Abstract

To evaluate the efficacy of extracorporeal shockwaves therapy (ESWT) in patients with chronic painful shoulder.a prospective design was used with a short term follow-up.data were collected at the Service of Physioterapy in Villa Stuart Clinic-Rome.72 patients of both sexes with an average age of 38 years (range 18 to 69) affected by chronic painful shoulder, whose symptoms lasted more than 6 months.all patients received on average, 2,000 impulses of ESWT at level 5 energy (0.21 mJ/mm2) according to Dornier Epos equipment one session per week for 8 weeks.all cases underwent an instrumental examination with X-ray conventional imaging and clinical assessment of pain (visual analogue scale) and functional limitation of the shoulder, two weeks before the treatment and at one month follow-up.53% of patient scored excellent results, 14% good, 13% fair and 20% poor. In the group of calcifying tendinitis we had a reduction in 37% and no changes in 63%.Even with a limited number of cases our early results show the efficacy and safety of ESWT in the treatment of chronic painful shoulder resistant to other conservative approaches.

Abstract

The aim of this study was to evaluate the differential localisation of glycoconjugates of bovine hyaline cartilage matrix by lectin histochemistry, to compare the results of lectin histochemistry with those that can be obtained in the same tissue with PAS and alcian blue. Frozen and paraffin sections were stained with HE, PAS and alcian blue (pH 1.8). Alcian blue staining was carried out also after 1 and 24 hour digestion with bovine testicular hyaluronidase. Peroxidase conjugated WGA, PNA and RS lectins were tested on all sections before and after 1 hour digestion with bovine testicular hyaluronidase. The results show that all the lectins used in this study react with sugars linked to proteoglycans of territorial matrix, the reaction being increased in territorial, and induced in interterritorial matrix by 1 hour hyaluronidase digestion. Alcian blue at pH 1.8 and PAS were complementary, the former staining territorial, and the latter interterritorial matrix. After 1 hour hyaluronidase digestion, alcian blue stained also the interterritorial matrix. These results suggest that lectins react with low molecular weight proteoglycans and that short hyaluronidase digestion causes depolymerization of high molecular weight proteoglycans without loss of their glucidic components, allowing: a) penetration of alcian blue molecules into the macromolecular proteoglycan network; b) an increase of sugar residuals available for lectin histochemistry. Lectin histochemistry can be useful for differential localisation of glycoconjugates in bovine cartilage, especially if associated with short hyaluronidase digestion and conventional histochemical techniques.

41. Clinical advantages with large diameter heads.

Abstract

A larger femoral head with a last generation ceramic on ceramic or metal on metal coupling is an useful recent introduction in THR. Most of the clinical advantages reported with the use of hip resurfacing (increased stability, improved ROM and lower dislocation rate) can be reproduced with large diameter joints and a conventional femoral implant. Some concerns still remains on the potential ions release by metal on metal in the long term. At the present time, not all patients in our practise receive large diameter THR. Restriction of the use of these devices is mostly influenced by the high cost of these components. It is possible that, in the future, large diameter, either metal on metal or ceramic on ceramic, will completely replace the conventional 28mm THR. [ABSTRACT FROM AUTHOR]

46. Intraoperative type 1 proximal femoral fractures

Abstract

We reviewed a series of 120 uncemented total hip replacements using the Omniflex stem with hydroxyapatite coating. Twenty minor intraoperative proximal fractures occurred. All fractures were treated with cerclage wiring after removal of the stem. Radiographic and clinical results of these 20 patients were compared with the remaining 100 implants in which this complication did not occur. In 20% of the cases of both groups, a migration of less than 2 mm was observed. No differences were detected in Harris Hip Scores, subsidence of the stem, and radiographic behavior. We concluded that a properly stabilized proximal femoral fracture above the lesser trochanter did not influence the clinical and radiographic results at more than 3 years follow-up.

Abstract

Background: Patient-reported outcome (PRO) measures are essential for evaluating disease-related quality of life. The International Hip Outcome Tool 12 (iHOT12) assesses various aspects of hip-related symptoms, function, sports participation, and social limitations. This study aimed to adapt and validate an Italian version of the iHOT12 according to COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) guidelines., Materials and Methods: A multicenter observational cohort study was conducted to translate and validate the iHOT-12 into Italian (iHOT-12-ita) and assess its psychometric properties. Following international guidelines, we translated and culturally adapted the iHOT-12-ita. Patients with hip pain included in the study completed the iHOT-12-ita and mHHS-ita questionnaires at three timepoints: initial consultation (T0), 10-20 days later (T1), and at least 2 months after treatment (T2): hip injection, hip arthroscopy or total hip replacement. The evaluation of the iHOT-12-ita psychometric properties was conducted following the COSMIN checklist, assessing its reliability, responsiveness, interpretability, and acceptability., Results: Between May 2019 to September 2022, 104 patients were included, from two tertiary hip surgery referral center clinics, they completed both questionnaires and were available for data analysis. The psychometric evaluations revealed robust test-retest reliability on 27 patients at T1, with a Lin coefficient of 0.709 [95% confidence interval (CI) 0.553-0.888] and a high Pearson correlation coefficient of 0.745. At T2 after treatment, there was significant responsiveness (p < 0.001, n = 72), with excellent correlation observed between the iHOT-12-Ita and mHHS-Ita questionnaires (pwcorr = 0.7971). All predefined hypotheses were confirmed., Conclusions: The Italian iHOT-12-ita proved reliable, valid, and responsive in assessing hip disease-related quality of life. Its implementation in routine clinical practice and research can enhance patient care and research quality in hip preservation surgery for the Italian orthopedic community., Level of Evidence: II., Competing Interests: Declarations.
