Bei dem Transforming growth Faktor (TGF)-ß handelt es sich um ein lokales Zytokin, das im Zusammenhang mit Heilungsprozessen und Fibrosierung von Gewebe sowie mit relevanter Bedeutung beim kardialen Remodeling und beim Herzversagen nach Myokardinfarkt steht. Die derzeitige Datenlage bezüglich der Funktion von TGF-ß ist nach Ischämie unklar. Infolgedessen haben wir den Effekt der Hemmung von TGF-ß durch Verabreichung von blockierenden Antikörpern bei Mäusen nach Induktion eines Myokardinfarkts getestet. Die Mäuse wurden mittels einer intraperitonialer Injektion von Anti-TGF-ß-Antikörpern (5mg/kg Körpergewicht 1d11, Genzyme) oder Plazebo entweder eine Woche vor oder 5 Tage nach der Ligatur der Arteria coronaria sinistra behandelt. Es wurden regelmäßig Echokardiographien am 1., 21. und 56. Tag nach Myokardinfarkt durchgeführt. Die linksventrikuläre Dilatation war signifikant verschlechtert bei Mäusen mit Anti–TGF–ß-Antikörper Behandlung im Vergleich zur Kontrollgruppe. Die Mortalität war bei Mäusen mit Anti–TGF–ß-Antikörper Behandlung nach acht Wochen signifikant höher als in der Kontrollgruppe. Die Expression von Kollagen und Matrixmetalloproteinase bei den mit Anti-TGF-ß-Antikörpern behandelten Mäusen wurde reduziert. Die Expression vom proentzündlichen Zytokin TNF wurde durch die Behandlung jedoch nicht beeinflusst. Die Behandlung mit Anti-TGF-ß-Antikörpern vor oder nach einem Myokardinfarkt erhöht also die Mortalität und aggraviert das linksventrikuläre Remodeling, möglicherweise durch Veränderungen der Extrazellulärmatrix., Background: Transforming growth factor (TGF)-ß is a locally generated cytokine involved in healing processes and tissue fibrosis, all relevant for cardiac remodeling and the development of heart failure after myocardial infarction (MI). However, data regarding the function of TGF after ischemic injury are inconclusive. Thus, we tested the effect of TGF inhibition by application of a blocking antibody in mice with myocardial infarction. Methods and results: Starting 1 week before or 5 days after coronary artery ligation mice were treated with intraperitoneal injections of an anti-TGF-antibody (5mg/kg bodyweight 1D11, Genzyme) or placebo. Serial echocardiography was performed at days 1, 21, and 56 after myocardial infarction (MI). Mortality over 8 weeks was significantly higher in the groups treated with the anti-TGF antibody. Both, pre or post MI treatments were associated with increased left ventricular dilatation after MI as measured by echocardiography. In anti-TGF treated mice collagen and matrix-metalloproteinase expression were profoundly changed. However, the expression of pro-inflammatory cytokine TNF was not altered by the treatment. Conclusion: Anti-TGF pre- or post-MI treatment increases mortality and worsens left ventricular remodeling in mice after myocardial infarction potentially mediated by changes in extracellular matrix remodeling.
