Abstract

Most of the world population lives in urban areas, the share of the urban population is estimated to increase in the future decades. The complexity of different land uses and services in urban areas, which composes the built environment, influences human health and wellbeing. Concerning urban areas, to a large extent, spatial planning has the potential to shape the built environment to become healthier. The necessity of addressing the built environment is coherent with the crucial need for human health to act in all policies effectively and outside healthcare to prevent negative impact and promote positive impacts across multiple sectors. Research and practice already addressed diverse health challenges related to the built environment, grouped under the large umbrella of "urban health". Policies, tools, projects, and programs have already been created in separate sectors to integrate health in governance. Despite these efforts, health is often seen as a restrain to development in spatial planning, encountering resistance in each of its phases: diagnosis, formulation, implementation, monitoring, and evaluation. In this research, we focused on diagnosis as a crucial driver for understanding the relationship between the built environment and health and advocating for healthier cities through data and spatial analysis. The call for a built environment diagnosis aims to jointly address most of the relevant urban health issues linked to urban areas and within urban areas. The following concepts are required to comprehend better and address the healthiness of the built environment: (i) a structured framework to understand the stratification of multiple built environment determinants of health (ii) the spatial distribution of those determinants to understand inequalities within urban areas (iii) the integration of the expert's perspectives in addressing multiple urban health issues, (iv) the understanding of the geospatial links between the built environment and health data. The thesis aims to assess the spatial distribution of characteristics of the built environment related to health in the state of Geneva (Switzerland). This work employs a holistic approach to explore how inequalities in the health-related built environment are distributed in space and compare it with geospatial health data adopting global and local geospatial approaches. Using multiple spatial analysis methods for both built environment and health data and building a framework for a broad assessment of urban health, three hypotheses are tested in three sections. Initially we develop a conceptual framework for urban health issues. Then the framework is adapted and employed to diagnose the healthiness of the BE in the canton of Geneva using a local and global method to generate synthetic indicators. Then we analyse the stakeholder perspective on urban health issues and on the integration of health in planning. In the last section, we analyse the spatial distribution of health data and study whether the healthy BE provide a good estimate of Cardiovascular risk factors.

18. Ultrasound and FibroScan® Controlled Attenuation Parameter in patients with MAFLD: head to head comparison in assessing liver steatosis.

Abstract

Background: Controlled attenuation parameter (CAP) has been suggested as a new non-invasive measurement performed during transient elastography (TE) to assess liver steatosis. The aim of this study was to evaluate CAP values head to head with ultrasound (US) as reference standard. Methods: A consecutive cohort of patients attending abdominal US in an outpatient liver unit was included in this study with simultaneous CAP determination using the FibroScan® M probe and fibrosis scored by TE. Patients were subdivided in four groups on the basis of risk factors for Metabolically Associated Fatty Liver Disease (MAFLD). Results: Four hundred thirty-five patients were included in the analysis: 221 (51%) were male; 117 (26.9%) were in control group, 144 (33.1%) in group 2 with inactive HCV or HBV infection and at low-risk for MAFLD, 134 (30.8%) in group 3 at high-risk of MAFLD, 40 (9.2%) in group 4 at high-risk of MAFLD and concomitant inactive HCV or HBV infection. Liver steatosis detected with US evaluation was observed in the 41% of the entire cohort; in particular in the 3.4%, 20.1%, 83.6% and 87.4% of the group 1, 2, 3 and 4, respectively (p < 0.001). In patients at high-risk factor for MAFLD (group 3 and 4), CAP median levels were found statistically different among the severity-grading groups for US steatosis (S0 [n.27], ≥S1 [n.59], ≥S2 + S3 [n.89]), observing higher CAP levels in patients with a higher steatosis grade (≥S2 + S3 327.5 [±40.6] vs ≥S1 277.7 [±45.6] vs S0 245.1 [±47.4]; p < 0.001 for the whole cohort analysis) (p < 0.001 between ≥S2 + S3 and ≥S1) (p < 0.001 between ≥S2 + S3 and S0) (p = 0.004 between ≥S1 and S0). ROC analysis showed that the global performance of the CAP median level ≥ 258 to predict liver steatosis (S0 vs S1–3), was excellent with an Area Under the Curve (AUC) value of 0.87 [CI 95% 0. 835–0.904] with an 84% of sensitivity and a 78% of specificity, and a positive predictive value (PPV) of 73% and negative predictive value (NPV) of 88%. A TE-kPa median value <8.0 was detected in the 100%, 84%, 83.6% and 60% of patients in group 1, 2, 3 and 4, respectively. A TE-kPa median value >13.0 was detected in the 0%, 4.2%, 5.2% and 17.5% of patients in group 1, 2, 3 and 4, respectively. Conclusions: CAP values are strongly associated with the standard US criteria for different degree of steatosis. Integrating TE up to 5% of patients may be identified at risk for advanced fibrosis. Abstract

We assessed the efficacy of a brief, 8-h teaching program, using an objective structured clinical examination (OSCE) for the use of a pocket ultrasound device (echoscope) focused on bed side diagnosis of subclinical ascites in chronic liver disease.Ten cirrhotic patients, five with subclinical ascites and five without, were selected during ultrasound examination performed with a high performance equipment for screening of hepatocellular carcinoma. Five post graduate medical doctors residents in Gastroenterology or Internal Medicine underwent a brief, 8-h theoretical and hands-on examinations training. The methodology to assess the correct competence consisted in two phases defined by actions and answers to questions.The evaluation of the technical efficacy was positive in all five students. The diagnostic efficacy for the ten patients examined consisted in no false positive diagnosis of ascites and one false negative.In this preliminary experience, the use of a pocket ultrasound device (echoscope) for diagnosis of subclinical ascites in the context of a short structured teaching program and examination was efficient.Abbiamo valutato l’efficacia didattica di un breve corso della durata di 8 ore secondo l’esame clinico strutturato ed oggettivo (OSCE) per l’uso di un ecografo tascabile (ecoscopio) per la diagnosi al letto dell’ascite subclinica in pazienti con epatopatia cronica. Pazienti e Metodi: Dieci pazienti cirrotici, cinque con ascite subclinica e cinque senza, sono stati selezionati durante esami ecografici effettuati per la sorveglianza dell’epatocarcinoma con apparecchiatura ecografica di alto livello. Cinque medici specializzandi in Gastroenterologia o Medicina Interna hanno ricevuto otto ore di insegnamento teorico e pratico. Il metodo per accertare la competenza acquisita ha consistito in due fasi definite da specifiche azioni e risposte a domande. Risultati: La valutazione della efficacia tecnica ha dato risultato positivo in tutti gli studenti. La efficacia diagnostica sulla presenza o assenza di ascite subclinica nei dieci pazienti esaminati è risultata in nessun caso falso positivo e un caso falso negativo. Conclusioni: in questa esperienza preliminare l’uso di un ecografo tascabile appreso nel contesto di un breve programma didattico strutturato per la diagnosi di ascite subclinica si è dimostrato efficace.

Abstract

Background & Aims Bacterial strains resistant to antibiotics are a serious clinical challenge. We assessed the antibiotic susceptibility of bacteria isolated from infections in patients with cirrhosis by a multicentre investigation. Results Three hundred and thirteen culture-positive infections (173 community acquired [CA] and 140 hospital acquired [HA]) were identified in 308 patients. Urinary tract infections, spontaneous bacterial peritonitis and bacteremias were the most frequent. Quinolone-resistant Gram-negative isolates were 48%, 44% were extended-spectrum beta-lactamase producers and 9% carbapenem resistant. In 83/313 culture-positive infections (27%), multidrug-resistant agents (MDRA) were isolated. This prevalence did not differ between CA and HA infections. 28. Ultrasound-guided insertion of the Elipse® gastric balloon: technical details, learning curve, and perioperative outcome in 36 cases.

Abstract

Aim: Our aim is to demonstrate the feasibility of real-time ultrasound-assisted insertion of the Elipse® intragastric balloon for the treatment of overweight and obese patients. Methods: A plastic gastric phantom filled with water was created to mimic the gastric lumen and to test the operator's ability to recognize the capsule containing the balloon inside. In the clinical phase, we tested the operator's ability to recognize the swallowed capsule and its progressive filling in the gastric lumen by means of ultrasound in 36 consecutive patients with a mean body mass index of 35 in an outpatient setting. Results: The ultrasound hyperechoic signal of the capsule in the gastric lumen was visible early on and was confirmed after the injection of a few milliliters of saline solution in the capsule with a cyst-like shape. The insertion of the balloon was successful in all cases at the first attempt, and the complete filling was monitored in real-time without the need for fluoroscopy. Postprocedure symptoms were limited to the first 24–48 h and were controlled by symptomatic therapy; endoscopic removal of the balloon because of painful distal gastric migration was necessary in one case after 3 months. Conclusion: After a learning curve, we were able to demonstrate the placement of the balloon capsule inside the fundus of the stomach under ultrasound guidance without fluoroscopy in all patients in an outpatient setting. Abstract

Background: Combination of the antiviral drug amantadine with interferon (IFN) may be more effective than IFN monotherapy for treatment of chronic hepatitis C. Methods: We randomised 93 patients with chronic hepatitis C to IFNIFN 6 MU 3 times/week for 24 weeks, followed by IFN 3 MU 3 times/week for further 24 weeks given in combination with amantadine 100 mg t.i.d. (regimen A, n=48) or as monotherapy (regimen B, n=45). Control liver biopsies were obtained 6 months post treatment. Results: At the end of the trial a similar proportion of patients had normal serum ALT levels (35% for regimen A, and 44% for regimen B) as measured by intention to treat criteria. Sustained biochemical response at 6 months post treatment was 15 and 20%, and sustained virological response was 10 and 20% for regimen A and B, respectively. The effect on liver histology was also similar: the inflammatory components of the Knodell score decreased by 1.3±0.6 points for regimen A and by 1.6±0.6 for regimen B, and score for fibrosis remained unchanged with both regimens. Conclusions: Combination of IFN therapy with amantadine does not enhance the effect of IFN as shown by biochemical, virological and histological criteria. [Copyright &y& Elsevier]
