A methodology is presented for developing nomograms for assessing and stratifying the risk of a clinical outcome based on the created virtual data set using the R software environment. The virtual data set included input numerical and factor variables (variable types correspond to the R software documentation) and outcome. For quantitative variables, descriptive statistics were calculated at all levels of the outcome variable, and mosaic diagrams were constructed for factor variables. As a model that describes the association of input variables with the outcome, a logistic regression model was used. A bootstrap method was applied to validate and evaluate the model performance. The calculated validity indicators showed an acceptable discriminatory ability of the predictive model. The statistical calibration demonstrated the proximity of the model’s calibration curve to the ideal calibration curve. Based on the logistic regression coefficients, a nomogram was constructed using which the risk value of a specific outcome was calculated for each subject (patient). It is shown that with the help of the presented technique it is possible to stratify patients effectively by the risk of an adverse outcome, thus adequately altering the diagnosis and treatment tactics. The use of a nomogram greatly simplifies risk assessment and can be used in paper form as a supplement to the patient examination protocol. The article contains the codes of the R programming language with explanations.

To raise the efficacy and the selectivity of known medicinal substances (MS), methods have recently been developed to create systems for their directed transport in the body using various carriers. The problem of decreasing the toxicity and increasing the selectivity of antitumor MSs is particularly relevant. In recent years, studies are underway for the development of conjugates of MSs and the natural polymers, in particular, the non-starch polysaccharides, many of which manifest antitumor and immunomodulatory properties. Owing to the unique combination of physicochemical and biological properties, arabinogalactan from larch wood (AG) is one of the most promising tools for the development of complex preparations with elevated bioavailability and reduced toxicity. The aim of the present work is to obtain mechanocomposites of cyclophosphamide (CPA) with the initial arabinogalactan (molecular mass 16 645 Da) and its fragments (molecular mass 8380 Da) and to study their physicochemical properties. According to IR and 13С, 15N and 31Р NMR spectroscopy, long-term mechanochemical treatment does not lead to a change in chemical composition of CPA molecules, only the formation of hydrogen bonds NH···OH is observed. The data on X-ray analysis, IR and multinuclear NMR spectroscopy show that, with a weight ratio of 1 : 5 and 1 : 10, disordering of CPA crystallographic structure and its molecular dispersion without chemical interaction of the components occur in the matrix of the polysaccharide with mechanical treatment of mixtures of CPA–AG over 4–12 hours.

The outcome of sarcoidosis of the respiratory system is closely linked to the state of homeostasis and reactivity. Monitoring their status is possible through immunological, biochemical and hormonal studies. Within common medical institutions re-executing these studies is not always possible for a number of reasons. Therefore, the research for the availability and informative criteria of homeostasis and reactivity state was initiated at the end of XIXth century and continues until now. At the Department of Phthisiopulmonology of the First St.-Petersburg state medical University Academy I. P. Pavlov. University, new accessible and informative diagnostic criteria for estimation the state of homeostatic balance of the organism are developed. They are based on research of the general adaptation syndrome and the discovery of antistress types of adaptive reactions of the organism. In this regard, based on the leukogram evaluation following diagnostic criteria were developed and tested: leukocyte-lymphocytic index, types of adaptive reactions, entropy and redundancy of formed elements of white blood cells, extent of disturbance of homeostasis, indices of proliferation and differentiation of monocytes and reactivity types. Unfortunately, in the pulmonology clinics, they are used today only in single cases. The monitoring of homeostasis and reactivity indicators in patients with respiratory system sarcoidosis had a high working performance. It is established that in sarcoidosis the best treatment effect with corticosteroids with minimal residual changes of respiratory system took place in patients with the appropriate type of reactivity and normal range of other indicators. Pathological reactivity of an organism, especially hyporeactive and areactive types, are always accompanied by formation of pronounced residual sarcoidosis changes in the respiratory tract, which is the basis for the subsequent exacerbation (recurrence) of sarcoidosis.

Features of adaptation after psychical traumatic stress in healthy military men who performed special tasks more than half a year ago were studied. It is shown that 40% of combatants have signs of partial posttraumatic stress disorder, which evidences about negative adaptation after psychical traumatic stress. Signs of partial posttraumatic stress disorder in all manifest as neurotic disorders, and in 7,5% of combatants are associated with signs of psychotic disorders. Among the signs of neurotic disorders, the manifestations of somato-vegetative disorder were encountered in 62,5%, affective tension - in 50%, sleep disorder - in 43,8% and anxiety-phobic disorder - in 43,8%. Among the signs of psychotic disorders, there were mildly pronounced manifestations of social phobia, panic disorder and depression. Soldiers with partial posttraumatic stress disorder have a statistically significant decrease in the self-assessment quality of life in such indicators as «general health», «mental health» and «social functioning». The negative correlation between scores of life quality and signs of partial posttraumatic stress disorder pieces of evidence that namely this disorder impacts on the quality of life in combatants. Comparison of servicemen groups with and without signs of partial posttraumatic stress disorder according to the expert evaluation indicators, exhibited by the unit commanders, and according to the indicators of the operator working, did not reveal a significant difference between them. Thus signs of partial posttraumatic stress disorder in combatants decrease self-assessment life quality but do not have an effect on the professional aspect of social functioning under favourable conditions. This military personnel are needed in medical and psychological correction and rehabilitation to prevent the development of maladaptation in stressful conditions.

Аbstract. Quality of life of young men in the view of existential compliance is considered. In its most general form, the term «existence» means «specifically human way of being», the main characteristic of which is the freedom of choice, that is the way of being a person, when he is constantly in a situation that asks him: «How does a man will be a person in this situation?». А person’s subjective assessment of his life was investigated by the technique of A. Langle and K. Orgler «Scale of Existence» in 42 practically healthy young men (24,12±1,1 years). Scores diagnosed on such scales as Self-distancing, Self-Transcendence, Freedom, Responsibility, Personality, Existentiality, and Fulfillment testify to the absence of grounds for the development of existential crisis (for a crisis of subjective evaluation of one’s own life). Thus, with the right medical and psychological support of young people, they have the ability to cope with themselves and with the world, and the feasibility of interaction with the internal and external demands and proposals presented to circumstances, correlating with their own values. At the same time, one should keep in mind that a possible conflict can lead to nervous breakdowns, decrease in the level of professional health (a professional fitness). In order to prevent this, it is necessary to identify persons at risk and prevent possible psychosocial adaptation disorders in advance, using direct methods of psychodiagnostic. To this end, both methods of psycho-counseling and solving personal problems through the provision of advice or other assistance are recommended.

Very weakly bound electron-ion pairs in a strong magnetic field are called guiding center drift atoms, since the electron dynamics can be treated by guiding center drift theory. Over a wide range of weak binding, the coupled electron-ion dynamics for these systems is integrable. This paper discusses the dynamics, including the important cross magnetic field motion of an atom as a whole, in terms of the system constants of the motion. Since the dynamics is quasi-classical, quantum numbers are assigned using the Bohr–Sommerfeld rules. Antimatter versions of these guiding center drift atoms likely have been produced in recent experiments.

In recent experiments ultracold plasmas were produced by photoionizing small clouds of laser cooled atoms. This paper presents the results of molecular dynamic simulations for the early time evolution of such plasmas. Contrary to earlier speculation, no evidence of strong electron–electron correlations is observed in the simulations even if the initial value of the coupling parameter (Γe=e2/akTe) is much larger than unity. As electron–electron correlations begin to develop, the correlation energy is released to heat the electrons, raising the electron temperature to the point where Γe∼1 and limiting further development of correlation. Further heating of the electrons occurs as a by-product of three-body recombination. When a model of laser cooling is added to the simulation, the formation of strong ion–ion correlation is observed. Contrary to earlier suggestion, the rate of three-body recombination is observed to be in reasonable agreement with the traditional formula, R=3.9×10−9 s−1[n(cm−3)]2[Te(K)]−9/2,...

A design of a setup for performing ICR isotope separation is examined. Its main components are based on new physical concepts. It is proposed that ionization of matter will occur in a discharge with crossed electric and magnetic fields. New hf antennas are proposed for ICR heating, making the heating process highly selective. The possibility of heating on the second harmonic of the ion cyclotron frequency is discussed. It is suggested that a curvilinear magnetic field be used for, separating the heated isotope, 6 figures, 24 references.

Discovery of superconducting materials that operate at high temperatures revive interest in the use of rf field for plasma confinement [1]. This paper discusses feasibility of a scheme where resonant rf cavities are attached to the mirror ends of an open system for plasma confinement.

The ATHENA and ATRAP groups at CERN recently reported the production of weakly bound antihydrogen atoms in a non-neutral positron-antiproton plasma. This Letter derives an equation of motion for weakly bound atoms in the electric and magnetic fields of the plasma and trap. The atoms are polarized by the electric field and can be trapped radially in the edge region of the plasma where the electric field is maximum.

In recent experiments, ultracold plasmas were produced by photoionizing small clouds of laser-cooled atoms. It has been suggested that the low initial temperature of these novel plasmas leads directly to strong correlation and order. In contrast, we argue that rapid intrinsic heating raises the electron temperature to the point where strong correlation cannot develop. The argument is corroborated by a molecular-dynamics simulation of the early-time plasma evolution.

The ApparaTus for High precision Experiment on Neutral Antimatter and antihydrogen TRAP collaborations have produced antihydrogen atoms by recombination in a cryogenic antiproton-positron plasma. This paper discusses the motion of the weakly bound atoms in the electric and magnetic field of the plasma and trap. The effective electric field in the moving frame of the atom polarizes the atom, and then gradients in the field exert a force on the atom. An approximate equation of motion for the atom center of mass is obtained by averaging over the rapid internal dynamics of the atom. The only remnant of the atom internal dynamics that enters this equation is the polarizability for the atom. This coefficient is evaluated for the weakly bound and strongly magnetized (guiding center drift) atoms understood to be produced in the antihydrogen experiments. Application of the approximate equation of motion shows that the atoms can be trapped radially in the large space charge field near the edge of the positron column. Also, an example is presented for which there is full three-dimensional trapping, not just radial trapping. Even untrapped atoms follow curved trajectories, and such trajectories are discussed for the important class of atoms that reach a field ionization diagnostic. Finally, the critical field for ionization is determined as an upper bound on the range of applicability of the theory.

The method of laser photoexcitation with registering spectral changes on the screen of an image-converter tube to study some primary phototransformations in photochromic spyropyranes in liquid and solid polymeric solutions has been used for the first time. The scheme of the device providing for the registrations of the absorption and luminescence spectra in the 400 to 750 nm band with time resolution up to IO-II s is presented. The results of the investigations show the polymeric matrix to have some specific effects upon the photocolouration of spyropyranes.

Summary form only given. We present a theoretical study of the isotope separation by means of isotopically selective ion cyclotron resonance plasma heating (ICRH). The special attention is devoted to the separation of gadolinium isotopes. The ions are supposed to pass through a device where they are heated by a full-turn-loop antenna that excites RF field with azimuthal number m=0. We calculate the distribution function of ions in a plasma stream at the orifice of the device.
