Abstract

Glutathione (GSH) is often upregulated in cancer, where it serves to mitigate oxidative stress. γ-glutamyl-transferase (GGT) is a key enzyme in GSH homeostasis, and compared to normal brain its expression is elevated in tumors, including in primary glioblastoma. GGT is therefore an attractive imaging target for detection of glioblastoma. The goal of our study was to assess the value of hyperpolarized (HP) γ-glutamyl-[1-13C]glycine for non-invasive imaging of glioblastoma. Nude rats bearing orthotopic U87 glioblastoma and healthy controls were investigated. Imaging was performed by injecting HP γ-glutamyl-[1-13C]glycine and acquiring dynamic 13C data on a preclinical 3T MR scanner. The signal-to-noise (SNR) ratios of γ-glutamyl-[1-13C]glycine and its product [1-13C]glycine were evaluated. Comparison of control and tumor-bearing rats showed no difference in γ-glutamyl-[1-13C]glycine SNR, pointing to similar delivery to tumor and normal brain. In contrast, [1-13C]glycine SNR was significantly higher in tumor-bearing rats compared to controls, and in tumor regions compared to normal-appearing brain. Importantly, higher [1-13C]glycine was associated with higher GGT expression and higher GSH levels in tumor tissue compared to normal brain. Collectively, this study demonstrates, to our knowledge for the first time, the feasibility of using HP γ-glutamyl-[1-13C]glycine to monitor GGT expression in the brain and thus to detect glioblastoma.

Abstract

Paretic side collisions frequently occur in stroke patients, especially while walking through narrow spaces. We determined whether training for walking through an opening (T-WTO) while entering from the paretic side would improve collision avoidance behavior and prevent falls after 6 months. Thirty-eight adults with moderate-to-mild hemiparetic gait after stroke who were hospitalized in a rehabilitation setting were randomly allocated to the T-WTO (n = 20) or regular rehabilitation (R-Control; n = 18) program. Both groups received five sessions of 40 min per week, for three weeks total. T-WTO included walking through openings of various widths while rotating with the paretic side in front, and R-Control involved normal walking without body rotation. Obstacle avoidance ability, 10-m walking test, timed Up and Go test, Berg Balance Scale, Activities-specific Balance Confidence, the perceptual judgment of passability, and fall incidence were assessed. Collision rate and time to passage of the opening in obstacle avoidance task significantly improved in the T-WTO group compared with those in the R-Control group. Contrast, T-WTO did not lead to significant improvements in other outcomes. T-WTO improved efficiency and safety in managing subacute stroke patients. Such training could improve patient outcomes/safety because of the paretic body side during walking. Abstract

Muroi et al. show that individuals with stroke have improved collision avoidance behavior when passing through an aperture while entering from the paretic-side of the body. However, the underlying mechanism remains unknown. We reanalyzed Muroi et al.'s data to reveal how individuals with stroke walk through an aperture by examining changes in walking velocity and behavioral complexity (i.e., sample entropy, an index of (ir)regularity of time series, regarded lower entropy as more regular and less complex) by focusing on the approaching process. The results showed that individuals with stroke reduced their walking velocity and behavioral complexity before passing through the narrow aperture when approaching from the paretic side. We interpreted that the improved obstacle avoidance when penetrating from the paretic side may be due to careful body rotation and adjusting the walking velocity in advance. [ABSTRACT FROM AUTHOR]

Abstract

Dynamic nuclear polarization (DNP) is a cutting-edge technique that dramatically enhances the detection sensitivity of molecules using nuclear magnetic resonance imaging (NMR/MRI). This methodology enables real-time imaging of dynamic metabolic status in vivo using MRI. To expand the targetable metabolic reactions, there is a demand for developing exogenous, i.e., artificially designed, DNP-NMR molecular probes; however, complying with the requirements of practical DNP-NMR molecular probes is challenging due to the lack of established design guidelines. Herein, we report Ala-[1-13C]Gly-d2-NMe2 as a DNP-NMR molecular probe for in vivo detection of aminopeptidase N (APN) activity. We developed this probe rationally through precise structural investigation, calculation, biochemical assessment, and advanced molecular design to achieve rapid and detectable responses to enzyme activity in vivo. With the fabricated probe, we successfully detected enzymatic activity in vivo. This report presents a comprehensive approach for the development of artificially derived, practical DNP-NMR molecular probes through structure-guided molecular design.

Abstract

Aminopeptidase N (APN) is an enzyme which preferentially cleaves peptides with neutral residues at N-terminus. This peptidase has attracted many interests as an important biomarker of cancers because of its relationship with malignancy, metastasis and angiogenesis. However, APN-activity detection in deep areas has not been achieved although several kinds of fluorescent and luminescent probes have been developed to date1. Nuclear magnetic resonance (NMR) is a powerful technique to monitor molecules in a body because this technique has advantages in high tissue penetration and low invasiveness, allowing a molecular detection at deep area. However, the sensitivity of NMR is severely low for in vivo application. To overcome this problem, dynamic nuclear polarization (DNP) has emerged. NMR signals are dramatically enhanced by using this method. Previously, a DNP-NMR molecular probe for detection of APN-activity has been reported by our group2. This probe was designed in points of view of APN-selectivity and spin-lattice relaxation time (T1) related with hyperpolarized lifetime. By using this probe, APN-activity was successfully detected in experiments in vitro. However, this probe has not been applicable in vivo. In this study, we have developed a practical molecular probe for DNP-NMR to detect APN-activity in vivo through rational molecular design based on the previous probe. Abstract

Although the dissolution dynamic nuclear polarization approach overcomes the relatively poor MR sensitivity of in vivo metabolites, the limited number of available hyperpolarized 13C probes reminds a drawback. Here, we synthesized a novel gamma-glutamyl transferase (GGT) probe as a promising hyperpolarizing probe. It is well known that dysregulation of GGT activities in malignant cells leads to more aggressive phenotypes by producing reactive oxygen species. GGT is important for glutathione homeostasis and has been used as a diagnostic marker for various pathologies. Abstract

A 79-year-old Japanese woman presented with a rare case of metastatic renal cell carcinoma to the left sphenoid sinus with left nasal bleeding. She had previously had right radical nephrectomy for renal cell carcinoma at the age of 64 years and brain and spinal cord infarction at 74 years. Endoscopic examination revealed no mass in the nasal cavity. CT and MRI revealed a tumor in the left sphenoid sinus. The size of the tumor increased gradually from 12 to 15 years after the radical nephrectomy. Complete resection with endoscopic surgery was performed without preoperative embolization. The tumor cells had clear cytoplasm and were arranged in a trabecular pattern lined by a layer of endothelial cells. These findings were identical to the pathological findings of the surgical specimen of the renal cell carcinoma from 15 years previous. A pathological diagnosis of metastatic renal cell carcinoma of clear cell type (grade 1) was made. PET-CT demonstrated no metastasis. The patient’s condition was successfully managed with excision of the tumor, and she remains well with no evidence of recurrence and metastasis 36 months after treatment. Metastatic renal cell carcinoma to the sphenoid sinus is rare, but it might be considered in the differential diagnosis of masses in the paranasal sinus even long after initial treatment of renal cancer.

Abstract

A 71-year-old Japanese male patient presented with a rare case of Glomangiopericytoma (GPC) of the left nasal with obstruction. Complete resection with endoscopic surgery was performed. Immunohistochemical staining for smooth muscle actin, β catenin, cyclin D1, vimentin, and factor 13 were helpful in establishing a definitive diagnosis. Extranasal treatment has been traditionally performed for successful management. However, recent advances in endoscopic treatment have enabled complete endoscopic resection of GPC, minimizing morbidity and facilitating subsequent surveillance for recurrence. Endoscopic management should be considered in suitable cases.

Abstract

It is well known that dysregulation of γ-glutamyl transferase (GGT) activities in malignant cells lead to more aggressive phenotypes by producing reactive oxygen species. GGT is important for glutathione homeostasis, and has also been used as a diagnostic marker for various pathologies in the liver, biliary system, and pancreas. Here, for the first time, a novel hyperpolarized 13C probe, γ-Glu-[1-13C]Gly, was demonstrated in vivo tumor xenografts, including human pancreatic ductal adenocarcinoma and ovarian adenocarcinoma, to detect real-time γ-glutamyl transferase activities as a prospective biomarker for monitoring the tumor progression and prognosis with/without various cancer therapeutic approaches.
