16. MEDICAL THERAPY FOR LARGE IDIOPATHIC FULL-THICKNESS MACULAR HOLES.

Abstract

Purpose: To describe the characteristics and outcomes of eyes with idiopathic fullthickness macular holes that underwent initial medical management. Methods: This retrospective study included eyes with full-thickness macular holes that were initially managed with 1 month of topical therapy. Eligible subjects were treated with dorzolamide 2% three times a day, nepafenac 0.1% twice a day, and prednisolone acetate 1% four times a day. The primary end points was hole closure at one month and secondary end point was change in best-corrected visual acuity. Results: Ten subjects (mean age: 62.80 years; female: 50%) with unilateral full-thickness macular holes were studied. The mean basal diameter of the entire cohort at baseline was 824.1 mm (median 828 mm). Four (40%) of the smaller holes (mean 698 mm; median 698.50 mm) closed after 1 month of topical therapy, whereas larger holes (mean 908.17 mm; median 889.50 mm) did not close. In one eye, the hole reopened 4 months after stopping the medication but closed again at 1 month after restarting the topical treatment. Median best-corrected visual acuity improved from 0.35 logMAR at baseline to 0.05 logMAR in eyes that closed but remained at 0.70 logMAR at 1 month in eyes that did not close. Conclusion: Topical corticosteroid, nonsteroidal antiinflammatory, and carbonic anhydrase inhibitor therapy may promote closure of small full-thickness macular holes, but large holes are less likely to respond. One month of topical therapy might avoid subjecting some patients to complex vitreoretinal surgery without compromising visual outcomes. Macular hole may reopen after stopping the topical therapy. 33. Clinical and Multimodal Imaging Study to Differentiate Amelanotic Choroidal Lesions

Abstract

Introduction:The aim of this study was to differentiate amelanotic choroidal lesions – amelanotic choroidal melanoma, choroidal metastasis, and choroidal granuloma using multimodal imaging. Methods:Retrospective comparative chart review was done. Patients diagnosed with the abovementioned choroidal lesions from 2015 to 2022 were included. Baseline lesion morphology and retinal layer changes on optical coherence tomography (OCT), lesion echogenicity and dimensions on ocular ultrasonography (USG), fundus autofluorescence (FAF), fundus fluorescein angiography (FFA), and indocyanine green angiography (ICG-A) patterns and findings were assessed. Results:Twelve eyes with melanoma, 22 eyes with metastasis, and 9 eyes with granuloma were included. On OCT, 83% of melanomas and 67% of granulomas, and 68% of metastasis had dome-shaped choroidal lesions. Presence of intraretinal fluid (IRF) and shaggy photoreceptors was 94% (95% CI: 79–99%) and 90% (95% CI: 74–98%) specific to differentiate melanomas from other choroidal lesions (AUC >0.75, p< 0.05). Similarly, presence of incomplete retinal pigment epithelium (RPE) and outer retinal atrophy (iRORA) was 77% (95% CI: 55–92%) sensitive and 77% (95% CI: 53–92%) specific (AUC = 0.8, p+ 0.03) and presence of lumpy-bumpy choroid was 55% (95% CI: 32–76%) sensitive and 95% (95% CI: 76–99%) specific (AUC = 0.75, p= 0.04) to distinguish metastasis from other choroidal lesions. Mean height: base ratio was more in melanoma compared to metastasis (0.54 ± 0.22 vs. 0.43 ± 0.12, p= 0.17) and both were hyperechoic on USG. On FAF, 100% metastasis and 60% granulomas were hypoAF, whereas 100% melanomas were hypoAF. Majority choroidal lesions were hyperfluorescent on FFA. Double vascular circulation was observed in melanomas (50%). On ICG-A, all choroidal lesions were hypofluorescent. Conclusion:Clinical and multimodal imaging features such as the presence of IRF, shaggy photoreceptors, iRORA, and lumpy-bumpy choroid can help diagnose and differentiate amelanotic choroidal lesions, thereby avoiding the need for choroidal biopsy. Further larger studies are needed to devise a standard imaging protocol to validate our findings.

Abstract

Purpose: To evaluate the follow up and treatment outcome of central serous chorioretinopathy (CSCR) based on the new multimodal imaging-based classification and identify the predictors for anatomic and visual outcome. Methods: Retrospective, multicentric study on 95 eyes diagnosed with CSCR and a follow up of at least 12 months were included. Eyes with macular neovascularization, atypical CSCR or any other disease were excluded. Results: At the baseline, observation was advised to 70% eyes with simple CSCR whereas photodynamic therapy (PDT) was performed in 49% eyes with complex CSCR. Over the follow up, decrease in CMT was significantly higher in simple CSCR as compared to complex CSCR (P = 0.008) and the recurrences were significantly more in eyes with lower CMT at baseline (P = 0.0002). Median time of resolution of SRF was 3 months and 6 months in simple and complex CSCR respectively (P = 0.09). For the 12 months follow up, the median fluid free period was greater (P = 0.03) while number of interventions performed was lesser in eyes with simple CSCR as compared to complex CSCR (P = 0.006). Multiple regression analysis showed baseline best corrected visual acuity (BCVA) and baseline persistent SRF to be significantly predictive of BCVA and persistent SRF at 12 months (P < 0.0001, 0.04) respectively. Conclusions: Complex CSCR more often required PDT, was associated with shorter fluid free interval and longer time for SRF resolution. Baseline BCVA and persistent SRF were predictive of final visual and anatomical outcome. The new multimodal imaging based classification is helpful in establishing objective criteria for planning treatment approaches for CSCR. © The Author(s) 2021.
