Summary: Introduction: Knowledge of the birth cycle is important in the organization of reproductive health services in a facility. The main objective was to study the determinants of the birth cycle at the Kalaban-Coro health center. However, it seems that there are other determinants of the birth cycle, hence the relevance of this study. Material and methods: This was a cross-sectional analytical study from October 1 to December 31, 2020. Results: The study included 977 parturients who gave birth in the department. It recorded more night deliveries (59.1%) than day deliveries (40.9%). The average age of the women was 25 years. Non-educated births were the most represented with 39% of cases. Nocturnal admissions were predominant (58.8%); 59% of parturients had a history of nocturnal delivery and 40% preferred to give birth at night, while 30% had no preference. At admission, 57.91% of parturients were in the active phase during the night, compared to 42.59% during the day; 52.6% of parturients had a nocturnal onset of labor, compared to 47.4% during the day. Conclusion: The study showed that childbirth is more frequent at night than during the day. It highlighted the admission nycthemer, the previous nycthemer of deliveries and the preference of the parturient as other determinants of the nycthemer of the delivery.

Introduction: Peripartum cardiomyopathy (PPCM) is a heart failure whose etiology is still unknown. The aim of work was to study peripartum cardiomyopathy in its epidemiological, clinical, paraclinical and therapeutic aspects at the Tombouctou hospital. Methods: This is a descriptive cross-sectional study carried out in the Medicine Department of the Timbuktu hospital from January 1 to December 31, 2019. It concerned patients who presented heart failure between the 8th month of pregnancy and the first 5 months postpartum. Results: During the study,23 patients were collected. Average age of the patients was 23.50 ± 3.50 years with extremes of 16 and 34 years. Incidence of Peripartum cardiomyopathy (PPCM) was 1/345 pregnancies. Average parity was 3.20 with extremes of 1 and 8. Symptoms appeared at postpartum with 87% of cases. A significant delay in diagnosis was observed. Global heart failure was the mode of decompensation with 70%. Electrocardiographic signs were mainly sinus tachycardia (87%) and left ventricular hypertrophy (83%). Cardiac ultrasound showed in all cases dilated cardiomyopathy and it was associated with thrombus in left ventricle cavity for three cases. Left ventricular ejection fraction was severely impaired in 70% of cases. Pulmonary hypertension was significant in 52%. Most commonly drugs we used in our series at acute stage were duretics: furosemide 100% and spironolactone 70% (100% and 70%) and ACE inhibitors (90%). Beta-blockers (bisoprolol and carvedilol) were used in 15 patients. Bromocriptine (prolactin inhibitor) was used for 2 patients. Conclusion: Peripartum cardiomyopathy is a serious cardiac complication of pregnancy of unknown cause, common in the African population.

Background: Pulmonary embolism (PE) is a severe form of venous thromboembolic disease. In Africa, prevalence of PE in hospitalized medical patients varies among studies. Objective: Aim of this work was to study the epidemiological, clinical, therapeutic and evolution aspects of PE in the medicine department of Mali’s Hospital Bamako-Mali. Methodology: This was a retrospective cross-sectional study carried out from January 01, 2017 to December 31, 2020 in the medicine department of Mali’s hospital Bamako, including all inpatients admitted for PE during the study period. Results: Of 1814 hospitalized patients, 54 patients had pulmonary embolism. Hospital frequency of pulmonary embolism was 2.97%. Predominance was female and sex-ratio M/F was 0.38. Mean age in our series was 54.24 ±1.19 years. Predisposing factors to pulmonary embolism were dominated by obesity 37%, high blood pressure 35.2% and history of cardiovascular disease 20.4%. Dominant signs were dyspnoea and chest pain in 83.3% and 70.4% of cases respectively. Probability of pulmonary embolism was high in 40.7% according to Wells score. Patients with right heart failure were 22.2%. EKG showed sinus tachycardia70.4% and S1Q3 aspect 7.4%. Heart right chambers were dilated at transthoracic echography 42.6%. Obstruction was bilateral at chest angio CT for 51.9% and proximal for 42.6%. Deep venous thrombosis was associated at EP in 16.6%. Treatment was low weight molecular heparin followed by vitamin K antagonist or direct oral anticoagulant. One patient was successfully treated by thrombolysis. Hospital mortality was 16.7%. Conclusion: PE is a serious disease probably underdiagnosed. It is responsible of important mortality.

Congenital heart diseases are abnormalities of heart that occur during intrauterine life. Our work aimed to study the epidemiological and echocardiographic aspects of congenital heart disease with Cardiology Department of the Tombouctou hospital. This was a descriptive cross-sectional study from November 2018 to December 31, 2019 on Doppler echocardiography reports from patients admitted to the Cardiology department. The probes used were 3.5 and 5 MHz on an Esaote Mylab40 device. The inclusion criteria concerned patients of both sexes under the age of 16, hospitalized or followed in the cardiology department for cardiac symptoms and having benefited from a cardiac Doppler ultrasound. Results: During the study period, 370 echocardiographic examinations were performed and revealed 20 cases of congenital heart disease with a prevalence of 5.45%. The female sex was the majority 13/7 cases which makes a sex ratio of 0.54. Heart murmurs were the main reason for requesting cardiac Doppler ultrasound with the 13 out of 20 cases of congenital heart disease (65%). Interventricular communication was represented by 15 cases (75%). Congenital heart disease is a reality in Africa. Its frequency and reported series are underestimated due to the inaccessibility of Doppler echocardiography.

Introduction: The partial ventricular atrioventricular canal is a congenital heart malformation of the ostium primum type of variable surface that can reach the single atrium where the interatrial septum is no more than a cephalic muscular vestige attached to the roof of the atrium. His diagnosis at the age of 73 is not a common situation. The interest of this clinical case that we report lies in the peculiarity of its mode of discovery and its evolution under medical treatment. Observation. He was a 73-year-old patient with no known cardiovascular risk factor and has since been at about 18 years of age with heart failure syndrome with progressive dyspnea, cough and edema of the lower limbs, but never explored for lack of financial means, and the evolution was enamelled by episodes of cardiac decompensation, the last of which dated back to 03/07/2018 justifying a hospitalization in our service. Cardiac ultrasound shows a partial atrioventricular canal. The care was medical. Conclusion: The atrioventricular canal is a congenital heart disease complex of poor prognosis.

8. Carbon dioxide evolution from snow-covered agricultural ecosystems in Finland

The release of CO2 from the snow surface in winter and the soil surface in summer was directly or indirectly measured in three different soil types (peat, sand and clay) in agricultural ecosystems in Finland. The closed chamber (CC) method was used for the direct and Fick's diffusion model (DM) method for the indirect measurements. The winter soil temperatures at 2-cm depth were between 0 and 1°C for each soil type. The concentration of CO2 within the snowpack increased linearly with snow depth. The average fluxes of CO2 calculated from the gradients of CO2 concentration in the snow using the DM method ranged from 10 to 27 mg CO2 m2 h -1 and with the CC method from 18 to 27 mg CO2 m -2 h -1. These results suggest that the snow insulates the soil thermally, allowing CO2 production to continue at soil temperatures slightly above freezing in the winter. Carbon dioxide formed in the soil can move across the snowpack up to the atmosphere. The winter/summer ratio of CO2 evolution was estimated to exceed 4%. Therefore, the snow-covered crop soil served as a source of CO2 in winter, and CO2 evolution constitutes an important part of the annual CO2 budget in snowy regions.;

Background Faecal calprotectin (FC) and faecal immunochemical tests (FIT) have the disadvantage of requiring faecal samples. It has been reported that prostaglandin E–major urinary metabolite (PGE-MUM) values correlate with Mayo endoscopic scores (MESs) for ulcerative colitis (UC). However, there has been no report that PGE-MUM can determine endoscopic remission under remission phase UC, nor comparative study of PGE-MUM with FC and FIT. Thus, we aimed to examine the association between PGE-MUM values and the colonoscopy (CS) results of patients in the remission phase of UC, and to compare the accuracy of using PGE-MUM vs. that of using FC or FIT values for determining endoscopic remission. Methods Patients diagnosed with UC who were under clinical remission and had planned to undergo CS from August 2017 to March 2019 were enrolled. FC levels were measured and FITs were performed on the day of CS; PGE-MUM was measured either the day before or after CS. Three physicians independently scored the CS findings (MES, Modified Mayo Endoscopic Score [MMES], and UC endoscopic index of severity [UCEIS]) while blinded from clinical information. We analysed the differences in PGE-MUM values between two groups, which were divided between those that did achieve and those that did not achieve the following scores: (1) MES 0 point, (2) MES 1 point, (3) modified MES 0 point, (4) modified MES ≤1 point, (5) UCEIS 0 point, (6) UCEIS ≤2 points. In addition, the accuracy of PGE-MUM, FC, and FIT with respect to determining the achievement of (1) through (6) were compared by using areas under the receiver operating characteristics curves. Patients with altered UC activity between the day of PGE-MUM measurement and CS, and those who received NSAIDs on the day of PGE-MUM measurement, were excluded from the analysis. Results Of the 125 enrolled subjects, 30 patients were excluded (urine specimens not submitted, 11; poor stool specimens, 6; NSAIDs users, 10; clearly altered UC activity, 3). The remaining 95 patients (average age 48.2 years, 57 males, and 54 patients with total colitis type), were eligible for analysis. The median PGE-MUM values (in µg/g·Cr) for groups that did or did not achieve (1) through (6) were as follows: (1) 14.6/17.2, p = 0.106; (2) 14.9/20.5, p = 0.039; (3) 14.5/17.4, p = 0.059; (4) 14.1/21.8, p < 0.001; 5) 14.5/17.4, p = 0.059; (6) 14.7/22.2, p = 0.003. The areas under the receiver operating characteristics curves for PGE-MUM/FC/FIT used for determining the achievement of 1) through 6) were as follows: (1) 0.597/0.664/0.682, (2) 0.692/0.74/0.825, (3) 0.613/0.686/0.692, (4) 0.794/0.82/0.786, (5) 0.613/0.686/0.692 and (6) 0.778/0.824/0.825. Conclusion PGE-MUM is equally as effective as FC and FIT for determining the achievement of endoscopic remission.

Water is a basic necessity for human’s life. Water, which is distributed to the public, should in decent condition, healthy, and protected from metal pollutants. In Indonesia, it is handled by a government institution, commonly is PDAM (Indonesian regional water utility company). A PDAM Tirta Kahuripan handles water distribution in Bogor area and part of Depok cities. Based on data, PDAM Tirta Kahuripan had approximately more than 46 % water loss, due to geological factor, human activity, etc. Therefore in this paper, we try to make a decision system of water loss at PDAM pipelines, using cluster Fuzzy C – Means method analysis. Then, mapped into ArcGIS software. Based on this method, we can be determine the region which shows the most water loss and also identify the highest leaks level from water pipelines at PDAM Tirta Kahuripan.

Functional improvement is an important concern for a composite allograft with motor function. As the procedure of rat composite limb transplantation is a well-established model, we focused on the effects of rehabilitation using this model. We performed the rat hindlimb transplantation in syngenic combination using a modified method confirming the processes of vessel anastomosis step-by-step that maintains good vascular patency. We studied the impact of treadmill training on hindlimb transplantated rats using Rodent Robot 3000 (Robomedica). Functional recovery was evaluated electro-physiologically after training. All transplants were conducted successfully. The “drop foot” condition improved in rats that were subjected to a training program. However, there were no significant differences in muscle atrophy improvement or denervation potential as determined by electromyography. This study is the first to detail the effects of rehabilitation on rat limb transplantation. This model is useful for the study of functional improvement for limb transplantation. © 2007 Wiley-Liss, Inc. Microsurgery 2007.

18. Methane in flooded soil water and the emission through rice plants to the atmosphere

Methane formation in flooded soil and emission from rice plants were examined by pot culture experiments. Rice straw treatment resulted in a two-fold increase in methane formation in flooded soil and a six-fold increase in methane emission rate from rice plants compared to that obtained with a straw compost treatment. Methane concentration in the surface water above the soil was extremely low as compared to that in the soil water. Temperature affected methane formation in flooded soil. Large amounts of methane formed in flooded soil when the redox potential was below −200 mV. In the high temperature (31°C) treatment, methane concentration in vegetated soil was about one-third of that in unvegetated pots; methane emission rate was 20 times higher in vegetated than in unvegetated pots. These results indicate that rice plants have a large capacity to transport methane from the rhizosphere to the atmosphere. Methane transport capacity of rice plants depends mainly on plant size. The present results indicate that the seasonally appearing peak of methane flux from paddy fields results from a combination of flooded soil and rice plant growth.

Mass treatment of onchocerciasis has changed radically in the last 20 years. With implementation of the Onchocerciasis Control Programme (OCP), use of insecticides to control vector larvae has been effective but has not been extended to all infected areas due to the cost. The consequences of this problem have been lessened thanks to ivermectin, an effective drug that can be administered in a single yearly dose. Although ivermectin does not appear to induce major side-effects, surveillance is necessary after administration in polyparasitized subjects living in zones where loaiasis is present and in hypermicrofilaremic subjects. To assist in the fight against onchocerciasis, ivermectin is distributed free of charge through the Mectizan Foundation by Merck Laboratories. Inexpensive community distribution programs with active participation of the populations at risk have demonstrated their usefulness. The results of mass treatment through the Mectizan Foundation have been excellent. Non-governmental organizations and in particular the Organization for the Prevention of Blindness (OPB) have become increasingly involved in the fight against onchocerciasis. The campaign conducted by the OPB in Mali, Senegal and Guinea illustrate this involvement. The role of non-governmental organizations expanded greatly with the implementation of the APOC programme supported by the World Bank.

Atrial fibrillation is the most frequent permanent rhythm disorder.Theaim of this work was to determine the epidemiological and clinical aspects of atrial fibrillation cases hospitalized in the cardiology department of the University Hospital of Kati., Materials and Methods: We carried out a descriptive retrospective study, from January 2018 to December 2019.Patients of all ages and both sexes with clinical and EKG atrial fibrillation admitted to the department during the study period were included in the study.The variables studiedwere: socio-demographic characteristics, clinical signs, classification of atrial fibrillation, and comorbidities associated., Results: A total of 52 patients were included in the studyof 203 cases hospitalized in the department, with a hospital prevalence of 25.61%, the female sex was predominant 69.23%. The main reason for consultation was heart failure syndrome (46.15%).Arterial hypertension (55.76%) was the mostassociatedcomorbidity. - Abstract
The rapid access of the population to basic and comprehensive health care determines the prognosis of the disease and decreases mortality and morbidity., Methodology: we carried out a prospective study over 12 months at the Bla Reference Health Center. All patients evacuated for obstetric complications were included. Women admitted on an emergency basis by self-referral were not included. The data was analyzed from the software Epi Info version: 3.5.4, the text entry from the software version Word 2013., Results: We recorded 430 evacuations or 42.36% of admissions to the maternity hospital in Bla. The patients were young under 20 years 17.9% (n = 77), nulliparous 22.1% (n = 95). They had not performed an antenatal consultation in 30.7% of cases (n = 132). Matrons evacuated in 50.2% of cases (n = 216). The transfer diagnosis was hemorrhage in 14.2% of cases. The main intervention performed was cesarean section (92.3% of cases). The evacuation time was greater than one hour in 95.1% (n = 21). The actors for the financing of the fund were the district council, the town halls and the Community Health Associations (ASACO). The mobilization rate of ASACOs was 82%, that of town halls 17%. Introduction: High blood pressure is a major cardiovascular risk factor. Patients with cardiovascular risk factors are at risk of developing COVID-19. The objective of this study was to determine epidemiology of Covid-19 infected in patients with high blood pressure., Patients and Method: Descriptive cross-sectional study from April 2020 to June 2020 about patients hospitalized for Covid 19 by PCR diagnosis at the Hopital du Mali Bamako and having high blood pressure. Admission registry and patient charts were used to collect data., Results: We collected 78 out of 484 in patients which mean hospital frequency of 16.11%. The mean age was 55.21 +/- 14.61 years. Sex ratio M / F was 1.36. Patients were followed for high blood pressure in 59% of cases. Medical history was ischemic heart disease in 2.6% and dilated cardiomyopathy in 2.6%. Main functional signs were cough in 41.02% and lost of taste in 11.53%. High blood pressure on admission was grade 2 in 37.2% and grade 3 in 3.8%. Treatments received were calcium channel blockers 41.02%, inhibitors of the reninangiotensinaldosterone system 16.66% and combinations 15.38%. Hospital mortality was 10.3%. There was no statistically significant difference in mortality between known hypertensive patients and de novo hypertensive patients. There was also no statistically significant difference in mortality by grade of hypertension., Conclusion: High blood pressure can be associated to Covid 19. Treatment is based on calcium channel blockers and reninangiotensinaldosterone system inhibitors. 25. An overview of the rangelands atmosphere-hydrosphere-biosphere interaction study experiment in northeastern Asia (RAISE).

Intensive observations, analysis and modeling within the framework of the rangelands atmosphere-hydrosphere-biosphere interaction study experiment in northeastern Asia (RAISE) project, have allowed investigations into the hydrologic cycle in the ecotone of forest-steppe, and its relation to atmosphere and ecosystem in the eastern part of Mongolia. In this region, changes in the climate have been reported and a market oriented economy was introduced recently, but their impact on the natural environment is still not well understood. In this RAISE special issue, the outcome is presented of the studies carried out by six groups within RAISE, namely: (1) Land-atmosphere interaction analysis, (2) ecosystem analysis and modeling, (3) hydrologic cycle analysis, (4) climatic modeling, (5) hydrologic modeling, and (6) integration. The results are organized in five relevant categories comprising (i) hydrologic cycle including precipitation, groundwater, and surface water, (ii) hydrologic cycle and ecosystem, (iii) surface-atmosphere interaction, (iv) effect of grazing activities on soils, plant ecosystem and surface fluxes, and (v) future prediction. Comparison with studies on rangelands in other parts of the world, and some future directions of studies still needed in this region are also summarized., (Copyright © 2006 Elsevier B.V. - Abstract
The seasonal pattern of growth and matter production of Pyrola incarnata, an evergreen herb on the forest floor in subalpine deciduous forests, was analyzed to understand the ecological significance of evergreenness in a subalpine climate with a short growing season and low temperature. Net production was highest under favorable light conditions in spring after the disappearance of snow cover, and 68% of the annual net production was attained before the canopy tree foliage had fully expanded. Most of the photosynthetic production in this period was carried out with over-wintered leaves. This appears to be an advantage of evergreenness. New leaves and inflorescences had developed in the period. Positive net production was maintained under deteriorating light conditions during summer, when 32% of the annual net production occurred. This production was used mainly for growth of fruits and underground organs. The net production of P. incarnata during summer was much higher than that of a related species that inhabits warm-temperate regions, because of its higher photosynthetic activity rather than its lower respiratory losses. The storage of dry matter in leaves and underground organs was not conspicuous. Unlike the warm-temperate species and another subalpine species that inhabits higher altitudes, P. incarnata is not strongly dependent on its reserve matter for the development of new organs.

29. Mechanism of Methane Transport from the Rhizosphere to the Atmosphere through Rice Plants.

- Abstract
To clarify the mechanisms of methane transport from the rhizosphere into the atmosphere through rice plants (Oryza sativa L.), the methane emission rate was measured from a shoot whose roots had been kept in a culture solution with a high methane concentration or exposed to methane gas in the gas phase by using a cylindrical chamber. No clear correlation was observed between change in the transpiration rate and that in the methane emission rate. Methane was mostly released from the culm, which is an aggregation of leaf sheaths, but not from the leaf blade. Micropores which are different from stomata were newly found at the abaxial epidermis of the leaf sheath by scanning electron microscopy. The measured methane emission rate was much higher than the calculated methane emission rate that would result from transpiration and the methane concentration in the culture solution. Rice roots could absorb methane gas in the gas phase without water uptake. These results suggest that methane dissolved in the soil water surrounding the roots diffuses into the cell-wall water of the root cells, gasifies in the root cortex, and then is mostly released through the micropores in the leaf sheaths.
