Rupture of an intracranial aneurysm leads to subarachnoid hemorrhage, a severe type of stroke. To discover new risk loci and the genetic architecture of intracranial aneurysms, we performed a cross-ancestry, genome-wide association study in 10,754 cases and 306,882 controls of European and East Asian ancestry. We discovered 17 risk loci, 11 of which are new. We reveal a polygenic architecture and explain over half of the disease heritability. We show a high genetic correlation between ruptured and unruptured intracranial aneurysms. We also find a suggestive role for endothelial cells by using gene mapping and heritability enrichment. Drug-target enrichment shows pleiotropy between intracranial aneurysms and antiepileptic and sex hormone drugs, providing insights into intracranial aneurysm pathophysiology. Finally, genetic risks for smoking and high blood pressure, the two main clinical risk factors, play important roles in intracranial aneurysm risk, and drive most of the genetic correlation between intracranial aneurysms and other cerebrovascular traits.

Background - To counteract a decreasing number of physician-scientists, a national intercalated Medical Student Research Programme (MSRP) was launched in Norway in 2002. We aimed to assess whether the students’ favourable perceptions and satisfaction with the program had prevailed since the inception in 2002 and until 2015, and to identify factors associated with pursuing a PhD. Methods - The study was an incorporation of data from two previous national evaluations of the MSRP performed in 2007 and 2015. We used electronic questionnaires to explore demographic characteristics, area and type of research, student satisfaction, and future scientific goals. In 2007, questionnaires were sent to all 208 students, and 183 (88%) replied. In 2015, the corresponding numbers were 279, and 240 (86%). Categorical data were analysed using either Kruskal-Wallis or Pearson’s chi square test. Differences between sample means were assessed with Student`s t-test while logistic regression was used to test associations between selected covariates and the students’ ambitions to pursue a PhD degree. Results - Overall, the student satisfaction was 79%. However, more students in 2015 received less regular and less supervision time and expressed a need for more of it. Seventy-seven per cent expressed an ambition to pursue a PhD. Students were more likely to have a PhD ambition if they were satisfied with the program, had a supervisor with high expectations for them, or had already published some of their results. At both time points, students (86% vs. 89%) responded that the MSRP had a positive impact on their regular curriculum achievements. Conclusions - The high degree of satisfaction with the national MSRP among undergraduate students has prevailed since the inception in 2002. By far, the program has also met its goal to increase the number of aspiring physician-scientists. However, to maintain that goal over time, adequate and personal supervision is a prerequisite.

Rupture of an intracranial aneurysm leads to subarachnoid hemorrhage, a severe type of stroke. To discover new risk loci and the genetic architecture of intracranial aneurysms, we performed a cross-ancestry, genome-wide association study in 10,754 cases and 306,882 controls of European and East Asian ancestry. We discovered 17 risk loci, 11 of which are new. We reveal a polygenic architecture and explain over half of the disease heritability. We show a high genetic correlation between ruptured and unruptured intracranial aneurysms. We also find a suggestive role for endothelial cells by using gene mapping and heritability enrichment. Drug-target enrichment shows pleiotropy between intracranial aneurysms and antiepileptic and sex hormone drugs, providing insights into intracranial aneurysm pathophysiology. Finally, genetic risks for smoking and high blood pressure, the two main clinical risk factors, play important roles in intracranial aneurysm risk, and drive most of the genetic correlation between intracranial aneurysms and other cerebrovascular traits.

Forekomst, dødelighet og risikofaktorer for subaraknoidalblødning Prospektive analyser av HUNT og Tromsøundersøkelsen Subaraknoidalblødning (SAB) utgjør omtrent 1-7 % av alle hjerneslag. Forekomsten har blitt estimert til å ligge mellom 7 og 10 per 100.000 personår. Det virker som forekomsten er relativt stabil over tid og mellom land, unntatt i Finland og Japan, hvor forekomsten er høyere. Noen studier har antydet at forekomsten har gått noe ned de senere tiår, mens sykeligheten og dødeligheten etter SAB fortsatt er høy. Siden blødningen er relativt sjelden, har det vært vanskelig å påvise risikofaktorer i prospektive studier, men røyking, høyt blodtrykk, høyt alkoholinntak og det å være kvinne er kjent å øke risikoen. Det er mer usikkerhet vedrørende kroppsmasseindeks (KMI) og serumlipider. Det er også lite kunnskap om hvilke kjennetegn ved pasientene som fører til økt risiko for død etter SAB. Vi registrerte alle som fikk SAB etter deltakelse i befolkningsundersøkelsene HUNT 1 og 2 (1984-86 og 1995-97) og Tromsø 3 og 4 (1985-87og 1994-95) ved hjelp av diagnoseregistrene på sykehusene som populasjonene sogner til og Dødsårsaksregisteret. Vi identifiserte til sammen 214 pasienter. Vi estimerte forekomst og overlevelse, og studerte effekten av risikofaktorer ved hjelp av Cox og Poisson regresjonsanalyser. De fire artiklene omhandler forekomst og dødelighet av SAB (artikkel I, HUNT 1 og 2 og Tromsø 3 og 4), risikofaktorer for SAB (artikkel II, HUNT 1), KMI og serum lipider og risiko for SAB (artikkel III, HUNT 2 og Tromsø 4) og kjønnsforskjeller i risikofaktorer for SAB (artikkel IV, HUNT 2 og Tromsø 4). Vi fant at forekomsten av SAB fra 1984 til 2007 var 10,3 per 100.000 personår, 13,3 hos kvinner og 7,1 hos menn. Insidensen kan ha økt litt siden 1984, noe som kan skyldes endring i diagnostikk, men den har vært stabil siden 1995. 30 dagers letalitet var 36 % og tenderte til å øke med alder, men forble stabil over de 23 årene i oppfølgningen. Vi fant en lineær og positiv sammenheng mellom systolisk og diastolisk blodtrykk og risiko for SAB. Røykere hadde høyere risiko for SAB enn dem som aldri hadde røykt, og det kan virke som om risikoen assosiert med røyking er høyere hos kvinner enn hos menn. Det kan også virke som om overvekt (KMI 25-29.9) er negativt assosiert med risiko for SAB. Derimot fant vi ingen sammenheng mellom totalkolesterol, HDL-kolesterol eller triglyserider og risiko for SAB i totalpopulasjonen, men hos deltakere under 50 år var HDL-kolesterol negativt assosiert med risiko.

- Abstract
Twice-daily (BID) thoracic radiotherapy (TRT) of 45 Gy per 30 fractions is recommended for limited-stage (LS) SCLC, but most patients are treated with once-daily (OD) schedules owing to toxicity concerns and logistic challenges. An alternative is hypofractionated OD TRT of 40 to 42 Gy per 15 fractions. A randomized trial by our group indicated that TRT of 45 Gy per 30 fractions is more effective than TRT of 42 Gy per 15 fractions, and because it was not more toxic, 45 BID replaced 42 OD as the recommended schedule in Norway. The aims of this study were to evaluate to what extent BID TRT has been implemented in Norway and whether this practice change has led to improved survival.

Introduction: It is not known whether increased breast cancer risk caused by menopausal hormone therapy (HT) depends on body mass patterns through life. Aims: To study body mass patterns from childhood until postmenopausal age, and assess whether the association of HT use with breast cancer risk could be modified by different patterns of body mass through life. Methods: In a prospective study of 483,241 Norwegian women aged 50-69 years at baseline, 7,656 women developed breast cancer during follow-up (2006-2013). We combined baseline information on recalled body mass in childhood/adolescence and current (baseline) body mass index (BMI) to construct mutually exclusive life-course body mass patterns. We assessed associations of current HT use with breast cancer risk according to baseline BMI and life-course patterns of body mass, and estimated relative excess risk due to interaction (RERI). Results: Within all levels of baseline BMI, HT use was associated with increased risk. Considering life-course body mass patterns as a single exposure, we used women who "remained at normal weight" through life as the reference, and found that being "overweight as young" was associated with lower risk (hazard ratio (HR) 0.85, 95% confidence interval (CI) 0.76-0.94), whereas women who "gained weight" had higher risk (HR 1.20, 95% CI 1.12-1.28). Compared to never users of HT who were "overweight as young", HT users who either "remained at normal weight" or "gained weight" in adulthood were at higher risk than expected when adding the separate risks (RERI 0.52, 95% CI 0.09-0.95, and RERI 0.37, 95% CI -0.07-0.80), suggesting effect modification. Conclusions: Thus, we found that women who remain at normal weight or gain weight in adulthood may be more susceptible to the risk increasing effect of HT compared to women who were overweight as young.
