10. Activities Forgone because of Chronic Breathlessness: A Cross-Sectional Population Prevalence Study

Abstract

Background: Chronic breathlessness is a prevalent disabling syndrome affecting many people for years. Identifying the impact of chronic breathlessness on people's activities in the general population is pivotal for designing symptom management strategies. Objective: This study aimed to evaluate the association between chronic breathlessness and activities respondents identify can no longer be undertaken (?activities forgone?). Design: This population-based cross-sectional online survey used a market research company's database of 30,000 registrants for each sex, generating the planned sample size?3000 adults reflecting Australia's 2016 Census by sex, age group, state of residence, and rurality. Setting/Subjects: The population of focus (n?=?583) reported a modified Medical Research Council (mMRC) breathlessness scale ?1 and experienced this breathlessness for ?3 months. Measurements: Activities forgone were categorized by mMRC using coding derived from the Dyspnea Management Questionnaire domains. Activities were classified as ?higher/lower intensity? using Human Energy Expenditure scale. Results: Respondents were male 50.3%; median age 50.0 (IQR 29.0); with 66% living in metropolitan areas; reporting 1749 activities forgone. For people with mMRC 1 (n?=?533), 35% had not given up any activity, decreasing to 9% for mMRC 2 (n?=?38) and 3% for mMRC 3?4 (n?=?12). Intense sport (e.g., jogging and bike riding) was the top activity forgone: 42% (mMRC 1), 32% (mMRC 2), and 36% (mMRC 3?4). For respondents with mMRC 3?4, the next most prevalent activities forgone were ?sexual activities? (14%), ?lower intensity sports? (11%), and ?other activities? (11%). Conclusions: People progressively reduce a wide range of activities because of their chronic breathlessness.

Abstract

Au Tchad, les entreprises publiques et parapubliques sont dans leurs majorités moins performantes. Elles sont caractérisées par un dysfonctionnement de leurs conseils d’administration (CA). Cette situation semble affaiblir l’efficacité du CA qui aurait une influence sur la performance de ces entreprises. Alors que le CA est l’un des mécanismes de gouvernance le plus approprié permettant d’apporter la solution à la performance des entreprises. Il semble que l’efficacité du CA qui impacte favorablement la performance dépend des spécificités cognitives de ces administrateurs. L’objectif de ce papier est de mettre en évidence l’influence de spécificités cognitives des administrateurs sur la performance financière des EPP au Tchad. Pour tester nos hypothèses en rapport avec les caractéristiques cognitives des membres du CA et la performance financière des EPP mesurée par ROA et ROE, nous avons utilisé le modèle de régression linéaire. A partir d’une enquête menée auprès des responsables de vingt (20) EPP au Tchad sur la période de 2017-2020, les résultats montrent qu’à l’exception des variables expérience et ancienneté des administrateurs, les autres variables explicatives de la performance et les variables du contrôle influencent positivement et significativement la performance financière des entreprises publiques et parapubliques au Tchad.

27. Same-day discharge after transvenous lead extraction: feasibility and outcomes.

Abstract

Aims Same-day discharge (SDD) is safe for patients undergoing electrophysiology procedures. There is no existing data regarding SDD for patients undergoing transvenous lead extraction (TLE). We report our experience with SDD for patients undergoing TLE. Methods and results The study group included patients undergoing TLE between February 2020 and July 2021 without an infectious indication. A modified SDD protocol for device implants/ablations was applied to TLE patients. Patient characteristics, extraction details, outcomes, and complications were reviewed. Of 239 patients undergoing TLE, 210 were excluded (94 infections and 116 did not meet SDD criteria). Of the remaining 29 patients, seven stayed due to patient preference and 22 were discharged home the same day. The SDD group had an average age of 65.9 ± 12 (47–84), 41% female, and LVEF of 52.2 ± 18% (10–80). The indication for TLE was malfunction (20), upgrade (4), advisory lead (2), and magnetic resonance imaging compatibility (1). Extractions included four implantable cardioverter-defibrillators (ICDs), 17 pacemakers (PPM), and one cardiac resynchronization therapy (CRT)-P system. The leads were 9.6 years (1.5–21.7) old, and 1.8 leads were removed per patient (1–3); the lead extraction difficulty (LED) score was 11.6 ± 7. Twenty underwent cardiovascular implantable electronic device (CIED) re-implantation (2 ICD, 3 CRT-D, 13 PPM, and 2 CRT-P). For CIED re-implants, patients sent a remote transmission the next day, and all patients received a next-day call. There were no procedure or device-related issues, morbidities, or mortalities in the 30 days after discharge. Conclusion Same-day discharge after TLE for non-infectious aetiologies is safe and feasible in a select group of patients with early procedure completion who meet strict SDD criteria. Abstract

Figure 1 sketches the basic graphene nanoribbon (GNR) fabrication steps for the prototypical armchair ribbon6 of width N = 7 obtained from 10,10'-dibromo-9,9'-bianthryl precursor monomers. Thermal sublimation ofthe monomers onto [...], Graphene nanoribbons--narrow and straight-edged stripes of graphene, or single-layer graphite--are predicted to exhibit electronic properties that make them attractive for the fabrication of nanoscale electronic devices (1-3). In particular, although the two-dimensional parent material graphene (4,5) exhibits semimetallic behaviour, quantum confinement and edge effects (2,6) should render all graphene nanoribbons with widths smaller than 10 nm semi-conducting. But exploring the potential of graphene nanoribbons is hampered by their limited availability: although they have been made using chemical (7-9), sonochemical (10) and lithographic (11,12) methods as well as through the unzipping of carbon nanotubes (13-16), the reliable production of graphene nanoribbons smaller than 10 nm with chemical precision remains a significant challenge. Here we report a simple method for the production of atomically precise graphene nanoribbons of different topologies and widths, which uses surface-assisted coupling (17,18) of molecular precursors into linear polyphenylenes and their subsequent cyclodehydrogenation (19,20). The topology, width and edge periphery of the graphene nanoribbon products are defined by the structure of the precursor monomers, which can be designed to give access to a wide range of different graphene nanoribbons. We expect that our bottom-up approach to the atomically precise fabrication of graphene nanoribbons will finally enable detailed experimental investigations of the properties of this exciting class of materials. It should even provide a route to graphene nanoribbon structures with engineered chemical and electronic properties, including the theoretically predicted intraribbon quantum dots (21), superlattice structures (22) and magnetic devices based on specific graphene nanoribbon edge states (3).

Abstract

Background The cyclin-dependent kinase inhibitor, p21, is well known for its role in cell cycle arrest. Novel anti-cancer agents that deplete iron pools demonstrate marked anti-tumor activity and are also active in regulating p21 expression. These agents induce p21 mRNA levels independently of the tumor suppressor, p53, and differentially regulate p21 protein expression depending on the cell-type. Several chelators, including an analogue of the potent anti-tumor agent, di-2-pyridylketone 4,4-dimethyl-3-thiosemicarbazone (Dp44mT), have entered clinical trials, and thus, their molecular mechanism of action is crucial to assess. Hence, this investigation examined how several iron chelators transcriptionally regulate p21 . Methods Promoter-deletion constructs; luciferase assays; RT-PCR; western analysis; gene silencing; co-immunoprecipitation. Results The transcriptional regulation of the p21 promoter by iron chelators was demonstrated to be dependent on the chelator and cell-type examined. The potent anti-cancer chelator, Dp44mT, induced p21 promoter activity in SK-MEL-28 melanoma cells, but not in MCF-7 breast cancer cells. Further analysis of the p21 promoter identified a 50-bp region between − 104 and − 56-bp that was required for Dp44mT-induced activation in SK-MEL-28 cells. This region contained several Sp1-binding sites and mutational analysis of this region revealed the Sp1-3-binding site played a significant role in Dp44mT-induced activation of p21 . Further, co-immunoprecipitation demonstrated that Dp44mT induced a marked increase in the interactions between Sp1 and the transcription factors, estrogen receptor-α and c-Jun. Conclusions and general significance Dp44mT-induced p21 promoter activation via the Sp1-3-binding site and increased Sp1/ER-α and Sp1/c-Jun complex formation in SK-MEL-28 cells, suggesting these complexes were involved in p21 promoter activation.
