[Abstract] Introduction: Silver-releasing dressings are used in the treatment of infected wounds. Despite their widespread use, neither the amount of silver released nor the potential in vivo toxicity is known. The aim of this study was to evaluate the cytotoxic effects and the amount of silver released from commercially available dressings with infected wounds. Methods: The review was conducted according to the PRISMA statement. The Web of Science, PubMed, Embase, Scopus, and CINAHL databases were searched for studies from 2002 through December 2022. The criteria were as follows: population (human patients with infected wounds); intervention (commercial dressings with clinical silver authorized for use in humans); and outcomes (concentrations of silver ions released into tissues and plasma). Any study based on silver-free dressings, experimental dressings, or dressings not for clinical use in humans should be excluded. According to the type of study, systematic reviews, experimental, quasi-experimental, and observational studies in English, Spanish, or Portuguese were considered. The quality of the selected studies was assessed using the JBI critical appraisal tools. Studies that assessed at least 65% of the included items were included. Data were extracted independently by two reviewers. Results: 740 articles were found and five were finally selected (all of them quasi-experimental). Heterogeneity was found in terms of study design, application of silver dressings, and methods of assessment, which limited the comparability between studies. Conclusion: In vivo comparative studies of clinical dressings for control of infection lack a standardized methodology that allows observation of all the variables of silver performance at local and systemic levels, as well as evaluation of its cytotoxicity. It cannot be concluded whether the assessed concentrations of released silver in commercial dressings for the topical treatment of infected wounds are cytotoxic to skin cell

Aim: The aim of this study is to evaluate the doctoral scientific production of Nursing in re-lation to impaired skin integrity, analyzing the subject matter and the types of lesions on which they focus.Methods: Descriptive, observational, retro-spective study. A search was performed in the Database of Doctoral Theses of the Spanish Mi-nistry of Universities with the keywords “Ulcer”, “Wound”, “Diabetic foot”, “Ostomy”, “Burns”, “Burn”, “Burned” and “Deterioration of skin in-tegrity”. A peer review was developed for thesis selection and data extraction, following the eligi-bility criteria.Results: 159 theses were selected, 56 of them by nurses. It was determined that most of the theses analyzed (54%) were performed between the academic years 2013-2014 and 2021-2022 and 47.6% of them were nurse theses. By type of injury 57.1% were pressure ulcers, 10.7% were vascular ulcers, 3.6% were ostomies, another 3.6% were diabetic foot ulcers, 1.8% were burns and 1.8% were surgical wounds.Conclusion: It is observed that Nursing doc-toral theses focus mainly on pressure ulcers and are mostly descriptive (prevention, incidence, quality of life, measurement of the level of know-ledge and determination of the burden of care), Objetivo: El objetivo de este estudio fue eva-luar la producción científica doctoral de enfer-mería relacionada con el deterioro de la integri-dad de la piel, analizando la temática y los tipos de lesiones en las que se centran.Metodología: Estudio descriptivo, observa-cional, retrospectivo. Regardless of the problems that this reuse and prolongation of use pose regarding the ability of personal protective equipment (PPE) to act as an effective barrier against infections, other associated risks are also emerging, in the form of adverse events, that has its incidence on the skin of contact with said PPE (mainly facial skin, nose and ears), and that are related to an overexposure of use for long periods of time. Not surprisingly, the FFP2 / N95 and FFP3 masks, which are the most widely used in hospitals, they are at particularly high risk of skin injury due to the requirements for a tight fit; such injuries can also occur as a result of friction and moisture buildup under the mask from extended use. This editorial includes measures for its prevention., Independientemente de los problemas que supone esta reutilización y prolongación del uso respecto a la capacidad de los equipos de protección individual (EPI) para actuar como barrera eficaz frente a infecciones, también están surgiendo otros riesgos asociados, en forma de eventos adversos, que tiene su incidencia sobre la piel de contacto con dichos EPI (principalmente la piel facial, la nariz y las orejas), y que guardan relación con una sobreexposición del uso por largos períodos de tiempo. No en vano, las máscaras FFP2/N95 y FFP3 que son las más utilizadas a nivel hospitalario, tienen un riesgo particularmente alto de lesiones cutáneas debido a los requisitos para un ajuste estanco; dichas lesiones también pueden ocurrir como resultado de la fricción y la acumulación de humedad debajo de la máscara por su uso extendido. Este editorial recoge medidas para su prevención.

To mitigate the potential transmission of the "VID-19 pandemic", health and public health authorities recommend frequent hand sanitization with soap and water and antiseptic disinfection with gels or hydroalcoholic solutions as an alternative or complementary measure when hand washing cannot be performed. Therefore, taking into account the nature of the same, the differences on its effectiveness are evident by its composition and antimicrobial effect; distinguishing between biocides, viricides and cosmetics. What this editorial article raises is the doubt of the useless effectiveness or low impact in the prevention of COVID-19 infection, due to the massive use and variability of gels of a cosmetic nature that we use daily and that do not usually reach the minimum standards of effectiveness, in addition to the dermatological risk involved in mixing different gels and solutions on a continuous basis., Para paliar los posibles contagios de la transmisión de la “pandemia del COVID-19”, las autoridades sanitarias y de Salud Pública recomiendan realizar una frecuente higienización de las manos, ya sea con agua y jabón, así como, la desinfección antiséptica con geles o soluciones hidroalcohólicas como medida alternativa o complementaria cuando no podamos llevar a cabo el lavado de manos. Por tanto, atendiendo a la naturaleza las mismas, las diferencias sobre su eficacia son evidentes por su composición y efecto antimicrobiano; distinguiendo entre biocidas, viricidas y cosméticas. Lo que plante este artículo editorial es la duda de la inútil eficacia o baja repercusión en la prevención del contagio por COVID-19, por el uso masivo y la variabilidad de geles de naturaleza cosmética que diariamente utilizamos y que no suelen alcanzar los mínimos estándares de efectividad, además del riesgo dermatológico que conlleva mezclar distintos geles y soluciones de forma continuada.

Objective: To evaluate and compare the quality of clinical practice guidelines (CPG) on prevention and treatment of acute radiodermatitis (RD). Methodology: Evaluation of methodological quality through the AGREE-II instrument. Search for CPG in specialized sources. Selection of CPGRD with an age of no more than 5 years. Review by three evaluators. Results: Of seven localized CPGs, 3 (two Mexican and one Canadian) were selected. The average quality achieved was 59% (sufficient quality). The first domain (scope and objective), obtained a good quality score (>75%), being the “worst” domain 5 (applicability) that was considered “low quality” (<50%) In all CPGGs evaluated. The rest of the domains achieved a sufficient quality (57.7%). A CPG was rated as “not recommended”, and no CPG was considered “highly recommended” by the reviewers. Conclusions: The CPG Mexicana of the IMSS and the Canadian GPC of the group MASCC, were considered as “recommended with conditions”; should improve key aspects such as: use a scale of evidence of greater methodological rigor, update the recommendations of evidence, include a viable implementation process and specify the editorial independence of the CPG., Objetivo: Evaluar y comparar la calidad de las guías de práctica clínica (GPC) sobre prevención y tratamiento de la radiodermatitis aguda (RDA). Metodología: Evaluación de la calidad metodoló- gica mediante el instrumento AGREE-II. Búsqueda de GPC en fuentes especializadas. Selección de GPC-RDA con antigüedad no superior a 5 años. Revisión por 3 evaluadores. Resultados: De siete GPC localizadas se seleccionaron 3 (dos mexicanas y una canadiense y cols.). La calidad media conseguida fue del 59% (calidad suficiente). Comparando los 6 Dominios AGREE, solo el primer dominio (alcance y objetivo), obtuvo calificación de buena calidad (>75%), siendo el “peor” el dominio 5 (aplicabilidad) que fue considerado de “baja calidad” (< 50%), en todas las GPC evaluadas. El resto de los dominios lograron una calidad suficiente (57,7%). Una GPC fue calificada como “no recomendable” y ninguna GPC fue considerada como “muy recomendada” por parte de los revisores. Conclusiones: La GPC Mexicana del IMSS y la GPC canadiense del grupo MASCC, fueron calificadas como “recomendables con condiciones”, debiendo mejorar aspectos clave como: utilizar una escala de evidencia de mayor rigor metodológico, actualizar las recomendaciones de evidencia, incluir un proceso de implementación viable y especificar la independencia editorial de la GPC.

Objective: To assess the proper condition of the skin through the measurement of cutaneous parameters by a non-invasive device multi probe in patients affected of radiodermitis (radiotherapy induced injury). Methodology: Descriptive observational study. Measurement of cutaneous parameters (hydration, erythema, melanin, elasticity) through a multi-probe device and, implementation of a protocol for the treatment of cure in humid environment to patients with radiodermitis. Descriptive statistics and dispersion. Results: We evaluated 6 clinical cases (4 men and 2 women) affected of radiodermitis grade 2-3. In 67% of the cases we obtained a decrease of 74,6% of lesion area. Of all the cutaneous parameters evaluated, erythema and hydration were the values that more variability presented in reference to the value of the control. Conclusions: The skin parameter information obtained by the non-invasive device multi-probe proved to be useful to determine the effectiveness of treatment of cure in humid environment in lesions induced radio, as well as for the followup, the evolution and prognosis of healing., Objetivo: Valorar el adecuado estado de la piel a través de la medición de parámetros cutáneos por un dispositivo no invasivo multi sonda en pacientes afectados de radiodermitis (lesión radio inducida). Metodología: Estudio observacional descriptivo. Medición de parámetros cutáneos (hidratación, eritema, melanina, elasticidad) a través de un dispositivo multi-sonda y, aplicación de un protocolo de tratamiento de cura en ambiente húmedo a pacientes con radiodermitis. Estadística descriptiva y de dispersión. Resultados: Se evaluaron 6 casos clínicos (4 hombres y 2 mujeres) afectados de radiodermitis de grado 2-3. En el 67% de los casos se obtuvo una disminución del 74,6% del área de lesión. De todos los parámetros cutáneos evaluados, el eritema y la hidratación fueron los valores que más variabilidad presentaron en referencia al valor de control. Conclusiones: La información de los parámetros cutáneos obtenidos por el dispositivo no invasivo multi-sonda resultó ser útil para determinar la efectividad del tratamiento de cura en ambiente húmedo en lesiones radio inducidas; así como, para el seguimiento, la evolución y el pronóstico de cicatrización.

Objective: To evaluate and compare the editorial independence and declaration of conflicts of interest of different Clinical practice Guidelines (CPG) on dermatological lesions. Methodology: Systematic review of CPG in active state (period 2011-2015), at the national and international level. Evaluation of item 22 and 23 the Domain 6 (editorial independence) of the AGREE Instrument II (Appraisal of Guidelines Research and Evaluation). Method of review by 4 evaluators. Descriptive statistic. Results: We assessed 30 GPC (4 on psoriasis; 2 on radiodermitis, 10 on atopic dermatitis, 5 on burns, and 9 on skin cancer Melanoma and non-melanoma). Of these, 14 GPC accomplished very well with the Domain 6 (editorial independence) of AGREE II; 11 did so partially and 5 have not reached the minimum level. Conclusions: The evaluation of item 22 (editorial independence) is the one least accomplished and the most difficult to assess by the reviewers of GPC. The points of view of the financing entity, usually does not appear as an explicit statement and/or concise in expressing its influence on the final result of the recommendations of the GPC; as proposed by the AGREE II., Objetivo: Valorar y comparar la independencia editorial y declaración de conflictos de interés de diferentes Guías de Práctica Clínica (GPC) sobre lesiones dermatológicas. Metodología: Revisión sistemática de GPC en estado activo (periodo 2011-2015), de nivel nacional e internacional. Evaluación del ítem 22 y 23 del Dominio 6 (independencia editorial) del instrumento AGREE II (Appraisal of Guidelines Research and Evaluation). Método de revisión por 4 evaluadores. Estadística descriptiva. Resultados: Se evaluaron 30 GPC (4 de psoriasis; 2 de radiodermitis, 10 sobre dermatitis atópica, 5 sobre quemaduras y 9 de cáncer de piel Melanoma y no-melanoma). De ellas, 14 GPC cumplieron muy bien con el Dominio 6 (independencia editorial) del AGREE II; 11 lo hicieron de forma parcial y 5 no alcanzaron el nivel mínimo. Conclusión: La evaluación del ítem 22 (independencia editorial) es el que menos se cumple y el más difícil de valorar por los revisores de GPC. Los puntos de vista de la entidad financiadora, generalmente, no aparecen como una declaración explícita y/o concisa a la hora de manifestar su influencia en el resultado final de las recomendaciones de la GPC; como propone el AGREE II.

Management and initial treatment combining a silver nanocrystalline dressing, a dressing of hydrogel in mesh and a secondary dressing hydrocellular adhesive. Satisfactory remission of infection to the 6 days of treatment., Presentación de un caso clínico característico de cómo aplicar los conceptos de control de la inflamación y de la infección en un tatuaje con sobrecarga bacteriana, en la extremidad inferior de un varón de 30 años. Abordaje y tratamiento inicial combinando un apósito de plata nanocristalina, un apósito de hidrogel en malla y un apósito secundario hidrocelular adhesivo. Remisión satisfactoria de la infección a los 6 días de tratamiento.
