Psychoanalysts and nonpsychoanalytic clinicians generally have a different view of the nature of symptoms. This contribution considers some aspects of maintaining a psychoanalytic perspective with regard to symptoms in the contemporary environment. [ABSTRACT FROM AUTHOR]

Presentación del caso: mujer de 73 años, independiente, con antecedentes médicos de psicopatología en tratamiento y no adherente. Tras una caída, sufre fractura de antebrazo y, en dicho contexto, se sospechó de un abuso de psicofármacos y de antiinflamatorios no esteroideos. Estudio y evaluación del caso: el estado de situación de la paciente se observó un abuso del tratamiento hipnótico-ansiolítico que puede estar relacionado con su reciente caída y fractura de antebrazo. Su mala adherencia terapéutica conllevó a un mal control de sus problemas de salud y a la aparición de posibles Resultados Negativos Asociados a la Medicación (RNM) que empeorarían su estado de salud. Intervención: se derivó a su médico de atención primaria con un informe de interconsulta farmacéutico, indicando los posibles RNM y alternativas terapéuticas en el tratamiento antihipertensivo y antidepresivo. Resultados: el médico de Atención Primaria relacionó la caída y subsecuente fractura del antebrazo, así como la descompensación de su patología basal, con el mal cumplimiento terapéutico. Se aceptó la propuesta de intervención farmacéutica, monitorizando la presión arterial para su correspondiente reajuste posológico. Se retiró el bupropión, se comenzó una desprescripción gradual de las dosis de benzodiazepinas y se sustituyó el antiinflamatorio por analgésicos opioides. Consecuentemente, la paciente mejoró significativamente su salud cardiovascular y aumentó su autonomía personal y autocuidado. Conclusiones: la comunicación multidisciplinar entre los profesionales sanitarios permite un mejor control de la enfermedad cardiovascular y una optimización de la farmacoterapia, lo que contribuye a mejorar su estado de salud.

Metal screw extrusion (MSE) is a continuous, solid-state forming method utilizing an inherently high degree of deformation to consolidate fragmented input materials into a solid bulk by breaking their oxide skins. Severe plastic deformation with equivalent strain in the range of 10–20 can be achieved depending on set process parameters. Rigorous mixing can be employed to form sophisticated materials like bulk composites, nanocomposites, particle-reinforced metals, and fine-grained materials. Furthermore, the inherent solid-state processing is well suited for recovery of difficult-to-recycle materials. A range of non-ferrous materials has been manufactured by MSE and further characterized in terms of microstructural evolution and mechanical and functional properties. Furthermore, MSE has been studied in terms of flow, accumulated strain, and environmental impact. The following review aims to critically highlight the existing work performed on MSE, compare it to existing and emerging technologies as well as explore future development and possible applications. MSE has the potential to be utilized for numerous commercial applications. A field experiment was conducted to study the effect of irrigation intervals and salicylic acid on root and seed production of carrot (Daucus carota L.) during Rabi seasons 2021-22 and 2022-23 at Horticulture Farm, S.K.N. College of Agriculture, Jobner, Rajasthan, India. The experiment consisted of twenty treatment combinations, including four levels of irrigation intervals and five foliar sprays of salicylic acid. The experiment was conducted in a strip plot design (SPD) and replicated thrice. Results revealed that application of irrigation with an interval of 6-day along with a double foliar spray of SA @ 75 ppm at 25 and 50 DAS significantly increased the quality attributes i.e., nitrogen (0.923 and 0.958%), phosphorus(0.289 and 0.290%), potassium (0.354 and 0.356%), TSS (9.00 and 9.20%) and β-carotene (6.70 and 7.14 mg/100 g) content of carrot, respectively, in comparison to the rest of the treatments except irrigation at 4-day and 8 day intervals in pooled analysis, which were statistically at par to each other. This article delves into the transnational aspects of the "Two Cultures" debate initiated by the British chemist and writer C. P. Snow, and explores how Italian and West German intellectuals localized and translated aspects of the debate within their respective political landscapes. Snow described the relationship between science and the humanities, and attributed a unique social responsibility to science. Prominent leftist thinkers, including Gino Martinoli, Adriano Buzzati Traverso, Aldo Visalberghi, Giulio Preti, Pier Paolo Pasolini, Karl Steinbuch, Hans Mohr, Hilde Domin, Jürgen Habermas, and Robert Jungk, engaged Snow's ideas, each formulating their stance on the role of science. These intellectuals were divided in their response. Some concurred with Snow, viewing scientific advancement as a cornerstone of social progress and considering the scientific ethos as a model for political emulation. Others, however, were critical, questioning the very notions of scientific progress, rationality, and modernization. This intellectual discourse foreshadowed the New Left's critique of scientism in the 1970s, a movement that significantly challenged the longstanding marriage between socialism and science. Al-Mg alloys are widely used as important engineering structural materials in aerospace engineering, transportation systems, and structural constructions due to their low density, high specific strength, corrosion resistance, welding capability, fatigue strength, and cost-effectiveness. However, the conventional Al-Mg alloys can no longer fully satisfy the demands of practical production due to difficulties caused by many defects. The high strength of Al-Mg alloys as non-heat treatment precipitation-strengthened alloys is achieved primarily by solid solution strengthening along with work hardening rather than precipitation strengthening. Therefore, severe plastic deformation (SPD) techniques can be often used to produce ultrafine-grained structures to fabricate ultra-high strength aluminum alloys. However, this approach often achieves the strengthening of material at the cost of reduced ductility. This paper comprehensively summarizes the various approaches of ultrafine/nanocrystalline materials for enhancing their plasticity, elaborates on the creation of a bimodal microstructure within the alloy, and discusses the formation of a nanotwin microstructure within the alloy and the incorporation of dispersed nanoparticles. The mechanisms underlying both the strengthening and toughening during large plastic deformation in aluminum alloys are summarized, and the future research direction of high-performance ultrafine crystalline and nanocrystalline Al-Mg aluminum alloys is prospected. This paper presents a continuous severe plastic deformation (SPD) process for sheet metal called Dual Rolls Equal Channel Extrusion (DRECE) is introduced. The DRECE process achieves maximum sheet metal deformation by combining bending deformation with channel-angular shear deformation. The evolution of the microstructure and mechanical properties of the CuZn37 alloy has been experimentally investigated as a function of the number of repeated DRECE passes. The DRECE process was repeated up to four times at room temperature. As the number of passes increased, the character of the microstructure was gradually strongly deformed by intensive shear deformation. Purpose: To review outcomes of spinopelvic dissociation treated with open lumbopelvic fixation. Methods: We reviewed all cases of spinopelvic dissociation treated at three Level-I trauma centers with open lumbopelvic fixation, including those with adjunctive percutaneous fixation. We collected demographic data, associated injuries, pre- and postoperative neurologic status, pre- and postoperative kyphosis, and Roy-Camille classification. Outcomes included presence of union, reoperation rates, and complications involving hardware or wound. Results: From an initial cohort of 260 patients with spinopelvic dissociation, forty patients fulfilled inclusion criteria with a median follow-up of 351 days. Ten patients (25%) had a combination of percutaneous iliosacral and open lumbopelvic repair. Average pre- and postoperative kyphosis was 30 degrees and 26 degrees, respectively. Twenty patients (50%) had neurologic deficit preoperatively, and eight (20%) were unknown or unable to be assessed. All patients presenting with bowel or bladder dysfunction (n = 12) underwent laminectomy at time of surgery, with 3 patients (25%) having continued dysfunction at final follow-up. Surgical site infection occurred in four cases (10%) and wound complications in two (5%). All cases (100%) went on to union and five patients (13%) required hardware removal. Conclusion: Open lumbopelvic fixation resulted in a high union rate in the treatment of spinopelvic dissociation. Approximately 1 in 6 patients had a wound complication, the majority of which were surgical site infections. Bowel and bladder dysfunction at presentation were common with the majority of cases resolving by final follow-up when spinopelvic dissociation had been treated with decompression and stable fixation. In this study, the invariant spectra and nuclear modification factors of direct photons produced in deuteron collisions at energies sqrt{s_{NN}} = 13.5 and 27.0 GeV/nucleon, relevant to the NICA project have been obtained by calculation. In doing this, both nuclear modified parton distribution functions and parton distribution ones found for free nucleons were used. Our analysis indicated that the predicted spectra of direct photons as a function of the transverse momentum, as well as the nuclear modification factors in these interactions were scarcely affected by accounting for the nuclear parton distribution function. It also turned out that the nuclear modification factors for direct photons generated in deuteron collisions at the specified energies depended slightly on the transverse momentum within the range from 1 to 5 GeV, differed from unity, exhibiting values around 0.8.

In recent years the ‘cost of governing’ has significantly increased for some mainstream political parties. In a context of financial uncertainty, multiple crises and growing constraints exerted by global forces, being a ‘natural’ party of government is no longer regarded as an electoral advantage. This is particularly true for parties that have moved from a position of clear dominance within ruling coalitions to a more subordinate role. In this article, focusing on the Social Democratic Party of Germany (SPD) and using an original dataset, we aim to provide a more nuanced assessment of the effects of incumbency by examining regional electoral performance since 1990. It appears that sub-national incumbency can be beneficial in regional elections, especially when a party faces significant costs of governing at the national level. However, this advantage is only applicable if the party holds a leading position in the regional executive. On the contrary, being a junior coalition partner at both national and regional levels may further exacerbate electoral decline for the party.

The detailed, high-resolution images captured by drones pose challenges to target detection algorithms with complex scenes and small-sized targets. Moreover, targets in unmanned aerial vehicle images are usually affected by factors such as viewing perspective, occlusion, and light, which increase the difficulty of target detection. In response to the above issues, we propose an improved SDMSEAFYOLOv8 for target detection based on YOLOv8, combined with a Bidirectional Feature Pyramid Network, to improve the sensing ability of the model for multiscale targets. A Space-to-depth layer replaces the traditional strided convolution layer to enhance the extraction of fine-grained information and small-sized target features. A Multi-Separated and Enhancement Attention module enhances the feature learning ability of the occluded target region, thus reducing missed and false detections. Four detection heads are employed for tiny target detection, each responsible for different size ranges, so as to improve the accuracy and robustness of small target detection. The conventional non-maximum suppression algorithm is improved, so as to reduce the problem of missed detections under a densely occluded scene by setting the attenuation function to adjust the confidence of the treated box based on the overlap between it and the highest-scoring box. Experiments demonstrate that the accuracy of SDMSEAF-YOLOv8 exceeds that of state-of-the-art models on the VisDrone2019-DET-val dataset, with a mAP of 42.9% at 640-pixel resolution, 14.8% over the baseline YOLOv8-x algorithm model, and 6.0% over the known state-of-the-art Fine-Grained Target Focusing Network model and with twice as fast detection. Surface defect detection of strip steel is an important guarantee for improving the production quality of strip steel. However, due to the diverse types, scales, and texture structures of surface defects on strip steel, as well as the irregular distribution of defects, it is difficult to achieve rapid and accurate detection of strip steel surface defects with existing methods. This article proposes a real-time and high-precision surface defect detection algorithm for strip steel based on YOLOv7. Firstly, Partial Conv is used to replace the conventional convolution blocks of the backbone network to reduce the size of the network model and improve the speed of detection; Secondly, The CA attention mechanism module is added to the ELAN module to enhance the ability of the network to extract detect features and improve the effectiveness of detect detection in complex environments; Finally, The SPD convolution module is introduced at the output end to improve the detection performance of small targets with surface defects on steel. The experimental results on the NEU-DET dataset indicate that the mean average accuracy (mAP@IoU = 0.5) is 80.4%, which is 4.0% higher than the baseline network. The number of parameters is reduced by 8.9%, and the computational load is reduced by 21.9% (GFLOPs). UAV aerial photos tend to have complicated backgrounds and dense targets that vary in size. Applying existing object detection algorithms to such images is often inaccurate and prone to misdetection and omission. To better improve the detection performance of UAV aerial photography, we proposed an improved small-target detection algorithm based on YOLOv5s: 1) We reconstructed the feature fusion network by introducing an upsampling layer, increasing the model’s focus on features from small targets and improving related detection accuracy. 2) We introduced the SPD convolutional building block to downsample the feature map without losing learning information, improving the model’s feature extraction ability. 3) We replaced the CIoU Loss function of the original model with EIoU to reduce the location loss during training and improve the regression accuracy. We experimented with the improved algorithm on the VisDrone2019 dataset and achieved mAP@0.5 of 44%, demonstrating a 10.7% improvement from the original model. The detection speed also increases to 99 FPS, indicating that the improved algorithm can maintain its real-time performance while improving its accuracy.

Los Sistemas Personalizados de Dosificación (SPD) son una herramienta eficaz, segura y homologada para el reacondicionamiento de fármacos en farmacia comunitaria. Estos implican la revisión del uso del medicamento (RUM) y la colaboración con el médico de atención primaria (MAP). En el presente artículo se describe el caso de una paciente de 57 años intervenida de lumbociatalgia en 2021 y 2022, con dolor crónico mal controlado y aturdimiento derivado del olvido y/o duplicidad de las tomas. Durante la dispensación habitual se detectan estos problemas relacionados con el medicamento (PRM) y se deriva a la paciente al servicio SPD. Tras la implantación del mismo, la paciente mejora a nivel cognitivo, eliminando el aturdimiento y controlando el dolor, lo que supuso un aumento en su calidad de vida. En conclusión, se destaca la importancia de los diferentes servicios disponibles en la farmacia para mejorar la calidad de vida del paciente, la adherencia al tratamiento y la detección de PRM.

Introduction: Platelet storage pool diseases (SPD) are a heterogeneous group of bleeding disorders associated with defects in the storage, secretion, or release of platelet granules. Patients with SPD present with a life-long mucocutaneous bleeding diathesis. Our goal was to study the clinical, laboratory, and ultrastructural changes in platelets of patients diagnosed with SPD using a transmission electron microscope (TEM). Methods: In this retrospective, cross-sectional cohort study, medical records of all patients referred for evaluation of a platelet function disorder were screened for an underlying diagnosis of SPD during the period 2010–2020. Results: Sixty-eight patients were identified, among whom 62 (91.2%) had a platelet function assay (PFA) study, of whom 21 (33.9%) were abnormal. Clinical, laboratory, and light transmission aggregometry (LTA) suggested that 10 (14.7%) patients had SPD; five had gray platelet syndrome (GPS), three had Hermansky Pudlak syndrome (HPS), and two had Chedia-Higashi syndrome (CHS). Most of these cases presented with mucocutaneous bleeding diathesis, but a few had oculocutaneous albinism. They were associated with variable abnormalities in the PFA and LTA studies. However, EM studies using TEM showed a reduction/absence of alpha or delta granules in GPS and HPS patients, respectively, but no abnormality in the granules of CHS patients. Conclusions: Although patients with SPD presented with bleeding diathesis, PFA and platelet aggregation studies were inconclusive. Abnormalities in platelet ultrastructure on EM studies demonstrated corroborative evidence for SPD with absent/reduced alpha or delta granules.
