We assessed the expected incidence rates of leukemia, malignant lymphoma, and multiple myeloma using national population estimates and data from the Kanagawa Cancer Registry.Patients and Methods: To evaluate the effects of community aging, we compared expected future incidences of hematological malignancies in Kanagawa with other three areas, namely the Yamagata, Osaka, and Nagasaki prefectures, which have different populations and predicted aging rates.Results: The total number of patients newly diagnosed with hematological malignancy in Kanagawa in 2010 was 1970. This was predicted to increase to 2581 by 2025 and to 2712 by 2040. Trends were very similar for all three hematological malignancies. These incidence rates were predicted to increase continuously in patients aged ≥ 65 years from 2010 to 2040, with a 169% increase in leukemia, a 167% increase in malignant lymphoma, and a 169% increase in multiple myeloma. A continuous increase in the population aged ≥ 65 years was also noted in the other three prefectures.Conclusion: The distribution demographic of patients with hematological malignancies is expected to change in the future as the number of elderly patients increases.Keywords: leukemia, myeloma, lymphoma, aging, hematopoietic stem cell transplantation

Abstract

Minute examination of local lattice structures in matter affected by impurity doping is of special importance for the development of functional materials. In order to obtain microscopic information on spinel ferrites, in the present work, we introduced nonmagnetic In3+ ions in Fe3O4 and probed their site selectivity and the doping effect on the local lattice structures and bulk magnetism by means of 57Fe Mössbauer spectroscopy and positron annihilation spectroscopies. The Mössbauer parameters of the area intensity and isomer shift (IS) show that In3+ ions predominantly reside in the tetrahedral A site, especially at low doping level. With increasing concentration of In ions, however, they gradually occupy the octahedral B site replacing Fe3+ ions. Along with the site information, the IS values confirmed that the introduced In ions squeeze the B-site Fe ions at their nearest neighbors. Supporting results were obtained from positron annihilation lifetime spectroscopy; positron lifetimes become shorter with increasing In concentration, signifying that the oxygen ions are pressed by the introduced In ions resulting in lowering the volume of the adjacent lattice vacancies. The results of Doppler broadening spectroscopy also support the squeezing effect; the positrons in the vacancies adjacent to In ions are more likely to annihilate with the inner shell electrons of the surrounding oxygen ions as a result of a reduction in the vacancy volume. Patients and methods: Flow cytometric analysis was performed to determine the expressions of PODXL and SCGB1D2 on the surface of cultured pancreatic cancer cells. Immunoblotting was performed to determine whether PODXL and SCGB1D2 were detectable in the media of cultured pancreatic cancer cells. A discovery-stage clinical study was performed in a cohort of 23 patients with pancreatic cancer and 51 control individuals without pancreatic disease who had been treated in the Department of Gastroenterology and Hepatology at Kochi Medical School Hospital from April 2014 to January 2016. Serum PODXL and SCGB1D2 levels were measured by enzyme-linked immunosorbent assay (ELISA). Results: PODXL and SCGB1D2 accumulated in the protrusions of cultured pancreatic cancer cells, and they were detectable both on the cell surface and in the cultured media from these cells. The discovery-stage clinical study showed that the area under the receiver-operating characteristic curve (AUC) was 0.96 (95% confidence interval [CI] 0.91–1.000) for PODXL, 0.80 (95% CI 0.67–0.94) for SCGB1D2, and 0.78 (95% CI 0.66–0.90) for CA19-9. The AUC for PODXL was thus significantly higher than that for CA19-9 (P = 0.006). The combination of SCGB1D2 with CA19-9 did not significantly increase the AUC (0.83; 95% CI 0.70–0.96) compared with the AUC for either SCGB1D2 or CA19-9 alone (P = 0.563). We present a geometric formulation of $(p,q)$-strings in which the $Sl(2;Z)$-doublet of the two-form gauge potentials is constructed as second order in the supersymmetric currents. The currents are constructed using a supergroup manifold corresponding to the $(p,q)$-string superalgebra, which contains fermionic generators in addition to the supercharges and transforms under the $Sl(2;Z)$. 12. Type-II Superstrings and New Spacetime Superalgebras

Abstract

We present a geometric formulation of type-IIA and -IIB superstring theories in which the Wess-Zumino term is second order in the supersymmetric currents. The currents are constructed using supergroup manifolds corresponding to superalgebras: the IIA superalgebra derived from M-algebra and the IIB superalgebra obtained by a T-duality transformation of the IIA superalgebra. We find that a slight modification of the IIB superalgebra is needed to describe D-string theories, in which the U(1) gauge field on the worldsheet is explicitly constructed in terms of D-string charges. Abstract

Co content (x) dependences of cationic distribution and local fields at the tetrahedral A-site and octahedral B-site nuclei in Co ferrites, CoxFe3−xO4 (x = 0–1.0), were investigated by means of hyperfine interaction techniques. A combined investigation by 57Fe transmission and 57Co emission Mössbauer spectroscopies revealed that about 6% of Co2+ ions occupy the A sites for the sample with Co contents of x = 0 and 1.0. This observation evidently suggests that the site selectivity of Co2+ ions is independent of x for samples prepared on the same heat treatment condition. For hyperfine interaction parameters, obvious x dependence was observed for the B-site Fe nuclei, reflecting variation of the atomic ratio of Fe3+/Fe2+ changing with x, whereas for the A-site Fe nuclei, the parameters hardly change along with x. Contrary to the x-independence for the A-site Fe nuclei, however, it was demonstrated by perturbed angular correlation spectroscopy with the nonmagnetic probe 111Cd(←111In) that the hyperfine field at the A site significantly changes along with x. These contradictory results on the A-site fields verify that the nonmagnetic probe can distinguish a subtle change in the local fields with higher sensitivity. [ABSTRACT FROM AUTHOR]

The early symptomatic syndromes eliciting neurodevelopmental clinical examinations-questionnaire (ESSENCE-Q) is a brief (12-item) screening questionnaire developed specifically for the purpose of speeding up the identification process of a wide variety of neurodevelopmental problems. The aims were to 1) estimate the reliability of the ESSENCE-Q, 2) evaluate the clinical cutoff levels suggested by the author of the ESSENCE-Q, and 3) propose optimal cutoff levels based on receiver operating characteristic analysis.Methods: The ESSENCE-Q was used for 1 year by a psychiatrist in Kochi, Japan, assessing children under the age of 6 years referred for developmental problems. The children were also clinically assessed with regard to whether or not they met criteria for a developmental disorder (diagnosis positive and diagnosis negative groups). We contrasted the results of the ESSENCE-Q and those of clinical diagnostic assessments in 130 cases.Results: Cronbach’s alpha was 0.82, sensitivity was 0.94 (95% confidence interval [CI]: [0.88, 0.98]), and specificity 0.53 (95% CI: [0.28, 0.77]), which are reasonable psychometrics for a first-step screening tool. Based on receiver operating characteristic analysis, we recommended an optimal cutoff level of yes ≥2 or maybe/a little ≥3 on the ESSENCE-Q (0.87 (95% CI: [0.79, 0.92]) sensitivity and 0.77 (95% CI: [0.50, 0.93]) specificity).Conclusion and implication: The ESSENCE-Q can be a good instrument for use as a screening tool for aiding in the process of early identification of neurodevelopmental disorders in clinical settings. To establish the broader validity and reliability of the ESSENCE-Q, case–control studies and general population studies of children in different age groups are needed. BackgroundPrior studies indicated a decrease in the incidences of aneurysmal subarachnoid haemorrhage (aSAH) during the early stages of the COVID-19 pandemic. We evaluated differences in the incidence, severity of aSAH presentation, and ruptured aneurysm treatment modality during the first year of the COVID-19 pandemic compared with the preceding year.MethodsWe conducted a cross-sectional study including 49 countries and 187 centres. We recorded volumes for COVID-19 hospitalisations, aSAH hospitalisations, Hunt-Hess grade, coiling, clipping and aSAH in-hospital mortality. Diagnoses were identified by International Classification of Diseases, 10th Revision, codes or stroke databases from January 2019 to May 2021.ResultsOver the study period, there were 16 247 aSAH admissions, 344 491 COVID-19 admissions, 8300 ruptured aneurysm coiling and 4240 ruptured aneurysm clipping procedures. Declines were observed in aSAH admissions (−6.4% (95% CI −7.0% to −5.8%), p=0.0001) during the first year of the pandemic compared with the prior year, most pronounced in high-volume SAH and high-volume COVID-19 hospitals. There was a trend towards a decline in mild and moderate presentations of subarachnoid haemorrhage (SAH) (mild: −5% (95% CI −5.9% to –4.3%), p=0.06; moderate: −8.3% (95% CI −10.2% to –6.7%), p=0.06) but no difference in higher SAH severity. The ruptured aneurysm clipping rate remained unchanged (30.7% vs 31.2%, p=0.58), whereas ruptured aneurysm coiling increased (53.97% vs 56.5%, p=0.009). There was no difference in aSAH in-hospital mortality rate (19.1% vs 20.1%, p=0.12).ConclusionDuring the first year of the pandemic, there was a decrease in aSAH admissions volume, driven by a decrease in mild to moderate presentation of aSAH. There was an increase in the ruptured aneurysm coiling rate but neither change in the ruptured aneurysm clipping rate nor change in aSAH in-hospital mortality.Trial registration numberNCT04934020.

Abstract

Background: The COVID-19 pandemic led to profound changes in the organization of health care systems worldwide. Aims: We sought to measure the global impact of the COVID-19 pandemic on the volumes for mechanical thrombectomy, stroke, and intracranial hemorrhage hospitalizations over a three-month period at the height of the pandemic (1 March–31 May 2020) compared with two control three-month periods (immediately preceding and one year prior). Methods: Retrospective, observational, international study, across 6 continents, 40 countries, and 187 comprehensive stroke centers. The diagnoses were identified by their ICD-10 codes and/or classifications in stroke databases at participating centers. Results: The hospitalization volumes for any stroke, intracranial hemorrhage, and mechanical thrombectomy were 26,699, 4002, and 5191 in the three months immediately before versus 21,576, 3540, and 4533 during the first three pandemic months, representing declines of 19.2% (95%CI, −19.7 to −18.7), 11.5% (95%CI, −12.6 to −10.6), and 12.7% (95%CI, −13.6 to −11.8), respectively. The decreases were noted across centers with high, mid, and low COVID-19 hospitalization burden, and also across high, mid, and low volume stroke/mechanical thrombectomy centers. High-volume COVID-19 centers (−20.5%) had greater declines in mechanical thrombectomy volumes than mid- (−10.1%) and low-volume (−8.7%) centers (p < 0.0001). There was a 1.5% stroke rate across 54,366 COVID-19 hospitalizations. SARS-CoV-2 infection was noted in 3.9% (784/20,250) of all stroke admissions. Conclusion: The COVID-19 pandemic was associated with a global decline in the volume of overall stroke hospitalizations, mechanical thrombectomy procedures, and intracranial hemorrhage admission volumes. Despite geographic variations, these volume reductions were observed regardless of COVID-19 hospitalization burden and pre-pandemic stroke/mechanical thrombectomy volumes. © 2021 World Stroke Organization.

Abstract

OBJECTIVE: To measure the global impact of COVID-19 pandemic on volumes of IV thrombolysis (IVT), IVT transfers, and stroke hospitalizations over 4 months at the height of the pandemic (March 1 to June 30, 2020) compared with 2 control 4-month periods. METHODS: We conducted a cross-sectional, observational, retrospective study across 6 continents, 70 countries, and 457 stroke centers. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases. RESULTS: There were 91,373 stroke admissions in the 4 months immediately before compared to 80,894 admissions during the pandemic months, representing an 11.5% (95% confidence interval [CI] -11.7 to -11.3, p < 0.0001) decline. There were 13,334 IVT therapies in the 4 months preceding compared to 11,570 procedures during the pandemic, representing a 13.2% (95% CI -13.8 to -12.7, p < 0.0001) drop. Interfacility IVT transfers decreased from 1,337 to 1,178, or an 11.9% decrease (95% CI -13.7 to -10.3, p = 0.001). Recovery of stroke hospitalization volume (9.5%, 95% CI 9.2-9.8, p < 0.0001) was noted over the 2 later (May, June) vs the 2 earlier (March, April) pandemic months. There was a 1.48% stroke rate across 119,967 COVID-19 hospitalizations. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was noted in 3.3% (1,722/52,026) of all stroke admissions. CONCLUSIONS: The COVID-19 pandemic was associated with a global decline in the volume of stroke hospitalizations, IVT, and interfacility IVT transfers. Primary stroke centers and centers with higher COVID-19 inpatient volumes experienced steeper declines. Recovery of stroke hospitalization was noted in the later pandemic months. © 2021 American Academy of Neurology.
