Abstract

BackgroundBecause of the improvements in survival rates for preterm infants, not only the rates of bronchopulmonary dysplasia (BPD) but also those of long-term respiratory complications of premature birth are increasing, resulting in financial and health burdens in developed countries. Thus far, the risk factors of respiratory morbidities in extremely preterm infants remain unknown. Furthermore, the definition and the predictive ability of BPD for long-term respiratory outcomes are yet to be determined. ObjectiveThe objective of our study, Extreme Prematurity and Pulmonary Outcomes Program in Saitama, is to develop the diagnostic criteria for BPD and to determine the prognostic factors contributing to the long-term pulmonary outcomes manifesting in extremely preterm infants. MethodsThe Extreme Prematurity and Pulmonary Outcomes Program in Saitama is an observational prospective cohort study performed by a consortium of six neonatal intensive care units (NICUs) in Saitama, Japan. The subjects included in this study are infants (from each clinical center) with gestational ages 22 to 27 weeks. The target is 400 subjects. This study aims to determine the definition of BPD and other perinatal factors that accurately predict the long-term pulmonary outcomes in survivors of extreme prematurity. Moreover, the association between BPD and postprematurity respiratory disease will be investigated using generalized linear models. ResultsThe protocol and consent forms were evaluated and approved on September 5, 2019, by the Ethics Committee of Saitama Medical Center, Saitama Medical University. Enrollment began on April 1, 2020. It is expected to end on March 31, 2023. The follow-up for 1 year corrected age is expected to continue through the middle of 2024. ConclusionsThe Extreme Prematurity and Pulmonary Outcomes Program in Saitama incorporates aspects of neonatal care in secondary- and tertiary-level NICUs to develop existing research studies on the definition of BPD, objective biomarkers, and outcome measures of respiratory morbidity in extremely preterm infants beyond NICU hospitalization, thereby leading to a novel understanding of the nature and natural history of BPD and potential mechanistic and therapeutic targets in at-risk subjects. Abstract

High-energy radiation caused by exoplanetary space weather events from planet-hosting stars can play a crucial role in conditions promoting or destroying habitability in addition to the conventional factors. In this paper, we present the first quantitative impact evaluation system of stellar flares on the habitability factors with an emphasis on the impact of Stellar Proton Events. We derive the maximum flare energy from stellar starspot sizes and examine the impacts of flare associated ionizing radiation on CO$_2$, H$_2$, N$_2$+O$_2$ --rich atmospheres of a number of well-characterized terrestrial type exoplanets. Our simulations based on the Particle and Heavy Ion Transport code System [PHITS] suggest that the estimated ground level dose for each planet in the case of terrestrial-level atmospheric pressure (1 bar) for each exoplanet does not exceed the critical dose for complex (multi-cellular) life to persist, even for the planetary surface of Proxima Centauri b, Ross-128 b and TRAPPIST-1 e. However, when we take into account the effects of the possible maximum flares from those host stars, the estimated dose reaches fatal levels at the terrestrial lowest atmospheric depth on TRAPPIST-1 e and Ross-128 b. Large fluxes of coronal XUV radiation from active stars induces high atmospheric escape rates from close-in exoplanets suggesting that the atmospheric depth can be substantially smaller than that on the Earth. In a scenario with the atmospheric thickness of 1/10 of Earth's, the radiation dose from close-in planets including Proxima Centauri b and TRAPPIST-1 e reach near fatal dose levels with annual frequency of flare occurrence from their hoststars., Comment: 37 pages, 19 figures, 4 tables. Abstract

Repeated or prolonged, but not short-term, general anesthesia during the early postnatal period causes long- lasting impairments in memory formation in various species. The mechanisms underlying long-lasting impairment in cognitive function are poorly understood. Here, we show that repeated general anesthesia in postnatal mice induces preferential apoptosis and subsequent loss of parvalbumin-positive inhibitory interneurons in the hippocampus. Each parvalbumin interneuron controls the activity of multiple pyramidal excitatory neurons, thereby regulating neuronal circuits and memory consolidation. Preventing the loss of parvalbumin neurons by deleting a proapoptotic protein, mitochondrial anchored protein ligase (MAPL), selectively in parvalbumin neurons rescued anesthesia- induced deficits in pyramidal cell inhibition and hippocampus-dependent long-term memory. Conversely, partial depletion of parvalbumin neurons in neonates was sufficient to engender long-lasting memory impairment. Abstract

Combination therapy of nivolumab and ipilimumab (NIVO + IPI) for metastatic renal cell carcinoma (mRCC) has shown efficacy, but approximately 20% of patients experience disease progression in the early stages of treatment. No useful biomarkers have been reported to date. Therefore, it is desirable to identify biomarkers to predict treatment responses in advance. We examined the tumor microenvironment (TME)-related gene expression in mRCC patients treated with NIVO + IPI, between the response and non-response groups, using tumor tissues, before administering NIVO + IPI. In TME-related genes, TNFSF9 expression was identified as a candidate for the predictive biomarker. Its expression discriminated between the response and non-response groups with 88.89% sensitivity and 87.50% specificity (AUC = 0.9444). We further analyzed the roles of TNFSF9 in TME using bioinformatics from The Cancer Genome Atlas (TCGA) cohort. An adaptive immune response was activated in the TNFSF9-high-expression tumors. Indeed, T follicular helper cells, plasma B cells, and tumor-infiltrating CD8+ T cells were increased in the tumors, which indicates the promotion of humoral immunity due to enhanced T-B interactions. However, as the number of regulatory T cells (Treg) increased in the tumors, the percentage of dysfunctional T cells also increased. This suggests that not only PD-1 but also CTLA-4 inhibition may have suppressed Treg activation and improved the therapeutic effect in the TNFSF9 high-expression tumors. Therefore, TNFSF9 may predict the therapeutic efficacy of NIVO + IPI for mRCC and allow more appropriate patient selection. [ABSTRACT FROM AUTHOR]
