Abstract

Objective To develop and validate the prognostic prediction model DU-VASC to assist the clinicians in decision-making regarding the use of platelet inhibitors (PIs) for the management of digital ulcers in patients with systemic sclerosis. Secondly, to assess the incremental value of PIs as predictor. Methods We analysed patient data from the European Scleroderma Trials and Research group registry (one time point assessed). Three sets of derivation/validation cohorts were obtained from the original cohort. Using logistic regression, we developed a model for prediction of digital ulcers (DUs). C-Statistics and calibration plots were calculated to evaluate the prediction performance. Variable importance plots and the decrease in C-statistics were used to address the importance of the predictors. Results Of 3710 patients in the original cohort, 487 had DUs and 90 were exposed to PIs. For the DU-VASC model, which includes 27 predictors, we observed good calibration and discrimination in all cohorts (C-statistic = 81.1% [95% CI: 78.9%, 83.4%] for the derivation and 82.3% [95% CI: 779.3%, 85.3%] for the independent temporal validation cohort). Exposure to PIs was associated with absence of DUs and was the most important therapeutic predictor. Further important factors associated with absence of DUs were lower modified Rodnan skin score, anti-Scl-70 negativity and normal CRP. Conversely, the exposure to phosphodiesterase-5 inhibitor, prostacyclin analogues or endothelin receptor antagonists seemed to be associated with the occurrence of DUs. Nonetheless, previous DUs remains the most impactful predictor of DUs. Conclusion The DU-VASC model, with good calibration and discrimination ability, revealed that PI treatment was the most important therapy-related predictor associated with reduced DU occurrence.

Abstract

Background: The current subclassification of systemic sclerosis into cutaneous subtypes does not fully capture the heterogeneity of the disease. We aimed to compare the performances of stratification into LeRoy's cutaneous subtypes versus stratification by autoantibody status in systemic sclerosis. Methods: For this cohort study, we assessed people with systemic sclerosis in the multicentre international European Scleroderma Trials and Research (EUSTAR) database. Individuals positive for systemic-sclerosis autoantibodies of two specificities were excluded, and remaining individuals were classified by cutaneous subtype, according to their systemic sclerosis-specific autoantibodies, or both. We assessed the performance of each model to predict overall survival, progression-free survival, disease progression, and different organ involvement. The three models were compared by use of the area under the curve (AUC) of the receiver operating characteristic and the net reclassification improvement (NRI). Missing data were imputed. Findings: We assessed the database on July 26, 2019. Of 16 939 patients assessed for eligibility, 10 711 patients were included: 1647 (15·4%) of 10 709 were male, 9062 (84·6%) were female, mean age was 54·4 (SD 13·8) years, and mean disease duration was 7·9 (SD 8·2) years. Information regarding cutaneous subtype was available for 10 176 participants and antibody data were available for 9643 participants. In the prognostic analysis, there was no difference in AUC for overall survival (0·82, 95% CI 0·81–0·84 for cutaneous only vs 0·84, 0·82–0·85 for antibody only vs 0·84, 0·83–0·86 for combined) or for progression-free survival (0·70, 0·69–0·71 vs 0·71, 0·70–0·72 vs 0·71, 0·70–0·72). However, at 4 years the NRI showed substantial improvement for the antibody-only model compared with the cutaneous-only model in prediction of overall survival (0·57, 0·46–0·71 for antibody only vs 0·29, 0·19–0·39 for cutaneous only) and disease progression (0·36, 0·29–0·46 vs 0·21, 0·14–0·28). The antibody-only model did better than the cutaneous-only model in predicting renal crisis (AUC 0·72, 0·70–0·74 for antibody only vs 0·66, 0·64–0·69 for cutaneous only) and lung fibrosis leading to restrictive lung function (AUC 0·76, 0·75–0·77 vs 0·71, 0·70–0·72). The combined model improved the prediction of digital ulcers and elevated systolic pulmonary artery pressure, but did poorly for cardiac involvement. Interpretation: The autoantibody-only model outperforms cutaneous-only subsetting for risk stratifying people with systemic sclerosis in the EUSTAR cohort. Physicians should be aware of these findings at the time of decision making for patient management. Funding: World Scleroderma Foundation.

Abstract

Gastrointestinal (GI) involvement is almost universal in patients with systemic sclerosis (SSc) and is associated with significant disease-related morbidity and mortality.1 The entire GI tract can be involved and other disease features (eg, low mood, terminal organ failure and functional hand impairment) can result in significant nutritional impairment. Severe GI involvement has been reported to occur in ~10% of patients with SSc and often occurs early in the course of the disease.2 However, identification of patients at high risk of clinically significant weight loss is extremely challenging, including from the high prevalence of GI symptoms in patients with SSc. Therefore, there is a need to understand high-risk patients including potentially modifiable risk factors, with a view to early intervention strategies. Against this background, the aim of this study was to examine potential clinical risk factors of significant weight loss in patients with SSc. We performed an analysis of patients with SSc enrolled in the multinational, longitudinal European League Against Rheumatism (EULAR) Scleroderma Trials and Research (EUSTAR) database. Abstract

Adenosine diphosphate (ADP)–ribosyl-transferases (ARTs) transfer ADP-ribose from nicotinamide adenine dinucleotide (NAD) onto target proteins. T cells express ART2.2, a toxin-related, glycosylphosphatidylinositol (GPI)–anchored ecto-enzyme. After the release of NAD from cells, ART2.2 ADP-ribosylates the P2X7 purinoceptor, lymphocyte function–associated antigen (LFA-1), and other membrane. Using lymphoma transfectants expressing either ART2.2 with its native GPI anchor (ART2.2-GPI) or ART2.2 with a grafted transmembrane anchor (ART2.2-Tm), we demonstrated that ART2.2-GPI but not ART2.2-Tm associated with glycosphingolipid-enriched microdomains (lipid rafts). At limiting substrate concentrations, ART2.2-GPI exhibited more than 10-fold higher activity than ART2.2-Tm. On intact cells, ART2.2-GPI ADP-ribosylated a small number of distinct target proteins. Strikingly, the disruption of lipid rafts by cyclodextrin or membrane solubilization by Triton X-100 increased the spectrum of modified target proteins. However, ART2.2 itself was a prominent target for ADP-ribosylation only when GPI anchored. Furthermore, cholesterol depletion or detergent solubilization abolished the auto-ADP-ribosylation of ART2.2. These findings imply that ART2.2-GPI, but not ART2.2-Tm, molecules are closely associated on the plasma membrane and lend support to the hypothesis that lipid rafts exist on living cells as platforms to which certain proteins are admitted and others are excluded. Our results further suggest that raft association focuses ART2.2 on specific targets that constitutively or inducibly associate with lipid rafts.

Abstract Einstein–Maxwell-scalar models allow for different classes of black hole solutions, depending on the non-minimal coupling function $$f(\phi )$$ f ( ϕ ) employed, between the scalar field and the Maxwell invariant. Here, we address the linear mode stability of the black hole solutions obtained recently for a quartic coupling function, $$f(\phi )=1+\alpha \phi ^4$$ f ( ϕ ) = 1 + α ϕ 4 (Blázquez-Salcedo et al. in Phys. Lett. B 806:135493, 2020). Besides the bald Reissner–Nordström solutions, this coupling allows for two branches of scalarized black holes, termed cold and hot, respectively. For these three branches of black holes we calculate the spectrum of quasinormal modes. It consists of polar scalar-led modes, polar and axial electromagnetic-led modes, and polar and axial gravitational-led modes. We demonstrate that the only unstable mode present is the radial scalar-led mode of the cold branch. Consequently, the bald Reissner–Nordström branch and the hot scalarized branch are both mode-stable. The non-trivial scalar field in the scalarized background solutions leads to the breaking of the degeneracy between axial and polar modes present for Reissner–Nordström solutions. This isospectrality is only slightly broken on the cold branch, but it is strongly broken on the hot branch.

Abstract

ART4 (CD297) is a member of the family of toxin-related ADP-ribosyltransferases (ARTs) and is the carrier of the Dombrock blood group alloantigens (Do). Two mouse monoclonal antibodies (MIMA-52 and MIMA-53), and two rat monoclonal antibodies (N0NI-B4 and NONI-B63) were obtained following immunization of mice with human Do/ART4-transfected cells and of rats with human Do/ART4 cDNA, respectively. All four mAbs recognize Do/ART4-transfected Jurkat cells but not untransfected cells by FACS analysis. Staining of Do/ART4-transfected cells by these mAbs was reduced following treatment of cells with PI-PLC, confirming that Do/ART4 is anchored in the cell membrane by linkage to glycosylphosphatidylinositol as predicted from its amino acid sequence. The four mAbs did not react with Gy(a-) (Dombrock null) erythrocytes but agglutinated other red blood cells. By flow cytometric analysis, all mAbs reacted prominently with erythrocytes, and weakly with peripheral blood monocytes and splenic macrophages, but not with B-lymphocytes or T-lymphocytes. The mAbs reacted weakly also with human umbilical vein endothelial cells and the basophilic leukemia KU-812. Immunohistology revealed staining of epithelia and endothelia on sections of tonsils. In FACS analyses NONI-B4 competed with MIMA-52 for binding to Do/ART4-transfected cells and erythrocytes, whereas NONI-B63 competed with MIMA-53. Neither of the mAbs reacted with mouse ART4-transfected cells, but NONI-B63 and MIMA-53 did react with a mouse/human ART4 chimera, indicating that the epitope recognized by these mAbs lies in the C-terminal half of the protein.

Abstract We calculate the quasinormal modes of static spherically symmetric dilatonic Reissner–Nordström black holes for general values of the electric charge and of the dilaton coupling constant. The spectrum of quasinormal modes is composed of five families of modes: polar and axial gravitational-led modes, polar and axial electromagnetic-led modes, and polar scalar-led modes. We make a quantitative analysis of the spectrum, revealing its dependence on the electric charge and on the dilaton coupling constant. For large electric charge and large dilaton coupling, strong deviations from the Reissner–Nordström modes arise. In particular, isospectrality is strongly broken, both for the electromagnetic-led and the gravitational-led modes, for large values of the charge.

Abstract

T cells proteolytically shed the ectodomains of several cell surface proteins and, thereby, can alter their responsiveness and can release soluble intercellular regulators. ART2.2 is a GPI-anchored ecto-ADP-ribosyltransferase (ART) related to ADP-ribosylating bacterial toxins. ART2.2 is expressed exclusively by mature T cells. Here we show that ART2.2 is shed from the cell surface in enzymatically active form upon activation of T cells. Shedding of ART2.2 resembles that of L-selectin (CD62L) in dose response, kinetics of release, and sensitivity to the metalloprotease inhibitor Immunex Compound 3, suggesting that ART2.2, like CD62L, is cleaved by TNF-α-converting enzyme or by another metalloprotease. ART2.2 shed from activated T cells migrates slightly faster in SDS-PAGE analyses than does ART2.2 released upon cleavage of the GPI anchor. This indicates that shedding of ART2.2 is mediated by proteolytic cleavage close to its membrane anchor. Shed ART2.2 is enzymatically active and ADP-ribosylates several substrates in vitro. Thus, shedding of ART2.2 releases a potential intercellular regulator. Finally, using a new FACS assay for monitoring ADP-ribosylation of cell surface proteins, we demonstrate that shedding of ART2.2 correlates with a reduced sensitivity of T cell surface proteins to ADP-ribosylation. Our findings suggest that by shedding ART2.2 the activated T cell not only releases a potential intercellular regulator but also may alter its responsiveness to immune regulation by ART2.2-mediated ADP-ribosylation of cell surface proteins.

9. The Development of Generosity From 4 to 6 Years: Examining Stability and the Biopsychosocial Contributions of Children’s Vagal Flexibility and Mothers’ Compassion

Individual differences in children’s prosocial behaviors, including their willingness to give up something of value for the benefit of others, are rooted in physiological and environmental processes. In a sample of 4-year-old children, we previously found evidence that flexible changes in respiratory sinus arrhythmia (RSA) were linked to donation behavior, and that these physiological patterns may support greater sensitivity to the positive effects of compassionate parenting on donation behavior. The current study focused on a follow-up assessment of these children at age 6. First, we examined the stability of individual differences in donation behavior and related parasympathetic nervous system (PNS) activity from age 4 to 6. Second, we examined associations between donation behavior and RSA at 6 years. Third, we examined whether the association between children’s RSA and donation behavior at age 6 varied depending on mothers’ compassionate love. We found low to modest stability in donation behavior and RSA reactivity from age 4 to 6. These findings provide preliminary evidence that stable individual differences in altruism, as reflected by generosity, and in some aspects of parasympathetic functioning during opportunities to be prosocial, emerge in childhood. In addition, we found that some of the same associations between donation behavior, RSA, and compassionate love that we previously observed in children at 4 years of age continued to be evident 2 years later at age 6. Greater decreases in RSA when given the opportunity to donate were associated with children donating more of their own resources which, in turn, were associated with greater RSA recovery after the task. Lastly, mothers’ compassionate love was positively associated with donation behavior in children who demonstrated stronger decreases in RSA during the task; compassionate parenting and RSA reactivity may serve as external and internal supports for prosociality that build on each other. Taken together, these findings contribute to the perspectives that individual differences in altruistic behaviors are intrinsically linked to healthy vagal flexibility, and that biopsychosocial approaches provide a useful framework for examining and understanding the environmental and physiological processes underlying these individual differences.

Abstract

We demonstrate that rotating matter sets the throat of an Ellis wormhole into rotation, allowing for wormholes which possess full reflection symmetry with respect to the two asymptotically flat spacetime regions. We analyze the properties of this new type of rotating wormholes and show that the wormhole geometry can change from a single throat to a double throat configuration. We further discuss the ergoregions and the lightring structure of these wormholes.

Abstract

In this work we present some new results obtained in a study of the phase diagram of charged compact boson stars in a theory involving a complex scalar field with a conical potential coupled to a U(1) gauge field and gravity. We here obtain new bifurcation points in this model. We present a detailed discussion of the various regions of the phase diagram with respect to the bifurcation points. The theory is seen to contain rich physics in a particular domain of the phase diagram.

Abstract

We consider charged black holes with scalar hair obtained in a class of Einstein–Maxwell– scalar models, where the scalar field is coupled to the Maxwell invariant with a quartic coupling function. Besides the Reissner–Nordström black holes, these models allow for black holes with scalar hair. Scrutinizing the domain of existence of these hairy black holes, we observe a critical behavior. A limiting configuration is encountered at a critical value of the charge, where space time splits into two parts: an inner space time with a finite scalar field and an outer extremal Reissner–Nordström space time. Such a pattern was first observed in the context of gravitating non-Abelian magnetic monopoles and their hairy black holes.
