Rationale & Objective: Complementary and alternative medicine (CAM) intake is widely prevalent in many parts of India. Heavy metals are known ingredients in some of these formulations. We studied the spectrum of glomerular diseases in patients using CAM. Study Design: Case series. Setting & Participants: Patients with proteinuria or unexplained acute kidney injury, who underwent a kidney biopsy between May 2021 and September 2022, and who provided a history of recent CAM intake were included in the study. For patients enrolled prospectively, blood and urine samples were analyzed using mass spectrometry for the presence of mercury, lead, arsenic and cadmium. The CAM formulation, when available, was analyzed using inductively coupled plasma-optical emission spectroscopy. Results: Twenty-eight patients were enrolled in the study, with a median duration of CAM intake of 4 months (interquartile range, 2-6 months). Heavy metal screening was performed in 17 patients, of whom 15 had elevated urine mercury levels, 10 had elevated blood mercury levels, and 1 had elevated blood and urine arsenic levels. Of the 6 CAM formulations that were analyzed, all had high levels of mercury. Kidney biopsy findings were membranous nephropathy (n = 19), minimal change disease (n = 8), and mesangial proliferative glomerulonephritis (n = 1). Of the 19 patients with membranous nephropathy, 14 were associated with neural epidermal growth factor-like protein 1 (NELL-1). With conservative management alone, 17 patients achieved complete remission. Limitations: Not all patients underwent blood and urine mercury testing, and only 6 patients provided the CAM samples for analysis. Furthermore, occupational and residential exposure to mercury could not be excluded. Conclusions: The most common kidney pathology noted in our study was membranous nephropathy, which was predominantly associated with neural epidermal growth factor-like protein 1. A significant proportion of the patients recovered completely after withdrawal of the offending agent and initiation of renin-angiotensin system blockade. We describe the clinical spectrum of kidney disease, among patients who had recently ingested CAM. All patients underwent a kidney biopsy, and the most common finding was an entity called “NELL-1-associated membranous nephropathy,” which is known to be associated with heavy metal toxicity and CAM intake. Of 17 patients screened for such heavy metals, 15 had greater-than-permissible levels of mercury. Furthermore, 6 patients provided the CAM formulations that they had consumed for analysis: all contained high levels of mercury. Most such patients recovered following withdrawal of the offending CAM agent.

Background. Infection-related glomerulonephritis (IRGN) in adults is witnessing a dramatic shift in its epidemiology and outcome. Adult IRGN studies are all retrospective in nature, and Indian studies are scarce. Methods. We did this prospective study (September 2016-April 2018) on all patients with biopsy-proven IRGN and age ≥18 years satisfying three of five diagnostic criteria. Patients with persistent hypocomplementaemia (>3 months) were excluded. We did electron microscopy in those without a minimum of three diagnostic criteria and did an extensive search for any occult infection in every patient. Results. Forty-five patients were studied with a mean (SD) follow-up of 45.7 (20) weeks. Their mean age was 41.5 years (18-70 years), with a female preponderance (1:1.25). At presentation, the majority had oedema (100%), oliguria (84.4%), hypertension (80%) and haematuria (77.8%). Of them, 86.7% had renal insufficiency and 35.6% required dialysis. Only 53.3% of them had evidence of antecedent/ current infection, with skin/subcutaneous focus being the most common site. Hypocomplementaemia was present in 82.2% of patients. Salient pathological features were endocapillary proliferation (93.3%), neutrophilic infiltration (88.9%), presence of crescents (17.8%), interstitial infiltration (24.4%), moderate-to-severe interstitial fibrosis with tubular atrophy (IFTA; 15.5%) and underlying diabetic glomerulosclerosis (8.9%). Only 66.7% of patients made complete renal recovery. By logistic regression analysis, the predictors of poor outcome were a requirement for dialysis at presentation (p=0.04) and presence of IFTA (p=0.03). Conclusion. Nephrology services across the world, already struggling to cope up with chronic kidney disease (CKD) of epidemic proportions, faced enormous challenges during the COVID pandemic. SARS CoV -2 virus affects kidney directly and indirectly through systemic effects. Also, the pandemic impacted almost all aspects of renal care services in several ways. This review article aims to discuss the impact of COVID on kidney and renal care services under the following headings.

Context: Kidney transplant recipients (KTRs) represent a high-risk population but were grossly underrepresented in COVID-19 vaccination trials. Aims: The aim was to study the immunogenicity of two COVID-19 vaccines (Covaxin and Covishield) among KTRs. Subjects and Methods: We conducted a prospective observational study among KTRs who consented to receive two doses of the COVID-19 vaccine – either Covaxin or Covishield. Patients who underwent kidney transplant during the previous 6 months or had received antirejection therapy during the previous 1 month were excluded from the study. Antibody titers to the spike protein of COVID-19 were measured at baseline and at 4 weeks after the second dose of the vaccine. Statistical Analysis: A univariate analysis was performed in the subgroup of patients who were seronegative at baseline, to identify factors that were associated with seroconversion. The Chi-square test or the Fisher's exact test was used to compare categorical variables, and the Mann–Whitney U-test was used to compare continuous variables. P < 0.05 was considered statistically significant. Results: A total of 150 KTRs were studied, of whom 98 received Covaxin and 52 received Covishield. At baseline, even before vaccination, 63.3% of patients (n = 95) were found to be seropositive. Among those who were seronegative (n = 55) at baseline, seroconversion was noted in 34.5% of patients (n = 19), of which 15.7% (n = 3) had a robust response with antibody titers >250 U/mL. Conclusions: Among KTRs who were seronegative at baseline, a seroconversion rate of 34.5% was demonstrated after two doses of Covaxin or Covishield.

Hypokalemic paralysis is an important and reversible cause of acute flaccid paralysis. The treating physician faces unique diagnostic and therapeutic challenges. We did a prospective study and included all patients with acute flaccid weakness and documented serum potassium of <3.5 mEq/L during the period between January 2009 and August 2015. We studied the incidence, etiology, clinical profile, and acid--base disturbances in patients presenting with hypokalemic paralysis and analyzed the significance of periodic and non-periodic forms of hypokalemic paralysis on patient's outcome. Two hundred and six patients were studied with a mean follow-up of 3.6 ± 1.2 years. Mean age was 37.61 ± 2.2 years (range 18--50 years). Males were predominant (M:F ratio 2.1:1). The nonperiodic form of hypokalemic paralysis was the most common (61%). Eighty-one (39%) patients had metabolic acidosis, 78 (38%) had normal acid--base status, and 47 (23%) patients had metabolic alkalosis. The most common secondary cause was distal renal tubular acidosis (RTA) (n = 75, 36%), followed by Gitelman syndrome (n = 39, 18%), thyrotoxic paralysis (n = 8, 4%), hyperaldosteronism (n = 7, 3%), and proximal RTA (n = 6, 4%). Patients with non-periodic paralysis had more urinary loss (40.1 vs. 12.2 mmol, P = 0.04), more requirement of potassium replacement (120 vs. 48 mmol, P = 0.05), and longer recovery time of weakness (48.1 vs. 16.5 h, P = 0.05) than patients with periodic paralysis. Background: Hypertension is more prevalent risk factor for cardiovascular morbidity and mortality among renal transplant recipients. Materials and Methods: It is a retrospective study conducted to assess the prevalence, clinical profiles, and outcome of hypertension in renal transplant recipients. Posttransplant hypertension was defined as systolic blood pressure (BP) ≥140 mmHg, diastolic BP ≥80 mmHg, or the need of antihypertensive medication. Donor and recipient demographical details were obtained from medical records. Patients who underwent second renal transplant or graft nephrectomy were excluded from the study. Results: Among 375 patients, 88% were male. The mean age of our study population was 35.82 ± 9.37 years. Almost 82.67% of patients had posttransplant hypertension, of which 80.97% had pretransplant hypertension. Nearly 19.03% patients developed hypertension posttransplant. Following transplantation, hypertension resolved in 12.85%. The prevalence of well controlled, poorly controlled, and resistant posttransplant hypertension was 49.68%, 50.32%, and 7.42%, respectively. Majority of them (90.97%) received calcium channel blockers. In univariate analyses, recipient sex, pretransplant hypertension, etiology of kidney disease, female donors, female donors of male recipient's subgroup, use of cyclosporine, left ventricular mass, weight gain, and presence of metabolic syndrome were statistically significant. In multiple logistic regression analyses, recipient sex, presence of metabolic syndrome, and use of cyclosporine were associated with posttransplant hypertension. Kaplan–Meier analyses showed low graft survival in posttransplant hypertensive patients when compared to normotensives. Conclusion: The prevalence of hypertension in our renal transplant recipients was 82.67%. Aim: The aim was to study the clinical characteristics of postrenal transplant cytomegalovirus (CMV) infection and analyze its risk factors and its impact on graft and patient survival. Materials and Methods: We reviewed medical records of 739 renal transplant patients over 17 years (2002–2018). The demographic characteristics of patients were collected and compared with and without CMV infection. Multiple logistic regression analysis was done to identify risk factors for posttransplant CMV infection. Kaplan–Meier survival curve analysis was performed to analyze graft and patient survival by CMV infection. Results: The prevalence of CMV infection in our center was 12.4%. The most common presentation of CMV infection posttransplant is CMV syndrome. The use of antirejection therapy (hazard ratio [HR] 4.2, 95% confidence interval [CI] 2.6–6.9, P = 0.00), and new-onset diabetes after transplantation (NODAT) (HR 5.95, 95% CI 3.4–10, P = 0.00) was independently associated with postrenal transplant CMV infection. In Kaplan–Meier survival analysis, death-censored graft survival was significantly superior in patients without CMV infection/disease (CMV group: 55.4% vs. non-CMV group: 70.6% at 140 months P = 0.046). Patient survival was also significantly superior in patients without CMV infection (CMV group :59.8% vs. non-CMV group: 75.9% at 140 months P = 0.016). Conclusions: The use of antirejection therapy and NODAT are strong risk factors for developing CMV infection. Posttransplant CMV infection has a significant negative impact on graft and patient survival.

Snake bite is mainly an occupational hazard and causes serious health problems in rural India. Acute kidney injury (AKI) occurs in 5-30% cases. Renal pathologic findings include acute tubular necrosis, cortical necrosis, interstitial nephritis, glomerulonephritis, and vasculitis. Thrombotic microangiopathy (TMA) occurrence after a snake bite is reported rarely. Here, we present two patients who developed TMA after viper bite treated with hemodialysisand plasmapheresis. Renal biopsy showed fibrin thrombi in glomeruli and arterioles with cortical necrosis. One patient progressed to end-stage renal disease and other was lost to follow-up. TMA should be considered as a possible pathogenesis of AKI after snake bite. The role of plasma exchanges in snake bite TMA is yet to be defined. [ABSTRACT FROM AUTHOR]

Deceased donor renal transplantation (DDRT) constitutes less than 5% of all kidney transplantais in India. A retrospective analysis of 173 deceased donor renal transplants performed in a public funded government hospital was done. Mean age of the recipients was 36 years (male:female ratio 2.4:1), and that of the donors was 32.3 years (male:female ratio 6:1). The cold ischemic time was 340 ±170 minutes. Mean follow-up period was 36 months. Forty one patients died, 75% of them in the first post-transplant year. Sepsis and cardiovascular disease were the most common causes of death. Twenty two percent had acute rejection. There was no significant difference in the incidence in the rate of acute rejection, bacterial, fungal infections and death rate between the cohorts of induction and non induction immunosuppression. We conducted a prospective study to assess utility of detection of antibodies to phospholipase A2 receptor (PLA2R) in the serum of patients with membranous nephropathy. Seventy five patients with biopsy proven membranous nephropathy admitted between January 2011 and September 2014 were studied. Serum anti- PLA2R was tested by indirect immunofluorescence. The test was positive in 45 out of 60 patients with primary membranous nephropathy (PMN) and in none of the 15 patients with secondary membranous nephropathy, with a sensitivity of 75% and specificity of 100% for PMN. Anti PLA2R positivity also showed a significant correlation with quantum of proteinuria and negative correlation with serum albumin.This study has validated detection of serum anti PLA2R in PMN as a non invasive diagnostic tool in Indian patients. Systemic lupus erythematosus (SLE) and myasthenia gravis (MG) are two autoimmune diseases that have a higher incidence in young females, relapsing--remitting course, and positive antinuclear antibodies. SLE and MG are two different clinical syndromes, which can coexist or precede each other; however, their occurrence in the same patient is rare. We report a 38-year-old female with biopsy-proven lupus nephritis on steroids and cyclophosphamide, later developed MG. Nerve conduction studies showed the decremental response of 15%--25% over facial muscles with no decremental response over limb muscles. Although antianticholinesterase receptor (AchR) antibodies were negative, she was treated with oral pyridostigmine 60 mg twice daily and clinical improvement of ocular symptoms was seen within 48 h. At present, she is on oral prednisolone and mycophenolate mofetil with follow-up creatinine of 1.4 mg/dl and no neurological symptoms. [ABSTRACT FROM AUTHOR]

Aim: The aim was to study the incidence of delayed graft function (DGF) in living donor kidney transplantation (LDKT), its risk factors, and its impact on graft and patient survivals. Materials and Methods: We retrospectively analyzed the medical records of 543 patients over 12 years of age who had received kidney transplant from a living donor between 2007 and 2018. The demographic characteristics of the patients, acute rejection rates, graft function at 1 year, graft survival, and patient survival were compared between patients with and without DGF. Results: The incidence of DGF was 10.1% in our study. Cold ischemia time (CIT) (76.61 ± 33.66 vs. 53.02 ± 17.55 min; P < 0.01) and the use of tacrolimus (6.3% vs. 4.2%; P < 0.01) were associated with greater incidence of DGF. Patients with DGF had a longer duration of hospital stay (23.03 ± 11.37 vs. 13.98 ± 5.81 days; P < 0.01). There were no statistically significant differences in acute rejection rates, death-censored graft survival, and patient survival in the two cohorts. Conclusions: Our study reiterates that CIT is an important predictive factor for DGF. This is pertinent because CIT is a modifiable risk factor. However, DGF did not have a significant impact on patient and graft survivals in LDKT recipients.

Introduction: Direct-acting antivirals (DAAs) are widely used in the treatment of hepatitis C virus (HCV) infection in renal transplant recipients. Aim: The aim was to study the efficacy and safety of these drugs in our renal transplant recipients. Study Design, Subjects, and Methods: A retrospective observational study was performed among the renal transplant recipients > 18 years of age who were treated with DAA for HCV infection. The viral genotype, DAA regimen, the viral load at various time intervals, FibroScan score at the start and at the end of therapy, the changes in graft function (estimated glomerular filtration rate) and in the dosage of calcineurin inhibitors during therapy, and side effects if any during therapy were documented from history and transplant records. The viral remission rates and the safety of DAA were analyzed. Statistical analysis was done with Medcalc statistical software version 12.7.0.0. Results: Thirty-three recipients were included in the study. The DAA regimens were sofosbuvir + ledipasvir (n = 17), sofosbuvir + daclatasvir (n = 8), and sofosbuvir + ribavirin (n = 8). The most common genotype was genotype 1 (n = 30, 90.9%). End-of-therapy response and sustained viral remission (SVR) at 12 weeks of completion of therapy (SVR12) were 100% in all the three DAA regimens. About 75% (n = 6) of the patients who underwent ribavirin therapy developed anemia, unlike the ribavirin-free regimens which had no side effects. The graft function remained stable during DAA therapy. At a mean follow-up of 3 years after initiation of sofosbuvir + ribavirin therapy and 2 years after initiation of sofosbuvir + daclatasvir and sofosbuvir + ledipasvir therapy, the viral remission was sustained. Conclusion: DAAs are safe and effective in achieving and sustaining viral remission in renal transplant recipients.

Collapsing glomerulopathy (CG) is a distinct histopathologic pattern of glomerular injury characterized by global/segmental wrinkling of the glomerular basement membrane with podocyte hyperplasia and hypertrophy along with tubulointerstitial changes. There is no specific treatment for CG due to etiological heterogeneity, and newer insights into the pathogenesis may lead to the development of targeted therapy. The most common form of CG is the primary or idiopathic followed by secondary (due to viral infections, autoimmune disease, drugs, etc.) and genetic causes. Thrombotic microangiopathy (TMA) is characterized by microangiopathic hemolytic anemia, thrombocytopenia, and organ failure of variable severity. We here present two young women with preeclampsia who presented with acute kidney injury, anemia, and schistocytes in peripheral smear suggestive of TMA. Renal biopsy showed interesting histopathology of CG in addition to TMA in the first patient and CG alone in the second. Both the patients received supportive therapy while the first patient also received plasmapheresis. One patient had complete recovery, and other had partial recovery of renal function at last follow-up. Combined histopathological lesion of CG with TMA has never been reported in postpartum period so far in literature. [ABSTRACT FROM AUTHOR]

Nonadherence to immunosuppressant medications leading onto poor graft outcome is frequent among renal transplant recipients. In this study, we sought to assess the prevalence and correlates of nonadherence to immunosuppressants and its impact on graft function. A singlecenter, retrospective cum cross-sectional study of renal transplant recipients of age >18 years and who had completed at least six months after transplantation was performed. Nonadherence was assessed based on the Immunosuppressant Therapy Adherence Scale questionnaire. Factors attributed to nonadherence were assessed based on the Immunosuppressant Therapy Barriers Scale (ITBS) questionnaire. Social, economic, demographic data, and all transplant related information were recorded. Two hundred and seventy-nine patients were included in the study, of whom 78% were male. Median follow-up period was 46 months (interquartile range – 24 months to 82 months). Seventy-four patients (26.5%) admitted nonadherence to immunosuppressants. The nonadherence was significantly related to the male gender, late acute rejection episodes, rise in serum creatinine from > 0.5 mg/dL from nadir level, lower blood levels of calcineurin inhibitor, and higher ITBS scores. Refill rates and use of alarm reminders were not significantly associated with better adherence.

Rheumatoid arthritis (RA) is one of the commonest rheumatological diseases. Renal involvement is not common but can occur as a result of chronic inflammation as part of disease process or drug toxicity. Thrombotic microangiopathy (TMA) is characterized by microangiopathic hemolytic anemia, thrombocytopenia, and organ failure of variable severity. Only a few cases of TMA in patients with RAwere reported to date. We describe a 45-year-old female patient with RAwho presented with oliguria and edema. We report a 21-year-old male who developed end-stage renal disease, probably due to immunoglobulin A nephropathy (IgAN), received a renal transplant from his mother, which was lost due to crescentic IgAN after 18 months. Two years later, he received a second transplant from a deceased donor. He developed rapidly progressive graft dysfunction 3 years later. Allograft biopsy revealed crescentic IgAN, which was successfully treated with intravenous steroids and cyclophosphamide. Angiomyolipomas (AML) of the kidney are non-encapsulated benign neoplasms with the incidence of 45-80% in patients with tuberous sclerosis and 1-3% in sporadic cases. There are very few case reports in the literature in which kidneys with AML have been used for transplantation. We report here a 27-year-old female patient who received a live related renal transplant from her mother with isolated angiomyolipoma in donor kidney and on follow-up after 5 years, has stable graft function and tumor size. Post-infectious glomerulonephritis (PIGN) still remains one of the most common glomerulonephritis in the developing world. We studied the epidemiology and clinical spectrum of PIGN in adults to identify the clinical, biochemical and histological factors that would predict renal outcome. Data of 102 adult PIGN patients treated between 2009 and 2011 with a mean fol-low-up of 12 months (6-36 months) were analyzed retrospectively. The mean age of the patients was 32.7 ± 15 years, with a male to female ratio of 1.2:1. At presentation, 99% of the patients had edema and oliguria, 73% had hypertension, 55% had macrohematuria and 60% had nephrotic range proteinuria. About 14% presented with complications (pulmonary edema-6%, seizure-1%, dialysis requiring renal failure-7%) and 9% had comorbid illness. Sixty percent of the patients had serum creatinine >2 mg/dL at presentation, which was persistent in 30% at the end of one week and 68% had hypo-complementemia. Renal biopsy revealed diffuse proliferative glomerulonephritis in 70% of the patients. At 12 months, 2% had persistent hypertension, 10% had persistent proteinuria and hematuria and 11% had serum creatinine >1.5 mg/dL. Univariate analysis with the Fischer Exact test revealed age >40 years, male gender, serum creatinine >2 mg/dL at one week, comorbid illness, requirement of dialysis, crescents in >30% glomeruli and persistent proteinuria and microscopic hematuria at 12 months as significant risk factors for poor renal outcome. Serum creatinine >2 mg/dL at one week and persistent proteinuria at 12 months were the independent risk factors that predicted poor renal outcome at one year.

Introduction: Hemodialysis (HD) units require large quantities of water. To reduce water consumption without compromising the adequacy and safety of dialysis, we studied a novel HD prescription with high temperature and low flow dialysate., Methods: This was a single-center nonrandomized open-label cross-over pilot trial in patients with end-stage kidney disease on maintenance HD. Each participant was subjected to 3 different dialysis prescriptions for 1 month each as follows: (i) normal temperature with normal flow dialysate (NTNF prescription), (ii) high temperature with normal flow dialysate (HTNF prescription), and (iii) high temperature with low flow dialysate (HTLF prescription). The primary outcome, assessed at the end of each dialysis session, was the delivery of "adequate" dialysis, as defined by a single-pool Kt/V (spKt/V) ≥1.2. Outcomes were evaluated by comparing the NTNF and HTLF prescriptions., Results: A total of 863 sessions of HD were performed in 30 patients over 3 months, with 287 to 288 sessions in each of the 3 dialysis prescriptions. The primary outcome was not significantly different between the NTNF prescription (202 sessions [70.14%]) and the HTLF prescription (198 sessions [68.75%]) (odds ratio, 1.07; 95% confidence interval, 0.75 to 1.52; P  = 0.45). The mean spKt/V and urea reduction ratio (URR) were not significantly different. Clinically evident hemodynamic instability occurred in only 1 dialysis session in the HTNF prescription., Conclusion: Increasing dialysate temperature while reducing dialysate flow rate (Q D ) can be used as a water conservation strategy without compromising the adequacy and safety of dialysis in young and hemodynamically stable patients.
