Background: The coronavirus disease 2019 (COVID-19) has evolved into a pandemic infectious disease transmitted by the severe acute respiratory syndrome coronavirus (SARS-CoV-2). Allergists and other healthcare providers (HCPs) in the field of allergies and associated airway diseases are on the front line, taking care of patients potentially infected with SARS-CoV-2. Hence, strategies and practices to minimize risks of infection for both HCPs and treated patients have to be developed and followed by allergy clinics. Method: The scientific information on COVID-19 was analysed by a literature search in MEDLINE, PubMed, the National and International Guidelines from the European Academy of Allergy and Clinical Immunology (EAACI), the Cochrane Library, and the internet. Results: Based on the diagnostic and treatment standards developed by EAACI, on international information regarding COVID-19, on guidelines of the World Health Organization (WHO) and other international organizations, and on previous experience, a panel of experts including clinicians, psychologists, IT experts, and basic scientists along with EAACI and the “Allergic Rhinitis and its Impact on Asthma (ARIA)” initiative have developed recommendations for the optimal management of allergy clinics during the current COVID-19 pandemic. These recommendations are grouped into nine sections on different relevant aspects for the care of patients with allergies. Conclusions: This international Position Paper provides recommendations on operational plans and procedures to maintain high standards in the daily clinical care of allergic patients while ensuring the necessary safety measures in the current COVID-19 pandemic. © 2020 EAACI and John Wiley and Sons A/S. The aim of the study was to assess the safety of nasal allergen challenge, and the use of certain parameters applied in assessing the condition of the respiratory system. We enrolled 30 patients diagnosed with allergy to common environmental allergens and 30 healthy controls. The safety of nasal challenge tests with an allergen was assessed by measuring the concentration of exhaled nitric oxide from the lower respiratory tract (eNO) and forced expiratory volume in one second (FEV1) in a spirometry test. In the early phase of the allergic reaction, extra-nasal symptoms were observed, namely cough and breathlessness. These measured symptoms using the VAS scale, were far more frequent in the patients diagnosed with perennial allergic rhinitis. The eNO and FEV1 values in the spirometry test did not show any statistically significant changes. No correlation was observed between the breathlessness and cough complaints to the eNO concentration levels (cough: r=0.019, p=0.921; breathlessness: r=-0.088, p=0.644) nor the FEV1 level (cough: r=0.002, p=0.983; breathlessness: r=-0.072, p=0.751) in the spirometry test. In the early phase of the allergic reaction, nasal allergen challenge do not cause any significant changes in the lower airways, as measured with the use of certain parameters applied in assessing the function of the lower airways’ function.

humanities - Abstract
The leading priority for the Polish Presidency of the Council of the European Union was to reduce health inequalities across European societies, and, within its framework, prevention and control of respiratory diseases in children. This very important paper contain proposal of international cooperation on the prevention, early detection and monitoring of asthma and allergic diseases in childhood which will be undertaken by the EU member countries as a result of EU conclusion developed during the Polish Presidency of the Council of the European Union. This will result in collaboration in the field of chronic diseases, particularly respiratory diseases, together with the activity of the network of national institutions and NGOs in this area. Paper also contains extensive analysis of the socio-economic, political, epidemiological, technological and medical factors affecting the prevention and control of childhood asthma and allergy presented during Experts presidential conference organized in Warsaw-Ossa 21-22 September 2011. © 2012 John Wiley & Sons A/S.

- Abstract
Background: Nasal allergen provocation tests are an important part of the diagnostics of allergic diseases triggered by environmental factors. Recently, increased attention has been paid to the potential use of this method in the diagnosis of food allergy. The objective of the study was to evaluate the usefulness of the nasal allergen provocation test in a group of subjects allergic to hen's egg white allergens., Methods: The material consisted of a group of 57 subjects (32 subjects with hen's egg white allergy and 25 healthy controls). The method consisted in a nasal allergen provocation test carried out with the use of hen's egg white allergen and assessed using the visual analog scale and optical rhinometry as well as by determination of sIgE and tryptase levels in nasal lavage fluid., Results: Subjective nasal symptoms and objective evaluations following the application of 100 µg of hen's egg white allergen revealed a moderately positive nasal mucosal response in optical rhinometry tests (ΔE = 0.34 OD)., Conclusions: Nasal food challenge with hen's egg white allergen is a good diagnostic alternative in the group of food allergy patients. 47. Comorbidity of Histamine Intolerance and Polyvalent Allergy: A Case Report and Literature Review.

- Abstract
Background/Objectives: Histamine intolerance is becoming a critical medical problem across numerous clinical specialties, due to the absence of a standardized diagnostic and therapeutic strategy to manage patients with a suspicion of or diagnosis of this condition. Histamine intolerance is a type of non-immune food hypersensitivity, characterized by heterogenous etiologies and a very broad range of symptoms. The condition is the result of an imbalance between the amount of histamine accumulated within the body and the body's systemic ability to degrade it. In regard to the diagnostics of histamine intolerance, the need to preliminarily exclude other potential conditions associated with increased histamine levels in the blood has been highlighted. The co-occurrence of allergies and histamine intolerance is not uncommon, and the similarity of the clinical manifestations can lead to diagnostic, as well as therapeutic, difficulties. This paper details the diagnostic and clinical workflow for a patient with histamine intolerance and polyvalent allergy comorbidity, with the aim being to help outline a protocol that may be helpful to clinicians managing patients with histamine intolerance. Case Presentation : This article presents the case of a 30-year-old patient with a polyvalent allergy and multimorbidity (allergic rhinitis, asthma, a food allergy, and eosinophilic esophagitis), with comorbid histamine intolerance. Due to the violent and severe symptoms, including facial erythema, urticaria, pruritus, abdominal pain, and tachycardia, experienced after meals, the patient received intramuscular epinephrine injections three times a week. The diagnostic protocol and the course of therapeutic management are presented. Conclusions : The diagnosis of histamine intolerance is difficult due to the high variability and heterogeneity of clinical symptoms in individual patients. Many studies on the issue recommend ruling out an allergic background in terms of the complaint. However, the possibility of the symptoms of an IgE-dependent allergy overlapping with those of histamine intolerance should be taken into account in every case. This is particularly important in patients presenting with an atypical and severe course of allergic diseases. The clinical case presented herein may be helpful for the daily practice of allergologists and physicians with other specialties, as an example of multimorbidity with both allergic and non-allergic backgrounds.

- Abstract
Introduction: Excessive body weight, particularly the abdominal distribution of adipose tissue, has remained an important public health concern for years., Objectives: The study aimed to assess the prevalence of general overweight status and obesity, as well as abdominal overweight status and obesity in the adult Polish population., Material and Methods: The results are based on the analysis of anthropometric data of 3735 people aged 19 and over who were surveyed under the 2016-2020 National Health Program., Results: Excessive body weight (BMI > 25 kg/m
2 ) was reported in 56.6% of the respondents. Abdominal overweight was found in 20.8% of respondents and abdominal obesity in 31.7% of the respondents. Abdominal obesity was more common in women than in men (44.3% vs. 18.0%, p < 0.001). Over half of women over 55 (approx. 60%) had abdominal obesity. The odds of becoming overweight/obese as measured by the BMI depended on the age and sex of the respondents. It was confirmed that men were over 28% more likely to develop such a condition than women (OR = 1.288, p < 0.001). The odds increased with age (OR = 1.029, p < 0.001). The type of person with obesity/found to be overweight in Polish population was most often a rural resident, aged 65.0-74.9, assessing their financial situation as average, being married/in a partnership, and not declaring the occurrence of cardiovascular diseases. The person with an overweight status had secondary education, and the person with obesity had primary/lower secondary/vocational education., Conclusions: Being overweight and having obesity, both general and abdominal, are still a major epidemiological problem in Poland. The results obtained in this study suggest that the prevalence of being overweight and having obesity has decreased over the last 10 years, though this assumption requires further investigation. Rural residents with lower education should be covered by educational activities adapted to their needs and possibilities, considering that obesity in young women, especially when having abdominal obesity, may be associated with an increased risk of infertility caused by ovulation disorders. Further research and educational activities are necessary. Excessive body weight affected almost 42% of the women from the youngest age group, and abdominal obesity was found in 21% of them.- Published

- Abstract
Introduction: Due to their widespread character, allergic diseases are a significant challenge in the field of public health and clinical practice. The available clinimetric tools, including standardized and validated questionnaires, play an important role in determining the incidence of a particular allergic disease in the targeted population., Aim: We attempted to evaluate the specificity and sensitivity of modified and standardized questions from the ISSAC and ECRHS questionnaires in the diagnosis of allergic diseases., Material and Methods: We examined a total number of 1,000 respondents classified into the following age groups: 6-10, 11-15, and 16-19 years. Modified ISSAC and ECRHS questionnaires were implemented to verify the effectiveness of diagnosing allergic diseases., Results: Among individuals aged from 6 to 10 years, the specificity and sensitivity of diagnosing allergic rhinitis were 0.84 and 0.50; for asthma 0.64 and 0.85, for atopic dermatitis 0.90 and 0.42, and for food allergy 0.42 and 0.85, respectively. Respondents aged 11-15 years presented the following values of specificity and sensitivity: 0.83 and 0.46 for allergic rhinitis, 0.45 and 0.87 for asthma, 0.89 and 0.43 for atopic dermatitis, and 0.61 and 0.80 for food allergies, respectively. Finally, among patients from 16 to 19 years of age, the specificity and sensitivity of validated questionnaires as assessment tools in diagnosing allergic rhinitis were 0.91 and 0.39, for asthma 0.35 and 0.89, for atopic dermatitis 1.0 and 0.45, and for food allergy 0.67 and 0.85, respectively., Conclusions: The modified and standardized questions included in the ISSAC and ECRHS questionnaires are a valuable tool for determining the scale of allergic diseases. 50. Assessing the impact of dietary choices on fiber deficiency: insights from the 2017-2020 Polish national adult nutrition survey.

- Abstract
Introduction: Dietary fiber is a key component of a healthy diet, associated with a reduced risk of cardiovascular disease, obesity, type 2 diabetes, certain cancers, chronic inflammation, or depression. The aim of the study was to perform an in-depth analysis of dietary fiber intake in the Polish population, taking account of the consumption of groups of products that are fiber sources and identify any age-related differences in the dietary fiber intake of the subjects., Methods: We analyzed data obtained from two representative cross-sectional studies on the diet and nutritional status of adult Polish residents including the total of 4,000 individuals aged 19 years and more. Two 24-h recalls were used per individual to assess the diet using the computer-assisted personal interview (CAPI) technique. Total fiber content and fiber contained in cereal products, vegetables, fruits, legumes, nuts and seeds were calculated. Fiber intake was compared to the recommendations: 25 g/d for adults up to 65 years of age and 20 g/d for those aged 66 years and older. All statistical analyses, including the Pearson's chi-squared test, the Student's t -test, and the Analysis of Variance (ANOVA), were conducted using STATISTICA
™ version 13.3, with the results being adjusted for demographic distribution biases to enhance the representativeness., Results: The average daily fiber intake was 17.83 ± 0.14 g/day (78% of the recommended intake), with 20.5% of respondents meeting the requirement. More men than women (27.05% vs. 14.3%;) met the requirement and men were characterized by a higher average intake (19.34 ± 0.20 g/day) than women (16.43 ± 0.19 g/day). The main fiber sources were cereals (44.1%), vegetables (23.6%), and fruits (16.0%). As regards men, the sources included refined bread (25.8%), vegetables (23.1%), and fruits (10.2%) and for women, they were vegetables (24.0%), fruits (17.2%), and refined bread (16.3%). Although refined bread is not recommended as a primary fiber source due to its lower fiber content compared to whole grain bread, its high consumption significantly contributed to the total fiber intake., Conclusion: The prevalence of widespread dietary fiber deficiency calls for the intensification of educational efforts that address the health advantages and sources of dietary fiber, as well as methods for its inclusion in daily meals., Competing Interests: The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest., (Copyright © 2024 Kucharska, Sińska, Panczyk, Samel-Kowalik, Szostak-Węgierek, Raciborski, Samoliński and Traczyk.)- Published
