Abstract

Minimum expected-cost solutions to the single-period inventory problem (the Newsboy problem) have been well known for many years, for risk cases where the distribution of demand is known. Also well known are minimax cost and Laplace solutions for the uncertainty case where only the range of demand is known. This study explores the use of orderstatistic- based quantile estimators as decision procedures while data gathers during the early decision periods. Simulation, using a variety of demand distributions and unit-cost values, provides recommendations on which period to leave the minimax procedure for a suggested data-based decision rule which will be asymtotic to the optimal risk procedure., http://archive.org/details/adecisionprocedu1094518393, Lieutenant, Turkish Navy, Approved for public release; distribution is unlimited.

Abstract

Background: Advanced age constitutes a difficult group for carotid surgery patients. The aim of this study was to evaluate the results of carotid disease in patients of advanced age. Materials and Methods: In our study, between January 2013 - April 2014, we discussed 250 patients underwent carotid disease. 72 patients have undergone carotid endarterectomy surgery (CEA) with safety under local anesthesia. 75 years and old 23 patients (14 male, 9 female, mean age:78.2±2.2) were retrospectively reviewed. Mean age of younger than 75 years old (n:49) patients was found to be 68.8±8.3 (39 male, 10 female). Each patient was discussed for carotid revascularization. Perioperative follow-up period was determined as 7 days. Patients were evaluated in terms of demographic data and operation specifications. Result: In this study, we found that there are risk factors of postoperative stroke positively associated with the octogenarians' group. Preoperative hypertension ratio was significantly higher in patients with ≥75 years old patients (p:0.04). In octogenarians' group, diabetes mellitus ratio was nearly the same but HbA1c levels were higher in patients with advanced age group.(P:0.002) Also, no differences was found with X clamp times (P: 0.5). Conclusions: Perioperative and surgical follow-up is important in patients with advanced maternal age. In octogenarians, CEA with regional anesthesia can do simple and successful results. Real time messaging and communication systems may be utilized for the solution of intervention during the complications in octogenarians. [ABSTRACT FROM AUTHOR]
