Abstract

Mustafa Kemal Paşa'nın önderliğinde oluşturulan I. Meclis 1920?1923 döneminde görev yapmıştır. Kurucu meclis niteliğindeki I. Meclis'in aracılığıyla Türkiye'nin bağımsızlık savaşı yürütülmüş ve zafere ulaşması sağlanmıştır. Ancak I. Meclis'te inkılâpların gerçekleştirilmesi mümkün görülmediği için seçime gidilmiş ve 1923- 1927 tarihleri arasında görev yapan II. Meclis oluşturulmuştur. II. Meclis'in Cumhuriyeti ilan etmesi ve laikliğe temel teşkil eden yasaları onaylamasından dolayı Türk siyasî hayatında önemi büyüktür. Bu nedenle çalışmada, II. Meclis'te görev alan Bolu milletvekillerinin biyografilerinin ve meclisteki faaliyetlerinin incelenmesi esas alınmıştır. Bu çerçevede seçimler, milletvekillerinin meclisteki çeşitli konulardaki siyasi yaklaşımları araştırmamızın konusunu oluşturmaktadır.Bu çalışma, giriş ve üç ana bölümden oluşmaktadır. Giriş bölümünde öncelikle, Türklerde parlamenter sistemden bahsedilmiş daha sonra Osmanlı Dönemi'nde ortaya çıkan anayasal gelişmeler, I. ve II. Meşrutiyet dönemleri incelenmiştir. Birinci bölümde, II. Meclis'in açılışı ve Bolu'daki seçimlerde seçilen milletvekilleriyle, II. Meclis döneminde yapılan inkılâpların açıklamaları ve Bolu milletvekillerinin bu inkılâplar hakkındaki düşüncelerine yer verilmiştir. İkinci bölümü, Bolu'dan II. Meclis'e seçilen milletvekillerinden Falih Rıfkı Bey, Mehmet Şükrü Bey, Cevat Abbas Bey, Mehmet Vasfi Bey ve Emin Cemal Bey'in özgeçmişleri incelenmeye çalışılmıştır. Araştırmamızın üçüncü bölümü ise Bolu milletvekillerinin meclisteki faaliyetlerinden oluşmaktadır. First Assembly which was formed under the leadership of Mustafa Kemal served between 1920 and 1923. As being a constituent, First Assembly conducted Turkey?s National struggle of Independence and carried the nation to the victory. But, it was seen impossible to perform the revolutions by the first assembly. That?s why an election was held and ?Second Assembly? was formed which worked between 1923 and 1927. Because of the establishment of Turkish Republic and the acceptance of the laws about secularism, Second Assembly has a great importance on Turkish political life. Thus this study is dealt with investigation of the biographies of Bolu representatives and their activities in assembly. In this framework election, attributes of the representatives and their political activities in different subject in Assembly are the subjects of our investigation.This study consists of introduction and three main sections. In introduction section parliamentary system of the Turks, constitutional developments appealed in Ottoman period and constitutional eras are studied. Opening of the second Assembly, explanations of the revolutions held in the period of second Assembly, opinions of Bolu representatives about these revolutions are presented in the first section. Second section consists of the biographies of Falih Rıfkı Bey, Mehmet Şükrü Bey, Cevat Abbas Bey, Mehmet Vasfi Bey and Emin Cemal Bey who were the representatives of Bolu in Second Assembly. The third section is about the activities of Bolu representatives in Assembly. 268

Abstract

Bu tez çalışması kapsamında kağıt sanayiinde laminasyon tutkalı olarak kullanılabilecek yeni bir tutkalın üretilmesi ve üretilen tutkalın standart tutkallardan farkının incelenerek belirlenmesi amaçlanmıştır. Bu çalışmada monomer olarak kullanılan Vinilasetatın polimerizasyonu incelenmiştir. Polimerizasyonda monomer-su oranı, sıcaklık ve bazı diğer etmenler değiştirilerek denemeler yapılmıştır. Bu denemeler sonucunda Viskozite, pH ve kuru madde yüzdesi bakımından en uygun olan oran sonucunda Viskozitesi 6400 mPa.S, pH' ı 2,9 ve kuru maddesi % 34,93 olan laminasyon tutkalı elde edilmiştir. Reaksiyonda kullanılan tüm maddelerin polimerizasyona etkisinden söz edilmiş olup, oluşan yeni tutkalın ne gibi farklılıklarının olduğu belirtilmiştir., This thesis includes production of new glue instead of using lamination glue and explaination of difference between new glue and standard glues in paper industry. The thesis?s purpose is searching of vinylacetate polymerization as a using monomer. It has some experiment while changing rate of monomer and water, temperature and other variables. As a result of this experiments,we obtain new lamination glue which has 6400 mPa.s in viscocity, 2.9 in pH, %34.93 solubility in solution. The thesis mentions about effection of all material ,which are using in the reaction, to polymerization.and defination the difference of the new glue from the other standard glues.

26. Kemik ve yumuşak doku sarkomlarında ploidi ve S-fazı fraksiyonunun belirlemenin hasta izlemine katkısı

Abstract

ÖZET Sunulan çalışma, sarkomlarda tümör davranışını öngördürücü ve adjuvan kemoterapi kullanımında yol gösterici bir yöntem olarak, maliyn doku örneklerinde flow sitometri ile DNA ploidi ve S-fazı fraksiyonu belirlemenin değerini araştırmak amacıyla planlanmıştır. Bu amaçla 89 sarkomlu (44 YDS, 45 kemik sarkomu) hastanın parafin bloklarındaki tümör örneklerinde flow sitometri ile DNA ploidi ve S-fazı fraksiyonu belirlenmiştir. Tüm grupta % 48, yumuşak doku sarkomlarında %48, osteosarkomda % 64, kondrosarkomda %14, Ewing sarkomunda %50, maliyn dev hücreli tümörde %25 oranında anöploidi saptanmıştır. Ortalama S-Fazı fraksiyonu yumuşak doku sarkomunda %11.1, osteosarkomda %11.4, kondrosarkomda % 8.1, Ewing sarkomunda %6.2, maliyn dev hücreli tümörde % 9 olarak hesaplanmıştır. Bu bulguların hastaların çeşitli özellikleri ile ilişkisi araştırılmıştır. Tüm grupta anöploidi ile yüksek greyd ve tanıda metastaz varlığı arasında korelasyon saptanmıştır. Bu bulgu DNA ploidi belirlemenin, sarkomlarda tümör davranışını öngördürücü, objektif ve tekrarlanabilir bir yöntem olarak kullanılabileceği düşüncesini desteklemiştir.Tedavi yönünden homojen bir grup oluşturan, yanlızca cerrahi tedavi uygulanmış yumuşak doku sarkomlu hastalarda saptanan DNA ploidi-sağkalım ve S-fazı fraksiyonu-hastalıksız yaşam süresi ilişkisi de bu hasta grubunda flow sitometrik analizin prognoz belirlemedeki önemini ortaya koymuştur. Bulgularımız ilgili literatür ile desteklenebilir niteliktedir. Sarkomlarda DNA ploidi ve S-fazı fraksiyonu belirlemenin histopatolojik greydlemedeki soruman aşmaya yardımcı ve adjuvan kemoterapi seçiminde yol gösterici bir yöntem olabileceği sonucuna varılmıştır. 50

ÖZET Bu çalışma yüksek doz medroksiprogesteron asetafın (YD-MPA) sitotoksik kemoterapi alan hastalarda myeloprotektif etkisinin varlığını ve yan etkilerini araştırmak amacıyla planlanmıştır. I.kür sonrası nötropeni geliştiren 22 hastada 1 gr/gün oral dozlarda MPA'nın 2.kür boyunca kullanılması ile ortalama lökosit, en düşük lökosit, ortalama nötrofil ve en düşük nötrofil sayılarında anlamlı koruyucu etki gözlenmiştir, ilacın yaklaşık bir aylık sürede Hb ve Htc değerini, trombosit sayılarını, karaciğer ve böbrek fonksiyonlarını, kan biyokimyasını ve elektrolitleri, anlamlı derecede etkilemediği, diastolik ve sistolik kan basıncı ve açlık kan şekerinde hafif yükselmelere yol açtığı gösterilmiştir. Bir hastada derin ven trombozu dışında ciddi bir yan etki saptanmamıştır. Bu bulgularla, YD-MPA'nın, daha geniş hasta gruplarında ileri çalışmalar yapılması kaydıyla, sitotoksik kemoterapi alan hastalarda myeloprotektif amaçla, poliklinik şartlarında kullanılabilecek ucuz bir seçenek olduğu sonucuna varılmıştır. 23 28

Abstract

Peripheral neuropathy (PN) is a common toxicity of frequently used chemotherapy agents. Pain in these patients may sometimes be a difficult problem to manage. To palliate symptoms we administered venlafaxine in our patients suffering from chemotherapy induced peripheral neuropathy. 12 patients with cancer, 10 of whom had unremitting painful chemotherapy-induced peripheral neuropathy and 2 had been suffering from severe paresthesia, numbness, walking difficulty without pain, used 75 mg venlafaxine extended release capsules for 8 weeks. Symptoms and adverse effects were evaluated before and after treatment. Median VAS score declined to 0 at the 8th week from its baseline median value of 45. This decline in pain severity was significant (P≤0.001). Other symptoms did not improve. As a side effect, drowsiness increased from 0% of cases reported baseline to 75% at the 8th week (P=0.041). Two patients gave up due to the toxicity. Venlafaxine seems to effectively improve chemotherapy induced neuropathic pain with moderate manageable toxicity. This observation warrants further study for comparison with other antidepressants. [ABSTRACT FROM AUTHOR]

Abstract

The purpose of this study was to determine the activity of third-line chemotherapy for non small lung cancer (NSCLC). This retrospective study included patients who had failed after a second-line chemotherapy. A total of 16 patients treated with third line chemotherapy were evaluated. Only one patient had a partial response (6.2%), with an additional 4 patients (24.8%) with disease stabilisation. The median time to progression was 46 days and the median overall survival time was 132 days after the onset of third-line therapy. The overall survival (OAS) after the third-line chemotherapy was linked with the presence of objective response achieved by the first-line treatment (p=0.009). Likewise, the progression free survival (PFS) was related with the presence of clinical benefit (complete + partial response + stable disease) after the first-line chemotherapy (p=0.044). However, other factors evaluated as potential predictors of PFS, or OAS were not found to be of significance. As a conclusion; highly selected patients, who have objective response to the first-line therapy may be offered a third-line chemotherapy regimen, provided that they have a good performance status. 41. A Novel Method of Reporting c-erbB-2 Overexpression: Correlation with Grade But Not with Other Prognostic Parameters in Breast Cancer.

Abstract

Overexpression of erbB-2 protein is a marker of clinically aggressive behavior in breast cancer patients. In general erbB- 2 status is reported on a scale from negative to 3+, depending on staining intensity, if more than 10% of the tumor cells are stained. However, the original definition does not take into account the percentage of stained cells in detail, since all samples score in the same way if they are above the cut-off, established at 10%. In this study, we evaluated the erB-B2 overexpression by immunohistochemistry in 169 patients and evaluated any correlation with the other well known prognostic parameters. We also tested whether there was a difference between the original definition of c-erbB-2 positivity (2+ or 3+) and a novel system, the total c-erbB-2 score, which takes into account both the percentage, and intensity of staining as a continuous variable. The total c-erbB-2 score was correlated with the grade (P = 0.006) but not with other parameters. When 2+ and 3+ levels of staining together were regarded as positive, a similar correlation with grade was demonstrated. However, when only 3+ cases were considered, there was no significant correlation. In conclusion reporting c-erbB-2 as a continuous variable may make more sense than treating a cut-off value as positive. [ABSTRACT FROM AUTHOR]

42. Reevaluation of baseline staging tests in breast cancer; what should be the standard?

Abstract

There has been an ongoing debate on staging in breast cancer. In this study, we evaluated the metastasis detection rate with baseline staging of breast cancer after operation. One hundred consecutive breast cancer patients were evaluated with liver ultrasonography, chest X-ray and bone scans. All patients were followed at least for 6 months. More than one third of our patients belong to the high risk group (axillary involvement >3). In 3 patients (3%) metastasis were detected. Two of them (5,8%) were in the group with pT4, N2 or N1 (N >3), one of them (2,7%) was in the group with pT2-3N0-1 (N ≤3). There was no metastasis in patients with pT1N0-1 (N ≤3). To detect one case of occult metastasis 2000 ∈ were spent. As a result, routine staging does not have a major impact on individual adjuvant treatment planning. We recommend routine staging only for patients with pT4 or N1, (N >3) or N2 disease, and we do not recommend staging for patients with pT1N0-1, (N ≤3) positive nodes. Decision about staging of the patients with pT2-3N0-1, (N ≤3) diseases should be individualized according to patients' and doctors' preferences and economic resources. [ABSTRACT FROM AUTHOR]

Abstract

Hematopoietic growth factors (HGFs) have been used to reduce the neutropenic complications of cytotoxic chemotherapy so that higher doses may be given. The authors have previously shown that endogenous serum granulocyte-colony stimulating factor (G-CSF) and granulocyte-macrophage–colony stimulating factor (GM-CSF) levels at night (p.m.) were significantly higher than those in the morning (a.m.). Twenty-four patients with soft tissue or bone sarcoma who were treated with high dose ifosfamide-based chemotherapy were enrolled in this study. Patients were randomized to either a.m. or p.m. treatment. GM-CSF was administered at a dose of 5 μg/kg/day at 10 a.m. or 10 p.m., beginning 36–48 hours after the last chemotherapy dose. GM-CSF therapy was continued until the neutrophil count exceeded 1000/mm3 for 2 consecutive days. Leukocyte, neutrophil, monocyte, and platelet counts were measured immediately before GM-CSF administration and exactly 12 hours after the first dose of GM-CSF, and every 24 hours until 3 days after the cessation of GM-CSF. The mean duration of Grade 3–4 neutropenia was 5.3 ± 0.4 days for the a.m. treatment arm and 6.5 ± 0.3 days for the p.m. treatment arm (P = 0.017). Although the duration of neutropenia in the a.m. arm was significantly shorter than in the p.m. arm, there were no differences related to the number of febrile neutropenic episodes or the duration of antibiotic administration. Also, there were no differences in the side effects observed in the a.m. and p.m. arms. The finding of 1.2 days' difference in the duration of Grade 3–4 neutropenia warrants further study of chronotherapy with HGFs. Cancer 2000;88:2033–6. © 2000 American Cancer Society.

Abstract

The aim of our study was to assess the relationship between microsatellite instability (MSI) and the clinical outcome in metastatic colorectal cancer (CRC) patients treated with irinotecan-based regimens. We assessed best objective response, progression free survival (PFS) and overall survival (OAS) in relation to MSI analysis that was performed using BAT-25, BAT-26, D5S346, D2S123, D17S250 markers in normal and tumor DNA. The best objective response was significantly and negatively related with the D17S250 (an adjacent locus to p53) microsatellite marker (p=0.047). However, MSI score was not related with the best objective response (p=0.88). There was again no relationship between PFS, OAS and MSI score. In conclusion, this study allowed us to establish in a prospective study design that MSI status did not predict survival in metastatic colorectal cancer patients treated with irinotecan-based regimens. Abstract

Meme kanseri kadınlarda en sık görülen kanser türüdür ve kansere bağlı ölümlerde deikinci sırada yer almaktadır. Tedavi olmuş hastalar yıllar sonra metastastatik hastalık ileortaya çıkabilmektedir. Meme kanserinde adjuvan tedavi gerekliliği prognostik faktörlerdikkate alınarak yapılmaktadır. Meme kanserinde çok çeşitli prognostik faktörler vardır.Ancak bilinen standart prognostik faktörler aksiller lenf nodu durumu, tümör çapı, histolojikgrade, histolojik altgrup, ER ve PR statusudur.Bu çalışmada, meme kanseri tedavisinde biyolojik belirteçlerin prognostik öneminibelirlemek ve bu doğrultuda literatüre katkıda bulunmak amaçlanmıştır.Bu amaçla çalışmanın birinci grubuna 65 adjuvan tedavi alan hasta alındı. Mevcutbilgileri retrospektif olarak dosyalarından tarandı. Dosya bilgilerinden yaş, menopoz durumu,tanı tarihi, adjuvan kemoterapi çeşidi ve radyoterapi, tümör boyutu, grade, histolojik tanısı,aksiller lenf nodu tutulumu, nüks tarihi, son kontrol tarihi ve durumu kaydedildi. Bazıhastaların tümör çapı ve grade'i bulunamadı. Ayrıca grade 1 ve 3 tümörlerin sayısı azdı. Bunedenlerle grade ve tümör çapının sağkalıma katkısına bakılamadı. İmmünhistokimyasalolarak ER, PR, cerbB2, p53 ve ki67 bakıldı.Çalışmanın ikinci grubuna 16 metastatik meme kanserli hasta alındı. Tedaviye objektifyanıt ile p53, ki67, Pgp ve MRP-3 arasında ilişki araştırıdı. Ayrıca İHC parametrelerinkorelasyonuna bakıldı.Çalışmanın birinci grubunda; p53, menopoz, aksiller lenf nodu tutulumu bağımsızprognostik faktörler olarak saptandı. P53 en anlamlı pronostik faktör olarak bulundu.İkinci grubta; p53, ki67, Pgp ve MRP-3 ile metastatik meme kanserli hastalarda tedaviyeobjektif yanıt arasında anlamlı ilişki saptanmadı.Sonuç olarak, p53 önemli bir prognostik faktör olabilir ve çeşitli ilaçlarla ilişkisi adjuvantedavi alan hastalarda daha ayrıntılı çalışılmalıdır. 33

Abstract

Background: Various biomarkers have prognostic value in non-small cell lung cancer (NSCLC). We aimed to identify the roles of P53, c-erb- B2 and p-glycoprotein (pgp) as prognostic factors, independently or in conjunction with each other, in operable NSCLC., Material/methods: Seventy operable NSCLC cases were retrospectively evaluated for P53, c-erb-B2 and pgp expression patterns by immunohistochemistry. An unsupervised hierarchical cluster analysis of the 3 biomarkers was conducted. Univariate and multivariate survival analyses were made in relation to cluster affiliation., Results: Cluster analysis yielded two distinct subgroups; group A of high biomarker expressors (n=26, 37%), and group B (n=44, 63%) of low expressors. Cluster affiliation with regard to tumor histology (interaction term) was independently associated with Recurrence- free survival (RFS) and Overall survival (OAS) with a Hazard Ratio (HR) of 5.88, P=0.003, and HR=4.68, P=0.012, respectively. The median OAS times for cluster A and B in the squamous cell carcinoma subgroup were 328 and 596 days, whereas the corresponding figures in the non-squamous cell carcinoma subgroup were non-measurable and 298 days., Conclusions: In operable NSCLC there may be different relationships of P53, c-erb-B2 and pgp with patient outcome for different tumor histologies. The prognostic utility of cluster affiliation with regard to these biomarkers, and in relation to tumor histology, deserves further testing.
