Abstract

Power quality is a critical concern in modern electrical grids, particularly with the increasing integration of grid-connected photovoltaic (PV) systems. Voltage sags and swells, which are common disturbances in such systems, can significantly impact the stability and efficiency of power supply. This paper introduces an advanced approach to enhancing power quality in grid-connected PV systems by employing a Type II Fuzzy Logic Controller (FLC) with Gaussian membership functions and integrating a Dynamic Voltage Restorer (DVR). The Type II FLC is specifically designed to handle the inherent uncertainties and nonlinearities in PV systems more effectively than traditional controllers. By utilizing Gaussian membership functions, the controller offers precise and adaptive control, addressing the variations in solar power output and grid conditions. The DVR complements the FLC by injecting compensating voltage to mitigate disturbances such as voltage sags and swells, ensuring a stable voltage profile at the point of common coupling (PCC). Simulation results demonstrate a significant enhancement in power quality, with a reduction in total harmonic distortion (THD) by approximately 35% and voltage sags and swells mitigated by up to 45%. This study provides valuable insights into advanced control strategies, showing that the proposed system significantly improves the overall performance and reliability of PV systems, thereby contributing to the maintenance of optimal power quality in renewable energy grids. Abstract

Objectives: The severity of pulmonary artery hypertension affects the outcomes of patients undergoing mitral valve replacement (MVR). Speckle-tracking derived strain is a new modality for the assessment of the right ventricular (RV) function as well as the longitudinal contractile pattern. Our main objective of this study was to evaluate the right ventricular (RV) function in patients undergoing MVR with pulmonary artery systolic pressure (PASP) of more than 50 mmHg at 1 and 4 weeks postoperatively. Material and Methods: This prospective cohort study included 40 patients with rheumatic heart disease (RHD) (mitral stenosis [MS] and mitral regurgitation [MR]) scheduled to undergo MVR between January 2022 and December 2023 in AIIMS, New Delhi. Serial 2D echocardiography, tissue Doppler imaging, as well as RV speckle-tracking echocardiography were performed, and serum brain natriuretic peptide (BNP) levels were measured during the pre-operative period, 1st week and 4th weeks postoperatively to evaluate RV function. Results: Tricuspid annular plane systolic excursion (TAPSE) and left ventricular ejection fractions (LVEFs) were significantly lower at 1 week after surgery, compared to pre-operative levels, and reached the pre-operative values at 4 weeks after surgery. The RV fractional area change (RVFAC) at 4 weeks postoperatively significantly increased compared to 1st week and baseline values. The tricuspid valve (TV) tissue velocity in systole (S′), TV velocity in diastole (E′), RV global strain (RVGLS), and RV free wall strain (RVFWLS) at post-operative 4 weeks were lower than pre-operative values, and this difference was statistically significant. Conclusion: Four weeks after MVR, in adult patients with RHD (MS and MR) having PASP >50 mmHg, it was observed that deformational indices or speckle-tracking echocardiography such as RVGLS and RVFWLS predict better RV dysfunction than linear indices like TAPSE. The BNP, PASP (delta TR), right atrial volume, left atrial volume, LV internal diameter in systole, and LV internal diameter in diastole decreased significantly in these patients. Abstract

Introduction: Lung transplant (LTx) is a potential treatment option for all patients with chronic, end‑stage respiratory disease, who are refractory to optimal medical therapy or where no medical therapy exists. In India, LTx is still in its evolving stages and published literature is sparse. The current study was carried out to study the selection criteria for lung transplant and to evaluate the clinical and socio‑economic profile of patients referred for the same at a tertiary health care facility. Methods: The study was a descriptive, prospective, observational study. All adults referred for lung transplant were evaluated for clinical and laboratory profiles. All enrolled patients were assessed for presence of referral criteria, listing criteria, contraindications, and willingness for lung transplant. These patients were followed up for 2 years for transplant‑free survival, and the Cox proportional hazards model was used to determine independent predictors of all‑cause mortality. Results: A total of 103 were included in study. The most common diagnosis was interstitial lung disease (57.2%), followed by bronchiectasis (17.5%) and COPD (13.6%). Most patients were referred for LTx at an advanced stage as 90% met listing criteria. Fifty‑four (52.4%) patients had an absolute or relative contraindication to transplant; however, the majority of those contraindications were modifiable. Patients with a lower socio‑economic status were less likely to be willing for LTx. The median survival was 757 days. A 6‑minute walk distance (6MWD) lesser than 250 m was found to be an independent predictor of mortality. Conclusion: Making patients aware about lung transplant early in their treatment may give them sufficient time to come to terms with their disease and understand the risk and benefits associated. Efforts should be focused on screening and early treatment of reversible contraindications for the eligible patients. Patients with 6MWD < 250 m are at increased risk of mortality. Abstract

Symmetrical peripheral gangrene is an unusual complication of extracorporeal membrane oxygenation (ECMO) which can arise due to low cardiac output state, high inotropic support, inadequate anticoagulation, disseminated intravascular coagulation, and sepsis. In symmetrical peripheral gangrene, there is no evidence of large vessel occlusion or vasculitis. It can be due to the absence of pulsatile flow in the peripheral arteries that result in stasis and microthrombi formation which causes occlusion of small arterioles. A 10-month-old male child who underwent anomalous left coronary artery from pulmonary artery repair and required central venoarterial ECMO support post-surgery due to severe myocardial dysfunction developed symmetrical peripheral ischemia with gangrenous changes in the right-hand fingers while on extracorporeal membrane oxygenation (ECMO). The limb ischemic changes improved to normal after the child convalesced from heart failure. We describe these changes and the management here. [ABSTRACT FROM AUTHOR]

Abstract

This review investigated depth literature survey on the removal of various heavy metals and dyes contamination. The current study is a broad review of the different methods of preparation of biowaste adsorbent from rice husk and rice straw that has been implemented for the adsorption of many hazardous heavy metals and dyes in order to reduce their harmful effect on the environment. The selection of rice waste-based material for the adsorption process was considered due to its cost-effectiveness, easy availability, high adsorption efficiency, and reusability. This study is a comprehensive review of the adsorption of toxic heavy metals and dyes using rice husk and rice straw-based adsorbents either in bare or in modified forms under different physicochemical processes. In addition, some parameters affecting adsorption capability like pH, initial dye concentration, equilibrium time, temperature, adsorbent dosage, and shaking or stirring speed influence the adsorption mechanism have been discussed thoroughly. The applicability of various adsorption isotherm models and adsorption kinetic models for dye adsorption by various rice husk and straw biomass adsorbents is also reported here.
