Abstract

Background and aim: Over 80% (365/454) of the nation’s centers participated in the Italian Society of Nephrology COVID-19 Survey. Out of 60,441 surveyed patients, 1368 were infected as of April 23rd, 2020. However, center-specific proportions showed substantial heterogeneity. We therefore undertook new analyses to identify explanatory factors, contextual effects, and decision rules for infection containment. Methods: We investigated fixed factors and contextual effects by multilevel modeling. Classification and Regression Tree (CART) analysis was used to develop decision rules. Results: Increased positivity among hemodialysis patients was predicted by center location [incidence rate ratio (IRR) 1.34, 95% confidence interval (CI) 1.20–1.51], positive healthcare workers (IRR 1.09, 95% CI 1.02–1.17), test-all policy (IRR 5.94, 95% CI 3.36–10.45), and infected proportion in the general population (IRR 1.002, 95% CI 1.001–1.003) (all p < 0.01). Conversely, lockdown duration exerted a protective effect (IRR 0.95, 95% CI 0.94–0.98) (p < 0.01). The province-contextual effects accounted for 10% of the total variability. Predictive factors for peritoneal dialysis and transplant cases were center location and infected proportion in the general population. Using recursive partitioning, we identified decision thresholds at general population incidence ≥ 229 per 100,000 and at ≥ 3 positive healthcare workers. Conclusions: Beyond fixed risk factors, shared with the general population, the increased and heterogeneous proportion of positive patients is related to the center’s testing policy, the number of positive patients and healthcare workers, and to contextual effects at the province level. Nephrology centers may adopt simple decision rules to strengthen containment measures timely.

Abstract

In this paper we study the evolution of a comet nucleus taking into account its orbital evolution. It is assumed that the nucleus undergoes several close encounters with Giant Planets and its final orbit is that of the near-Earth object 4015 Wilson-Harrington. In paper I we presented results of a comet evolution model establishing the conditions under which the nucleus becomes dormant or extinct; we also identified a range of physical conditions leading to long-lasting activity that inexorably lead to the consumption of the icy material. The assumption in paper I was that the nucleus model is composed of a mixture of ices of water, C[O.sub.2], and CO and dust particles; the [H.sub.2]O ice is initially crystalline. In this paper we analyze if the initial status of ice, the presence of CO, and the spin period affect the evolutionary history of the body, and if part of the original material is preserved in the inner part of the nucleus when the comet becomes a short-period comet.

Abstract

What is the ultimate fate of a comet? Excluding impacts with other bodies, two possibilities are foreseen: either long-lasting activity, accompanied by nucleus reduction, or the formation of a stable crust that inhibits dust emission and strongly reduces volatile emissions. In the first case the comet could disintegrate, whereas in the second case it could become dormant or extinct, assuming an asteroidal appearance; in both cases the comet could be reactivated. In this paper we present results of a comet evolution model trying to establish the conditions under which the nucleus becomes dormant or extinct and under which it continues its activity up to the consumption of the icy material. Our nucleus model is composed of a mixture of ices of water, C[O.sub.2], and CO and dust particles. The [H.sub.2]O ice can be either amorphous or crystalline; the solid matrix is assumed to be porous. The evolution of the body is determined by the solar energy reaching its surface and by the heat transfer in the interior. The propagation of the heat through the nucleus is modeled by means of the heat transfer and gas diffusion equations, coupled via the condensation-sublimation terms that are seen as sinks or sources of energy and matter, respectively. Particular attention is given to the variations of porosity and to the changes in composition of the superficial layers due to sublimation-condensation phenomena, to gas diffusion processes through the pore system, and to the ejection of dust particles. At the beginning of the evolution of the nucleus the crust is never present and the interior of the comet is not differentiated. We have seen that the evolution can proceed essentially in two ways: (1) if the body is dark and rich in volatiles and the dust grains are fluffy, then the upper layers are usually removed at the same rate at which the C[O.sub.2] upper boundary sinks; (2) in the opposite case, sometimes a dusty crust is formed, sometimes not, but in any case the C[O.sub.2] interface sinks deeply. We conclude that in the first case the body will remain active for several orbits, while in the second case the gradual reduction of any activity can produce a dormant or extinct comet, maybe with the appearance of an asteroid.

Abstract

BACKGROUND: Relative survival, a methodology previously used in epidemiologic studies of cancer, compares the observed survival of a patient cohort with expected survival derived from general population life tables. We examined relative survival in patients treated by long-term dialysis in the Italian Dialysis and Transplantation Registry in order to determine the prognosis of dialysis patients. STUDY DESIGN: Cohort study drawn from a registry. SETTING & PARTICIPANTS: Patients enrolled in the Italian Dialysis and Transplantation Registry. FACTORS: Sex, age, primary kidney disease, renal replacement therapy modality, and main comorbid conditions. OUTCOMES: Death from any cause. MEASUREMENTS: Relative survival ratio (the ratio of observed survival in the population of interest to the survival expected given the age- and period-specific mortality of the general population) and excess mortality rate (difference between observed and expected mortality rates). RESULTS: In January 2000 to December 2008, a total of 27,642 patients were included. The 5-year relative survival estimate was 55.6% (95% CI, 54.7%-56.5%). The excess mortality rate showed a peak at 3 months (21 deaths/100 patient-years), then decreased, becoming constant from the end of year 1 to year 8, with leveling off at about 10 deaths/100 patient-years. Older age, systemic diseases, and diabetes showed the strongest association with excess mortality. Peritoneal dialysis was associated with a lower relative excess risk in only the first year of treatment. LIMITATIONS: The patient cohort comprises about half the Italian patients beginning dialysis therapy in the period. CONCLUSIONS: This study highlights the applicability of relative survival methods in dialysis patients. This measure allows estimation of disease prognosis and severity comparisons among chronic diseases. The excess mortality rate appears to be a more sensitive and informative measure than the simple proportion of survivors.

Abstract

L’Eco-design, ovvero una progettazione orientata ai principi della sostenibilità ambientale, rappresenta uno strumento di importanza strategica, in quanto consente di prevenire, invece di rimediare, le conseguenze ambientali negative della produzione, dell’uso e dello smaltimento dei prodotti, contribuendo a definirne, oltre che la configurazione formale e tecnica, il profilo ambientale complessivo (qualità e quantità dei materiali impiegati; tecnologie e processi di produzione; modalità d’uso e manutenzione; modalità e scenari di dismissione, ad esempio riuso, riciclo, recupero energetico, compostaggio, ecc.). Infatti definire gli obiettivi e i requisiti ambientali di un prodotto, fin dalla fase di “design”, di ideazione, determina significativi effetti positivi durante tutte le altre fasi del processo di sviluppo del prodotto stesso. La crescente convinzione dell’importanza e dell’urgenza di saper organizzare forme di sviluppo sostenibile, nel rispetto delle limitate risorse ambientali e della salute umana, ha orientato la ricerca verso l’utilizzo delle fibre vegetali.

Abstract

The Italian Society of Nephrology (SIN) promoted a national survey in order to obtain detailed information from all Renal and/or Dialysis Units using the on-line questionnaire (158 items) regarding structural and technological resources, medical workforce organisation and activity features. The purposes of this initiative were to obtain regional benchmarks as references for renal units and to describe the current Italian renal network in order to plan further interventions for the next 5 years. In this paper data of the first three Italian Regions (Piemonte, Liguria and Valle d'Aosta) which completed the survey (100% of the units) are reported. Main findings in the 3 Regions. A) Epidemiology: prevalence of dialysis patients = 709, 720, 787 pmp (per million population); prevalence of transplanted patients = 325, 387, 279 pmp; incidence of dialysis patients = 166, 191, 156 pmp; gross mortality of dialysis patients = 13.7, 15.0, 13.0%; distribution of vascular access in prevalent dialysis patients: arteriovenous fistula = 74, 83, 76%, central venous catheter = 18, 12, 15%, vascular graft = 8, 5, 9%. B) Structural resources: hospital's number of beds = 49, 72, 49 pmp, dialysis places = 166, 158, 164 pmp. C) Personnel resources: renal physicians = 44, 47, 41 pmp, renal nurses = 186, 194, 205 pmp; each renal physician takes care of 16, 15, 19 dialysis patients and each renal nurse cares for 3.8, 3.7, 3.8 dialysis patients. D) Activity: admission to hospital = 1507, 2392, 1606 pmp, renal biopsies = 109, 133, 57 pmp. Despite discrepancies in population density in the three Regions, most indexes are surprisingly similar and show the satisfactory level of renal care attained in the Northwestern Italian area. Further improvements in health care management can be predicted as a consequence of a direct comparison between needs and results in the various Regions of the Country.

Abstract

The Italian Registry of Dialysis and Transplantation (RIDT) was born in 1996 under the aegis of the Italian Society of Nephrology, and it is organized as a federation of regional registries. This study aimed to completely revise the epidemiological data collected during the first 5 yrs (1996-2001) of RIDT activity to evaluate the trends of the main epidemiological features. During this period, regional registries were not always able to assure complete and exhaustive information according to RIDT requirements, owing to different levels of organization and functioning. To avoid any possible error in data analysis, information inadequately assessed was refused. The incidence of end-stage renal disease (ESRD) patients on renal replacement therapy (RRT) in Italy has increased from 114 pmp in 1996 to 139 pmp in 2001, that means an increase of 3.5%/yr, corresponding to 5718 patients during 1996 and 8000 patients during 2001. Primary renal diseases (according to the EDTA) in incident ESRD patients are vascular and diabetic nephropathy. Main dialysis modality in incident patients was hemodialysis (HD) (85%), while peritoneal dialysis (PD) was only 15%; pre-emptive transplantation was a very unusual modality. The prevalence of ESRD patients at 31 December was 693 pmp in 1996 and 827 pmp in 2001; among dialysis patients, the corresponding rates were 575 pmp and 657 pmp, respectively. Consequently, the number of dialyzed patients increased, respectively, from 28892 to 37919. The prevalent dialysis modality was bicarbonate dialysis in 74% of cases, followed by hemodiafiltration (HDF) in 15%, continuous ambulatory peritoneal dialysis (CAPD) in 7% and APD in 3%. The gross mortality rate in dialyzed patients was stable during this period, at approximately 14%, the main causes of death being cardiovascular diseases and cachexia.

Abstract

Aim of this study is to analyse the incidence of diabetic patients starting dialysis in Piedmont (Italy) during the period 1981-1996 and to evaluate, in a subgroup of patients, the causes of uremia (diabetic nephropathy or other), and the type and seriousness of comorbid factors, in order to define the clinical conditions and try to explain the causes incidence increase.Data are taken from the RPDT (Regional Registry of Dialysis and Transplantation of Piedmont).Total incidence of new patients starting dialysis in this Region increased from 65 pmp in 1981-1982 to 116 pmp in 1995-1996 and the mean age increased from 55.4 +/- 15.5 years in 1981-1982 to 61.5 +/- 15.6 in 1995-1996; 49% of patients had at least one of the 13 conditions of comorbidity considered by the Registry (including severe vascular, cardiac, systemic diseases and diabetes). In the same period the incidence of diabetic patients increased from 6 pmp (1981-1982) to 19 for males and 12 pmp for females (1995-1996); this increase is higher for males and limited to patients with ageor = 60 years (for example: group 70-79 years from 7 to 56 pmp). A study performed in a group of 64 patients (52 type 2 and 12 type 1) showed the incidence of multiple comorbid factors: the most important in type 2 are vascular diseases (44/52) and heart diseases (20/52); blindness and amputations are relatively rare (2 each). An important comorbid factor in type 1 diabetes is blindness (3/12) and in this group the most frequent cause of uremia is diabetic nephropathy (DN) (9/12); in the group of type 2 patients nephroangiosclerosis and a clinical picture of progressive renal failure, without nephrotic syndrome, represents 48% of all diagnoses.These results underline the necessity of a strict collaboration with diabetologists and of an improvement of dedicated structures in order to meet the increase of this critical population.

Abstract

The parameters used at present by the Dialysis and Transplant Registries of various countries to evaluate dialyzed patients' comorbidity show great differences, mostly owing to the different epidemiological, social and racial characteristics of the studied populations. Moreover, the typology of the dialyzed patient is changing: the mean age is increasing, patients with high-risk conditions as vasculopathy and diabetes are widely accepted to the treatment. Thus the Piedmont Registry will be implemented as follows: new fields about comorbidity for clinical (blindness, cachexy and dementia), social (smoking, alcohol and drugs addiction) and diagnostic (type of diabetes, of neoplasm, of cardiovascular problem) parameters, and questions needing dichotomic response (vasculopathy yes or no) will be added. The exact time of appearance of any risk factor will be requested, and a field for "others" risk factors will be added as well, trying to deeply identify the dialytic population not affected by any comorbidity factor. Finally, a new improved control system of the collected data will be used: our registry needs to be implemented in the future by such evaluations, to go on giving useful informations about epidemiology of the dialyzed patients.

Abstract

In the Italian language, the term cachexia is a rather picturesque synonym of "marasma senile", "senile marasmus", an old definition involving not only old-age, but specifically senility, the end of the ageing process and marasmus, a stagnant and hopeless situation in which all superior organised functions have disappeared. The problem of cachexia during dialysis is complex and several discordant opinions exist at this regard, partly accounted by different definitions of this sluggish entity (or non entity). Actually, the basic question is very simple: is cachexia the cause or the effect of failure of dialysis treatment? The aim of this study was an evaluation of epidemiological data from the Dialysis and Transplantation Registry of Piedmont, a northern Italian Region with about 4,350,000 inhabitants, 22 public dialysis Centers, open acceptance to dialysis since the mid seventies, a multiple choice dialysis system developed in the eighties. In the period 1981-1995, 764 patients died in conditions of cachexia. This figure is 20.9% of all deaths recorded, 27.4% over age 65 and 34.7% over age 75. Despite a likewise significant increase in age and presence of comorbid factors, an improvement of patients survival, that reach statistical significance in the old age group (or = 65 yrs), was observed.
