This work focuses on predicting near-term onset of hypotension prior to onset using convolutional neural networks. Based solely on the arterial blood pressure curve, our initial attempt can predict an onset with 60% sensitivity and 80% specificity 5-15 minutes before onset. Clinical relevance Hypotension is common during large surgery. By identifying and treating hypotensive episodes early, preferably even before onset, hypotension and its associate post- surgery complications are reduced. Even a prediction with 80% sensitivity/specificity is valuable for the anesthesiologist. QC 20211029

The healthcare system has been placed under an enormous burden by the SARS-CoV-2 (COVID-19) pandemic. In addition to the challenge of providing sufficient care for COVID-19 patients, there is also a need to ensure adequate care for non-COVID-19 patients. We investigated neurosurgical care in a university hospital during the pandemic. We examined the second wave of the pandemic from 1 October 2020 to 15 March 2021 in this retrospective single-center study and compared it to a pre-pandemic period from 1 October 2019 to 15 March 2020. Any neurosurgical intervention, along with patient- and treatment-dependent factors, were recorded. We also examined perioperative complications and unplanned readmissions. A statistical comparison of the study groups was performed. We treated 535 patients with a total of 602 neurosurgical surgeries during the pandemic. This compares to 602 patients with 717 surgeries during the pre-pandemic period. There were 67 fewer patients (reduction to 88.87%) admitted and 115 fewer surgeries (reduction to 83.96%) performed, which were essentially highly elective procedures, such as cervical spinal stenosis, intracranial neurinomas, and peripheral nerve lesions. Regarding complication rates and unplanned readmissions, there was no significant difference between the COVID-19 pandemic and the non-pandemic patient group. Operative capacities were slightly reduced to 88% due to the pandemic. Nevertheless, comprehensive emergency and elective care was guaranteed in our university hospital. This speaks for the sufficient resources and high-quality processes that existed even before the pandemic.

Berichten des Bundeskriminalamtes zufolge konnten im Jahr 2016 insgesamt 72 Betroffene des Menschenhandels und des grenzüberschreitenden Prostitutionshandels erkannt werden. Fünf Betroffene kamen aus Nigeria, chinesische Betroffene gab es keine. Erlaubt diese sehr niedrige Zahl identifizierter nigerianischer und chinesischer Betroffener – die im Falle der Nigerianerinnen gleichzeitig die höchste Zahl seit 2012 ist – den Schluss, dass bis 2016 der nigerianische und chinesische Menschenhandel kaum eine Rolle gespielt hat? Ein Bericht von IOM aus dem Jahr 2017 vermittelt ein anderes Bild: So schätzt IOM, dass 80 % der nigerianischen Frauen, die in Italien ankommen, von Menschenhandel betroffenen sind. Könnte diese Schätzung auch für Österreich zutreffen? Wie haben sich die Zahlen weiterentwickelt? Könnte es sein, dass ein Großteil der Betroffenen nicht erkannt wird? Und falls ja: Was ist es, das nigerianische und chinesische Betroffene daran hindert, sich als Opfer an die Polizei zu wenden bzw. eine Aussage zu machen? Die Organisation Herzwerk arbeitet seit 12 Jahren in der aufsuchenden Sozialarbeit mit Menschen in der Prostitution und betreut zahlreiche Nigerianerinnen und Chinesinnen, die von sexueller Ausbeutung betroffen sind, aber meist nicht gegen die Täterinnen aussagen. Im EU­ geförderten internationalen Forschungsprojekt „Best Practices in Tackling Trafficking Nigerian Route" (BINls) wurden Betroffene aus Nigeria in den Fokus genommen und unter anderem Faktoren, die oft einer Aussage im Wege stehen, aufgezeigt. Im Rahmen des laufenden EU­ geförderten Projekts „lntersectional Approach to the Process of Integration in Europe for Survivors of Human Trafficking" (lNTAP) kommen nigerianische und chinesische Betroffene selbst zu Wort. Der Beitrag möchte ein Licht auf das Dunkelfeld drittstaatsangehöriger Betroffener des Menschenhandels zur sexuellen Ausbeutung in Österreich werfen.

The occurrence of gbm in adult members of the same family and in the absence of tumor syndromes is extremely rare. We describe the cases of a brother and a sister with multifocal gbm diagnosed at the age of 63 years. We discuss three further paired gbm in adult patients from the literature.Patients and results: The sister was diagnosed with multifocal primary gbm in 2014 at the age of 63 years and 6 months. In 2018, her younger brother had to be operated on for a multifocal primary gbm at the age of 63 years and 9 months. Extended neuropathological examination revealed most markers to be similar, except for the percentage of O6,-methylguanine-DNA methyltransferase promotor methylation, the presence of intratumoral immune cells and the immunohistochemical expression of C12ORF75. Comparison with further published cases of familial adult GBM reveals that most of these patients are male, about 65 years old and the tumor is localized predominantly in the left temporal lobe.Conclusion: Paired adult familial gbm occurs mainly in the elderly male patient with an integrative diagnosis of primary gbm. Whereas a statistical coincidence seems to be most likely in these rare cases, supplementary and improved genetic studies may identify pathogenetic causes of gbm.Keywords: multifocal glioblastoma, familial tumor clustering, IDH-1 wildtype, tumor syndrome

Die vorliegende Diplomarbeit mit dem Titel „Selbst- und Fremdermächtigung von Frauen in der honduranischen Gesellschaft: Welche Plattformen der Ermächtigung haben Frauen in Honduras genutzt und welche Rolle spielt die westliche Entwicklungszusammenarbeit?“ beschäftigt sich mit den verschiedenen, im Bereich „Frauenthemen“ engagierten, Akteuren – Frauen- und feministische Bewegung, Frauen in der Politik, EZA – und setzt sich kritisch mit ihnen auseinander. Grundsätzlich stellt sich unter anderem die Frage: Ist es überhaupt möglich, jemand anderen „zu ermächtigen“, oder stellt dies bereits einen Widerspruch dar? An der Frage wird deutlich, dass ein entwicklungskritischer Ansatz – neben den verschiedenen Ansätzen postkolonialer Feministinnen, wie Peggy Antrobus, Chandra Talpade Mohanty und Srilatha Batliwala sowie Werken honduranischer Autorinnen wie Breny Mendoza und Rina Villars – die theoretische Basis der Arbeit bildet. Mit Hilfe der Ergebnisse einer zwei-monatigen empirischen Forschung in Form von Interviews mit den erwähnten Akteuren wird der Stand der Frauen- und Entwicklungsforschung in Honduras aktualisiert und um neue Elemente, die sich aus den politischen Veränderungen nach dem Putsch 2009 ergeben haben, erweitert. Die zwei Hypothesen, die der Arbeit zugrunde liegen, fokussieren sich zum Einen auf Machtbeziehungen, die zwischen den verschiedenen Akteuren entlang von Grenzen wie race, class und gender existieren, und zum Anderen auf die Frage nach dem Empowerment-Potential der Plattformen, die Frauen in Honduras für sich nutzen. So wird davon ausgegangen, dass die westlichen EZA-„Partner“ aufgrund der bestehenden finanziellen Abhängigkeit zwischen „Gebern“ und „Nehmern“ einen starken Einfluss auf die honduranischen Akteure und ihre Zielsetzungen ausüben können. Auch zwischen Frauen in der Politik und Frauen der sozialen Bewegung werden Machtbeziehungen erwartet. Als Plattformen werden neben der Frauen- und feministischen Bewegung und der honduranischen Politik auch die westliche EZA und die Widerstandsbewegung Frente Nacional de Resistencia Popular, die nach dem Putsch 2009 gegründet wurde, analysiert. Letzterer wird ein besonderes Potential bezüglich der Idee einer Selbst-Ermächtigung zugeschrieben, während angenommen wird, dass die westliche EZA eher „fremdermächtigt“. So zeigt die vorliegende Diplomarbeit nicht nur auf, welchen spezifischen Herausforderungen Frauen in Honduras begegnen und welchen Einfluss die Veränderungen politischer Rahmenbedingungen ausüben, sondern verhilft darüber hinaus zu einem allgemeinen Verständnis von Machtbeziehungen und Abhängigkeitsverhältnissen, die zwischen dem „Norden“ und dem „Süden“, Frauen und Männern und auch zwischen Frauen selbst bestehen., The presented diploma thesis entitled “Self- and External Empowerment of Women in the Honduran Society: Which empowerment platforms have women in Honduras used and what role does Western development cooperation play?” deals with the different actors involved in the area of “woman’s issues” - the women’s and feminist movement, women in politics, development cooperation (hereafter 'DC') - and critically evaluates them. Fundamentally, one of the questions being asked is: Is it even possible to “empower” another person, or does this present a contradiction in itself? This question makes clear that an approach evaluating development – besides the various approaches of post-colonial feminists such as Peggy Antrobus, Chandra Talpade Mohanty and Srilatha Batliwala as well as works of Honduran authors such as Breny Mendoza and Rina Villars – builds the theoretical basis of the paper. Using the results of two months of empirical research in the form of interviews with the above-mentioned actors, the state of research in the areas of women and development is updated and expanded by new elements that resulted from the political changes after the coup of 2009. The two hypotheses which form the foundations of the diploma thesis focus on the one hand on the power relationships that exist between the different actors along the lines of race, class and gender, and on the other hand deal with the issue of the empowerment potential of the platforms that women in Honduras use for themselves. It is then assumed that the Western DC “partners”, on the basis of the existing financial dependency between “donors” and “recipients”, can render a strong influence upon the Honduran actors and their objectives. Power relationships are also expected between women in politics and women of the social movement. The platforms of the women's and feminist movement and Honduran politics are analyzed as well as the Western DC and the resistance movement Frente Nacion de Resistencia Popular, which was founded after the coup of 2009. A particular self-empowerment potential is ascribed to the latter while it is presumed that the Western DC is mainly empowering “externally”. Thus the diploma thesis not only highlights the specific challenges women in Honduras face and the influence changes in political conditions exert, but additionally helps provide a general understanding of power relationships and independent relationships that exist between the north and the south, women and men and between women themselves.

The organisation of unscheduled health care in France is complex and constantlyevolving. It counts many players, who either work in hospitals or independently. General practitioners have a crucial role in the support provided to patients and in the unscheduled health care system, given their position as the first medical link in the emergency chain. Their role is reinforced in rural areas, which suffer from a growing medical desertification. Indeed they need to ensure ? the right medical care? to the entire population. This medical support in the first remedy area requires an active participation of GPs and the need for training. The aim of our study is to assess the impression of GPs, via questionnaires, concerning medical support for emergency situations, in their daily practice. The population counts 162 independent GPs, who work in the Meuse. 67 GPs answered our questionnaire. The majority of them don? t feel at ease, or not very much, when faced with an emergency situation and 47 of them are favorable to a tailored training course provided by the CESU 55. In order to improve health care, particularly unscheduled care in rural areas, it would be. useful to provide emergency actions training at the CESU to GPs who are interested. Weshould also continue ongoing action regarding the development of health care homes. These suggestions are part of the current territorial Health project and of the coordination between hospital and independent medicine, based on a health care network; L'organisation du système de soins non programmés en France est complexe et en constante évolution. Elle regroupe de nombreux acteurs qu'ils soient hospitaliers ou libéraux. Le médecin généraliste tient une place primordiale dans la prise en charge globale du patient et dans le système de soins non programmés, en tant que premier maillon médical de la chaîne des secours, en particulier dans les zones rurales permettant ainsi d'assurer « le juste soin » à l'ensemble de la population.Notre étude propose d'évaluer, au moyen d'un questionnaire, le ressenti des médecins généralistes concernant la prise en charge de situations d'urgence dans leur pratique quotidienne. La population source regroupe 162 médecins généralistes libéraux exerçant sur le territoire meusien. Parmi cette population, les médecins ayant une activité d'urgence (MCS et MSP) sont surreprésentés par rapport à la population médicale globale du département. 67 des médecins interrogés ont répondu à ce questionnaire avec une prédominance de médecins en fin de carrière. La majorité d'entre eux se sentent peu ou pas à l'aise face à ces situations. Paradoxalement, ils semblent plus à l?aise avec un certain nombre de techniques utilisables en situation d'urgence. 47 d'entre eux sont favorables à uneformation adaptée proposée par le CESU 55. Afin d'améliorer l'offre de soins notamment concernant les soins non programmés dans les zones rurales, il est souhaitable d'offrir la possibilité d?une formation aux gestes et soins d'urgence aux médecins généralistes qui le souhaitent au sein des CESU. Il convient également, et plus que jamais, de poursuivre les actions en cours concernant le développement des maisons de santé. Ces propositions s'inscrivent dans le projet global actuel de Politique de Santé des territoires et dans la coordination entre médecine hospitalière et libérale selon un fonctionnement interactif des réseaux de soins
