Abstract: Among multimessenger observations of the next Galactic core-collapse supernova, Super-Kamiokande (SK) plays a critical role in detecting the emitted supernova neutrinos, determining the direction to the supernova (SN), and notifying the astronomical community of these observations in advance of the optical signal. In 2022, SK has increased the gadolinium dissolved in its water target (SK-Gd) and has achieved a Gd concentration of 0.033%, resulting in enhanced neutron detection capability, which in turn enables more accurate determination of the supernova direction. Accordingly, SK-Gd’s real-time supernova monitoring system has been upgraded. SK_SN Notice, a warning system that works together with this monitoring system, was released on 2021 December 13, and is available through GCN Notices. When the monitoring system detects an SN-like burst of events, SK_SN Notice will automatically distribute an alarm with the reconstructed direction to the supernova candidate within a few minutes. In this paper, we present a systematic study of SK-Gd’s response to a simulated Galactic SN. Assuming a supernova situated at 10 kpc, neutrino fluxes from six supernova models are used to characterize SK-Gd’s pointing accuracy using the same tools as the online monitoring system. The pointing accuracy is found to vary from 3° to 7° depending on the models. However, if the supernova is closer than 10 kpc, SK_SN Notice can issue an alarm with three-degree accuracy, which will benefit follow-up observations by optical telescopes with large fields of view.

Abstract: We report the first search result for the flux of astrophysical electron antineutrinos for energies ${ \mathcal O }(10)\,\mathrm{MeV}$ in the gadolinium-loaded Super-Kamiokande (SK) detector. In 2020 June, gadolinium was introduced to the ultrapure water of the SK detector in order to detect neutrons more efficiently. In this new experimental phase, SK-Gd, we can search for electron antineutrinos via inverse beta decay with efficient background rejection thanks to the high efficiency of the neutron tagging technique. In this paper, we report the result for the initial stage of SK-Gd, during 2020 August 26, and 2022 June 1 with a 22.5 × 552 kton · day exposure at 0.01% Gd mass concentration. No significant excess over the expected background in the observed events is found for the neutrino energies below 31.3 MeV. Thus, the flux upper limits are placed at the 90% confidence level. The limits and sensitivities are already comparable with the previous SK result with pure water (22.5 × 2970 kton · day) owing to the enhanced neutron tagging. Operation with Gd increased to 0.03% started in 2022 June.

5. The increasing impact of length of stay 'outliers' on length of stay at an urban academic hospital

Abstract: Abstract Background As healthcare systems strive for efficiency, hospital “length of stay outliers” have the potential to significantly impact a hospital’s overall utilization. There is a tendency to exclude such “outlier” stays in local quality improvement and data reporting due to their assumed rare occurrence and disproportionate ability to skew mean and other summary data. This study sought to assess the influence of length of stay (LOS) outliers on inpatient length of stay and hospital capacity over a 5-year period at a large urban academic medical center. Methods From January 2014 through December 2019, 169,645 consecutive inpatient cases were analyzed and assigned an expected LOS based on national academic center benchmarks. Cases in the top 1% of national sample LOS by diagnosis were flagged as length of stay outliers. Results From 2014 to 2019, mean outlier LOS increased (40.98 to 45.11 days), as did inpatient LOS with outliers excluded (5.63 to 6.19 days). Outlier cases increased both in number (from 297 to 412) and as a percent of total discharges (0.98 to 1.56%), and outlier patient days increased from 6.7 to 9.8% of total inpatient plus observation days over the study period. Conclusions Outlier cases utilize a disproportionate and increasing share of hospital resources and available beds. The current tendency to exclude such outlier stays in data reporting due to assumed rare occurrence may need to be revisited. Outlier stays require distinct and targeted interventions to appropriately reduce length of stay to both improve patient care and maintain hospital capacity.

6. Osteochondral Allografts for Large Oval Defects of the Medial Femoral Condyle: A Comparison of Single Lateral Versus Medial Femoral Condyle Oval Grafts Versus 2 Overlapping Circular Grafts

Abstract: Background: Studies have demonstrated the acceptability of using a contralateral nonorthotopic lateral femoral condyle (LFC) graft for a circular medial femoral condyle (MFC) osteochondral defect up to 20 to 25 mm in diameter. Larger oblong defects can now be managed using either overlapping circle grafts or a single oblong-shaped osteochondral allograft (OCA). Purpose: To determine if an oblong contralateral nonorthotopic LFC OCA can attain an acceptable surface contour match compared with an oblong ipsilateral MFC OCA or an overlapping circle technique for large oblong defects of the MFC. Study Design: Controlled laboratory study. Methods: A total of 120 fresh-frozen human femoral condyles were matched by tibial width into 30 groups of 4 condyles (1 recipient MFC, 3 donor condyles). The recipient MFC was initially imaged using nano–computed tomography (nano-CT). A 17 × 36–mm oblong defect was created in the recipient MFC. Overall, 3 donor groups were formed: MFC oblong, LFC nonorthotopic oblong, LFC or MFC overlapping circles. After each transplant, the recipient condyle underwent nano-CT and was digitally reconstructed, which was superimposed on the initial nano-CT scan of the native recipient condyle. Dragonfly 3D software was used to determine the root mean square (RMS) of both the surface height deviation and the circumferential step-off height deviation between the native and donor cartilage surfaces for each graft. Results: RMS surface height deviations were as follows: 0.59 mm for MFC oblong grafts, 0.58 mm for LFC oblong grafts, and 0.78 mm for overlapping circle grafts. The MFC and LFC oblong grafts had significantly less surface height deviation than the overlapping circle grafts ( P = .004 and P = .002, respectively). RMS step-off height deviations were as follows: 0.68 mm for MFC oblong grafts, 0.70 mm for LFC oblong grafts, and 0.85 mm for overlapping circle grafts. The MFC and LFC oblong grafts had significantly less step-off height deviation than the overlapping circle grafts ( P < .001 and P = .002, respectively). The majority of this difference was on the medial segment of the overlapping circle grafts. Conclusion: Oblong ipsilateral MFC OCAs and oblong contralateral nonorthotopic LFC OCAs produced a significantly better surface contour match to the native MFC than overlapping circle grafts for oblong defects 17 × 36 mm in size. Clinical Relevance: Size-matched contralateral nonorthotopic LFC grafts are acceptable for MFC defects, which may allow for a quicker match, earlier patient care, and less wastage of valuable donor tissue.

Abstract: BACKGROUND When treating Medicare beneficiaries, orthopaedic surgeons must follow Centers for Medicare & Medicaid Services (CMS) policies regarding whether to perform surgical treatments under inpatient or outpatient status. Recently, most orthopaedic and spinal procedures were removed from the CMS's "inpatient-only" list (IPOL). We investigated differences in hospital payments under the Diagnosis Related Group (DRG)/Ambulatory Payment Classification (APC) system when common orthopaedic/spinal procedures are done under outpatient rather than inpatient status. We compared these differences under the DRG/APC model with differences in payments to Maryland hospitals, which are paid under the alternative Global Budget Revenue model. METHODS We used the CMS Inpatient Pricer and CMS Addendum B to retrieve the mean duration-of-stay data, estimated DRG (inpatient) payment, and APC (outpatient) payment for eight common orthopaedic/spinal procedures for four non-Maryland hospitals (2 urban academic hospitals and 2 neighboring community hospitals). We retrieved Maryland's Health Services Cost Review Commission hospital rates for the same eight procedures done under inpatient or outpatient status to estimate hospital charges for a Maryland urban academic hospital and a neighboring community hospital. Abstract: Super-Kamiokande has been searching for neutrino bursts characteristic of core-collapse supernovae continuously, in real time, since the start of operations in 1996. The present work focuses on detecting more distant supernovae whose event rate may be too small to trigger in real time, but may be identified using an offline approach. The analysis of data collected from 2008 to 2018 found no evidence of distant supernovae bursts. This establishes an upper limit of 0.29 yr−1 on the rate of core-collapse supernovae out to 100 kpc at 90% C.L. For supernovae that fail to explode and collapse directly to black holes the limit reaches to 300 kpc.

12. Bone Morphology and Vascular Supply of Pedicled Distal Radius Bone Using Nano-Computed Tomography

Abstract: Background: The goal of this study was to use nano-computed tomography to describe the intraosseous vascularity and structural characteristics of commonly used distal radius vascularized bone grafts for treatment of scaphoid nonunion. Methods: We obtained 8 fresh frozen human cadaver forearm specimens for infusion of barium contrast. Specimens were scanned and segmented to quantify the vascular volume and trabecular density within 3 common graft regions, including 1, 2 intercompartmental supraretinacular artery (1,2 ICSRA), fourth extensor compartment artery (4 ECA), and volar carpal artery (VCA), as well as thirds of the scaphoid. Outcomes also included mean and maximum cortical thickness and number of cortical perforators. Single-specimen analyses were also performed comparing vascularity and trabecular density of each graft with scaphoid regions of a single specimen. Statistical analysis was performed using analysis of variance with post hoc Tukey testing when P value was less than .05. Results: There was no significant difference between groups in the mean percent vascularity ( P = .76). The ratio of trabecular bone in each graft to scaphoid thirds was less than 1. The mean cortical thickness (0.79 mm, 95% confidence interval [CI], 0.66-0.93 mm) and maximum cortical thickness (1.45 mm, 95% CI, 1.27-1.63 mm) of VCA grafts were both significantly greater than those of 4 ECA and 1,2 ICSRA ( P < .001). Conclusions: There were no differences between vascular density of the 3 grafts and the scaphoid. Pedicled distal radius bone grafts have similar vascularity but morphometric differences such as cortical thickness and trabecular density which have unclear clinical implications.

Abstract: In order to improve Super-Kamiokande's neutron detection efficiency and to thereby increase its sensitivity to the diffuse supernova neutrino background flux, 13 tons of $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ (gadolinium sulfate octahydrate) was dissolved into the detector's otherwise ultrapure water from July 14 to August 17, 2020, marking the start of the SK-Gd phase of operations. During the loading, water was continuously recirculated at a rate of 60 m$^3$/h, extracting water from the top of the detector and mixing it with concentrated $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ solution to create a 0.02% solution of the Gd compound before injecting it into the bottom of the detector. A clear boundary between the Gd-loaded and pure water was maintained through the loading, enabling monitoring of the loading itself and the spatial uniformity of the Gd concentration over the 35 days it took to reach the top of the detector. During the subsequent commissioning the recirculation rate was increased to 120 m$^3$/h, resulting in a constant and uniform distribution of Gd throughout the detector and water transparency equivalent to that of previous pure-water operation periods. Using an Am-Be neutron calibration source the mean neutron capture time was measured to be $115\pm1$ $\mu$s, which corresponds to a Gd concentration of $111\pm2$ ppm, as expected for this level of Gd loading. Abstract: We present the development of neutron-tagging techniques in Super-Kamiokande IV using a neural network analysis. The detection efficiency of neutron capture on hydrogen is estimated to be 26%, with a mis-tag rate of 0.016 per neutrino event. The uncertainty of the tagging efficiency is estimated to be 9.0%. Measurement of the tagging efficiency with data from an Americium-Beryllium calibration agrees with this value within 10%. The tagging procedure was performed on 3,244.4 days of SK-IV atmospheric neutrino data, identifying 18,091 neutrons in 26,473 neutrino events. Abstract: The Super-Kamiokande detector can be used to search for neutrinos in time coincidence with gravitational waves detected by the LIGO-Virgo Collaboration (LVC). Both low-energy ($7-100$ MeV) and high-energy ($0.1-10^5$ GeV) samples were analyzed in order to cover a very wide neutrino spectrum. Follow-ups of 36 (out of 39) gravitational waves reported in the GWTC-2 catalog were examined; no significant excess above the background was observed, with 10 (24) observed neutrinos compared with 4.8 (25.0) expected events in the high-energy (low-energy) samples. A statistical approach was used to compute the significance of potential coincidences. For each observation, p-values were estimated using neutrino direction and LVC sky map ; the most significant event (GW190602_175927) is associated with a post-trial p-value of $7.8\%$ ($1.4\sigma$). Additionally, flux limits were computed independently for each sample and by combining the samples. 17. The increasing impact of length of stay 'outliers' on length of stay at an urban academic hospital

Abstract: Background As healthcare systems strive for efficiency, hospital “length of stay outliers” have the potential to significantly impact a hospital’s overall utilization. There is a tendency to exclude such “outlier” stays in local quality improvement and data reporting due to their assumed rare occurrence and disproportionate ability to skew mean and other summary data. This study sought to assess the influence of length of stay (LOS) outliers on inpatient length of stay and hospital capacity over a 5-year period at a large urban academic medical center. Methods From January 2014 through December 2019, 169,645 consecutive inpatient cases were analyzed and assigned an expected LOS based on national academic center benchmarks. Cases in the top 1% of national sample LOS by diagnosis were flagged as length of stay outliers. Results From 2014 to 2019, mean outlier LOS increased (40.98 to 45.11 days), as did inpatient LOS with outliers excluded (5.63 to 6.19 days). Outlier cases increased both in number (from 297 to 412) and as a percent of total discharges (0.98 to 1.56%), and outlier patient days increased from 6.7 to 9.8% of total inpatient plus observation days over the study period. Conclusions Outlier cases utilize a disproportionate and increasing share of hospital resources and available beds. The current tendency to exclude such outlier stays in data reporting due to assumed rare occurrence may need to be revisited. Outlier stays require distinct and targeted interventions to appropriately reduce length of stay to both improve patient care and maintain hospital capacity.

Abstract: The radioactivity background are among the most dangerous background for low energy neutrino analysis in Super-Kamiokande (SK), like the solar neutrino analysis. Among them, the main contribution is coming from $^{222}$Rn, which is spread in the detector's water due to the water source and to the photo multiplier (PMT) emanations. Up to now, its exact distribution in the detector was not known. Using our knowledge of the radon concentration in the detector water, and the SK-IV solar data, we developed a model of the radon distribution in the detector. The uncertainty on the Rn concentration associated with this model was estimated to be $\sim0.1$ mBq/m$^{3}$

Abstract: This manuscript describes the commissioning of the Mini-CAPTAIN liquid argon detector in a neutron beam at the Los Alamos Neutron Science Center (LANSCE), which led to a first measurement of high-energy neutron interactions in argon. The Mini-CAPTAIN detector consists of a Time Projection Chamber (TPC) with an accompanying photomultiplier tube (PMT) array sealed inside a liquid-argon-filled cryostat. The liquid argon is constantly purified and recirculated in a closed-loop cycle during operation. The specifications and assembly of the detector subsystems and an overview of their performance in a neutron beam are reported., 21 pages, 27 figures

Abstract: A search for neutrinos produced in coincidence with Gamma-Ray Bursts(GRB) was conducted with the Super-Kamiokande (SK) detector. Between December 2008 and March 2017, the Gamma-ray Coordinates Network recorded 2208 GRBs that occurred during normal SK operation. Several time windows around each GRB were used to search for coincident neutrino events. No statistically significant signal in excess of the estimated backgrounds was detected. The $\bar\nu_e$ fluence in the range from 8 MeV to 100 MeV in positron total energy for $\bar\nu_e+p\rightarrow e^{+}+n$ was found to be less than $\rm 5.07\times10^5$ cm$^{-2}$ per GRB in 90\% C.L. Upper bounds on the fluence as a function of neutrino energy were also obtained., Comment: 29 pages, 16 figures, 5 tables, accepted by PTEP

Abstract: Super-Kamiokande (SK) is a 50-kt water Cherenkov detector, instrumented with $\sim 13$k photo-multipliers and running since 1996. It is sensitive to neutrinos with energies ranging from 4.5 MeV to several TeV. A new framework has been developed for the follow-up of gravitational wave (GW) alerts issued by the LIGO-Virgo collaboration (LVC). Neutrinos are searched for, using a 1000-second time window centered on the alert time and in both SK low-energy and high-energy samples. Such observation can then be used to constrain the neutrino emission from the GW source. The significance of potential signals has been obtained by comparing neutrino direction with the localization of the GW. The computation of limits on incoming neutrino flux and on the total energy emitted in neutrinos by the source has been performed for the different neutrino flavors. The results using the LVC GWTC-2 catalog (covering O3a period) are presented, as well as the outlooks for the future real-time public release of follow-ups for the O4 period (in 2022) and beyond.

Abstract: We have searched for proton decay via p→e+π0 and p→μ+π0 modes with the enlarged fiducial volume data of Super-Kamiokande from April 1996 to May 2018, which corresponds to 450 kton·years exposure. We have accumulated about 25% more livetime and enlarged the fiducial volume of the Super-Kamiokande detector from 22.5 kton to 27.2 kton for this analysis, so that 144 kton·years of data, including 78 kton·years of additional fiducial volume data, has been newly analyzed. No candidates have been found for p→e+π0 and one candidate remains for p→μ+π0 in the conventional 22.5 kton fiducial volume and it is consistent with the atmospheric neutrino background prediction. We set lower limits on the partial lifetime for each of these modes: τ/B(p→e+π0)>2.4×1034 years and τ/B(p→μ+π0)>1.6×1034 years at 90% confidence level.

Abstract: Rarely, if ever, does a national healthcare system experience such rapid and marked change as that seen with the COVID-19 pandemic. In March 2020, the president of the United States declared a national health emergency, enabling the Department of Health & Human Services authority to grant temporary regulatory waivers to facilitate efficient care delivery in a variety of healthcare settings. The statutory requirement that Medicare beneficiaries stay three consecutive inpatient midnights to qualify for post-acute skilled nursing facility coverage is one such waiver. This so-called Three Midnight Rule, dating back to the 1960s as part of the Social Security Act, is being scrutinized more than half a century later given the rise in observation hospital stays. Despite the tragic emergency circumstances prompting waivers, the Centers for Medicare & Medicaid Services and Congress now have a unique opportunity to evaluate potential improvements revealed by COVID-19 regulatory relief and should consider permanent reform of the Three Midnight Rule. 31. VIRTUAL, ON-SITE AND HYBRID...HOW DO THE NEW MODELS OF CARDIAC REHAB IN NOVA SCOTIA COMPARE TO THE PRE-COVID PROGRAMS?

Abstract: BACKGROUND In Nova Scotia (NS) the arrival of COVID-19 led to immediate closures of all cardiac rehabilitation (CR) programs and redeployment of staff. Doors opened again two months later with CR teams needing to quickly develop and implement a virtual version of CR. Months following, with the reduction of COVID cases, programs were transitioned back to in-person on-site/hybrid models. Throughout transitions, the goal was to develop safe, effective and sustainable CR models of care with measured outcomes comparative to core elements of traditional on-site CR. METHODS AND RESULTS The first version of Virtual Cardiac Rehab (VCR 1) was an eight-week program for 99 patients with individual weekly case management calls from the CR team. Two On-Site CR (O-SCR) programs were later implemented;O-SCR1, an 8 week model with 5 patients/class (n=125) and O-SCR 2, a 12 week model with 9 patients per class (n=98). Due to the 2nd wave of COVID in winter 2020, all programming reverted back to virtual care with a new structured VCR2, a 12 week model (n=96) integrating nurses, dietitians and physiotherapists for individual case managed calls, behavior change counselling, Zoom for Healthcare group sessions and rigorous modeling to emulate the physical program as closely as possible, including exercise prescription. In all models; risk factors, medications and medical history were assessed and when possible blood work and exercise stress tests were completed, along with health behaviour change evaluation for activity and eating patterns. There were no adverse events in the virtual groups. 72% of patients completed the VCR1 model with positive changes in eating and activity patterns and effective medical management. 92% of patients completed VCR2 with improved outcome measures including physiologic measures (currently in analysis phase). The O-SCR1 demonstrated 80% attendance, but pandemic restricted stress testing and blood work resulted in most outcome reports as qualitative, based on questionnaires; showing a significant improvement in dietary fat, fibre, sodium and sugar intake patterns, along with 90% of patients reporting improvements in self-management of eating and exercise goals. With O-SCR2, greater stress and lab data availability demonstrates improved exercise tolerance (METS), lipid profiles and food scores. See Results in Table 1. CONCLUSION Keeping CR doors open virtually and on-site can be challenging during a pandemic but the NS models are feasible with measurable outcomes that are comparable to on-site CR when modeled as such. Further modelling continues with building safe, sustainable and effective programming options.

Abstract: Cognitive control and working memory processes have been found to be influenced by changes in motivational state. Nevertheless, the impact of different motivational variables on behavior and brain activity remains unclear.The current study examined the impact of incentive category by varying on a within-subjects basis whether performance during a working memory task was reinforced with either secondary (monetary) or primary (liquid) rewards. The temporal dynamics of motivation-cognition interactions were investigated by employing an experimental design that enabled isolation of sustained and transient effects. Performance was dramatically and equivalently enhanced in each incentive condition, whereas neural activity dynamics differed between incentive categories. The monetary reward condition was associated with a tonic activation increase in primarily right-lateralized cognitive control regions including anterior prefrontal cortex (PFC), dorsolateral PFC, and parietal cortex. In the liquid condition, the identical regions instead showed a shift in transient activation from a reactive control pattern (primary probe-based activation) during no-incentive trials to proactive control (primary cue-based activation) during rewarded trials. Additionally, liquid-specific tonic activation increases were found in subcortical regions (amygdala, dorsal striatum, nucleus accumbens), indicating an anatomical double dissociation in the locus of sustained activation.These different activation patterns suggest that primary and secondary rewards may produce similar behavioral changes through distinct neural mechanisms of reinforcement. Further, our results provide new evidence for the flexibility of cognitive control, in terms of the temporal dynamics of activation.

Abstract: Hospitalists and other providers must classify hospitalized patients as inpatient or outpatient, the latter of which includes all observation stays. These orders direct hospital billing and payment, as well as patient out-of-pocket expenses. The Centers for Medicare & Medicaid Services (CMS) audits hospital billing for Medicare beneficiaries, historically through the Recovery Audit program. A recent U.S. Government Accountability Office (GAO) report identified problems in the hospital appeals process of Recovery Audit program audits to which CMS proposed reforms. In the context of the GAO report and CMS's proposed improvements, we conducted a study to describe the time course and process of complex Medicare Part A audits and appeals reaching Level 3 of the 5-level appeals process as of May 1, 2016 at 3 academic medical centers. Of 219 appeals reaching Level 3, 135 had a decision--96 (71.1%) successful for the hospitals. Mean total time since date of service was 1663.3 days, which includes mean days between date of service and audit (560.4) and total days in appeals (891.3). Government contractors were responsible for 70.7% of total appeals time. Overall, government contractors and judges met legislative timeliness deadlines less than half the time (47.7%), with declining compliance at successive levels (discussion, 92.5%; Level 1, 85.4%; Level 2, 38.8%; Level 3, 0%). Most Level 1 and Level 2 decision letters (95.2%) cited time-based (24-hour) criteria for determining inpatient status, despite 70.3% of denied appeals meeting the 24-hour benchmark. These findings suggest that the Medicare appeals system merits process improvement beyond current proposed reforms. Journal of Hospital Medicine 2017;12:251-255.

They are notable for their large masses and charged particle detection capabilities. While current WC detectors reconstruct charged particle tracks over a wide energy range, they cannot efficiently detect neutrons. Gadolinium (Gd) has the largest thermal neutron capture cross section of all stable nuclei and produces an 8 MeV gamma cascade that can be detected with high efficiency. Because of the many new physics opportunities that neutron tagging with a Gd salt dissolved in water would open up, a large-scale R&D program called EGADS was established to demonstrate this technique’s feasibility. EGADS features all the components of a WC detector, chiefly a 200-ton stainless steel water tank furnished with 240 photo-detectors, DAQ, and a water system that removes all impurities from water while keeping Gd in solution. Abstract: We report the first measurement of the neutron cross section on argon in the energy range of 100-800 MeV. The measurement was obtained with a 4.3-hour exposure of the Mini-CAPTAIN detector to the WNR/LANSCE beam at LANL. The total cross section is measured from the attenuation coefficient of the neutron flux as it traverses the liquid argon volume. A set of 2,631 candidate interactions is divided in bins of the neutron kinetic energy calculated from time-of-flight measurements. These interactions are reconstructed with custom-made algorithms specifically designed for the data in a time projection chamber the size of the Mini-CAPTAIN detector. The energy averaged cross section is $0.91 \pm{} 0.10~\mathrm{(stat.)} \pm{} 0.09~\mathrm{(sys.)}~\mathrm{barns}$. Abstract: Observation stays are increasingly commonplace, yet no standard method to identify observation stays in Medicare claims is available, including events with status change. To determine the claims patterns of Medicare observation stays, define comprehensive claims-based methodology for future Medicare observation research and data reporting, and identify policy implications of such definition, we identified potential observation events in a 2014 20% random sample of Medicare beneficiaries with both Part A and B claims and at least one acute care stay (1,667,660 events). Observation revenue center (ORC) and Healthcare Common Procedure Coding System codes occurring within 30 days of an inpatient hospitalization were recorded. In total, 125,920 (7.6%) events had an ORC code, 75,502 (4.5%) in the outpatient revenue center. Claims patterns varied tremendously, and almost half (47.3%, 59,529) of ORC codes were associated with an inpatient claim, indicating status change, and demonstrating need for clarity in observation policy. The proposed University of Wisconsin method identified 72,858 of 75,502 (96.5%) events with ORC codes as observation stays, and provides a comprehensive, reproducible methodology.

Abstract: Neutral current (NC) interactions of atmospheric neutrinos on oxygen form one of the major backgrounds in the search for supernova relic neutrinos with water-based Cherenkov detectors. The NC channel is dominated by neutrino quasi-elastic (NCQE) scattering off nucleons inside $^{16}$O nuclei. In this paper we report the first measurement of NCQE cross section using atmospheric neutrinos at Super-Kamiokande (SK). The measurement used 2,778 live days of SK-IV data with a fiducial volume of 22.5 kiloton water. Within the visible energy window of 7.5-29.5 MeV, we observed $117$ events compared to the expected $71.9$ NCQE signal and $53.1$ background events. Weighted by the atmospheric neutrino spectrum from 160 MeV to 10 GeV, the flux averaged NCQE cross section is measured to be $(1.01\pm0.17(\text{stat.})^{+0.78}_{-0.30}(\text{sys.}))\times10^{-38}$ cm$^2$., Comment: 11 pages, 9 figures, submitted to PRD

Abstract: Far-infrared astronomy has advanced rapidly since its inception in the late 1950's, driven by a maturing technology base and an expanding community of researchers. This advancement has shown that observations at far-infrared wavelengths are important in nearly all areas of astrophysics, from the search for habitable planets and the origin of life, to the earliest stages of galaxy assembly in the first few hundred million years of cosmic history. The combination of a still developing portfolio of technologies, particularly in the field of detectors, and a widening ensemble of platforms within which these technologies can be deployed, means that far-infrared astronomy holds the potential for paradigm-shifting advances over the next decade. In this review, we examine current and future far-infrared observing platforms, including ground-based, sub-orbital, and space-based facilities, and discuss the technology development pathways that will enable and enhance these platforms to best address the challenges facing far-infrared astronomy in the 21st century., Invited review article, submitted to the Journal of Astronomical Telescopes, Instruments, and Systems. Abstract: Background— The aim of this study is to examine the relationship between time in the therapeutic range (TTR) and clinical outcomes in heart failure patients in sinus rhythm treated with warfarin. Methods and Results— We used data from the Warfarin versus Aspirin in Reduced Cardiac Ejection Fraction (WARCEF) trial to assess the relationship of TTR with the WARCEF primary outcome (ischemic stroke, intracerebral hemorrhage, or death), with death alone, ischemic stroke alone, major hemorrhage alone, and net clinical benefit (primary outcome and major hemorrhage combined). Multivariable Cox models were used to examine how the event risk changed with TTR and to compare the high TTR, low TTR, and aspirin-treated patients, with TTR being treated as a time-dependent covariate. A total of 2217 patients were included in the analyses; among whom 1067 were randomized to warfarin and 1150 were randomized to aspirin. The median (interquartile range) follow-up duration was 3.6 (2.0–5.0) years. Mean (±SD) age was 61±11.3 years, with 80% being men. The mean (±SD) TTR was 57% (±28.5%). Increasing TTR was significantly associated with reduction in primary outcome (adjusted P P =0.001), and improved net clinical benefit (adjusted P P =0.082 for ischemic stroke and adjusted P =0.109 for major hemorrhage). Conclusions— In patients with heart failure in sinus rhythm, increasing TTR is associated with better outcome and improved net clinical benefit. Patients in whom good quality anticoagulation can be achieved may benefit from the use of anticoagulants. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . Abstract: Outpatient versus inpatient status determinations for hospitalized patients impact how hospitals bill Medicare for hospital services. Medicare policies related to status determinations and the Recovery Audit Contractor (RAC) program charged with postpayment review of such determinations are of increasing concern to hospitals and physicians. We present an overview and discussion of these policies, including the recent 2-midnight rule, the effect on status determinations by the RAC program, and other recent and pertinent legislative and regulatory activity. Finally, we discuss the future direction of Medicare status determination policies and the RAC program, so that physicians and other healthcare providers caring for hospitalized Medicare beneficiaries may better understand these important and dynamic topics.

Abstract: We have searched for proton decays into a charged antilepton (e+, μ+) plus a meson (η, ρ0, ω) and for neutron decays into a charged antilepton (e+, μ+) plus a meson (π−, ρ−) using Super-Kamiokande I-IV data, corresponding to 0.316 megaton⋅years of exposure. This measurement updates the previous published result by using 2.26 times more data and improved analysis methods. No significant evidence for nucleon decay is observed and lower limits on the partial lifetime of the nucleon are obtained. The limits range from 3×1031 to 1×1034 years at 90% confidence level, depending on the decay mode.

Abstract: A search for boosted dark matter using 161.9 kt yr of Super-Kamiokande IV data is presented. We search for an excess of elastically scattered electrons above the atmospheric neutrino background, with a visible energy between 100 MeV and 1 TeV, pointing back to the Galactic center or the Sun. No such excess is observed. Limits on boosted dark matter event rates in multiple angular cones around the Galactic center and Sun are calculated. Limits are also calculated for a baseline model of boosted dark matter produced from cold dark matter annihilation or decay. This is the first experimental search for boosted dark matter from the Galactic center or the Sun interacting in a terrestrial detector.

Abstract: Using 5326 days of atmospheric neutrino data, a search for atmospheric tau neutrino appearance has been performed in the Super-Kamiokande experiment. Super-Kamiokande measures the tau normalization to be 1.47±0.32 under the assumption of normal neutrino hierarchy, relative to the expectation of unity with neutrino oscillation. The result excludes the hypothesis of no-tau appearance with a significance level of 4.6σ. The inclusive charged-current tau neutrino cross section averaged by the tau neutrino flux at Super-Kamiokande is measured to be (0.94±0.20)×10−38 cm2. The measurement is consistent with the Standard Model prediction, agreeing to within 1.5σ.

49. Selenomethionine and Total Methionine Ratio is Conserved in Seed Proteins of Selenium-Treated and Nontreated Soybean, Flax, and Potato

Abstract: Crop biofortification with Se is widely accepted. However, little is known about the efficiency of selenomethionine (Se-Met) incorporation into the seed and tuber protein fraction of crops. Here, we report on the efficiency of Se incor - poration into the seed and tuber protein fraction of soybean, flax, and potato and the putative identity of some of the proteins that are likely targets for this random pretranslation modifica - tion. Soybean, flax, and potato plants received Se using different sodium selenate application methods and rates, and Se-Met was deter- mined in seed, tuber, and their protein frac- tions. Selenium content in seed and tuber tis- sues and in protein fractions was increased by 2 to 86 times. The methionine (Met) pool was increased, as was the proportion of Se-Met in the Met pool, but the ratio of Se-Met:total Met was well conserved (1:1) between the treated and nontreated plants. After two-dimensional (2D) protein electrophoresis, five protein spots were identified as glycinin, trypsin inhibitor (soy - bean), patatin-3-Kuras-1, patain-B2, and phos- phoenol carboxy kinase (potato), none of which showed S to Se substitution in their primary structure. Linking Se and three stress markers in fresh tuber extracts, glutathione (GSH) level, glutathione peroxidase (GPx), and glutathione reductase (GR) activities were increased by 20 to 35%. The data showed low protein modifi - cation in these non-Se-accumulator crops and indicated that rates of 5 to 10 g ha -1 Se, through foliar application, may ensure not only an ade- quate daily intake levels for humans but may also activate antioxidant enzyme systems within the tuber tissue.
