Background: Colostomy formation as part of the Hartmann's procedure is often performed during emergency surgery as a damage limitation measure where attempts at bowel anastomosis and continuity are contraindicated. Hartmann's reversal (HR) remains challenging and can be attempted through open surgery and various minimally invasive techniques (laparoscopic and robotic platforms). We aimed to analyse outcomes of conventional multi-port laparoscopy (CL) versus single-incision approach (SILS) in patients undergoing HR. Methods: A comprehensive online search of various databases was conducted in accordance with PRISMA guidelines including Medline, PubMed, Embase, and Cochrane. Comparative studies of patients undergoing CL and SILS for HR were included. Analysed primary outcomes were total operative time and mortality rate. Secondary outcomes included post-operative complications, length of hospital stay, risk of visceral injury intra-operatively, and re-operation rate. Combined overall effect sizes were calculated using the random-effects model, and the Newcastle–Ottawa Scale (NOS) was used to assess bias. Results: Two observational studies matching our inclusion criteria with a total of 160 patients (SILS 100 vs. CL 60) were included. Statistical difference was observed for one outcome measure: operative duration (MD − 44.79 CI − 65.54– − 24.04, P < 0.0001). No significant difference was seen in mortality rate (OR 1.66 CI 0.17–16.39, P = 0.66), overall post-operative complications (OR 0.60 CI 0.28–1.32, P = 0.20), length of stay (MD − 0.22 CI − 4.25–3.82, P = 0.92), Clavien-Dindo III + complications (OR 0.61 CI 0.15–2.53, P = 0.50), risk of visceral injury (OR 1.59 CI 0.30–8.31, P = 0.58), and re-operation rates (OR 0.73 CI 0.08–6.76, P = 0.78). Conclusion: Accounting for study limitations, the SILS procedure seems to be quicker with non-inferior outcomes compared with the conventional multi-port approach. This may lead to better patient satisfaction and cosmesis and potentially reduce the risk of future incisional hernia occurrence. When the fighter's platform altitude is low or when the fighter radar detects downward-direction targets, the air-to-air target detection range of the radar is limited by ground clutter. To overcome the air-to-air target detection range limit caused by ground clutter, a high-pulse repetition frequency (HPRF) waveform that can detect air-to-air targets in clutter-free areas can be used. We present an HPRF waveform design that uses an M-of-N Pulse Repetition Frequency (PRF) schedule for long-range air-to-air target detection of a fighter radar. Laparoscopic fenestration is the preferred treatment for symptomatic splenic cysts because it is curative and spleen‐sparing. We report a case of a 25‐year‐old female who underwent laparoscopic fenestration for a giant splenic cyst using a single‐incision plus one‐port approach. She presented to our hospital with repeated vomiting. Imaging showed a 23 × 18 cm splenic cyst with no solid components, strongly compressing the stomach to the right side. Laparoscopic fenestration was initiated through a 2.5‐cm umbilical incision, and a 5‐mm port was added in the left abdomen intraoperatively. The drain placement was deemed necessary to prevent abscess formation and post‐operative bleeding because of a thick cyst component and unexpectedly thick wall. Splenic cysts are typically benign and commonly develop in young people, hence, organ preservation and cosmetic results are crucial. Laparoscopic fenestration using single‐incision plus one‐port is considered to be an appropriate procedure for giant splenic cysts with non‐serous contents. [ABSTRACT FROM AUTHOR]

Aim: Adnexal mass is one of the most common surgical indications in gynecology clinics, and laparoscopy is generally accepted as a gold standard in the management of adnexal mass.Single incision laparoscopic surgery (SILS) is an evolving endoscopic approach in the era of minimal access surgery. Recently, it has been reported that adnexal mass may also be managed by SILS. This study aimed to present huge adnexial masses managed by SILS. Materials and Methods: In this report, we present 3 symptomatic patients with a huge adnexal mass operated by a single port laparoscopic approach. Age, clinicodemographic characteristics, size of the adnexal masses, CA-125 levels, and the duration of the operations were recorded. Results: The mean duration of the operations 95 minutes (range 75-135 minutes). None of the patients converted to laparotomy. All patients discharged on postoperative day 1. None of the patients required readmission to the hospital. Conclusion: Postoperatively all patients were satisfied with their incision and cosmetic results. We propose a method to detect and counter deceptive jamming when an enemy fighter uses deceptive jamming against radar tracking the enemy fighter target. Our proposed countering method assesses the presence and type of deceptive jamming for the tracking target designated by the pilot, maintaining tracking of the target and preventing deceptive jamming. UCI Digital Scholarship Services and Cataloging & Metadata Services have been collaborating on a demonstration project to examine the possibilities of using bibliographic metadata in digital humanities research. his presentation provides an overview of our goals and workflows, and questions about how our system-wide bibliographic data may or may not be useful to researchers beyond record discovery, especially in the context of the Systemwide ILS Project (SILS).

In fall 2020, the Systemwide Integrated Library System (SILS) Shared Governance Task Force (SGTF) developed and secured endorsement of its proposed UC Libraries Advisory Structure (UCLAS)-integrated SILS organizational chart from both the SILS Working Group and Council of University Librarians (CoUL); this chart served as the basis for the SILS governance and operations model presented herein.

In order to prepare for a smooth transition from the phase 4 SILS governance and operations structure to the UCLAS-integrated SILS structure, a Transition Task Force is being charged by the SILS Working Group to oversee and coordinate the preparation plans and activities necessary to nominate, onboard, kickoff and support the new SILS cohort as they begin their work within the new governance structure. The new structure was designed by SGTF, approved by CoUL, and will be socialized to the broader UC Libraries staff community prior to this task force beginning work.

As part of its phase 4 charge, the Systemwide Integrated Library System (SILS) Working Group shared this mid-term report outlining work completed so far with the University of California (UC) Direction and Oversight Committee (DOC) and Council of University Librarians (CoUL) on April 23, 2021 for their review. This report covers work completed during the first 12 months of phase 4 from March 2020 through March 2021.

A consistent set of expectations about responsible and appropriate use of patron data is needed for SILS, in conjunction with information security and other compliance obligations. These expectations will underpin uniform policies and procedures and systemwide governance of local campus data for SILS.

CDL (California Digital Library) manages multiple kinds of data related to consortial licensing of electronic resources, and has anumber of shared services generating related workflows and data: shared acquisitions, sharedcataloging, SFX management, etc. The quantity and complexity of these workflows and data require aspecialized group of CDL staff and UC Libraries stakeholders to review and assess in order to bestmigrate current practices and data into the new Alma environment.

To investigate how to configure Regional Library Faciliteis (RLF) materials in the upcoming Shared Integrated Library System (SILS), an RLF Configuration Task Force (RCTF) has been charged by the SILS Working Group to carry out an analysis and make recommendations to the SILS Working Group.

Harmonization will involve aligning practices, procedures, policies and workflows where appropriate to best take advantage of the operational and service opportunities afforded through a SILS. The harmonization pilot process was created to test out possible harmonization processes: gathering experts, selecting topics, discussing potential areas of alignment, and coming to some agreement on next steps.

Systemwide Integrated Library System (SILS) shared governance is a model of practice in which the structure and environment for planning and decision-making about the implementation and ongoing management of the SILS is based on widespread partnership, accountability and ownership. Shared governance focuses on the process of decision-making as well as the outcomes. All staff in the UC Libraries have responsibility for helping to create an environment that supports participation in the shared governance process.

The systemwide Integrated Library System (SILS) working group has been charged with governance planning for phase 4 of the project and for planning ongoing, post implementation oversight of the SILS. As work on the current phase of the project (phase 3) has gotten underway, it has become clear that SILS governance planning should not be done in isolation of the current UC Libraries Advisory Structure (UCLAS). Accordingly, the SILS working group determined that a representative task force should be charged to focus on developing SILS shared governance.

The purpose of this Request for Proposal (the “RFP”) is to invite qualified suppliers to prepare and submit proposals to the University of California (“UC”) to provide a systemwide integrated library system (SILS) (the “Services”) all in accordance with Federal and State of California laws and the requirements of the UC as further detailed in this RFP.

The need for data cleanup activities across the UC Library system prior to implementation of a systemwide ILS (SILS) has come to the attention of the SILS Working group (WG). Proposed Co-chairs Aislinn Sotelo (UCSD) and Lynne Grigsby (UCB) suggest a process whereby representatives from across the UCs meet regularly to determine what data cleanup activities may need to be undertaken.

Answers to library employee questions about the UC Systemwide Integrated Library System.

The Systemwide Integrated Library System (SILS) working group will deliver potential SILS cost sharing models to the Council of University Librarians (CoUL) at the end of phase 3. In consultation with CoUL, the SILS working group has determined that a task force should first develop future-oriented SILS cost sharing principles. While this initial SILS Cost Sharing Task Force charge (i.e. part 1) will focus on principles, it is likely that the task force will subsequently be charged to define SILS cost sharing metrics and ultimately cost sharing models for the SILS working group and CoUL to review and endorse.

Abstract Background Silver ions have potent broad-spectrum antimicrobial activity and are widely incorporated into a variety of products to limit bacterial growth. In Enterobacteriaceae, decreased silver susceptibility has been mapped to two homologous operons; the chromosomally located cus operon and the plasmid based sil operon. Here we characterised the mechanisms and clinical impact of induced silver tolerance in Klebsiella pneumoniae. Results In K. pneumoniae carriage of the sil operon alone does not give elevated silver tolerance. However, when exposed to increasing concentrations of silver nitrate (AgNO3), K. pneumoniae strains which contain the sil operon, will preferentially mutate SilS, resulting in overexpression of the genes encoding the RND efflux pump silCBA. Those strains which do not carry the sil operon also adapt upon exposure to increasing silver concentrations through mutations in another two-component regulator CusS. Secondary mutations leading to disruption of the outer membrane porin OmpC were also detected. Both routes result in a high level of silver tolerance with MIC’s of >512 mg/L. When exposed to a high concentration of AgNO3 (400 mg/L), only strains that contained the sil operon were able to survive, again through mutations in SilS. The AgNO3 adapted strains were also resistant to killing by challenge with several clinical and commercial silver containing dressings. Conclusions This study shows that K. pneumoniae has two possible pathways for development of increased silver tolerance but that the sil operon is preferentially mutated. This operon is essential when K. pneumoniae is exposed to high concentrations of silver. The potential clinical impact on wound management is shown by the increased survivability of these adapted strains when exposed to several silver impregnated dressings. This would make infections with these strains more difficult to treat and further limits our therapeutic options.

The incorporation of the broadest possible university library collection into the Systemwide Integrated Library System (SILS) ensures more comprehensive discovery and access for UC users, facilitates the development of campus and systemwide services, and reduces UC’s overall software licensing and systems administration costs. There are a number of affiliated library collections in the University of California (UC) system, such as professional school and subject libraries. Whatever your definition of an “affiliated library collection” (for example, by budget line, reporting structure, or systems architecture), for the purposes of this document we are using the term to refer to any collection that might conceivably be included in the SILS.

The Systemwide ILS Planning Project Working Group shared this report with the University of California (UC) Direction and Oversight Committee (DOC), first on April 26th and then again on May 23rd, for their review and feedback. Once finalized, it was submitted to the UC Council of University Librarians (CoUL) on June 01st, 2018 for their review and endorsement. CoUL endorsed the phase 2 report and recommendations on June 15th, 2018.

Assessment of functional health literacy (FHL) is not integrated into clinical settings in Jordan possibly because relevant psychometric studies are lacking. The aim of this secondary analysis of data on family caregivers (N = 115) was to evaluate the internal consistency reliability and construct validity of three measures of FHL among Jordanians: the Short Test of Functional Health Literacy in Adults (S-TOFHLA), the Rapid Estimate of Adult Literacy in Medicine-Revised (REALM-R), and the Single Item Literacy Screener (SILS). Cronbach's alpha was excellent for the S-TOFHLA (.92), but suboptimal for the REALM-R (.67). In bivariate analysis, FHL measured by the S-TOFHLA and the REALM-R, but not the SILS, was positively associated with educational attainment (p <.05) and negatively related to self-reported anxiety (p <.05). Among Jordanians, psychometric rigor was fully demonstrated for the S-TOFHLA and partially for the REALM-R, but not for the SILS. [ABSTRACT FROM AUTHOR]

Purpose: Laparoscopic appendectomy for pediatric acute appendicitis is commonly performed by pediatric surgeons. A single incision approach has been investigated, but the lack of technical uniformity has resulted in conflicting data. We hypothesized that an initial single incision laparoscopic-assisted extracorporeal appendectomy (SILEA) approach is associated with similar complication rates as compared to the standard three-incision laparoscopic appendectomy (TILA). Methods: Approximately 1300 laparoscopic appendectomies were retrospectively reviewed for acute appendicitis over a 5 year period. Patients were split into TILA or SILEA cohorts. Propensity score matching identified 102 matched cases in both cohorts. Case and control cohort comparisons were then analyzed. Results: Successful SILEA was associated with no difference in post-operative complication rate. SILEA was associated with decreased postoperative narcotic dosage and shorter operative time than TILA. Conclusions: An initial SILEA approach is safe and associated with similar complication rates as TILA. Based on this data, the authors advocate adoption of the an initial SILEA approach for uncomplicated, freely mobile, acute appendicitis with seamless conversion to TILA if the appendix is not amendable to SILEA. Background: SILS (single incision laparoscopic surgery) and NOTES (natural orifice transluminal endoscopic surgery) are considered breakthroughs in minimally invasive surgery, the first consisting in the surgeon working via a single entrance site and the second via a natural orifice (e.g., oral cavity). Methods: Since 2000 until 2022, the original articles published in the online databases were analyzed. Eligible studies included information about the current therapy of patients with liver surgical pathology and how the two new techniques improve the surgical approach. Results: A total of 798 studies were identified. By applying the exclusion criteria, nine studies remained to be included in the review. Two out of nine studies examined the NOTES approach in liver surgery, whereas the other seven focused on the SILS technique. The age of the patients ranged between 24 and 83 years. Liver resections for hepatocellular carcinoma or colorectal metastases were undertaken and biliary or hydatid cysts were removed. The mean procedure time was 95 to 205 min and the average diameter of the lesions was 5 cm. Conclusions: When practiced by multidisciplinary teams, transvaginal liver resection is feasible and safe. The goals of SILS and NOTES are to be less intrusive, more easily tolerated and aesthetic. [ABSTRACT FROM AUTHOR]

At its annual planning retreat in June 2017, the University of California Council of University Librarians (CoUL) discussed the UC Libraries Direction and Oversight Committee (DOC) report on the Future of Resource Sharing (June 9, 2017) and the report of the Shared Regional Library Facility ILS Six Month Exploration Period (June 12, 2017). Together these reports made a compelling case for a more thorough exploration of a systemwide or consortial Integrated Library System (ILS) for the UC Libraries.

Abstract Background The arcuate line is the inferior margin of the posterior layer of rectus abdominis sheath. An arcuate line hernia is a parietal interstitial hernia consisting of ascending protrusion of intraperitoneal contents above the arcuate line. Arcuate line hernias are rare, and fewer than 20 cases undergoing surgical repair have been reported. Various surgical approaches were used in previous cases, and there is no consensus regarding the ideal repair method. We report the first case of an arcuate line hernia repaired using single-incision laparoscopic surgery. Case presentation The patient was a 78-year-old man who presented with a history of intermittent lower abdominal quadrant pain of more than 2 month’s duration. He had not previously undergone abdominal surgery, but had a history of mycobacterial lung disease and asthma. His vital signs were normal on presentation, and he experienced no vomiting or nausea. On palpation, his abdomen was flat and soft, and no mass was palpable. However, there was slight tenderness in the right lower quadrant. Blood laboratory test results were within normal ranges. Computed tomography revealed small bowel protrusion between the rectus abdominis and the posterior rectus sheath, and an arcuate line hernia was suspected and subsequently confirmed intraoperatively. The patient underwent single-incision laparoscopic repair with the intraperitoneal onlay mesh technique with tacks and with care to avoid the inferior epigastric vessels. The operation time was 30 min, and no intra- or post-operative complications occurred. Surgery relieved his symptoms, with no recurrence within 1 year postoperatively. Conclusions Single-incision laparoscopic surgery was performed easily and successfully in this rare patient with arcuate line hernia. Arcuate line hernia should be considered in patients presenting with abdominal symptoms, and single-incision laparoscopic repair should be considered for repair.
