Food Insecurity constitutes a significant challenge for public health, since it is experienced mainly in individuals belonging to vulnerable and historically backward groups, such as the case of Mexican indigenous groups. The structuralist theory of Lévi-Strauss in 1969, proposes a perspective approach to nutrition and its complexity as a process socially affected by macro and micro influences that impact the nutrition of family groups. The article objective was to describe food insecurity in indigenous Yoreme-Mayo adults of northwest Mexico. The study was descriptive and transversal study. The sample is 212 adults aged 18 years and older who live in a Yoreme-Mayo indigenous community. Sampling was by snowball. The Mexican Food Safety Scale, anthropometric measurements of weight, height and waist circumference and a sociodemographic data sheet were used. An average age of 38.78 (18 - 78) was found, men predominated (f = 115; 54.2%). The activities with salary were the domain occupation (f = 151; 71.2%) and 82.1% (f = 174) received less than 6,000 pesos M.N. monthly. Likewise, the general prevalence of food insecurity was 89.2% (f =189) where the most affected groups were women (f = 87, 51.8%), people aged 18 to 29 (f = 67, 31.6%) and individuals with high BMI (f = 120, 56.6%). Conclusion: Food insecurity occurs in the Yoreme-Mayo ethnic group without distinction of social and demographic characteristics. Correlation studies are recommended to identify factors that are related to the phenomenon studied. [ABSTRACT FROM AUTHOR]

La salud bucal colectiva es una perspectiva contrahegemónica de pensamiento y acción en salud bucal, en donde se integran posiciones políticas y experiencias comunitarias en la Atención Primaria en Salud. Objetivo: Caracterizar la experiencia de integración de la perspectiva de la Salud Bucal Colectiva en una estrategia de Atención Primaria en Salud dirigida a las comunidades Pijao de Ortega, Tolima. Métodos: Reflexión derivada de una sistematización etnográfica sobre la experiencia de la Facultad de Odontología de la Universidad Nacional de Colombia con comunidades indígenas Pijao entre 2016 y 2021. Resultados: Se integró la perspectiva de salud bucal colectiva en acciones de atención primaria en salud, principalmente de salud oral y comunicativa, con un enfoque en el buen vivir de las comunidades. Esto incluyó acciones de promoción y educación en salud, y la exigencia del derecho a la salud bucal desde un análisis de los modos de vida desde la determinación social de la salud. Se utilizó la interculturalidad como base del diálogo con las comunidades y sus servicios de salud propios, y se construyeron proyectos conjuntos de cuidado de la naturaleza, desde la participación comunitaria y la formación de estudiantes, fortaleciendo sus capacidades de participación en acciones colectivas en salud. Conclusiones: La salud bucal colectiva se integró en la experiencia de atención primaria en salud con comunidades Pijao del municipio de Ortega, mediante la integración de principios teóricos, metodológicos y praxeológicos que potenciaron una gestión alternativa de la salud de las comunidades comprometida con el buen vivir.

5. Factores asociados con la infección por el virus de la hepatitis B en comunidades indígenas de Colombia

Introducción. Colombia alberga dos millones de indígenas, que viven en condiciones de pobreza y tienen deficiencias en salud, por lo cual están expuestos a contraer infecciones virales como la hepatitis B. El departamento del Amazonas presenta una gran prevalencia del virus y barreras para acceder a la vacunación; por esto, parte de la población es propensa a la infección. Objetivo. Identificar factores asociados con la infección por el virus de la hepatitis B en indígenas colombianos. Materiales y métodos. Se llevó a cabo un estudio de casos y controles en mayores de 18 años de cuatro departamentos del país. Los casos se identificaron mediante el registro nacional de notificación de hepatitis B (2015-2022). Los controles seleccionados de manera concurrente fueron pareados con los casos por edad, sexo, etnia y departamento. En una encuesta se consignaron las características sociodemográficas, los factores asociados con el contacto con sangre y fluidos, las prácticas socioculturales y los antecedentes de vacunación. El proyecto fue aprobado por Comité de Ética de la Universidad de Antioquia. Resultados. Participaron 75 casos y 150 controles de 13 grupos étnicos. El departamento del Amazonas aportó el 49 % de los participantes (83 % mujeres) con una mediana de edad de 30 años (RIC = 27-37). Los factores asociados con una mayor probabilidad de contraer la infección fueron el antecedente de algún familiar infectado con el virus de la hepatitis B (OR ajustado = 2,61) (IC95%: 1,09-6,27) y número de embarazos en mujeres, (OR ajustado = 1,61) (IC95%: 1,02-2,54). La vacunación mostró un efecto protector sin asociación significativa. Conclusión. Los aspectos asociados con la convivencia familiar y el número de embarazos contribuyen a una potencial transmisión vertical y horizontal del virus. No se identificaron prácticas culturales asociadas. Se requieren estrategias novedosas y diferenciales para reducir la transmisión del virus de la hepatitis B en poblaciones indígenas.

The Indigenous Tutorial Education Program (PET)--Actions in Health has been developed since 2010 at the Federal University of São Carlos, Brazil, and seeks to train agents capable of collaborating in the improvement of indigenous health conditions and in popular communities. Several indigenous people have passed through the group since its creation. There are also some indigenous peoples who have already graduated and are present professionally in different contexts. Thus, this article presents research that sought to map the group's former members, in the first twelve years of its existence, and to know their current experiences and the period in which they were participants. This is a study with a quantitative and qualitative approach, using documental survey, sessions and semi-structured interviews. There were 46 participants in the group, 39 of whom were graduates, with a great diversity of self-employed courses, regions of Brazil and indigenous peoples. They developed several activities in research, teaching and extension, with interesting relationships with their graduations and with their permanence in the institution. The effects of participation in the group and the relationship between their participation and daily professional practices were noticed, including those related to the COVID-19 pandemic. La Menopause Rating Scale (MRS) se compone de tres dimensiones que evalúan síntomas somáticos, emocionales y urogenitales relacionados con la menopausia. La utilidad de las escalas varía según las características poblacionales y se desconocen investigaciones que corroboren estas dimensiones en población indígena. El objetivo fue evaluar la utilidad de las dimensiones y confiabilidad de MRS en indígenas colombianas. Se realizó análisis del patrón de respuesta de MRS en 914 mujeres indígenas, 507 posmenopáusicas y 407 premenopáusicas, entre 40-60 años, media 50,3 años (DE=5,9). Se estimó alfa de Cronbach para las dimensiones originales y para las que emergieron en el análisis factorial mediante el método de máxima verosimilitud y rotación oblicua promax. MRS mostró alfa de Cronbach: 0,86; la dimensión somática 0,63, la psicológica 0,75 y la urogenital 0,84. Introducción. El pueblo hitnü vive en condiciones sanitarias precarias, con inseguridad alimentaria y víctima de la violencia sociopolítica en Arauca (Colombia). Además, se sospecha que pueden estar afectados por la exposición a los hidrocarburos del petróleo. Objetivo. Identificar los eventos de salud del perfil de morbilidad y mortalidad de los indígenas hitnü que podrían asociarse con la exposición a petróleo crudo. Materiales y métodos. Se realizó un estudio transversal con indígenas hitnü, durante febrero y marzo de 2021, época de sequía. Se aplicó un cuestionario de hogares y uno individual para recolectar datos del ambiente peridomiciliario, ocupaciones y otras actividades, así como datos sociodemográficos, signos, síntomas y hallazgos de un examen médico. La potencial asociación con los hidrocarburos se exploró considerando tres grupos, según su localización: cabecera de Arauca, resguardo Aspejená (no expuestos) y resguardos de San José del Lipa y La Vorágine (expuestos por su cercanía al río Ele y afluentes). Con listados libres, se exploraron las causas de muerte. El estudio incorporó un riguroso manejo intercultural en todos sus componentes. Resultados. Participaron 576 indígenas de 16 asentamientos. El agua consumida pudo servir como medio de exposición a los hidrocarburos. Los problemas de salud fueron muy variados e incluían enfermedades infecciosas y crónicas, malnutrición y trauma. Las masas en el cuello se asociaron con residir en los resguardos ancestrales (RP=3,86; IC95% 1,77-8,39), territorios potencialmente expuestos al petróleo. Las causas de muerte más relevantes fueron el homicidio, los tumores y la tuberculosis. Conclusión: Por su posible asociación con los hidrocarburos, es prioritario el estudio intercultural de linfoadenopatías entre indígenas potencialmente expuestos al petróleo.

RESUMEN En 2016 comenzó en Colombia la implantación de un nuevo modelo de salud con foco en la atención primaria, siendo Guainía, en la Amazonía colombiana, la primera región en iniciar el test piloto. El objetivo de este estudio fue comprender el cotidiano de implementación de la atención primaria, en el marco del Modelo Integral de Atención en Salud, desde la perspectiva de indígenas y profesionales de la salud en Guainía, Colombia. Se trata de un estudio de caso único, con abordaje cualitativo, fundamentado en la sociología comprensiva del cotidiano. Los datos, recolectados mediante observación directa y entrevistas semiestructuradas con 26 profesionales de salud y 22 usuarios indígenas, fueron sometidos a análisis de contenido temático. Emergieron cinco categorías: ‘vivir en el puesto’, ‘resolver solo’, ‘el desafío de la cobertura territorial’, ‘comisiones de salud: solventando vacíos’ y ‘apoyo de líderes locales’. A pesar de la propuesta reestructurante traída por el nuevo modelo, se evidenció en el cotidiano la persistencia de un enfoque asistencialista y basado en la lógica curativista en la prestación de los servicios. Aunque los atributos de la atención primaria se ven comprometidos, se identificaron acciones cotidianas que pueden favorecer la integralidad en la atención y aportar a una transición exitosa.

Objetivo: identificar en la literatura las acciones de cuidados de enfermería para la salud de las poblaciones indígenas. Método: se trata de una revisión de alcance, cuya búsqueda fue realizada en septiembre de 2022 en las siguientes bases de datos: Base de Datos de Enfermería (BDENF), Catálogo de Tesis y Disertaciones de la CAPES, Google Scholar, Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS), Pan American Health Organization (PAHO), National Library of Medicine and National Institutes of Health (PUBMED) y Scientific Electronic Library Online (SCIELO). Los resultados se evaluaron resumiendo los artículos seleccionados. Resultados: las principales acciones dirigidas a la población indígena fueron: consultas de enfermería y actividades de promoción de la salud y prevención. La principal dificultad encontrada en relación a la población indígena es la barrera lingüística. Conclusión: el equipo de enfermería encuentra dificultades como: la difícil localización de las aldeas; la elevada sobrecarga de trabajo debido a la falta de profesionales médicos; la barrera lingüística; y las condiciones inadecuadas.

Introducción: La desnutrición crónica es un problema de difícil erradicación con grandes repercusiones en la población infantil. En el Ecuador 1 de cada 5 niños menor de cinco años, presenta desnutrición crónica. El objetivo del presente estudio fue determinar incidencia y factores asociados al desarrollo de desnutrición crónica en una cohorte de niños menores de 5 años durante un periodo de seguimiento de un año, en la provincia de Chimborazo, 2018-2019. Métodos: Se realizó un estudio de cohorte prospectivo, con seguimiento de 2018-2019. La población estuvo conformada por 1228 niños, de los cuales se tomó una muestra de 138 niños. Se tuvieron en cuenta variables como la edad, sexo, ubicación geográfica, nivel socioeconómico, acceso a servicios de salud, servicios básicos y tierras, prácticas de alimentación, antropometría, desnutrición crónica, talla de la madre, infecciones, y parasitosis. Se calculó el Riesgo Relativo (RR) y sus respectivos intervalos de confianza al 95% usando modelos lineales generalizados (familia binomial). Resultados: La incidencia de desnutrición crónica fue de 20,3%. Los factores asociados fueron: dificultad para acceder a los servicios de salud [RR: 4.87; IC 95%: 1.89-12.55]; y LME < 6 meses [RR: 3.44, IC 95%: 2.14-5.55]. Conclusiones: La incidencia de desnutrición crónica en menores de cinco años en la provincia Chimborazo es elevada; por lo que se precisa de intervenciones que garanticen el acceso a los servicios de salud y educación nutricional para eliminar este problema de salud.

Food autonomy allows communities to determine their food practices, starting with agricultural production in their territories, guaranteeing their own economy and harmony with Mother Earth. In this context, the COVID-19 pandemic generated changes in social practices in the communities. indigenous communities, including their food practices. Objective: Describe food autonomy practices revitalized following the COVID-19 confinement in a Nasa indigenous community in southern Colombia. Method: Qualitative study oriented on community-based participatory research (CBPR) carried out in a Nasa indigenous community in southern Colombia. The collection and interpretation of information was carried out with qualitative research techniques: open and axial coding and until reaching the description of emerging categories. Results: Ancestral practices of the food system were strengthened to improve access to healthy foods produced in their territories, among which the following stand out: tul (family garden), barter, hand-exchange, Nasa market and traditional recipes. Conclusions: Confinement represented an opportunity to resume and revitalize the community's ancestral food practices that responded to specific health and food needs, strengthening the social fabric and indigenous identity, actions that can transcend public policies, life plans and aspirations for a good life. Resumen Este estudio describe experiencias y percepciones sobre interculturalidad en Atención Primaria de Salud (APS) desde la perspectiva de trabajadores/as y usuarios/as de salud mapuche. Se realizó una sistematización cualitativa de experiencias de un Programa de Salud Intercultural en APS en una comuna urbana de Chile. Participaron 19 usuarios/as y 13 trabajadores/as en entrevistas individuales y tres entrevistas grupales, respectivamente. Se realizó un análisis de contenido semántico. Para los participantes, la salud mapuche es percibida positivamente, los/as usuarios/as la asimilan al concepto de interculturalidad, mientras que los/as trabajadores/as señalan que, si bien se respeta, no se promueve un trabajo integrado. Los/las participantes identifican como barreras aspectos administrativos, falta de integración y cuestionamientos científicos. Se requiere reconocimiento de la salud indígena y mayor formación de trabajadores/as sobre salud indígena e interculturalidad.

Objective: to understand the construction process of the National Program for Access to Potable Water in Indigenous Lands, through socio-epidemiological factors that drove its institutionalization. Method: descriptive and socioepidemiological study, based on secondary data from the Special Secretariat for Indigenous Health (SESAI), with a pre-pandemic and pandemic time frame. Data analysis followed the foundations of David Easton's political system theory. Results: the program's construction process considered the volume of notifications of waterborne diseases, the low access to potable water and the difficulty in controlling its quality in the villages served by SESAI. After the construction of the PNATI, the importance of this instrument for planning, management and social control in guaranteeing the supply of potable water in indigenous lands and changes in the health reality of the indigenous peoples of Brazil was verified. Conclusion: PNATI fills an existing gap in terms of legal support, with the opportunity to reverse the scenario of insufficient environmental sanitation, with a view to universalizing access to water, in quantity and quality in indigenous communities, and contributing to the improvement of the health of this population. RESUMEN Introducción. En un país multicultural como Colombia, la etnoeducación es un importante elemento en la educación médica, pues favorece la calidad de vida de minorías étnicas a partir de potencialidades comunitarias, prácticas, hábitos, experiencias y enfoques que promuevan el sentido global de la salud. El objetivo de este artículo fue realizar una revisión narrativa sobre los antecedentes de la etnoeducación y su impacto en la salud. El desarrollo de competencias interculturales en los estudiantes del área de la salud permitiría a los futuros profesionales desempeñar su rol con adecuada pertinencia cultural, respetando los valores, tradiciones e historia de las comunidades. Temas tratados: A nivel mundial, la etnoeducación ha sido un aspecto relevante, organizaciones internacionales han trabajado durante décadas para su implementación. A nivel nacional, se han creado diversos instrumentos normativos y reglamentación para la implementación de la etnoeducación, sin embargo, cerca del 86% de las poblaciones étnicas no cuentan con acceso a educación que cumpla con los principios establecidos. La importancia de la etnoeducación se ha evidenciado en varios países, brindando resultados positivos como la disminución de la morbimortalidad con actividades educativas de promoción de la salud y prevención de la enfermedad. Para ello, es crucial que las acciones planteadas se integren con las percepciones culturales de las comunidades. Conclusión: Existen muchos lineamientos a nivel local, nacional e internacional, sin embargo, la etnoeducación continúa siendo un reto. The purpose of this article is to perform a narrative review on the background of ethnoeducation and its impact on health. The development of inter-cultural skills among students in health fields would enable the future professional to perform their roles with adequate cultural relevancy, respecting the values, traditions and history of the communities. Topics discussed: Worldwide, ethnoeducation has been highly relevant, and some international organizations have worked on its implementation for decades. In Colombia, several legal and regulatory instruments have been developed to implement ethnoeducation. 23. Factors associated with hepatitis B virus infection in indigenous communities of Colombia

Introduction. Colombia is home to 2 million indigenous people who live in conditions of poverty and with health deficiencies, making them vulnerable to contracting hepatitis B (HBV). Amazonas has a high virus prevalence, and there are barriers to accessing vaccination; thus, part of the population is susceptible to infection.Objective. To identify factors associated with HBV in Colombian indigenous people.Materials and Methods. A case-control study of people over 18 years from four departments of Colombia. Cases were identified through the national hepatitis B notification registry (2015-2022). Controls were selected and matched to cases (2:1) by age, sex, ethnicity, and department. Sociodemographic characteristics, factors associated with contact with body fluids, cultural practices, and vaccination history were identified by means of a survey. The ethics committee of the Universidad de Antioquia approved the project.Results. Seventy five cases and 150 controls from 13 ethnic groups were surveyed. Amazonas contributed 49% of participants, 83% were women, and the median age ofcases was 30 years (IQ range: 27-37). The associated factors were a family history of hepatitis B [adjusted OR: 2.61 (95% CI: 1.09-6.27)] and, in women, the number ofpregnancies [adjusted OR: 1.61 (95% CI 1.02- 2.54)]. The vaccination history showed a protective effect, but the association was not significant.Conclusion. Aspects associated with family life and unprotected sexual relations seem to be responsible for the potential transmission of the virus. It was not possible to identify associated cultural practices. Innovative and differential strategies are required for indigenous people to achieve a reduction of HBV., Introducción. Colombia alberga dos millones de indígenas, que viven en condiciones de pobreza y tienen deficiencias en salud, por lo cual están expuestos a contraer infecciones virales como la hepatitis B. El departamento del Amazonas presenta una gran prevalencia del virus y barreras para acceder a la vacunación; por esto, parte de la población es propensa a la infección.Objetivo. Identificar factores asociados con la infección por el virus de la hepatitis B en indígenas colombianos.Materiales y métodos. Se llevó a cabo un estudio de casos y controles en mayores de 18 años de cuatro departamentos del país. Los casos se identificaron mediante el registro nacional de notificación de hepatitis B (2015-2022). Los controles seleccionados de manera concurrente fueron pareados con los casos por edad, sexo, etnia y departamento. En una encuesta se consignaron las características sociodemográficas, los factores asociados con el contacto con sangre y fluidos, las prácticas socioculturales y los antecedentes de vacunación. El proyecto fue aprobado por Comité de Ética de la Universidad de Antioquia.Resultados. Participaron 75 casos y 150 controles de 13 grupos étnicos. El departamento del Amazonas aportó el 49 % de los participantes (83 % mujeres) con una mediana de edad de 30 años (RIC = 27-37). Los factores asociados con una mayor probabilidad de contraer la infección fueron el antecedente de algún familiar infectado con el virus de la hepatitis B (OR ajustado = 2,61) (IC95%: 1,09-6,27) y número de embarazos en mujeres, (OR ajustado = 1,61) (IC95%: 1,02-2,54). La vacunación mostró un efecto protector sin asociación significativa.Conclusión. Los aspectos asociados con la convivencia familiar y el número de embarazos contribuyen a una potencial transmisión vertical y horizontal del virus. No se identificaron prácticas culturales asociadas. Se requieren estrategias novedosas y diferenciales para reducir la transmisión del virus de la hepatitis B en poblaciones indígenas.

Introducción: en Colombia, la mayoría de los pueblos indígenas se encuentran en riesgo de extinción física y cultural. Además, existe una deficiencia de estudios relacionados con el análisis de narrativas propias sobre los asuntos que rodean la salud indígena. Tal ausencia deriva en las dificultades de análisis e intervención culturalmente apropiada a problemas indígenas. Así, se planteó como objetivo develar las narrativas sobre el proceso vital humano en pueblos indígenas de Colombia construidas por estudiantes universitarios indígenas migrantes en Bogotá. Materiales y métodos: esta investigación es cualitativa con enfoque narrativo y está basada en el paradigma hermenéutico-interpretativo. Se entrevistó a ocho estudiantes que cursan el pregrado en Bogotá, con edades entre 18 y 40 años, pertenecientes a los pueblos kamentsá, pastos, pijao, kankuamo, nasa y misak. Resultados: se identificó la vida en armonía, anclada al territorio de origen, como el concepto clave organizador de todas las comprensiones relativas a la salud, la enfermedad, la muerte y el cuidado. Conclusiones: el diseño de intervenciones sanitarias apropiadas dirigidas a indígenas requiere considerar sus propias comprensiones sobre el proceso vital humano, en relación intrínseca con la armonia integral entre persona, comunidad y territorio.

Introduction: Chronic malnutrition in children under 5 years old is defined as stunting due to recurrent nutritional deficiencies associated with poverty, maternal health, and nutrition. This study aimed to identify the prevalence of chronic malnutrition and associated factors in children under 5 years old from the indigenous Awá community, at Barbacoas, Nariño in 2019. Materials and methods: A cross-sectional study was conducted, which included 527 children under 5 years old from the Awá community who attended a health institution for growth and development program. Chronic malnutrition was evaluated as an outcome, whereas sociodemographic, nutritional and other health characteristics as associated factors. Chi-square, Fisher’s exact, and Mann–Whitney’s U test were used to determine statistical association. Additionally, the relationship was adjusted through a multiple logistic regression model. Results: Chronic malnutrition was found in 17.6% of children. The statistical association was found in children of the adolescent mother, incomplete vaccination schedule, and age and birth weight. Conclusion: In children under 5 years old in the Awá community, the frequency of chronic malnutrition is above the average for the general population in Colombia. Prioritizing the active search for chronic malnutrition in children whose mothers are adolescents and those with low birth weight and incomplete vaccination is important.

Introduction: Public instances use indigenous languages in communications about COVID-19; but the availability of such materials is still very limited and sometimes are not accessible enough. Objective: To generate health communications according to the reality of the most numerous indigenous group in Mexico, the maya. Methodology: We conformed a multidisciplinary working team, and to identify the necessities of information, we performed a search for official materials and a survey in Mayan communities. We generated and adjusted the communications considering linguistic and sociocultural conditions. Results: All products were spoken in maya because the majority of maya speakers do not read in their language. We generated three videos and a podcast series of six episodes, in digital format and for traditional radio. They include general health information and topics with limited coverage in maya language. Conclusion: With our communication materials, we expect to contribute to equity in information about COVID-19 and to make more visible the importance of maya language in health messages. To describe the experience of military nurses in Operation Xavante in coping to the new Coronavirus. This is an experience report on the role of military nurses in Operation Xavante. As part of a multidisciplinary team, military nursing in Operation Xavante was primordial on several fronts of action, which allowed, within the proposed context, the proper fulfillment of the mission and the success of the planned health actions, standing out in the following aspects: reception and screening; host; health education and immunological tests for COVID-19. It ends by emphasizing that these actions are part of the professional performance of nurses who are members of the Armed Forces, working in different situations and contexts, making it possible to reaffirm nursing practices in indigenous health care, amid a moment of extreme challenge experienced by the world population. Introduction. In a multicultural country such as Colombia, ethnoeducation is an important component of health education, because it promotes the quality of life of ethnic minorities based on the community's potential, practices, habits, experiences and approaches that promote overall health awareness. The purpose of this article is to perform a narrative review on the background of ethnoeducation and its impact on health. The development of inter-cultural skills among students in health fields would enable the future professional to perform their roles with adequate cultural relevancy, respecting the values, traditions and history of the communities. Topics discussed: Worldwide, ethnoeducation has been highly relevant, and some international organizations have worked on its implementation for decades. In Colombia, several legal and regulatory instruments have been developed to implement ethnoeducation. However, close to 86 % of the ethnic populations do not have access to education in accordance with the established principles. The importance of ethnoeducation has been highlighted in several countries in that it has achieved positive results such as a reduction of morbidity and mortality through educational activities that promote health and help prevent diseases. To achieve this, it is essential that the planned activities be integrated into the communities' cultural perceptions. Conclusion: Although local, national and international guidelines have been established, ethnoeducation continues to be a challenge. It is necessary to increase efforts in order for ethnoeducation to achieve the objectives that have been set out from a theoretical perspective. [ABSTRACT FROM AUTHOR]

En Argentina, la ocurrencia de diabetes y otras enfermedades crónicas en población indígena aún no ha sido suficientemente estudiada. Este trabajo es resultado de una exploración etnográfica de las determinaciones socioculturales y de género de los procesos de atención y prevención de diabetes en indígenas tapietes de la provincia de Salta. La investigación combinó observación participante con entrevistas a varones y mujeres de edad adulta y con diagnóstico de diabetes, y entrevistas a enfermeras y médicas de un centro de atención primaria de Misión Los Tapietes y de un hospital de Tartagal, entre los meses de agosto y septiembre de 2019. En la comunidad se ha observado la creciente presencia de diabetes, junto con la falta de articulación de las intervenciones sociosanitarias con las prácticas nativas de salud-enfermedad-atención; los problemas en la comunicación del diagnóstico y tratamiento; y las dificultades de acceso al sistema de salud. El artículo discute la complejidad de la transición epidemiológica, los sentidos de la enfermedad crónica para los varones y las mujeres, la emergencia traumática de la diabetes y las dificultades en el ejercicio del derecho a la salud de los pueblos originarios.

Objetivo: Conocer el significado que otorga el usuario mapuche, adscrito al Programa Salud Cardiovascular 2016, Consultorio Makewe, Temuco, Chile, a la hipertensión arterial. Metdología: Investigación bajo el paradigma cualitativo, con base en la teoría fundamentada. La muestra fue conformada por veintidós usuarios, no probabilística, intencionada de casos por criterios y conveniencia, de acuerdo con criterios de inclusión. La recolección de información se hizo mediante dos grupos focales y ocho entrevistas en profundidad, hasta llegar al punto de saturación, previa firma de consentimiento informado. El análisis se caracterizó por ser dinámico y creativo, basado en la estrategia de método comparativo constante, realizando codificación y análisis simultáneo. La validez se comprobó con criterios de rigor y fue triangulada por investigadores. La investigación fue autorizada por el Comité Ético Científico de la Universidad de La Frontera. Resultados: Para hipertensión arterial emergieron cuatro categorías: “Significado” (definición del concepto), “Causa” (percepción), “Convivir” (cambios) y “Tratamiento” (farmacológico, herbolaria, machi —curandera o consejera—). Conclusiones: Sin haber definido la patología, los mapuches la asocian a signos, síntomas y causas; confían en tratamientos otorgados por un equipo de salud, pero no en las/los machis, reconociendo algunas de ellas como buenas, las que por alta demanda y costo son inaccesibles. En general, se ha descuidado la Ley Indígena 19253 de 1993, que da lugar a la institucionalización del Programa de Salud con Población Mapuche (promap), dependiente del Servicio de Salud Araucanía.

Objective: To analyze the challenges faced by nursing professionals in health care for indigenous peoples in the Rio Tapajós Special Sanitary District. Methods: A quantitative and qualitative descriptive study submitted to content analysis technique. Results: The prevalence of females and individuals aged between 31 and 35 years (60%) was identified. It was found that 60% of respondents work between 6 to 10 years at Rio Tapajós Special Sanitary District. Among the difficulties of indigenous health, the location of the villages and the language barrier are factors of concern for nurses, as well as the high turnover of professionals and the lack of training during academic training to work in an intercultural context. - Abstract
Knowledge of the feeding process allows the design of specific programs on food and nutritional security for different social groups, as is the case of indigenous communities that deserve a differential approach. Objective: to understand the experiences of the Curripaco indigenous people settled in the Tierra Alta reservation in relation to the feeding process and the meaning they give to it. Methodology: qualitative study with an ethnographic approach and techniques such as participant observation and semi-structured interviews for collecting information. Results: the following emerging categories were obtained: obtaining food, with the subcategories the conuco, the river or pipe and the jungle; food preparation, which includes the process from the time when the food is obtained to the final dish; special foods, with two subcategories, sacred foods and forbidden foods; and serving the food which includes, among other things, the temperature of the food and the hierarchical order within the family group to serve it. Conclusion: the preservation of ancestral practices related to the feeding process is an important element that has contributed to the conservation of the health of its members over time, so that it is necessary to tend to protect these practices and include them as an essential component of health promotion in the food and nutritional security plans that are developed in the territory. Objective. To determine the prevalence and risk factors for syphilis transmission of the male indigenous population of Paraguay during 2017. Materials and methods. An observational, descriptive cross-sectional study was conducted with multi-epic stratified probabilistic sampling. They were included in the study indigenous male, over 15 years of age, of five linguistic families resident in Paraguay during the study period. Results. The prevalence of syphilis in men of the indigenous population of Paraguay was 4.50%. Indigenous people who were 15 to 20 years old had 2.70 times the probability of presenting syphilis, as well as the participants who reported multiple partners in the last year, transaccional sex and consumed alcohol in the last sexual intercourse had 2.69, 3.06 and 2.16 times the probability of presenting syphilis. Conclusions. A moderate prevalence of syphilis was found in the male indigenous population. In turn, an opportunity was found twice more to acquire syphilis infection for those who had more than two sexual partners, transactional sex and those who involved alcohol during sexual practice. Objetivo: analizar la capacidad de respuesta comunitaria (CR) hacia la obesidad, diabetes e hipertensión (ODH) para el desarrollo de iniciativas de promoción de la salud en una comunidad de origen náhuatl en México. Material y métodos: estudio basado en el Modelo de Capacidad de Respuesta Comunitaria (MCRC) con abordaje cualitativo. Entrevistamos ocho informantes clave. Realizamos la asignación de valores por dimensión utilizando la escala del MCRC y el análisis de contenido para comprender los factores involucrados en la CR. Resultados: la etapa de CR identificada fue de "concientización vaga" (valor 3). Se reconocieron los elementos que dificultan el involucramiento de la comunidad. Discusión: valorar la CR es imprescindible para generar estrategias apropiadas. El MCRC resultó útil para profundizar en la ODH como fenómeno social en un contexto como el de Xoxocotla. El análisis cualitativo fue indispensable para comprender la CR y las consideraciones necesarias para generar iniciativas efectivas. [ABSTRACT FROM AUTHOR]

Introduction: The cultural and ethnic diversity of indigenous peoples in Colombia makes it necessary to adopt a differential approach to the health of different cultures. Objective: To understand the meaning of puerperium assigned by the Embera Katío, from the municipality of Tierralta, Córdoba, 2017-2018. Method: ethnographic study. Theoretical sample with 13 key participants. Participant observation methods and in-depth interviews were used. The analysis was guided by Leininger's proposal. Ethical aspects were considered. Results: The puerperium, called "Recién Parida", is a period of change and uncertainty between happiness and danger of getting sick or dying, which consists of four cultural patterns: 1. Just after giving birth my body changes physically and mentally; 2. Right after giving birth I feel happy to be accompanied and alive; 3. Just after giving birth, I am in danger and I am afraid of getting sick and die; 4. A hatch of attention and teachings to take care of me. Conclusions: Embera Katío puerperal women consider puerperium as a process of change and uncertainty which can be dangerous due to natural, physical and emotional factors which generate fear of illness or death. They recognize that their body must recover and close after childbirth in order not to suffer from diseases, and they require a hatch of accompaniment between the care provided especially by the family and self-care. Objective. To reflect on the process of committing to participation in the implementation of a health strategic plan, using Participative Systematization of Social Experiences as a tool. Materials and methods. Our study was a qualitative research-intervention study, based on the Dialectical Methodological Conception approach. We designed and implemented a two-day workshop, six hours daily, using Systematization methodology with a Community Work Group (CWG). During the workshop, women systematized their experience, with compromise as axis of the process. Using Grounded Theory techniques, we applied micro-analysis to data in order to identify and strengthen categories that emerged during the systematization process. We completed open and axial coding. Results. The CWG identified that commitment and participation itself is influenced by group dynamics and structural determinants. They also reconsidered the way they understood and exercised commitment and participation, and generated knowledge, empowering them to improve their future practice. Conclusions. Commitment and participation were determined by group dynamics and structural factors such as socioeconomic conditions and gender roles. These determinants must be visible and understood in order to generate proposals that are aimed at strengthening the participation and organization of groups.

Over the last 26 years, the Mexican government has developed a number of activities and discourses around what has been called “intercultural health,” directed especially at indigenous peoples in Mexico (some 62, according to linguistic criteria). In this way, the government has built health care institutions (rural centers, clinics, and hospitals) in states like Puebla, Nayarit, Oaxaca, Chiapas, Queretaro, and Jalisco, proposing the implementation of cultural pertinence indicators (which are minimal and inadequate). Nevertheless, the health conditions among indigenous populations and the quality of health care provided by public institutions continue to be precarious in terms of human and material resources (health personnel, drugs, etc.) and discriminatory with respect to the form and content of the provided services. This paper describes some of the governmental interventions that purport to be institutional improvements in the field of interculturality, but that actually represent the continuity of arbitrary and exclusive policies.

Se recuperó el proceso de construcción del esquema conceptual, referencial y operativo (ECRO) de los wixáritari sobre la diabetes mellitus tipo 2 en una comunidad de Jalisco, México, desde la aparición del primer síntoma hasta una posible explicación de la enfermedad. Se realizó un estudio cualitativo con el método fenomenológico a partir de entrevistas en profundidad para la recolección de información. Se contó con siete participantes. Los datos se procesaron con el método de análisis del discurso de Bajtín, a partir de la teoría del vínculo de Pichon-Rivière. Se encontró que el wixárika con diabetes construye su esquema conceptual, referencial y operativo durante un periodo de tres a cinco años, a partir de tres vínculos: los síntomas en el cuerpo, la explicación del wixárika a sus síntomas, y las explicaciones del espacio social a su enfermedad. El wixárika construye una serie de vínculos y un esquema conceptual, referencial y operativo que le permite validar una explicación de lo que experimenta en el cuerpo, en medio de un proceso confrontativo y doloroso, lleno de afirmaciones y negaciones hasta llegar a una síntesis, que le permita comprender su situación e instrumentar una conducta para el cuidado de su salud.

Objective: to analyze the way in which the prevention of infection by the human immunodeficiency virus (HIV) is configured under the view of women from the Potiguara ethnic group. Method: a qualitative study involving 256 women from three indigenous villages in Paraíba-Brazil The information was collected in 2015, through the Free Word Association Test and semi-structured interviews. The corpus was analyzed from the Descending Hierarchical Classification, Content Analysis, and Similitude Analysis. Results: three classes of content related to HIV prevention were identified: "Vulnerability to HIV infection"; "Protective measures"; and "Need for health education". The word "condom" occupied the central position in the similarity analysis. Conclusion: although they recognize the means for preventing HIV, it is difficult to perform protected sexual practices. Such condition is crossed by social and gender inequalities, which extend to the reality of nonindigenous women. [ABSTRACT FROM AUTHOR]
