This paper is about treason and anxiety in a small country. In particular, it examines how anxiety of treason as a core political affect and as a form of collective sentimentality has been shaping Georgia's political culture, especially since independence. I approach treason first and foremost as a category embedded in the collective memory narratives to demonstrate how it has been used to frame political processes. At the same time, I treat controversies involving accusations of treason as ramifications of collective anxiety over the notions of peoplehood and sovereignty in a small country. The aim of this study is to explore the current practice in Cyprus regarding the introduction and reimbursement of innovative pharmaceuticals through Managed Entry Agreements (MEA), assess its operational context, and suggest approaches toward spanning the knowledge gap consequential to these efforts, especially the barriers of a small country context. The recent introduction of a National Health System (NHS), brought about fundamental reforms in Cyprus' Healthcare sector. Among such reforms, of particular interest, has been the introduction of a Managed Entry Agreements (MEA) mechanism. The first preliminary results indicate that despite being a small and unattractive market, Cyprus can apply a substantial MEA program. Concomitantly, it annotates the need to design an operational framework which should include, the definition of important technical parameters, clear demarcation of the scope, cooperation principles ensuring the effective operation of scientific committees, and clear delineation of what 'value' is. Moreover, in the context of the unified healthcare market, budget transfers should be considered, which could alleviate the inordinate budget impact of new products, which nevertheless will cut down on hospital expenditures. Narrative synthesis and health policy analysis-related resources were used. The implementation of MEA in Cyprus provides an ideal testing ground for innovative reimbursement approaches. This will streamline the country's efforts toward reimbursement of innovation, while concomitantly add to the collective MEA experience. [ABSTRACT FROM AUTHOR]

• Little is known of epilepsy care in small economically developed countries (pop:500 k) • This study is on clinical & risk features of adults with epilepsy (68) in Gozo, Malta. • 92% were compliant with their anti-seizure medication (ASM) with most on just one ASM. • 19% were on antipsychotics, 18% had an epilepsy care plan & none had SUDEP advice. • Stigma & shame play a significant role in small communities & access to care. People with epilepsy are at increased risk of multiple co-morbidities that may influence risk of adverse outcomes including impact on quality of life and premature mortality. These risk factors include potentially modifiable clinical characteristics associated with sudden unexpected death in epilepsy (SUDEP). For services to tackle risk, the clinical complexity of the target epilepsy population needs to be defined. While this has been comprehensively studied in large, economically developed countries little knowledge of these issues exist in small economically developed countries, like Malta (population: 500,000). This was a single centre study focused exclusively on patients attending Gozo General Hospital (GGH) Malta. STROBE guidance for reporting cross sectional studies was used to design and report the study. This was a retrospective review of standard care and SUDEP and seizure risks provided to all adults (over 18 years) with epilepsy attending GGH (2018–2021). The review identified 68 people and 92% were compliant with their anti-seizure medication. A fifth (21%) had an intellectual disability. Despite only one patient having a psychotic illness, 19% were on antipsychotic medication. Only 18% of patients had a specific epilepsy care plan, 6% nocturnal surveillance and none had received advice on SUDEP. Patient outcomes may be improved with increasing rates of personalized epilepsy care plans, appropriate nocturnal surveillance and reducing the prescription of antipsychotic medication as it is associated with greater risk of mortality. The author has developed new criteria for the inclusion of African countries in the group of small countries. It is noted that the criteria for assignment to small countries in the developed world cannot fully meet the criteria applicable to the African continent. On the basis of a comparison of the population of Africa with some countries of Europe, small states of the African continent are singled out. Emphasize the fact that a single criterion for attributing to small countries has not yet been developed. In the author’s opinion, it is incorrect to evaluate African countries using only economic indicators, since economic growth rates do not always coincide with demographic indicators, sociocultural transformations, and ethnic and tribal characteristics in Africa. The main types of foreign economic strategies of small African countries are identified and considered. It is shown that the states of the African continent often use several strategies for the development of their countries. The small countries of the African continent are actively involved in integration processes that fit into the overall concept of African modernization and development. An important role in the processes of modernization in Africa is played by international financial, educational, innovative and technical programs. Often, it is precisely small countries that become a kind of transport hub at the regional level, which contributes to the strengthening of foreign economic relations between the countries of the regions of the African continent, attracts considerable financial, resource, and innovative capital to African countries. The historical and ethno-tribal characteristics of some African states, especially the long and bloody wars, hinder the progressive development of these countries. A vivid example of such tragic events is Rwanda (attributed to the small states by the author of the article), where incessant civil wars have undermined the economic and political stability in the region. In such cases, substantial international assistance will be required with the involvement of Rwanda’s neighbouring countries for the implementation of possible major regional programs and investment projects. Since innovative infrastructure projects in African countries are possible only with the involvement of all countries of the region, in this regard, Russia will need new management approaches and interesting projects for the integrated development of the African continent to actively participate in ambitious African projects.

The majority of research into Managed Clinical Networks (MCN) has been undertaken in large countries; little is known about the implementation of MCNs in small countries. Using a prospective, longitudinal, qualitative method, this three-year case study explored the development and implementation of a New Zealand palliative care MCN, particularly focusing on the establishment, mid-point, and completion time points. The analysis showed throughout the three years, the MCN failed to implement the guiding principles of MCNs. This may have been influenced by the small county context where clinicians and health provider organizations know each other well and compete for government-allocated resources and other funding (e.g. grants and donations). MCNs remain widely supported as a tool to achieve clinical representation in governance, particularly where change is needed to support equitable service delivery. However, it is possible in small country settings that MCNs require a different approach to leadership and development to achieve effective governance and implementation. [ABSTRACT FROM AUTHOR]

In 1951, Paul Samuelson showed that a surprisingly rich class of economies the production possibility frontier is a linear and even with neoclassical substitution possibilities, as outputs adjust, firms continue to use inputs and outputs in the same proportions. This model is often thought to be of limited practical value since it assumes that there is only one non-produced factor, and no joint production. The single factor assumption rules out economies in which agriculture and mining are important and the lack of joint production leaves no good way to handle durable capital goods. In this paper, I extend the Samuelson model to small trading economies with many nontradeable goods, some of which may be fixed factors, and where joint production is allowed. In my model, althought the production possibility frontier may be curved, the consumption possibility frontier after trade is linear. Output prices and wages of labor are determined independently of factor endowments or demand. The prices of other non-traded factors may depend on endowments but are independent of demand.

Normally, economists take the size of countries as an exogenous variable. Nevertheless, the borders of countries and their size change, partially in response to economic factors such as the pattern of international trade. Conversely, the size of countries influences their economic performance and their preferences for international economic policies – for instance smaller countries have a greater stake in maintaining free trade. In this paper, we review the theory and evidence concerning a growing body of research that considers both the impact of market size on growth and the endogenous determination of country size. We argue that our understanding of economic performance and of the history of international economic integration can be greatly improved by bringing the issue of country size at the forefront of the analysis of growth.
