Abstract Background Time-restricted eating (TRE) is a dietary regimen that limits food intake for at least 12 h daily. Unlike other fasting protocols, TRE does not dictate what or how much to eat but rather focuses on the timing of meals. This approach has been previously demonstrated to improve body composition in individuals with obesity or metabolic impairments. However, its impact on body composition and cardiometabolic factors in healthy individuals remains unclear. Furthermore, the optimal fasting duration is still debated. Thus, we aimed to compare the effects of 8 weeks of different fasting durations on body composition and biochemical parameters in metabolically healthy, non-trained individuals using a parallel randomized controlled trial. Methods Forty-one volunteers were randomly assigned to one of the four experimental groups: TRE 16:8 (16 h of fasting,8 h of eating), TRE 14:10 (14 h of fasting,10 h of eating), TRE 12:12 (12 h of fasting,12 h of eating) or a normal diet group (ND; no dietary restriction). Participants underwent body composition measurements and blood tests for lipid profiles (i.e., total cholesterol, LDL, HDL, and triglycerides), fasting glucose, leptin, and anabolic hormones (i.e., insulin and testosterone) levels. Data were analyzed using both intention-to-treat (ITT) and per-protocol (PP) analysis to account for compliance. A two-way ANOVA for repeated measures was employed to assess interactions between time and group. Results In the ITT analysis, TRE 16:8 reduced body mass (-2.46%, p = 0.003) and absolute fat mass (-8.65%, p = 0.001) with no changes in lean soft tissue and in calorie intake. These results were consistent with the PP analysis which included 8 participants in TRE 16:8, 5 in TRE 14:10, 9 in TRE 12:12, and the entire ND group. Participants in the TRE 16:8 group spontaneously reduced their total caloric intake, although this reduction was not statistically significant. None of the other measurements significantly changed after 8 weeks. Conclusions Our results suggest that a 16-hour fasting window, even without caloric restriction, may be a viable strategy for improving body composition in healthy and non-trained individuals, whereas a shorter fasting period may be insufficient to produce significant changes in a healthy population. Background and Objective:: Oropharynx Squamous Cell Carcinoma (OPSCC) linked to Human Papillomavirus (HPV) exhibits a more favorable prognosis than other squamous cell carcinomas of the upper aerodigestive tract. Finding reliable non-invasive detection methods of this prognostic entity is key to propose appropriate therapeutic decisions. This study aims to provide a comprehensive method based on pre-treatment clinical data for predicting the patient’s HPV status over a large OPSCC patient cohort and employing explainability techniques to interpret the significance and effects of the features. Materials and Methods:: We employed the RADCURE dataset clinical information to train six Machine Learning algorithms, evaluating them via cross-validation for grid search hyper-parameter tuning and feature selection as well as a final performance measurement on a 20% sample test set. For explainability, SHAP and LIME were used to identify the most relevant relationships and their effect on the predictive model. Furthermore, additional publicly available datasets were scrutinized to compare outcomes and assess the method’s generalization across diverse feature sets and populations. Results:: The best model yielded an AUC of 0.85, a sensitivity of 0.83, and a specificity of 0.75 over the testing set. The explainability analysis highlighted the remarkable significance of specific clinical attributes, in particular the oropharynx subsite tumor location and the patient’s smoking history. The contribution of each variable to the prediction was substantiated by creating a 95% confidence intervals of model coefficients by means of a 10,000 sample bootstrap and by analyzing top contributors across the best-performing models. Conclusions:: The combination of specific clinical factors typically collected for OPSCC patients, such as smoking habits and the tumor oropharynx sub-location, along with the ML models hereby presented, can by themselves provide an informed analysis of the HPV status, and of proper use of data science techniques to explain it. Future work should focus on adding other data modalities such as CT scans to enhance performance and to uncover new relations, thus aiding medical practitioners in diagnosing OPSCC more accurately.

RESUMO As atitudes colaborativas propiciam o cuidado integral, compartilhado e centralizado no usuário, na família e na comunidade, superando a assistência fragmentada. O presente estudo objetivou identificar atitudes colaborativas e fatores associados dos profissionais que atuam nas equipes da Estratégia Saúde da Família da Região Metropolitana de São Luís, Maranhão. Trata-se de um estudo transversal, analítico e quantitativo. A amostra foi constituída por 311 participantes. A coleta de dados ocorreu por meio de formulário eletrônico e aplicação da Escala Jefferson de Atitudes Relacionadas à Colaboração Interprofissional. As análises estatísticas foram realizadas pelo teste Qui-Quadrado, considerando p < 0,05. Os resultados demonstraram que os profissionais apresentaram atitudes colaborativas moderadas. Verificou-se, ainda, que o tipo de especialização (p = 0,008) e tempo de trabalho nas equipes da Estratégia Saúde da Família (p = 0,02) associaram-se a atitudes mais colaborativas, reforçando a importância do trabalho em equipe e da mão de obra qualificada para o desenvolvimento de atitudes positivas para colaboração em saúde.

ABSTRACT Neisserial adhesin A (NadA) is a meningococcal surface protein included as recombinant antigen in 4CMenB, a protein-based vaccine able to induce protective immune responses against Neisseria meningitidis serogroup B (MenB). Although NadA is involved in the adhesion/invasion of epithelial cells and human myeloid cells, its function in meningococcal physiology is still poorly understood. To clarify the role played by NadA in the host-pathogen interaction, we sought to identify its cellular receptors. We screened a protein microarray encompassing 2,846 human and 297 mouse surface/secreted recombinant proteins using recombinant NadA as probe. Efficient NadA binding was revealed on the paired sialic acid-binding immunoglobulin-type lectins receptors 5 and 14 (Siglec-5 and Siglec-14), but not on Siglec-9 therein used as control. The interaction was confirmed by biochemical tools with the determination of the KD value in the order of nanomolar and the identification of the NadA binding site by hydrogen-deuterium exchange coupled to mass spectrometry. The N-terminal domain of the Siglec-5 that recognizes the sialic acid was identified as the NadA binding domain. Intriguingly, exogenously added recombinant soluble Siglecs, including Siglec-9, were found to decorate N. meningitidis surface in a NadA-dependent manner. However, Siglec-5 and Siglec-14 transiently expressed in CHO-K1 cells endorsed NadA binding and increased N. meningitidis adhesion/invasion while Siglec-9 did not. Taken together, Siglec-5 and Siglec-14 satisfy all features of NadA receptors suggesting a possible role of NadA in the acute meningococcal infection.IMPORTANCEBacteria have developed several strategies for cell colonization and immune evasion. Knowledge of the host and pathogen factors involved in these mechanisms is crucial to build efficacious countermoves. Neisserial adhesin A (NadA) is a meningococcal surface protein included in the anti-meningococcus B vaccine 4CMenB, which mediates adhesion to and invasion of epithelial cells. Although NadA has been shown to bind to other cell types, like myeloid and endothelial cells, it still remains orphan of a defined host receptor. We have identified two strong NadA interactors, Siglec-5 and Siglec-14, which are mainly expressed on myeloid cells. This showcases that NadA is an additional and key player among the Neisseria meningitidis factors targeting immune cells. We thus provide novel insights on the strategies exploited by N. meningitidis during the infection process, which can progress to a severe illness and death.

Abstract Background Benzalkonium chloride (BAC) is a quaternary ammonium compound (QAC), that can be found in a wide variety of household products–from disinfectants to medicaments and home fragrances–but also professional products. In pets, cats have long been reported as more sensitive than dogs to QACs; in fact, signs of irritation such as oral ulcerations, stomatitis and pharyngitis can be observed after contact with concentrations of 2% or lower. In a review of 245 cases of BAC exposure in cats, reported by the Veterinary Poisons Information Service (United Kingdom) only 1.2% of the cases died or were euthanized. Nevertheless, BAC toxidromes in cats can result in transitory CNS and respiratory distress, as well as severe mucosal and cutaneous lesions. Currently, only a few reports are available concerning BAC poisoning in this species. Case presentation A 4 month-old kitten presented with severe glossitis, lameness in the hindlimbs and episodes of vomiting and diarrhoea. The cause was unknown until the owners reported use of a BAC-containing mould remover (5%) 4 days later. The patient developed severe oral burns requiring a pharyngeal tube for feeding and severe cutaneous chemical burns. The kitten was managed with supportive therapy and required hospitalization for 10 days. The symptoms disappeared completely 3 weeks after exposure. Conclusions BAC is a very common compound contained in several household and professional products but, to the best of our knowledge, no previous case had been reported in Italy. We hope that this report will help raise awareness on the hazards of BAC products for cats in both domestic and work contexts.

This study explores the perception of a “sense of place” among tourists visiting Hegra, an archaeological site in northern Saudi Arabia, through reviews on TripAdvisor. The 267 reviews on TripAdvisor between 2020 and 2023 were analyzed using the VADER sentiment polarity analysis tool and object modeling using the NMF machine learning algorithm. The results highlight positive factors linked to the history and uniqueness of the place while showing some critical issues related to isolation, cost, privatization, and competitiveness. The originality of the research lies in the type of case study chosen, an archaeological site of a country that has recently opened its doors to tourism, and in the pragmatic nature of the investigation, oriented towards the search for possible solutions to be adopted in terms of heritage management based on the feedback received for the development of the tourist destination.

Abstract Background The wild boar (Sus scrofa) and the Apennine wolf (Canis lupus italicus) are two wild species that have both increased their presence in the Italian territory, albeit in varying numbers. They can be occasionally found in peri-urban areas as well. Both of these species can serve as intermediate hosts for Toxoplasma gondii, as they can become infected either through the consumption of oocysts found in water, soil, or on vegetables, or through the ingestion of meat containing bradyzoites. Consequently, these animals can be regarded as key indicators of Toxoplasma presence in the wild or peri-urban environment. In our study, we examined a total of 174 wild boar meat juice and 128 wolf sera from Italy for the detection of T. gondii IgG using the indirect fluorescent antibody test (IFAT). Results The results showed that 40 (22.6%) of the wild boar meat juice and 34 (26.6%) of the wolf serum samples tested positive. Interestingly, there were no significant differences in seropositivity with respect to gender, age group, or the region of origin in both species. Conclusions Overall the results indicate a moderate exposure in both the species under investigation, highlighting the spread of T. gondii in sylvatic and periurban environments. The prevalence of T. gondii in wild boar is consistent with findings from other studies conducted in Europe. Our study, with a considerably larger sample size compared to the available research in European context, provides valuable data on the seroprevalence of T. gondii in wolves.

Background The recommendations of both the World Cancer Research Fund (WCRF) and the American Institute for Cancer Research (AICR) for the prevention of cancer are important public health tools. These recommendations for the prevention of specific cancers are related to body weight maintenance; physical activity; foods and drinks that promote weight gain; plant foods; foods of animal origin; alcoholic beverages; preservation, processing, and preparation of food; food supplements; and breastfeeding. Methods This study was a descriptive, cross-sectional, retrospective study. All patients provided written informed consent prior to enrollment in the study. Stratified random sampling was carried out with a convenience sample size of ≥280 participants. The characteristics of the participants were investigated using validated questions. Knowledge about the WCRF/AICR recommendations for the primary prevention of cancer was determined using 14 multiple choice questions validated in this study. Group A included participants who answered that cancer can be prevented and that lifestyle is the main factor related to the onset of cancer; the remaining participants were assigned to Group B. The χ2 test and Mann‒Whitney U test were used to determine differences in the groups. A p-value of ≤0.05 was considered statistically significant. A multiple linear regression analysis with gamma response was performed to assess the knowledge score. Results A total of 289 participants were included; 168 (58%) participants were in group A, and 121 (42%) participants were in group B. Using a 0 to 14 scale, the median (P25, P75) number of correct answers was 11 (10, 12). There was no evidence of a difference between groups in sex, relationship status, literacy skills, years of education, occupation, monthly income per person, and BMI. Most of the participants reported that they did not consume tobacco (n = 259/289) or alcohol (n = 261/289) in the week prior to completing the survey. Discussion Most of the participants (58%) considered cancer preventable and agreed that lifestyle factors were the most important factors in cancer development. The results also showed a high level of public awareness of some evidence-based associations between cancer and lifestyle factors, such as tobacco use. Some confusion among the public on other risk factors was also identified: waist circumference, a variable related to excess weight, was not identified as a risk factor for cancer by most of the participants, but the consumption of foods and beverages high in calories was identified as a risk factor by the majority of participants. Awareness of cancer protective and risk factors may lead to positive behavioral changes and eventually reduce the burden of cancer.

Abstract Coronavirus disease 2019 (COVID‐19) caused by the severe acute respiratory syndrome coronavirus 2n first appeared in Wuhan, China in 2019. Soon after, it was declared a pandemic by the World Health Organization. The health crisis imposed by a new virus and its rapid spread worldwide prompted the fast development of vaccines. For the first time in human history, two vaccines based on recombinant genetic material technology were approved for human use. These mRNA vaccines were applied in massive immunization programs around the world, followed by other vaccines based on more traditional approaches. Even though all vaccines were tested in clinical trials prior to their general administration, serious adverse events, usually of very low incidence, were mostly identified after application of millions of doses. Establishing a direct correlation (the cause‐effect paradigm) between vaccination and the appearance of adverse effects has proven challenging. This review focuses on the main adverse effects observed after vaccination, including anaphylaxis, myocarditis, vaccine‐induced thrombotic thrombocytopenia, Guillain–Barré syndrome, and transverse myelitis reported in the context of COVID‐19 vaccination. We highlight the symptoms, laboratory tests required for an adequate diagnosis, and briefly outline the recommended treatments for these adverse effects. The aim of this work is to increase awareness among healthcare personnel about the serious adverse events that may arise post‐vaccination. Regardless of the ongoing discussion about the safety of COVID‐19 vaccination, these adverse effects must be identified promptly and treated effectively to reduce the risk of complications.
