In industrial environments, forklift safety management systems using various smart sensors such as light detection and ranging (LiDAR), laser, ultrasound, radio frequency identification (RFID) sensors, or much more have been developed to prevent forklift accidents arising from collisions. However, these approaches tend to give a big burden to the introduction of the forklift safety management system because they typically require the high initial costs for its implementation and the complexity of the system installation, configuration, and maintenance. Hence, in this study, the authors propose a simple, cost-effective, and straightforward mechanism and system architecture based on the artificial intelligence of things (AIoT). The proposed system and algorithm have been designed for the early detection of potential hazards and dangerous environments involving moving forklifts and pedestrians. In this paper, we describe a prototype implementation of AIoT-based forklift safety management system and its performance evaluation results. From the laboratory and field tests, it was shown that the spatial-perspective-based algorithm proposed in this study improves the F1 score value, which indicates the critical metric for safety assessment, by approximately 26% and 2.4% in the laboratory and the field test, respectively, when compared to the distance-based algorithm only based on the calculated distance. It was analyzed that this significant discrepancy in the F1 score's performance improvement between the laboratory and the field test was attributed to the frequency of scenes with pedestrians near forklifts. Consequently, the field test demonstrated that the cost-effective 2D camera and AIoT-based hazard detection using the proposed algorithm has effectively and correctly identified various hazardous situations that happen in the real manufacturing workspaces, including the scenarios that the pedestrians are partially detected in the images while approaching the forklift so closely. 6. Constructing Safety Management Systems in Modern Industry and Trade Enterprises: A STAMP-Based Approach.

With the burgeoning landscape of new enterprises and business paradigms, industrial and trade enterprises are facing escalating pressure to ensure operational safety. Conventional safety management mechanisms have proven to be inadequate for adapting to the dynamic market demands and intricacies of modern production environments. To improve safety management practices, this study integrates complex network theory to dissect the causal chains underlying accidents in industry and trade enterprises. A network model is established to elucidate the factors contributing to accidents and leverage datasets from safety inspections to construct a repository of latent safety risks. To address deficiencies in extant safety frameworks, a comprehensive safety management evaluation system is formulated, comprising ten primary evaluation indices and 30 secondary metrics. Based on the established frameworks, such as ISO 45001 for occupational health and safety management systems (OHSMS), standardized safety production protocols, and risk hierarchical management and control systems and hidden hazard identification and treatment systems (dual prevention systems), a holistic safety management system (SMS) is synthesized on the basis of system-theoretic accident model and process (STAMP) theory. This systematic approach culminates in a robust framework tailored to modern industrial and trade enterprises, fostering flexibility and efficacy in safety management capabilities. This case analysis underscores the model's ability to enhance its safety management proficiency, thereby amplifying its relevance in fortifying enterprise operations and fostering sustainable growth. This study represents a pivotal step toward augmenting safety management capacities within the industrial and trade enterprises to safeguard enterprise vitality and advance sustainable business practices. 7. Association between marine corps safety management system assessment results and injury rates and outcomes.

Occupational health and safety management systems (OHSMSs) have been proposed as a method of managing human health and safety risk and reduce injuries and illnesses. The investigation objective was to determine the association between OHSMS performance, as measured by scores from assessments and inspections, and mishap rates and mishap outcomes. Results for 55 OHSMS assessments/inspections across 15 organizations from the years 2017–2022 were analyzed. Results were paired with 545 mishaps and according to mishap classification, mishap category, and mishap severity. Correlation analysis was conducted to determine strength and significance of association between OHSMS performance and mishap rates. Improved OHSMS performance, as measured by scores from assessments and inspections, was associated with lower severity mishap occurrence such as reduced rates of fatalities, permanent total and partial disability, and property damage. Mishaps involving fatalities and permanent total disability had the strongest association with measured OHSMS performance. This research indicates that higher levels of OHSMS performance may help reduce severe worker injuries and illnesses and reduce mishap costs. [ABSTRACT FROM AUTHOR]

This research examines the influence of safety culture on aviation safety through the implementation of the Safety Management System (SMS) in Papua. The focus of this study is on safety risk management and crew training at airports in Papua, which face unique challenges such as extreme weather and difficult terrain. The research method used is quantitative analysis by collecting data through surveys and interviews. The results indicate that the implementation of a strong and effective safety culture can significantly improve aviation safety. Additionally, well-implemented SMS helps in identifying and managing risks and enhancing the quality of training and personnel qualifications. The study also emphasizes the importance of involving all stakeholders, including local communities, to create an environment that supports aviation safety. 9. Practices and challenges of safety management in outsourced facilities management.

• Three cases of outsourced facility management between national or international companies were investigated. • Safety management practices in outsourced facility management conform to those in safety critical industries. • However, the practice of review was not found, limiting learning. • Regulatory failures contribute to safety management challenges in outsourced facility management. Introduction: Outsourcing is a commonly occurring organizational activity, but one associated with negative occupational safety outcomes. Improving the management of safety in workplaces where contractors are employed is vital, but under-researched in the service sectors. The aims of this paper were to investigate both the practices and challenges of safety management in outsourced facility management (FM), an important global service sector. Method: Twenty-three semi-structured interviews were conducted with clients and contractors in three different FM outsourcing arrangements between large corporations in the UK. Data were thematically coded against frameworks derived from existing literature to identify deployed safety management practices and reveal challenges associated with safety management in these outsourced relationships. Results: Safety management practices in outsourced FM conformed to known practices clustering into four previously identified categories (planning, selecting, on-site working, and checking). A fifth category (reviewing) was not observed. Operating across national boundaries, applying national contracts locally, working with mandated KPIs, and contract specifications all created new challenges for safety management not previously reported. Other known challenges associated with economic pressure and disorganization were observed. Conclusion: Safety management practices observed in safety critical industries also apply in FM. However, the challenges of safety management in these three cases included regulatory failures that have not been routinely identified in other empirical studies of safety in outsourcing arrangements. Practical application: Adopting widely accepted safety management practices support safer working in outsourced FM and encourage cross-sector learning. New challenges for safety management noted here encourage consideration of unintended consequences of contract terms and conditions, require corporate agreement on how to ensure safety compliance when working transnationally, and a review of decision-making and processes and procedures to enable effective and safe working locally. This paper investigates the usage of a systemic neutralized taxonomy for understanding human factors to foster a restorative culture when studying adverse events. The available studies supporting accident analysis from a system‐theoretic perspective with human factors‐based methods are grounded on error taxonomies and do not reinforce the non‐judgemental dimension that is encouraged by modern safety science. We propose a methodology to integrate the system‐theoretic accident analysis technique, Causal Analysis based on System Theory, with a neutralized taxonomy of human explanatory factors. The proposed methodology has been applied to an aviation accident involving a military aircraft and a glider. This case revealed various critical interactions among system components, which require dedicated safety recommendations that go beyond the identification of single points of failures and root causes, leading to a deeper understanding of socio‐technical orchestrations. Despite the use case in aviation, the proposed methodology remains suitable for various types of safety occurrences in diverse domains and industrial settings. Norwegian county authorities’ management of road safety has undergone major changes since 2020, when a structural reform was implemented. County authorities merged into fewer administrative units, and took over the responsibility for administering the county roads. Because of this, county authorities had to establish new systems for managing road safety. The aims of the study are to examine: 1) Whether four county authorities have implemented living systems for road safety management, three years after the 2020 regional reform, and 2) Factors influencing whether safety management systems are living, including the impact of extensive reforms. The study is based on qualitative research interviews (n = 42) and an online survey (n = 392). The extent to which the county authorities have managed to implement living safety management systems and maintain their key road safety roles differs substantially. Multivariate analyses show that the most important factors influencing whether county authorities have been able to implement living systems are the safety cultures in the county authorities, and the extent to which respondents believe that the reorganizations related to the structural reform have required a lot of time and energy.

Purpose: The main purpose of the article is to analyze accident rates and the ocean of correlation of accident events in the period 2022-2023, as well as to identify relationships affecting changes in the number of accident events. Design/methodology/approach: The research methods used in the article were quantitative methods, which included empirical surveys and qualitative methods including conducting interviews/questionnaires with representatives of the surveyed companies. An additional approach to the research referred to the statistical data. Findings: The considerations presented in the publication made it possible to analyze the accident rates for the surveyed 25 companies in the logistics industry, statistical data in correlation with both the level of employment and the measures taken that are aimed at the complete elimination of accidents at work. The article seeks to analyze in detail the correlations that occur in the surveyed enterprises in relation to the decreases in accident rates, which were evident in the whole set at 52.98%, with a slight decrease in employment determined for the total at 2.443%. Thus, new courses of action were verified and established, through which companies will eliminate accidental events, affecting the provision of very safe working conditions. Originality/value: The carried out assessment of accident rates in the surveyed companies as well as on the basis of statistical research indicates minimization and strives for complete elimination of accident events in the logistics sector. The main influence on this is the raising of awareness by employees as well as the continuous spreading of safety culture through the implementation of new technical, organizational, analytical or management solutions. The focus on the development, implementation and maintenance of safety management at the highest level, as well as its continuous improvement, significantly affects the decrease in accident rates, which should continue to be maintained and introduced in the logistics industry, but also in other branches of the economy. Purpose: The purpose of this study is to understand the air traffic control (ATC) threats that cause airline pilots to commit errors during normal operations by identifying the prevalence of ATC threats to reduce ATC communication errors for safe flights. This study deals with ATC communication threats effect on the human performance of pilots through LOSA (line operations safety audit) implemented by Airlines. Methods: This study deals with ATC communication threats effect on the human performance of pilots through LOSA implemented by two domestic Airlines. Results: The prevalence of ATC threats shows that there are 60% of flights with at least one ATC-related threat. The frequent ATC threats have been identified as difficulties in understanding controller accents, changes of the runway, errors by ATC controllers, difficult clearance of altitude change and multiple clearances in one call. The most prevailing ATC threats occurred during the Descend, Approach & Land phase of the flight. Conclusion: In order to prevent accidents related with ATC and improve threats and human errors in the actual environment of pilots, it is necessary to identify and analyze the actual threats and errors in the cockpit regarding ATC. It is required to improve the management of ATC threats proactively. According to LOSA result, ATC threats are a frequent threat to pilots, and the most frequent type of ATC for pilots is difficult Accent or language to understand controller. These findings could be trained in the simulators and cockpit in a similar environment and workload as in actual flight for effective countermeasures.

[Objective] Unsafe behaviors among personnel are significant factors contributing to safety accidents in rail transit engineering. To address the existing issues of video image monitoring systems, such as reliance on manual identification, delayed detection of safety problems, and inadequate real-time alarms, research on metro construction personnel safety management system is required. [Method] The construction scheme of the metro construction personnel safety management system is elaborated from the aspects of overall architecture, equipment installation for data collection modules, establishment of BIM (building information modeling), and software platform. The application of the above management system in specific engineering pilot projects is analyzed to verify the intelligent identification accuracy of the system. [Result & Conclusion] The application of the system on construction sites demonstrates that the system can intelligently identify unsafe behaviors of construction personnel, such as not wearing safety helmets, unauthorized entry into restricted areas, and sleeping or leaving posts, for which scenarios the accurately triggered alarms take up to 98%, significantly enhancing the precision and efficiency of identifying safety hazards. Relevant personnel can access surveillance videos in real-time through interactive terminals, receive alarm information promptly, swiftly locate the occurrence of hazards, and take necessary actions.

The fire safety and protection of a multistory building seems not only national, but a worldwide concern. To sort out this issue, there is a special need to pay attention towards fire safety management system of buildings. In recent decades no. of fire incidents to whole over the world increased to a very high level and yearly a large number of users died and a substantial property damage takes place. In this regard, effective fire safety management system plays an important role towards the fire safety of building and to avoid fire incidents and reduce its effects. The major aim of this article is to evaluate the existing fire safety measures by designing a fire safety audit, for a public sector multistory building, in which all the components of fire safety system will be inspected by considering relevant NFPA standards and UAE fire and life safety codes. By conducting fire safety audit, conclusions are derived and safety gaps are identified in existing fire safety management system, which may be rectified for effective fire safety controls. Accordingly, recommendations drawn on the basis of findings, analysis and conclusions for proper fire safety management system.

20. The Safety Management and Organizational Resilience System Maturity of Aviation Organizations during the COVID-19 Pandemic: Comparison of Two Approaches to Achieving Safety.

The coronavirus pandemic crisis highlighted the critical importance of comprehensive safety management for all organizations. Safety management literature delineates two approaches to achieving safety, characterized as safety management through centralized control, known as the safety management system (SMS), and safety management through guided adaptability, known as organizational resilience (OR). Each of these approaches plays a pivotal role in establishing and maintaining the safety and sustainability of an organization. This paper aimed to compare the maturity of SMS with the maturity of OR, identifying the relationship between aspects of SMS and OR in the context of the crisis of the pandemic. Based on a literature review, the author presents adopted concepts of SMS and OR, as well as a customized maturity model for both. The survey methodology involved two questionnaires on SMS and OR, consisting of 26 and 18 questions, respectively. The survey was conducted in three approved training organizations (ATOs) in the Greater Poland voivodeship. When comparing key aspects of both approaches to safety management (SMS vs. OR), significant differences in ratings were observed. Additionally, a moderate correlation was found between aspects of SMS and OR. This discrepancy was reflected in the maturity models. According to the survey results, SMS achieved the fourth level of maturity, labeled proactive safety management, while OR attained the third level of maturity, termed a fairly agile organization. Furthermore, the results showed that while the guided adaptability approach is more difficult to achieve in an organization, the centralized control approach is insufficient. Husein Sastranegara Bandung International Airport was previously an airport called Andir (Andir Airfield) during the Dutch East Indies era in 1920. As time progressed, Husein Sastranegara Bandung International Airport was managed by PT. Angkasa Pura II in 1994. Even though it has been processed by PT. Angkasa Pura II and many improvements have been made, but there are still facilities that are not available in accordance with Minister of Transportation Regulation Number KM 21 of 2005 concerning the Implementation of Indonesian National Standards (SNI) 03-7095-2005 concerning Markings and Signs in Aircraft Movement Areas at Airports as a Mandatory Standard, namely Service Road markings. The aim of this research is to provide an overview of the actualization (actual condition) of the Service Road in order to fulfill the Safety Management System at Husein Sastranegara Bandung International Airport. The research method used is qualitative, the informant validation tool uses NVivo Softwear, and data validation uses triangulation, and the tool to answer research questions uses fishbone to answer "why (epistemology)" and triangulation to answer "how (axiology)". The results of this research are that the Service Road is a marking on the apron which is mandatory due to ICAO (International Civil Aviation Organization) regulations. The Service Road at Husein Sastranegara Bandung International Airport has not yet been built so the Safety Management System is still not fulfilled, but the management of PT. Angkasa Pura II has created a Service Road design plan in accordance with KP 777 of 2016 concerning the Master Plan for Husein Sastranegara International Airport in Bandung City, West Java Province. Management systems that are recognized as key tools to improve business management and the results associated with it have spread at the business level during the last 50 years. Regarding Safety Management Systems (SMSs), despite having specific international standards, there are no complete studies that analyze the degree of effectiveness of SMS, and even less in construction, a sector that concentrates a large part of registered labor accidents worldwide. The present investigation is an analysis of SMS effectiveness from an empirical study carried out over 48 months in five countries with a total of more than 23 million work hours between 2009 and 2012. Additionally, it is implied that the impact of SMS implementation in a certain organization must be complemented by a statistical qualitative analysis of its effect on the distribution of accidents. Both analyses are developed in the present study, thus contributing relevant implications when assessing both quantitatively and qualitatively the effects of developing and implementing a health and safety management system in this sector. The findings from this research can contribute to understanding how SMS implementation can help reduce accidents in this industry as well as to enhance SMS implementation in a high-risk sector. For organisations to take preventive measures and eliminate potential accidents, the information gained through voluntary reporting is essential. Employees do not, however, report voluntarily for a number of reasons. In this study, we examine why train drivers, who are vital to maintaining rail safety, fail to report hazardous occurrences, leading to employee silence. The measurement tool, which has already been developed specifically for aviation employees, has been applied to 346 train drivers working on Turkish Railways. The scale used for research purposes has proven to be valid and reliable for organisations involved in railways. As a result, it was determined that the drivers did not participate in voluntary reporting due to relational and prosocial, disengaged, quiescence and acquiescence and fear and defensive factors. The highest score for the reasons for non-reporting was observed in the dimension of quiescence and acquiescence. The results of the correlation analysis between dimensions, which are assumed to be the reasons for non-reporting, point to strong positive relationships between each dimension.

27. Systematic Review of Safety Management Systems in the United States General Aviation.

A safety management system may benefit General Aviation by increasing safety and reducing accident rates. This paper aims to systematically review the existing literature in targeted databases for gaps concerning general aviation safety management systems. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology provided the overall structure. Science Direct®, Embry Riddle Aeronautical University's Scholarly Commons, and the Federal Aviation Administration's (FAA) website were the three publicly available sources. Initial search criteria included PDF research articles (2006-2022), including the terms safety management system and general aviation. Additionally, keyword searches for the terms policy, promotion, risk management, and assurance guided the analysis. A final case review was conducted to eliminate faulty connections and false positive word matches. Bias may exist in this project due to the single author and lack of additional reviewers. However, two strategies were employed to maximally reduce unintended bias: (1) software-generated keyword searches and (2) a 1-week delay between reviews. The initial search returned 197 listings. Twenty-nine records remained after multiple rounds of elimination and review. The results suggest that general aviation participants receive minimalistic exposure to the benefits of the safety management system. Seven FAA publications showed little evidence of a safety management system within general aviation. Twenty-two research articles demonstrated a strong understanding of the safety management system's components, especially safety risk management. As a result of this review, it is recommended that the FAA explore the feasibility of incorporating additional safety management system education, awareness, and best practices. Additionally, future research should explore contributing factors related to the benefits of establishing a safety management system in general aviation operations. The fatigue of pilots is the most threatening factor in aviation safety internationally, requiring continuous management. In order to prevent aircraft accidents that may occur due to fatigue, this paper analyzes fatigue-related accident cases and previous research and suggests ways to improve the fatigue management of pilots. Problems in major fatigue accidents were active communication failure, lack of leadership of the captain, inadequate contingency management, lack of workload management, and the most important factors in accidents were inadequate decision making of continuing an approach without going around. It is necessary to proactively improve and establish go-around culture in the organization and manage human factors with training and procedures of Threat and Error Management and Crew Resource Management. In order to fundamentally prevent accidents related to fatigue fundamentally, it is necessary to establish Fatigue Risk Management and Safety Management System, and identify the actual threats and errors in the cockpit of the aircraft.

In the life of companies operating with hazardous substances, the daily task is to maintain the safety on a high level and to introduce actions to increase safety performance. The companies covered by the Disaster Prevention Act have a special task, to operate a safety management system that also satisfies industrial safety aspects. To make this as efficient as possible, good practices from other safety management systems can be built into the currently used management system at the company. Perhaps the most frequently operated safety management systems for organizations producing and processing hazardous substances are the Occupational Health and Safety Management according to the ISO 45001 standard and the Process Safety Management systems. A comparison of the latter with what is required by law points to several points that may designate areas for improvement in the certified safety management system. In this way, they provide a basis for the development of a new system, that complying with the prescribed requirements, helps to maintain a high level of safety, to prevent the occurrence of accidents, and to work with safety-conscious employees and suppliers.

31. Overcoming barriers to smart safety management system implementation in the construction industry

The reliance on networked technologies in construction safety and health management is expected to increase; however, studies have reported potential resistance to the broad implementation of smart technologies within structured construction safety management systems (SMS). This study examines the challenges and drivers impacting the implementation of smart SMS in the construction industry using quantitative and qualitative research methods. The current state of safety knowledge and practices was reviewed and critical insights were obtained from construction industry practitioners. To identify barriers and solutions to implementing smart technologies in construction safety management, a literature review was conducted. Based on this review and semi-structured expert interviews, a survey was developed and distributed to U.S. construction industry management to assess their perceptions of these barriers and strategies. The study revealed 24 implementation barriers of which 14 were considered critical with mean normalized values ≥ 0.50. These barriers were categorized into four groups – organization, infrastructure, cost, and system integration-related barriers based on the correlation between the four different groups. Sixteen strategies that could help overcome these challenges and improve the integration of IoT-SMS technologies were uncovered and discussed. The findings from this study extend technology implementation theories by proposing an organization-infrastructure-cost-system integration framework for SMS implementation. Moreover, the study creates a foundation for evaluating barriers and strategies associated with implementing technology-driven SMS in the construction industry, thereby advancing research at the intersection of SMS and emerging technologies.

The main tool of cockpit crew performance evaluation is the recorded flight data used for flight operations safety improvement since all certified airlines require implementation of a safety and quality management system. The safety performance of a flight has been a challenging issue in the aviation industry and plays an important role to acquire competitive benefits. In this study, an integrated multi-class classification machine learning models and Markov chain were developed for cockpit crew performance evaluation during their flights. At the outset, the main features related to a flight are identified based on the literature review, flight operations expert’s statements, and the case study dataset (as numerical example). Afterwards, the flights’ performance is evaluated as a target column based on four multi-class classification models (Decision Tree, Support Vector Machine, Neural Network, and Random Forest). The results showed that the random forest classifier has the greatest value in all evaluation metrics (i.e., accuracy = 0.90, precision = 0.91, recall = 0.97, and F1-score = 0.93). Therefore, this model can be used by the airline companies to predict flight crew performance before the flight in order to prevent or decrease flight safety risks.

The fire safety and protection of a multistory building seems not only national, but a worldwide concern. To sort out this issue, there is a special need to pay attention towards fire safety management system of buildings. In recent decades no. of fire incidents to whole over the world increased to a very high level and yearly a large number of users died and a substantial property damage takes place. In this regard, effective fire safety management system plays an important role towards the fire safety of building and to avoid fire incidents and reduce its effects. The major aim of this article is to evaluate the existing fire safety measures by designing a fire safety audit, for a public sector multistory building, in which all the components of fire safety system will be inspected by considering relevant NFPA standards and UAE fire and life safety codes. By conducting fire safety audit, conclusions are derived and safety gaps are identified in existing fire safety management system, which may be rectified for effective fire safety controls. Today's air traffic represents the safest form of transportation globally. Its functioning is based on the integration of various elements that together constitute a coordinated system, including people, procedures, the environment, and various systems. With the increase in air traffic, potential safety hazards also grow, making the safety management system a crucial factor. This paper explains the application of the safety management system, the implementation of which enables risk control and reduces the number of accidents in daily operations. Proper application of the safety management system allows, among other things, for the safe flow of air traffic. International standards in air traffic provide general guidelines and norms to ensure a high level of safety. However, for these standards to be effectively applied, each country needs to adopt national documents that more precisely regulate organizations involved in providing services in air navigation and the safety management system. 36. Determination of Indicators of Implementation of Sea Transportation Safety Management System for Traditional Shipping Based on Delphi Approach.

The implementation of the safety management system (SMS) in traditional shipping in Indonesia, known as "Pelra", has not been implemented. The nature of the operation pattern and characteristics of Pelra, which are still traditional (non-convention), means that international maritime rules such as SOLAS, MARPOL, LLC, Collreg, and STCW, as well as Indonesian government regulations related to shipping safety, are not suitable for Pelra. As a result, Pelra ship accidents continue to occur every year in Indonesian waters without any efforts to deal with safety management. The aim of this study is to find indicators to assess the implementation of Pelra's SMS that adopt the specific characteristics of Pelra sea transportation. The analysis was conducted with a Delphi approach, based on expert opinion in the field of ship safety, which was tested using non-parametric statistics (Kendall's W test). The consensus results obtained 9 factors described in 44 assessment indicators to implement SMS on Pelra vessels. Several new indicators were found, including indicators related to ship stability, ship construction, and safety and navigation equipment. The level of expert agreement from the concordance W coefficient value is in the range of 0.3 (moderately strong) to 0.7 (very strong), which suggests that these findings are valid and feasible to be used as assessment indicators in order to implement SMS Pelra. It characterizes process safety management and safety management system at technical installations. It describes methodology for ensuring the safety of technical installations. The highest quality of each object (i.e. the State, human community, technical installation etc.) is safety, so the highest priority for the object is the overall (integral) safety. This means that it is not only about partial safety such as internal, external, environmental, fire, economic, etc., but about the safety of the whole, which includes partial safety so that the condition of the whole, is optimal. Due to the systemic nature of today's world, integral safety cannot be built without regard to others. • Resilience of safety management systems has a positive impact on safety performance in building R&M companies. • Resilience of safety management systems can be measured by people resilience, place resilience, and system resilience. • Safety performance is affected by the interactions between people resilience and place resilience. • Safety performance is affected by the interactions between place resilience and system resilience. Introduction : The complex and dynamic nature of building repair and maintenance (R&M) work tends to create safety risks for workers. Resilience engineering approach is identified as a complementary to conventional safety management techniques. Resilience of safety management systems is characterized by the capabilities to recover from, respond during, and prepare for unexpected situations. This research aims to conceptualize resilience of safety management systems in the building repair and maintenance context by introducing resilience engineering principles into the safety management system concept. Method: Data were collected from 145 professionals in building repair and maintenance companies in Australia. The structural equation modeling technique was utilized to analyze the collected data. Results: The results confirmed three dimensions (i.e., people resilience, place resilience, and system resilience) with 32 measurement items for assessing resilience of safety management systems. The results revealed that safety performance of building R&M companies was significantly affected by the interactions between people resilience and place resilience and the interactions between place resilience and system resilience. Conclusions: Theoretically, this study contributes to the safety management knowledge by theoretically and empirically supporting the development of concept, definition, and purpose of resilience for safety management systems. Practical Applications: Practically, this research proposes a framework for assessing the level of resilience of safety management systems that is characterized by employees' capabilities, supportiveness of workplace, and supportiveness of management to recover from safety incidents, react during unexpected situations, and prepare for preventative actions before occurrence of undesirable events. 42. A Scoping Literature Review of Natural Language Processing Application to Safety Occurrence Reports.

Safety occurrence reports can contain valuable information on how incidents occur, revealing knowledge that can assist safety practitioners. This paper presents and discusses a literature review exploring how Natural Language Processing (NLP) has been applied to occurrence reports within safety-critical industries, informing further research on the topic and highlighting common challenges. Some of the uses of NLP include the ability for occurrence reports to be automatically classified against categories, and entities such as causes and consequences to be extracted from the text as well as the semantic searching of occurrence databases. The review revealed that machine learning models form the dominant method when applying NLP, although rule-based algorithms still provide a viable option for some entity extraction tasks. Recent advances in deep learning models such as Bidirectional Transformers for Language Understanding are now achieving a high accuracy while eliminating the need to substantially pre-process text. The construction of safety-themed datasets would be of benefit for the application of NLP to occurrence reporting, as this would allow the fine-tuning of current language models to safety tasks. An interesting approach is the use of topic modelling, which represents a shift away from the prescriptive classification taxonomies, splitting data into "topics". Where many papers focus on the computational accuracy of models, they would also benefit from real-world trials to further inform usefulness. Safety management systems (SMSs) are widely applied across many industrial sectors, and a large body of literature has been published addressing their design, implementation, effectiveness, and associated challenges. This article presents a high-level analysis of the SMS research domain, guided by a set of questions addressing the contents, structure, and evolution the research domain, its dominant themes and focus topics, the key scientific domains and journals contributing to its development, and the key publications serving as an intellectual basis for SMS related research. The results show a rapidly increasing volume of research outputs and a shift from research based in North America and Europe to Asia and Australia. There is only a limited number of institutions enduringly contributing to the field, and there are relatively few stable research collaborations, with the number of Chinese institutions publishing SMS related research fast expanding in recent years. The domain is strongly interdisciplinary and embedded in applied domains of science, with industrial engineering the most contributing category, as well as categories focusing on the industrial application domains. A temporal evolution of the research activity in different application domains is apparent, with an initial focus on occupational health and safety, followed by process safety, patient safety, food safety, and construction safety. SMS research has a strong relation to safety culture and safety climate research, and while safety and risk management concepts and theories form an important knowledge base for most application domains, the dominant views on accident causation differ between these. Research on SMS in the food industry is relatively separated from the other application domains. Based on the findings, various future research directions are discussed.

This type of research is research and development. The design of the development model in this study uses the ADDIE step (Analysis, Design, Develop, Implementation, Evaluation). However, this article only discusses up to the analysis stage, namely to find out student needs for the development of Introduction to Safety Management System learning through MOOCs. The study was conducted in May-June 2022. The samples in this study were 146 Palembang Poltekbang students who took the Safety Management System course. Sampling is done by purposive sampling technique. The research instrument used was a closed questionnaire consisting of 20 positive statements which is a modification of the Likert Scale with 4 answer choices, namely strongly agree with a score of 4, agree with a score of 3, disagree with a score of 2 and strongly disagree with a score of 1. The results of this study indicate that student agrees to do learning development Introduction To Safety Management System through MOOCs to improve mastery of aviation safety concepts.

The human factors become the most important factor in aviation safety. In order to prevent accident, pilots should manage errors in the cockpit. International Civil Aviation Authority (ICAO) recommends airlines to implement line operation safety audit (LOSA) to identify the management of errors by pilots and understand real errors and the competencies. The LOSA may provide metrics of the prevalence of errors by each types and rate of trapping errors to management enhancing errors. Airlines may understand how crew respond and manage errors. The operator may enhance safety once they identify errors and train crew how they manage to reduce the most mismanagement errors. This study is to provide operators eight recommendations on how to enhance errors and LOSA understanding human errors collected through LOSA and enhancing the implementation of LOSA with guidance according to ICAO and Federal Aviation Administration.

Purpose: The COVID-19 pandemic has wide spread to aviation. This study was conducted to find out the perception of flight training for student pilots in the flight training center in a university with COVID-19 pandemic. Methods: It analyzed through qualitative research by aviation subject matter experts in Korea. Results: The advantages of COVID-19 were the increased flight training opportunities due to significant decrease of aircraft operations in the airports, reduced ground school hours due to Social Distancing concerns of infection, difficulty in communication due to wearing masks, difficulty in purchasing parts for the maintenance, deterioration of skills due to reduced face to face class hours, and laxity in lifestyle were found. Conclusion: It was analyzed that the longer the corona pandemic, the lower the motivation of students due to uncertainty in the job market.

The hydropower station real-time safety monitoring is not only related to the safety, but also to the generation benefit of the power station. However, as the amount of monitoring data is very huge, and the dam structure is very complex, real-time monitoring cannot be carried out in most of the dam. It is useful to develop a high-performance safety monitoring system to assist the dam safety management work. As such, the paper introduces an expert system for dam safety management with a mode which can carry out the entire process of real-time dam safety monitoring, covering abnormal importing data, abnormal data changes, monitoring work, dam safety. The system has abundant functions such as dam safety analysis, visual query and remote consultation, etc. The system has been successfully applied to more than ten dam projects in China. The application shows that reliable data evaluation can identify data anomalies such as single point jump, multi point outlier and step change. Trend tracking can find possible structural changes. And clustering comprehensive evaluation can give a reasonable evaluation of the safety state of different parts of the dam. Complete online system can provide a supporting decision-making platform for the safety monitoring of the dams and improve the efficiency of power station management.

Management systems that are recognized as key tools to improve business management and the results associated with it have spread at the business level during the last 50 years. Regarding Safety Management Systems (SMSs), despite having specific international standards, there are no complete studies that analyze the degree of effectiveness of SMS, and even less in construction, a sector that concentrates a large part of registered labor accidents worldwide. The present investigation is an analysis of SMS effectiveness from an empirical study carried out over 48 months in five countries with a total of more than 23 million work hours between 2009 and 2012. Additionally, it is implied that the impact of SMS implementation in a certain organization must be complemented by a statistical qualitative analysis of its effect on the distribution of accidents. Both analyses are developed in the present study, thus contributing relevant implications when assessing both quantitatively and qualitatively the effects of developing and implementing a health and safety management system in this sector. The findings from this research can contribute to understanding how SMS implementation can help reduce accidents in this industry as well as to enhance SMS implementation in a high-risk sector.

Objective To design a safety management system for antitumor drugs, which can intercept or warn the overdose of medical orders from the prescription source, and ensure the safety of clinical application of antitumor drugs. Methods The threshold of interception and warning strategy of 88 kinds of antitumor drugs were developed, and the safety management system of antitumor drugs was established to intercept or warn the overdose orders. The interception and warning data from January 2021 to June 2021 in the safety management system of antitumor drugs were extracted and analyzed. Results A total of 665 medical orders were intercepted or warned by the safety management system of antitumor drugs, and the majority of the intercepted or warned drugs were oral tablets(74.29%). The intercepted orders were 642 (96.54%), and the warned orders were 23(3.46%). Among the intercepted medical orders, frequency error (64.22%) has the highest proportion, followed by route error (11.73%) and dose error (9.92%). A total of 564 medical orders were intercepted, involving 28 kinds of antitumor drugs. Methotrexate tablets ranked first in overdose quantity, with an average of seven times. Pemetrexed ranked first in overdose multiples, with an average overdose of 1000 times. 78 medical orders were intercepted for over pathway, involving 5 kinds of antitumor drugs. Conclusion The safety management system of antitumor drugs can eliminate the error of overdose in prescription and guarantee the safety of antitumor drugs.
