáreo no 26º dia de prenhez, com três incisões em cada corno uterino. As histerorrafias foram realizadas com fio Vicryl® 00, com suturas distintas (pontos separados, sutura contínua e contínua ancorada). No 60º dia pós-parto, realizamos histerectomia total abdominal e anexectomia bilateral, para avaliação das cicatrizes cirúrgicas. Na macroscopia, avaliamos o grau de retração cicatricial (longitudinal e transversal), o depósito de fibrina, presença de aderências e integridade dos fios de sutura. Na microscopia, utilizamos coloração de hematoxilina-eosina, para contagem de vasos sangüíneos e fibroblastos, e a coloração do tricômio de Masson, para quantificação do colágeno. Para a análise comparativa das cicatrizes, utilizamos os testes de Friedman e exato de Fisher, adotando nível de significância de 5%. RESULTADOS: foram obtidas 18 cicatrizes, seis para cada tipo de sutura. Obtivemos as seguintes médias 0,5/0,4/0,5 (p=0,069) para os graus de retração longitudinal e 0,3/0,4/0,3 (p=0,143) para os graus de retração transversal, respectivamente para as suturas com pontos separados, contínuo e contínuo ancorado. Todas as suturas apresentaram depósito de fibrina regular, ausência de aderência e reabsorção integral dos fios. Na microscopia, apuramos a média de vasos sangüíneos (158,5/139,3/172,1; p=0,293), de fibroblastos (351,6/345,8/354,3; p=0,311) e da porcentagem de tecido colágeno (44,0/45,5/48,5; p=0,422), respectivamente para as suturas com pontos separados, contínuo e contínuo ancorado. CONCLUSÕES: a técnica de histerorrafia na cesárea de coelhas (pontos simples, sutura contínua e contínua ancorada) não determinou diferenças estatísticas significantes em relação aos parâmetros macroscópicos e microscópicos avaliados. PURPOSE: to compare macro and microscopically, surgical uterine sutures in female rabbits, after caesarean section utilizing separate, continuous and continuous anchored suture stitches. METHODS: three New Zealand female rabbits in their first pregnancy were used. The caesarean section was carried out at the 26th day of gestation and three incisions were performed in each uterus. The hysterorrhaphy was performed with a 00 Vicryl® thread, and a different suture technique was employed for each incision. Total hysterectomy and adnexectomy were done at the 60th day post-delivery with the preservation of eventual adhesions for the evaluation of the surgical scars. The extent of scar retraction, amount of fibrin deposit and the suture integrity were evaluated through macroscopy. For the evaluation through microscopy, hematoxylin eosin technique was used for cellular colorimetry, and Masson's trichrom to evidence collagen. The statistical non-parametric Friedman's test was employed for the matching hypothesis, and Fisher's exact test to verify the homogeneity of the techniques (level of significance: 5%). RESULTS: a total of 18 scars were obtained (six scars per suture). The following mean values were obtained for the longitudinal (0.5/0.4/0.5, p=0.069) and transversal retraction degrees (0.3/0.4/0.3, p=0.143) respectively for separate, continuous and continuous anchored suture techniques. All sutures presented regular fibrin deposit, no adhesions and integral absorption of the stitches. The mean value of the blood vessels (158.5/139.3/172.1; p=0.293), fibroblasts (351.6/345.8/354.3; p=0.311) and of collagen percentage (44.0/45.5/48.5; p=0.422) were calculated through microscopy, respectively for separate, continuous and continuous anchored suture techniques. CONCLUSIONS: the type of hysterorrhaphy technique of caesarean section in female rabbits did not generate any significant statistical difference in the macroscopic and microscopic parameters evaluated.

Background The fourth-year Obstetrics and Gynaecology course at our institution had previously been taught using theory classes alone. A new teaching model was introduced to provide a better link with professional practice. We wished to evaluate the impact of the introduction of case discussions and other practical activities upon students' perceptions of the learning process. Methods Small-group discussions of cases and practical activities were introduced for the teaching of a fourth-year class in 2003 (Group II; 113 students). Comparisons were made with the fourth-year class of 2002 (Group I; 108 students), from before the new programme was introduced. Students were asked to rate their satisfaction with various elements of the teaching programme. Statistical differences in their ratings were analysed using the chi-square and Bonferroni tests. Results Group II gave higher ratings to the clarity of theory classes and lecturers' teaching abilities (p < 0.05) and lecturers' punctuality (p < 0.001) than did Group I. Group II had greater belief that the knowledge assessment tests were useful (p < 0.001) and that their understanding of the subject was good (p < 0.001) than did Group I. Group II gave a higher overall rating to the course (p < 0.05) than did Group I. However, there was no difference in the groups' assessments of the use made of the timetabled hours available for the subject or lecturers' concern for students' learning. Conclusions Students were very receptive to the new teaching model.

Introduction The decision of a woman to get pregnant at later age of her reproductive cycle has be a phenomenon around the world. Epidemiology data show frequent increase of clinical complications in direct proportion in advanced age motherhood, hypertensive disturbances being more prevalent. Objectives Analyse the prevalence of hypertensive syndromes in pre-determined age groups at Hospital Guilherme Alvaro in Santos, Sao Paulo, Brazil showing the different segments in each one. Methods From data collected in the outpatient department of Hospital Guilherme Alvaro of High Risk Pre-natal between 04/06/2008 and 30/05/2011, a prevalence transversal study was carried out where data were obtained from 628 patients aged between 16 and 46 years. Procedures of homogeny analysts were set out, always collecting data such as age and disorder for high risk gestation. According to age, patients were divided into groups: precocious (up to 19 years old), middle age (between 20 and 34) and late pregnancies (over 35). Results In the precocious pregnancies, clinical illnesses/no hypertension (31%) were observed in first place, 25% (8) twin pregnancy in second place, 19% (6) showed hypertensive disturbances. Concerning pregnancies between 20 and 34 years old, 36% (144 patients) showed hypertensive syndromes, 23% (92 patients) showed endocrine disturbances, 22% (90 patients) showed clinical illnesses/no hypertension, and 9% twin pregnancy. Regarding late pregnancies, the most frequent disturbance was isolated hypertensive syndromes: 44% (88 patients) in first place, only endocrine disturbances, 24% (47 patients) in second place followed by association between hypertensive syndromes and endocrinopathy with 13% (26 patients). Conclusion About precocious pregnancies, greater prevalence showed clinical illnesses/no hypertension, whereas middle age and late pregnancies showed greater hypertensive syndrome prevalence, results, which are compatible with other studies, have been observed that due to advance of age, hypertensive syndromes are more frequent. For late pregnancies, the prevalence of clinical illnesses/no hypertension was a lower percentage regarding the other two groups: precocious pregnancies (31% = 10 women), middle age pregnancies (22% = 90 women), and late pregnancies (2% = 7 women). Considering the fact that the occurrence of pregnancy is more and more late in life, it can be concluded that the professionals must be prepared to attend pregnancies on women with hypertensive disturbances and their eventual complications.

Objectives Analyze epidemiological data of postpartum women who had or not preeclampsia in a reference hospital to evaluate possible differences between these two groups. Methods It is a descriptive and retrospective study of medical records of 287 pregnant women with and without preeclampsia admitted at the Guilherme Alvaro Hospital Santos/SP – Brazil (January–February 2013). Patients with preeclampsia were included in the study group according to the criteria of National High Blood Pressure Education Program – 2000: systolic blood pressure (⩾140 mmHg) and/or diastolic blood pressure (⩾90 mmHg) two times, associated to proteinuria/24 h ⩾ 2 g/dL or tape reagent ⩾1 cross. The control group consisted of healthy women admitted during the same period. The data analyzed was: maternal age, mode of delivery, maternal presence of comorbidities (such as diabetes, collagen diseases, anemia, lung and heart diseases), gestational age, birth weight, Apgar score 1′5′ and admission to the neonatal intensive care unit (ICU). For categorical variables we used the Person’s Chi-square test and Fisher’s exact and for numerical variables from the research, we used the Student t test. The data analysis was performed by calculating odds ratio hypothesis rejection level of adoption of 0.05. Introduction The impact of preeclampsia (PE) of pregnancy is seen as a global concern. Despite their importance, PE does not have well-established causes. Several studies point to obesity as a predisposing factor for PE [1] . In a systematic review Duckitt and Harrington showed that in women with BMI ⩾ 30 the relative risk of developing PE is equal to 2.1 [2] . Bianco et al. demonstrated that the chance of patients with BMI ⩾ 35 developing PE is more than 4 times [3] . Furthermore, it is believed that the intracellular accumulation of triglycerides cause mitochondrial dysfunction, more electron transport and generation of ROS (Reactive Oxygen Species) [4] , causing a harmful effect on the endothelium. Objectives To compare the incidence of potential lipotoxicity markers among normal pregnant women and with pre-eclampsia. Methods A case–control study, including pregnant women from October 2011 to January 2012 at Hospital Guilherme Alvaro in Santos/Brazil. The study group consisted of 27 mothers with pre-eclampsia, according to the criteria of NHBPEP (2000) and the control group consisted of 27 mothers with normal pregnancy, who gave birth vaginally, at term without hypertension. The exclusion criteria were diagnosis of collagen diseases, smoking, diabetes, twin pregnancy and fetal malformations. Blood samples were collected immediately after delivery and the results were analyzed in conjunction with maternal data, according to the following variables: age, body mass index (BMI), serum cholesterol total and fractions, glucose, triglyceride and creative protein. Analysis of SF data was performed by calculating the odds ratio adopting an hypothesis rejection level of 0.05. Results The PE group had BMI > 30 (or), serum triglycerides > 150 (or) and LDL cholesterol > 100 (or) more frequently than normal mothers, and as regards statistical significance comparing the age of the women, glucose level, HDS cholesterol, there was no significant difference between the groups. Conclusion In pregnancy, lipotoxicity reduces the ability of trophoblastic invasion, causing maternal systemic endothelial and placental dysfunction causing alterations in metabolic functions. All such changes maintain a close relationship with the pathophysiology of PE. Monitoring pregestational period of obese patients or those with lipid disorders is part of the medical arsenal against PE by monitoring and advising on the best time to conceive.

Introduction The hypertensive pregnancy disorders affect 7.5% of Brazilian women, being a major cause of maternal and perinatal morbidity and mortality, affecting 6–10% of primigravidae. They represent a significant cause of prematurity birth, with elective responsible for 20–30% of premature births, with higher incidence of neonatal morbidity due to high rates of complications in short and long term. Objectives Identify the repercussions in a short time, on caused by elective obstetric parity decisions in women with hypertensive neonatals pregnancy syndromes. Methods Retrospective analysis of medical records of pregnant women admitted to Hospital Guilherme Alvaro, in the city of Santos/SP - Brazil, between Jan/2005 to Jan/2012. Calculations were performed for dichotomous variables and an epidemiological analysis with data obtained on pregnant women with hypertension with a single fetus, subjected to delivery before 37 weeks. Classification of Hypertensive Syndromes followed the criteria of the NHBPEP – 2000, after blood pressure measurements were calibrated following standard technique: Chronic hypertension (CH), Pre-eclampsia, superimposed pre-eclampsia and late gestational hypertension and multiple births were excluded, premature labor and fetal malformations, totaling 102 cases. Results Throughout the study, it was observed that 67.6% are appropriate for gestational age, birth weight 1500–2500 grams (43.1%). Support in the neonatal were excluded ICU was necessary in the vast majority (75.7%), and length of stay exceeding 28 days in 29.3% of cases. The main indication was respiratory distress syndrome (63.6%) requiring intubation orotraquel and jaundice in 18.2% of premature infants was a common comorbidity (39.4%). Even with all the support in-hospital, the neonatal mortality was present in 8.1% of cases. The main indication of the resolution was for obstetric maternal complications (61.8%) where pre-eclampsia and preeclampsia superimposed were expressions which caused more frequent (42.2 and 40.2% respectively), showing the level of proteinuria between two -4.9 g (32.1%). Gestational age at delivery was 32–34 weeks (43.1%), corticosteroid therapy was not performed (54.9%) and cesarean delivery was (94.1%). The most frequent age group was 20–34 years (53.9%), Caucasian (54.9%), primigravida (62.7%) and less than five prenatal visits (36.3%) were performed. Conclusion The hypertensive pregnancy are risk factors for preterm elective delivery and is more frequent before 34 weeks, resulting in high perinatal morbidity and mortality, especially in cases of preeclampsia with severe clinical presentation and long hospital stays, with consequent costs and conducting excessive procedures in the neonate. Enable early referral to specialist services and consequent prenatal follow up, enables the adoption of early action, such as clinical support in a timely manner to facilitate the handling of cases which are still stable, optimizing the opportunity for administration of antenatal corticosteroids. It is necessary to review decisions about the mode of delivery in prematurity related to hypertension cases, with the aim of increasing the occurrence of vaginal deliveries.
