In the fields of engineering, nanoscience, and biomechanics, thin structural members, such as beams, plates, and shells, that are supported by an elastic medium are used in several applications. There is a possibility that these thin structures might buckle under severe loading conditions; higher-order, complicated elastic buckling modes can be found owing to the balance of rigidities between the thin members and elastic supports. In this study, we have shown a new and simple ‘power law’ relation between the critical buckling strain (or loads) and rigidity parameters in structural members supported by an elastic medium, which can be modelled as a Winkler foundation. The following structural members have been considered in this paper: i) a slender beam held by an outer elastic support under axial loading, ii) cylindrical shells supported by an inner elastic core under hydrostatic pressure (plane strain condition), and iii) complete spherical shells that are filled with an inner elastic medium.

The goal of this study was to investigate analytically the buckling phenomena of slender beam structures embedded in an elastic medium. In such cases, there is a possibility that spiral-shaped buckling modes can occur due to the restraining effect of the surrounding medium. As a result of analytical investigations, helical buckling occurs radically. Furthermore the pitch of the helix does not fluctuate as the radius of the helix increases. The interesting mechanical implications of the embedding effect are demonstrated by the theoretical approach established in this study.

The Surface Ocean CO2 Atlas (SOCAT) is an effort by the international marine carbon research community. It aims to improve access to carbon dioxide measurements in the surface oceans by regular releases of quality controlled and fully documented synthesis and gridded fCO2 (fugacity of carbon dioxide) products. SOCAT version 2 presented here extends the data set for the global oceans and coastal seas by four years and has 10.1 million surface water fCO2 values from 2660 cruises between 1968 and 2011. The procedures for creating version 2 have been comparable to those for version 1. The SOCAT website (http://www.socat.info/) provides access to the individual cruise data files, as well as to the synthesis and gridded data products. Interactive online tools allow visitors to explore the richness of the data. Scientific users can also retrieve the data as downloadable files or via Ocean Data View. Version 2 enables carbon specialists to expand their studies until 2011. Applications of SOCAT include process studies, quantification of the ocean carbon sink and its spatial, seasonal, year-to-year and longer-term variation, as well as initialisation or validation of ocean carbon models and coupled-climate carbon models.

Using 80 gastric biopsy specimens from patients with various gastroduodenal diseases, the isolation rate of H. pylori and urease activity of the biopsy specimens were examined. The sensitivity and specificity of the rapid urease test was 84.6% and 75.9%, respectively, for the cultivation of H. pylori. The mean number of H. pylori detected in urease-positive and negative biopsy specimens was 10(6.1 +/- 0.9) and 10(4.5 +/- 1.2) cfu/g specimens, respectively.

Previous studies have shown that hypertension causes endothelial dysfunction. To study the influence of exogenous nitric oxide(NO) on endothelial dysfunction produced by hypertension, we administered a non-depressor dose of nipradilol to two-kidney, one-clip renovascular hypertensive rats(2K1C). Sprague-Dawley rats underwent either sham surgery(G-1) or clipping of the left renal artery. From day seven, 2K1C were randomized into 3 groups, placebo treatment(G-2), nipradilol treatment(G-3,) and propranolol treatment(G-4). Urinary NO2- + NO3-(NOx) excretion (UNOx V) was measured 4 weeks after clipping, and then, acetylcholine(Ach), A23187, or sodium nitroprusside(SNP)-induced relaxation were measured in the aorta. Blood pressure was increased in G-2, G-3, and G-4 compared to G-1. UNOx V was lower in G-2, G-3, and G-4 compared to G-1, but UNOx V was higher in G-3 compared to G-2 and G-4. Although Ach or A23187-induced relaxation was significantly decreased in isolated artery from G-2, G-3, and G-4 compared with those from G-1. Ach- or A23187-induced relaxation was improved in G-3. SNP-induced relaxation did not differ among the 4 groups. These results suggest that exogenous NO from nipradilol reduces the endothelial dysfunction caused by hypertension without changing the blood pressure.

It is known that cutaneous electrogastrography (EGG) undergoes a change after food ingestion, showing increases in frequency and amplitude compared with preprandial values, but the factors regulating such changes remain to be elucidated. Paying special attention to gastrin, one of gastrointestinal peptides released after food ingestion, we administered tetragastrin in an exogenous manner and evaluated its effects on EGG in the present study. In healthy subjects, the intramuscular injection of tetragastrin significantly increased EGG frequency dose-dependently, but caused no significant change in amplitude. These results suggest that the increase in endogenous gastrin release is one of the mechanisms which underlies the increase in EGG frequency after food ingestion.

We investigated the differences in background factors, clinical features, and gastric function tests among gastric ulcers of various depths.The subjects were 68 patients (male 64, female 4) who were diagnosed as having a gastric ulcer at the angulus. The ulcers were classified according to depth based on the following: UL2 (shallow) ulceration to the submucosa; UL3 (intermediate), to the muscularis propria; and UL4 (deep excavation), beyond the muscularis propria. The depth of each ulcer was determined by endoscopic ultrasonography and/or ordinary endoscopic findings. We assessed clinical features, age, gender, smoking habit, alcohol consumption, ulcer history, presence of H. pylori, gastric acid secretion, gastric emptying, serum gastrin level, healing rate, and recurrence rate.Patients with UL4-type ulcers had a higher rate of recurrence and a significantly higher incidence of H. pylori infection. Patients with hyperacidity and currently smoking or consuming alcohol were significantly more likely to have UL4-type ulcers than of UL2 or 3 ulcers. Furthermore, a close relationship was recognized between recurrence, intractability and deeply excavated ulcers. Ulcer depth was not correlated significantly with any of the following factors: 1) patient's profile; including gender and hemorrhagic symptoms; 2) gastric function; including gastric emptying and serum gastrin levels.Smoking, alcohol consumption, recurrence of ulcers, hyperacidity and H. pylori infections are important factors associated with deep ulcers.

This study was aimed to clarify the endoscopic findings and the decline of serum IgG titer after successful eradication of H. pylori in long-term (from 2 to 7 years). Forty-six H. pylori-positive peptic ulcer (22 GU and 26 DU) cases were eradicated with antimicrobial therapy. Sixty-nine non-eradicated DU cases who received maintainance therapy with H2-blocker were control group. Biopsy urease test (BUT) and culturing was performed to diagnose the H. pylori infection. Anti-H. pylori IgG titer (EIA) were also measured in some cases, pre and 6, 12, 24 months after the eradication. In 3 cases, H. pylori were recrudescent and only in a case, DU recurred during 5 years after eradication. Meanwhile, in 55% of control cases, DU were recurred during same periods. In 62% of eradicated cases, serum IgG-antibody to H. pylori declined below the cut-off level during 2 years after eradication. It was certified that eradication therapy against H. pylori prevents ulcer recurrence for long time, and re-rise of serum IgG titer to H. pylori might predict a recrudescence of infection.

It has been suggested that changes in the micro circulatory system are related to the early production of acute gastric mucosal injury and inflammatory factors such as prostaglandins, histamine, etc., have been considered as contributing to the development of the injury. We assessed the permeability of the gastric mucosa in rats with ethanol-induced acute mucosal injury by measuring the leakage rate of 51chronium-ethylene-diamine-tetraacetic acid (51Cr-EDTA) into the gastric juice. Histamine concentrations in the gastric mucosa was measured by high performance liquid chromatography. The enterochromaffin-like (ECL) cell counts in the gastric mucosa was performed following histamine staining with an enzyme-labeled antibody, and the histamine released due to degranulation was observed. We also investigated the kinetics of endogenous histamine in the gastric mucosa. Five minutes after the administration of ethanol, an increase in permeability, an increase in histamine concentration, and a decrease in ECL cell count were found in the gastric mucosa. These results suggest that endogenous histamine in the gastric mucosa is closely related to the early development of acute gastric mucosal injury.

The contribution of the endogenous nitric oxide (NO) to the distal colonic motility was investigated by inhibiting NO production using NO synthase inhibitor, NG-monomethyl-L-arginine (L-NMMA). The experiment was carried out in the anaesthetized rat, which was vagotomized and pretreated with guanethidine, phentolamine and propranolol. The colonic motility was evaluated by changes in the increase in intraluminal pressure of the distal colon. L-NMMA infusion (100mg/kg iv) in the presence of atropine (1 mg/kg iv) resulted in a significant increase in arterial blood pressure and intraluminal pressure. The following L-arginine infusion (100mg/kg iv) restored blood pressure and intraluminal pressure to the control level. In contrast, L-NMMA infusion in the absence of atropine resulted in no change in intraluminal pressure with a significant increase in arterial blood pressure. These results indicate that there are NO-dependent and nondependent mechanisms to regulate the distal colonic motility.

Changes in gastric acid secretion and fasting gastrin levels (FGL) during the healing of duodenal ulcers were investigated. Duodenal ulcers were classified into three stages, with regard to healing, active, healing, and healed (scar). As controls, 49 individuals without gastro-duodenal lesions were used. We found that gastric acid secretion was highest during the active stage, thereafter decreasing during the healing and healed stages. Gastric acid secretion was lowest in the normal subjects, and significant differences were found between the 3 stages of the duodenal ulcers. FGL was also highest in the active stage, thereafter decreasing during the healing and healed stages. In the follow-up of 10 doudenal ulcer patients, gastric acid secretion and FGL were higher in the active than in the healed stage. It was concluded that gastric acid secretion and FGL change during the healing of duodenal ulcers. The increase of gastric acid secretion and FGL seen in the active stage appears to be implicated in the pathogenesis of the duodenal ulcer.

We sought a good indicator cell line to use in detecting the vacuolating toxin produced by Helicobacter pylori and in examining the characteristics of the toxin.Using five cell lines [baby hamster kidney (BHK-21), human amnion (FL), human uterine cancer (HeLa), rabbit kidney (RK-13) and African green monkey kidney (Vero)], cytotoxicity assays were performed with 33 H. pylori strains. To analyse the proteins produced by toxigenic and non-toxigenic strains, immunoblotting was done with the sera of patients with gastroduodenal diseases.When RK-13, FL, Vero, BHK-21 and HeLa cells were used for the assay, 73, 61, 27, 27 and 21% of strains, respectively, were toxigenic. The vacuolating toxin is sensitive to heat at 70 degrees C. By immunoblot analysis, under non-denaturing conditions, immune sera recognized proteins with molecular weights of 600,000 and 500,000 in the toxigenic but not in the non-toxigenic strains.H. pylori produces vacuolating toxin, which is a heat-sensitive protein at 70 degrees C and is easily detectable by cytotoxicity assay using the RK-13 cell line. Toxigenic H. pylori strains produce various proteins, including this toxin, which are recognized by sera from patients with gastroduodenal diseases.

Stress ulcer formation is reportedly much less frequent in SHR than in normotensive control rats (Wistar Kyoto Rat: WKY). The purpose of this study was to investigate the mechanism of maintenance of gastric mucosal blood flow (GMBF) during imposed stress in SHR. In stressed-only SHR, GMBF did not significantly change during water immersion and restraint conditions and ulcer index (UI) was significantly lower than that of WKY. Stress conditions led to a fall in blood pressure and a gradual fall in heart rate in WKY and SHR. It was assumed that the changes in blood pressure and heart rate during stress were due to vagal hyperfunction. The catecholamine level in the fundic gland of the gastric tissue was higher in the non-stressed SHR than in the non-stressed WKY. The administration of 6-hydroxydopamine to SHR produced a significant reduction in GMBF during stress conditions and UI was significantly higher in this group than in the stressed-only SHR. In SHR treated with nifedipine, UI was lower than that of the control group and GMBF showed no significant change compared with the stressed-only SHR. However, the administration of verapamil produced a significant reduction in GMBF during stress conditions and increased UI. The norepinephrine and dopamine levels of the groups treated with verapamil were significantly lower than those in the groups treated with nifedipine. These results suggest that local regulation of gastric mucosa mediated by sympathetic hyperfunction in SHR is more important for the maintenance of GMBF during stress conditions than changes in peripheral artery resistance.

The pharmacokinetics and dose proportionality of mosapride citrate ((+-)-4-amino-5-chloro-2-ethoxy-N-[[4- (4-fluorobenzyl)-2-morpholinyl]methyl]benzamide citrate dihydrate, AS-4370, CAS 112885-42-4) were investigated in healthy male volunteers. The subjects were given a single oral dose (5, 10, 20 and 40 mg, each of 5 subjects) and a multiple oral dose (20 mg t.i.d. for one day, and 10 and 20 mg t.i.d. for 8 days, each of 5 subjects). Food effect on the pharmacokinetics of mosapride was also evaluated after a single oral 10 mg dose by an open, two-way crossover method. Mean plasma levels of mosapride reached a peak 0.5-1 h after single dosing of 5, 10, 20 and 40 mg. The peaks were dose-related with values of 25.1, 51.2, 157.8 and 280.6 ng/ml, respectively, and were followed by a first-order decrease with apparent half-lives of 1.4-2.0 h. The Cmax and AUC increased in proportion to the dose, indicating linear pharmacokinetics of mosapride up to 40 mg. The Cmax of M-1, a des-4-fluorobenzyl metabolite, was 1/6 of that of the unchanged drug. Urinary excretion of the unchanged mosapride and M-1 during 48 h after single dosing accounted for 0.1-0.4% and 7.0-11.0% of the dose, respectively. There were no significant changes in the plasma concentration-time profiles and urinary excretions between single and multiple doses, indicating that the pharmacokinetics of mosapride in man was not altered by its multiple administration. Plasma levels of mosapride reached steady state on day 2 of multiple administration.(ABSTRACT TRUNCATED AT 250 WORDS)

One hundred and fourty four patients with non-ulcer dyspepsia (NUD), as defined by the working party of AGA in 1987, (67 men and 77 women, 16-76 years, mean age 42.9 +/- 1.2 years) and 34 asymptomatic controls (25 men and 9 women, 17-75 years, mean age 50.6 +/- 2.4 years) parameters of gastrophysiological function (gastric acid secretion, postprandial gastric emptying-acetaminophen method, serum gastrin levels and cutaneous electrogastrography (EGG)) and the prevalence of Helicobacter pylori (Hp) (histological and urease test of biopsy specimens) were investigated. Based on symptom patterns, there were 68 patients with dysmotility-like dyspepsia, 27 with ulcer-like dyspepsia, 17 with reflux-like dyspepsia, 6 with aerophagia and the 26 with nonspecific or idiopathic dyspepsia. The age distribution of NUD was predominant in the fourth decade, and the sex distribution was not significantly different. In general, hypersecretion of gastric acid and hypergastrinemia were rare in NUD patients. There was no significant difference in gastric acid secretion, basal and food stimulated serum gastrin levels and prevalence of Hp between the two groups. But 51 of 144 NUD patients (41.1%) had delayed gastric emptying (p0.05) compared to controls. Indeed gastric emptying was markedly prolonged in patients with dysmotility-like (58.1%) and reflux-like (42.9%) dyspepsia. On EGG, about a half of NUD patients showed evidence of bradygastria or tachygastria, in particular in the postprandial state, which was related to delayed gastric emptying. By chronic administration of cisapride, score of symptoms was significantly decreased and postprandial gastric emptying was significantly accelerated in delayed gastric emptying cases. We conclude that in NUD patients, in particular those with dysmotility-like dyspepsia, tests of postprandial gastric emptying and/or EGG are useful for investigation of gastric motor disorder and therapeutic effects of several prokinetic drugs clinically.

In 240 patients with symptoms of dyspepsia, recruited consecutively and investigated in 12 hospitals in Japan, 24.2% were diagnosed having organic dyspepsia; 75.8% had functional dyspepsia, of whom 63.2% were diagnosed by the investigator having dysmotility-like, 13.7% ulcer-like, 11.5% reflux-like, and 11.5% non-specific dyspepsia. There was, however, considerable overlap of symptom profiles. Cisapride therapy initiated in functional dyspeptic patients resulted in moderate or marked improvement in 79.1% of the patients with the highest response rates for dysmotility-like (85.2%), reflux-like (81.0%), and non-specific dyspepsia (76.1%) (versus 52.0% for ulcer-like dyspepsia).

To investigate the effect of 6 weeks administration of proton pump inhibitor (omeprazole) and H2-receptor antagonist (cimetidine) on gastric histamine synthesis and acid secretion, we studied experimentally in the rat stomach. Then gastric mucosal histamine concentration, histidine decarboxylase (HDC) activity and serum gastrin concentration, and HDC positive cell number were examined in time-course. The 6 weeks administration of omeprazole caused more increase of HDC positive cells than cimetidine. After cessation of administration with omeprazole, high plasma gastrin level immediately reduced. However the increase of gastric mucosal HDC activity and histamine concentration were prolonged, compared with cimetidine. These findings suggest to affect differently between omeprazole and cimetidine on gastric acid secretion after cessation of long-term treatment.

Hemorrhage into a pancreatic pseudocyst is a rare event, but is the most rapidly lethal complication of chronic pancreatitis. Visceral-vessel aneurysms are an unexpectedly common finding in arteriography of patients with chronic pancreatitis. This case report describes bleeding from an anterior superior pancreaticoduodenal artery aneurysm, caused by chronic pancreatitis. The aneurysm was successfully treated by embolization with a steel coil.

A-49-year-old man was admitted because of a lower abdominal mass. During the five days prior to admission, he had noted fever and lower abdominal pain. Palpation of the abdomen revealed a tender mass, 10 cm in diameter, in the suprapubic region. An x-ray revealed an irregular collection of gas, 9 cm in diameter, in the pelvic cavity, which appeared as a mirror image when the patient was upright. Based on the physical finding and the results of a barium enema, abdominal CT, MRI, and small intestine imaging, a diagnosis of leiomyosarcoma of the ileum was made. Excision of the tumor and part of the small intestine was performed. A saccular tumor, 11 x 6 x 5 cm, was found 1.0 m from the cecum and growing out of the wall of the ileum. A large amount of pus was found inside the cavity. The pathological diagnosis was leiomyosarcoma.

The long-team effects of cisapride on gastric emptying and hormonal interaction in diabetic patients without gastrointestinal symptoms was evaluated. Gastric emptying was determined by the N-acetyl-p-amino-phenol (APAP) method before and after cisapride treatment. Gastric emptying improved significantly, from 5.95 to 8.04 micrograms/ml, after 6 weeks of treatment. Plasma concentrations of immunoreactive insulin (IRI), after 30 and 45 minutes in the gastric emptying test, increased significantly after treatment. It appeared that improvement of gastric emptying after loading with cisapride might be related to improved insulin secretion in diabetic patients without subjective symptoms of gastroparesis.

Mock-up flow tests were run on the 10 Mw(th) natural uranium heavy water JRR-3 reactor. Equipment consisted of a mock-up core tank, circulating pump, flow control system, and water level regilating system. Tests were run on the flow resistance and vibration of the model fuel element, leakage around the fuel element seat, flow distribution from lower plenum, core tank water level, fuel element flow, effect of flow control mechanism on the flow characteristics of the total system, and errors of flow measurement with orifice. A dog model was used for direct measurement of the sphincter of Oddi using endoscopy. The effects of anticholinergic agents and a selective antimuscarinic agent on the canine sphincter of Oddi motility were studied. Manometric tracing at the canine sphincter showed rhythmical phasic contractions which were not affected by respiration. The mean values of the basal motility were as follows: the peak pressure was 61.5 +/- 2.2 (m +/- SE) cmH2O, trough pressure was 2.18 +/- 1.2 cmH2O, and the frequency was 17.4 +/- 0.2 waves/min. Intravenous administration of atropine sulfate, butropium bromide, butylscopolamine bromide and also pirenzepine significantly reduced the amplitude and frequency of canine sphincter contractions. From these results, it appeared that the cholinergic autonomic nervous system might be the one of the regulators of the canine sphincter of Oddi activity.

Preoperative carcinoembryonic antigen (CEA) levels in 36 colorectal carcinoma patients were corrected in accordance with pathological stage and tumor location. Increasing levels of CEA were found in advanced stages of the disease. There were no significant differences among the CEA levels of right-sided colon tumors, left-sided tumors and rectal tumors.

The records of a total of 60 gastritis patients who showed hematemesis and/or melena are reviewed. The mean age was 40. The ratio of males to females was two to one. During the examination, 19 upper G-I lesions were confirmed in addition to gastritis. Sixteen patients had severe underlying diseases which were thought to be the cause of the bleeding tendency. Thirteen patients took alcohol or drugs which induced bleeding. Prognoses of the patients were relatively good when they were treated medically.

We have reported that collagenase activity of the gastric mucosa increased significantly in acetic acid ulcers of dogs. The present study revealed that acetic acid ulcers in showed that the active collagenase level in the ulcer tissue increased rapidly with a peak (412.5%) on the 3rd day after induction of the acetic acid ulcer. Thereafter, the collagenase activity levels decreased gradually by the 7th day and returned practically to the control levels by the 14th day. On the other hand, collagen (hydroxyproline content) decreased by the 7th day (-16.5%) and recovered by the 14th day after induction of the acetic acid ulcer. The peak of active collagenase on the 3rd day coincident with the maximum ulcer index, suggesting involvement of collagenase activity in ulceration. In addition, destruction and production of collagen are considered to take place not only at the base of the ulcer and its margin, but also along its circumference.

Three cases of type A gastritis associated with multiple polypoid lesions of the body of the stomach are presented. The endoscopic congo-red test showed the red to black color changes of the sprayed congo-red only on the top of the polypoid lesions. The endoscopic methylene-blue test revealed no dye absorption on the antral mucosa or on the top of the polypoid lesions, while the dye was absorbed on the flat mucosa surrounding the top of the polypoid lesions. Histological examinations revealed well-preserved oxyntic glands on the top of the polypoid lesions. The flat mucosa showed marked atrophy or intestinal metaplasia. There have been several reports of experimental models of type A gastritis, where chief and parietal cells decreased in considerable number. These findings and the experimental models suggest that the polypoid lesions resulted from atrophic changes of type A gastritis. The top of the polypoid lesions is the residual oxyntic glands. The lesions, therefore, should be called "gastric pseudopolyposis." The spontaneous disappearance of the polypoid lesions seen in one case is thought to be a result of a further progression of the atrophic gastritis. A pronounced G-cell increase was noted in the antrum and lower body of the stomach. This is considered a secondary phenomenon following the marked reduction of acid secretion.

Gastric acid secretion and plasma gastrin response to test meals were examined in 110 patients with gastric cancer and a comparative study was made in accordance with depth of invasion, macroscopic shape, histological type, location and size of the lesion. Cases were classified into two large groups by the depth of invasion: early cancer (invasion was limited to the mucosa and submucosa) and advanced cancer (invasion reached the muscularis propria or deeper). Patients showed hypoacidity on the whole. Cases of early cancer showed significantly higher acid secretion than cases of advanced cancer, although no significant differences were observed in gastrin release. Among the cases of early cancer, cases of the elevated shape showed significantly lower acid secretion and higher gastrin release than cases of the depressed shape, and cases of the histologically differentiated type showed significantly higher acid secretion than cases of the undifferentiated type although no significant differences were observed in gastrin release. Among the cases of advanced cancer, cases with large carcinomas in the corpus tended to show low acid secretion and cases with large carcinomas in the antrum tended to show low gastrin release. High gastrin release was observed in cases of the Borrmann IV type with giant folds.
