Abstract

We study a recently introduced base $b$ polynomial analog of Stern's diatomic sequence, which generalizes Stern polynomials of Klavar, Dilcher, Ericksen, Mansour, Stolarsky, and others. We lift some basic properties of base $2$ Stern polynomials to arbitrary base, and introduce a matrix characterization of Stern polynomials. Abstract

The structural evolution of Andean-type orogens is strongly influenced by the geometry of the subducting slab. This study focuses on the flat-slab subduction of the Nazca Ridge and its effects on the South American Plate. The process of flat slab subduction impacts the stress distribution within the overriding plate and increases plate coupling and seismic energy release. Using the finite element method (FEM), we analyse interseismic and coseismic deformation along a 1000 km transect parallel to the ridge. We examine stress distribution, uplift patterns, and the impact of megathrust activity on deformation. To better define the crust's properties for the model, we developed a new thermal model of the Nazca Ridge subduction zone, reconstructing the thermal structure of the overriding plate. The results show concentrated stress at the upper part of the locked plate interface, extending into the Coastal and Western Cordilleras, with deeper stress zones correlating with seismicity. Uplift patterns align with long-term rates of 0.7–1 mm/yr. Cooling from flat-slab subduction strengthens the overriding plate, allowing far-field stress transmission and deformation. These findings provide insights into the tectonic processes driving stress accumulation, seismicity, and uplift along the Peruvian margin. [ABSTRACT FROM AUTHOR]

Abstract

Background: Breast cancer represents one of the leading causes of death among women. Surgery can be effective, but once breast cancer has metastasized, it becomes extremely difficult to treat. Conventional therapies are associated with substantial toxicity and poor efficacy due to tumor heterogeneity, treatment resistance and disease relapse. Moreover, immune checkpoint blockade appears to offer limited benefit in breast cancer. The poor tumor immunogenicity and the immunosuppressive tumor microenvironment result in scarce T-cell infiltration, leading to a low response rate. Thus, there is considerable interest in the development of improved active immunotherapies capable of sensitizing a patient's immune system against tumor cells. Methods: We evaluated the in vitro anti-tumor activity of a personalized vaccine based on dendritic cells generated in the presence of interferon (IFN)-α and granulocyte-macrophage colony-stimulating factor (IFN-DC) and loaded with an oxidized lysate from autologous tumor cells expanded as 3D organoid culture maintaining faithful tumor antigenic profiles. Results: Our findings demonstrate that stimulation of breast cancer patients' lymphocytes with autologous IFN-DC led to efficient Th1-biased response and the generation in vitro of potent cytotoxic activity toward the patients' own tumor cells. Conclusions: This approach can be potentially applied in association with checkpoint blockade and chemotherapy in the design of new combinatorial therapies for breast cancer. [ABSTRACT FROM AUTHOR]

18. Normal-fault stress and displacement through finite-element analysis

Abstract

We compute displacement and stress due to a normal fault by means of two-dimensional plane-strain finite-element analysis. To do so, we apply a system of forces to the fault nodes and develop an iterative algorithm serving to determine the force magnitudes for any slip distribution. As a sample case, we compute the force magnitudes assuming uniform slip on a 10-km two-dimensional normal fault. The numerical model generates displacement and stress fields that compare well to the analytical solution. In fact, we find little difference in displacements (<5%), displacement orientation (<15 DEG), and stress components (<35%, half of which due to slip tolerance). We analyze such misfit, and discuss how the error propagates from displacement to stress. Our scheme provides a convenient way to use the finite-elements direct method in a trial-and-error procedure to reproduce any smooth slip distribution., Comment: 14 pages, 5 figures. Abstract

COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March–April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients’ management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons’ personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon–patient/family communication. Abstract

Introduction: The impact of COVID-19 pandemic represents a serious challenge for ‘frail’ patients' populations with inflammatory autoimmune systemic diseases such as systemic sclerosis (SSc). We investigated the prevalence and severity of COVID-19, as well the effects of COVID-19 vaccination campaign in a large series of SSc patients followed for the entire period (first 38 months) of pandemic. Patients and method: This prospective survey study included 1755 unselected SSc patients (186 M, 1,569F; mean age 58.7 ± 13.4SD years, mean disease duration 8.8 ± 7.3SD years) recruited in part by telephone survey at 37 referral centers from February 2020 to April 2023. The following parameters were carefully evaluated: i. demographic, clinical, serological, and therapeutical features; ii. prevalence and severity of COVID-19; and iii. safety, immunogenicity, and efficacy of COVID-19 vaccines. Results: The prevalence of COVID-19 recorded during the whole pandemic was significantly higher compared to Italian general population (47.3 % vs 43.3 %, p < 0.000), as well the COVID-19-related mortality (1.91 % vs 0.72 %, p < 0.001). As regards the putative prognostic factors of worse outcome, COVID-19 positive patients with SSc-related interstitial lung involvement showed significantly higher percentage of COVID-19-related hospitalization compared to those without (5.85 % vs 1.73 %; p < 0.0001), as well as of mortality rate (2.01 % vs 0.4 %; p = 0.002). Over half of patients (56.3 %) received the first two plus one booster dose of vaccine; while a fourth dose was administered to 35.6 %, and only few of them (1.99 %) had five or more doses of vaccine. Of note, an impaired seroconversion was recorded in 25.6 % of individuals after the first 2 doses of vaccine, and in 8.4 % of patients also after the booster dose. Abstract

Purpose: Mixed cryoglobulinemia syndrome (MCs) is a rare immunoproliferative systemic disorder with cutaneous and multiple organ involvement. Our multicenter survey study aimed to investigate the prevalence and outcome of COVID-19 and the safety and immunogenicity of COVID-19 vaccines in a large MCs series. Methods: The survey included 430 unselected MCs patients (130 M, 300 F; mean age 70 ± 10.96 years) consecutively collected at 11 Italian referral centers. Disease classification, clinico-serological assessment, COVID-19 tests, and vaccination immunogenicity were carried out according to current methodologies. Results: A significantly higher prevalence of COVID-19 was found in MCs patients compared to Italian general population (11.9% vs 8.0%, p < 0.005), and the use of immunomodulators was associated to a higher risk to get infected (p = 0.0166). Moreover, higher mortality rate was recorded in MCs with COVID-19 compared to those without (p < 0.01). Patients’ older age (≥ 60 years) correlated with worse COVID-19 outcomes. The 87% of patients underwent vaccination and 50% a booster dose. Of note, vaccine-related disease flares/worsening were significantly less frequent than those associated to COVID-19 (p = 0.0012). Impaired vaccination immunogenicity was observed in MCs patients compared to controls either after the first vaccination (p = 0.0039) and also after the booster dose (p = 0.05). Finally, some immunomodulators, namely, rituximab and glucocorticoids, hampered the vaccine-induced immunogenicity (p = 0.029). Conclusions: The present survey revealed an increased prevalence and morbidity of COVID-19 in MCs patients, as well an impaired immunogenicity even after booster vaccination with high rate of no response. Therefore, MCs can be included among frail populations at high risk of infection and severe COVID-19 manifestations, suggesting the need of a close monitoring and specific preventive/therapeutical measures during the ongoing pandemic.

Abstract

New-generation mRNA and adenovirus vectored vaccines against SARS-CoV-2 spike protein are endowed with immunogenic, inflammatory and immunomodulatory properties. Recently, BioNTech developed a noninflammatory tolerogenic mRNA vaccine (MOGm1 psi) that induces in mice robust expansion of antigen-specific regulatory T (Treg) cells. The Pfizer/BioNTech BNT162b2 mRNA vaccine against SARS-CoV-2 is identical to MOGm1 psi except for the lipid carrier, which differs for containing lipid nanoparticles rather than lipoplex. Here we report that vaccination with BNT162b2 led to an increase in the frequency and absolute count of CD4(pos)CD25(high)CD127(low) putative Treg cells; in sharp contrast, vaccination with the adenovirus-vectored ChAdOx1 nCoV-19 vaccine led to a significant decrease of CD4(pos)CD25(high) cells. This pilot study is very preliminary, suffers from important limitations and, frustratingly, very hardly can be refined in Italy because of the >90% vaccination coverage. Thus, the provocative perspective that BNT162b2 and MOGm1 psi may share the capacity to promote expansion of Treg cells deserves confirmatory studies in other settings.

26. 2D Geothermal model across the Peru-Chile trench and the Andean Cordillera above the Nazca Ridge subduction

Abstract

The Nazca Ridge is a wide aseismic ridge subducting beneath the South American margin at latitude about 15°. The buoyancy of the thickened oceanic crust of the Nazca Ridge produces localized flat subduction influencing the geometry and the geological history of the whole area. With the aim of analysing the spatio-temporal evolution of the deformation and uplift/subsidence history of the lithosphere above the Nazca Ridge flat slab, we have started from the study of the geothermal structure of the upper plate. We have built a crustal section with a length of 1000 km that reaches a depth of about 130 km. The section runs from the top of the Nazca Ridge in the west to the Amazonian Basin in the east, progressively crossing the Peru-Chile trench, the East Pisco Basin and the Andean Cordillera. Thereafter we have elaborated a 2D geothermal model based on the crustal section. We have considered the whole lithosphere composed of two main geological units: (i) crystalline basement, (ii) sedimentary cover (including the whole lithostratigraphic succession). For each unit we have assigned the following parameters: thickness, density, heat production and thermal conductivity. Moreover, we have also taken into account the friction coefficient, the convergence rate of the plates, the heat flux of the Moho, and the slip rate of the megathrust. Model parameters have been set up in order to obtain the best simulation of the heat flow contribution due to the large reverse fault responsible for the coastal seismic event of November 12, 1996, with epicentre on the section trace. Using these parameters and applying an analytical methodology we have calculated isotherms and geotherms. The resulting model may provide an important contribution on the investigation of the effects of the Nazca Ridge subduction and the associated flat slab geometry on the tectonic evolution of the area.

Abstract

Laboratory Medicine has undergone significant evolution in recent decades, shifting its focus from emphasizing high quality in technical aspects to achieving diagnostic excellence. The "Manifesto" proposed by Plebani et al. some years ago, underscores the central role of Laboratory Medicine in the diagnostic-therapeutic process, outlining the challenges and opportunities facing the discipline. It emphasizes the crucial role of laboratory professionals in addressing these challenges and promoting its advancement. The competencies and roles of laboratory professionals in different laboratory activities need continuous development and re-evaluation to effectively manage the rapid advancement of science and technology, the complex organizational models, the increasing burden of chronic diseases, decentralized healthcare systems, and disparities in Health Information Technology access. Furthermore, the ability to collaborate with clinical specialists is emphasized as essential for highlighting the value of laboratory medicine in the clinical pathway. Similarly, the support of professionals from different disciplines in using new management tools and technologies, such as artificial intelligence and big data, can ensure the reliability of derived information and compliance with ethical considerations, data privacy, and security. In conclusion, the Manifesto advocates for a patient-centered approach and the recognition of laboratory professionals' skills and abilities to secure a promising future for Laboratory Medicine. Abstract

Autoimmune systemic diseases (ASD) show impaired immunogenicity to COVID-19 vaccines. Our prospective observational multicenter study aimed at evaluating the seroconversion elicited by COVID-19 vaccine over the entire vaccination cycle including the booster dose. Among 478 unselected ASD patients originally evaluated at the end of the first vaccination cycle (time 1), 344 individuals were re-evaluated after a 6-month period (time 2), and 244 after the booster vaccine dose (time 3). The immunogenicity of mRNA COVID-19 vaccines (BNT162b2 and mRNA-1273) was assessed by measuring serum IgG-neutralizing antibody (NAb) on samples obtained at the three time points in both patients and 502 age-matched controls. In the 244 ASD group that received booster vaccine and monitored over the entire follow-up, the mean serum NAb levels (time 1, 2, and 3: 696.8 ± 52.68, 370.8 ± 41.92, and 1527 ± 74.16SD BAU/mL, respectively; p < 0.0001) were constantly lower compared to controls (p < 0.0001), but they significantly increased after the booster dose compared to the first two measurements (p < 0.0001). The percentage of patients with absent/suboptimal response to vaccine significantly decreased after the booster dose compared to the first and second evaluations (time 1, 2, and 3: from 28.2% to 46.3%, and to 7.8%, respectively; p < 0.0001). Of note, the percentage of patients with absent/suboptimal response after the booster dose was significantly higher compared to controls (19/244, 7.8% vs 1/502, 0.2%; p < 0.0001). Similarly, treatment with immune-modifiers increased the percentage of patients exhibiting absent/suboptimal response (16/122, 13.1% vs 3/122, 2.46%; p = 0.0031). Overall, the above findings indicate the usefulness of booster vaccine administration in ASD patients. Abstract

This study monitored the anti-spike-receptor-binding domain (RBD) and neutralizing antibodies induced by the Pfizer/BioNTech mRNA BNT162b2 vaccine in a cohort of 163 healthcare workers aged ≤60 years. We have taken advantage of two study groups, both of whom received the first two doses in the same time window, but Group 1 (54 HCWs) received the third dose 2 months before Group 2 (68 HCWs) did. The cohorts were monitored from the 12th day after the first vaccine dose up to 1 month after the third vaccine dose for a total of eight time points and about 1 year of surveillance (T1 = 12 days after the first dose; T2 = 10 days after the second dose; T3 = 1 month after the second dose; T4 = 3 months after the second dose; T5 = 4 months after the second dose; T6 = 5 months after the second dose; T7 = 7 months after the second dose; T8 = 1 month after the third dose for Group 1; T8* = 9 months after the second dose for Group 2; T9 = 1 month after the third dose for Group 2). The mean value of anti-spike antibodies decreased faster over time, but at T7, its decline was significantly slowed (T7 vs. T8*). After the third dose, the anti-spike titer rose about 34-fold (T7 vs. T8 and T8* vs. T9) and the booster improved the anti-spike titer by about three times compared with that of the second dose (T3 vs. T8 and T3 vs. T9), and no difference was noted between the two groups. The neutralizing titer was evaluated at T3, T7, T8, and T9. Anti-spike and neutralizing antibodies were found to be strongly correlated (r2 = 0.980; p < 0.001). At T3, 70% of the participants had a neutralizing antibody titer >91% of total anti-spike antibodies that increased to 90% after the third dose (T8 and T9). However, when the anti-spike titer reached its lowest value (T7), the neutralizing antibody levels decreased even further, representing only 44% of total anti-spike antibodies (p < 0.0001).
