Background: Although FD may affect up to 10% of the general population, the therapy for FD is not standard. Recently, ginger-based food supplements have been proposed in order to restore FD symptoms. Our aim was to assess the efficacy of a new nutraceutical formulation containing extract of gingerol and thymus as a possible natural treatment in managing the symptoms of functional dyspepsia (FD). Methods: We retrospectively analyzed the efficacy and safety profiles of a nutraceutical formulation containing Zingiber officinalis root extract and a standardized Thymus extract. It was administered as 1 ml/day twice a day for 90 days. Patients were assessed at baseline and after 1, 2 and 3 months of treatment, following a month of pharmacological washout by completing a questionnaire reporting the trend of the following symptoms: epigastric pain, epigastric heaviness, early satiety, belching, and regurgitation. Every symptom was assessed by a Visual Analogic Scale (VAS), ranging from 0= absence to 10= maximal severity. Results: We enrolled 272 patients (99 males and 173 females; median IQR age 49.5, 36-64 yrs). Obesity (BMI>30) was present in 28 (12.5%) patients; smokers were 83 (30.5%); and comorbidities were present in 107 (39.3%) patients. Improvement of symptom scores during treatment and one month after its suspension was extremely significant (p Conclusion: This large study found that nutraceutical formulation could be one of the tools for an empirical approach to treat patients with FD, especially when a non-conventional drug treatment is preferable for the patient and considered suitable by the physician.

- Abstract
Proximal humeral fractures represent the third most common observed osteoporotic fracture; the treatment in three and four-part proximal humeral fractures in patients over 65 years is still controversial. Among the treatments described in literature, open reduction and internal fixation (O.R.I.F) and reverse shoulder arthroplasty (RSA) are gaining an increasing popularity. To investigate the correct treatment for three and four-part proximal humeral fractures according to psychological aspects. It was conducted a prospective study with a series of 63 patients treated with O.R.I.F. (group A) and with RSA (group B) for three and four-part proximal humeral fractures according to Neer classification system. A conservative treatment group, as control, was finally introduced. One independent observer performed clinical and a psychological evaluation at one (T0), six (T1) and twelve months (T2) postoperatively. The Constant's score and The Disabilities of the Arm, Shoulder and Hand (DASH score) were used for clinical evaluation, while General Anxiety Disorder-7 (GAD-7) and Caregiver Strain Scale (CSS) were used for psychological evaluation. At one month follow up in group A the mean values were DASH score 50.8, Constant score 36.1, GAD-7 score 5.4, CSS 5.0. For the group B, the average values at T0 were: DASH score 54.6, Constant score 32.0; GAD-7 score 6.4, CSS 6.2. At six months in group A the average values were DASH score 42.1, Constant score 47.3, GAD-7 score 4.3, CSS 3.9. For the group B, the average values at T1 were: DASH score 39.1, Constant score 43.2, GAD-7 score 5.7, CSS 5.5. At twelve months in the group A, the mean values were DASH score 32.8, Constant score 60.0, GAD-7 score 3.2, CSS 3.1. For the group B shown these mean values: DASH score 33.6, Constant score 52.9, GAD-7 score 4.3, CSS 4.5. We demonstrated a better clinical and psychological outcome at T2 in the group treated with osteosynthesis compared to the group treated with arthroplasty (ConstantBoth open osteosynthesis and reverse shoulder arthroplasty are valid treatments for proximal humeral fractures. According to the best osteosynthesis results the authors suggested to perform a psychological analysis for each patient in order to choose the appropriate treatment.

Meniscectomy significantly change the kinematics of the knee joint by reducing the contact area between femoral condyles and the tibial plateau, but the shift in the contact area has been poorly described. The aim of our investigation was to measure the shift of the tibiofemoral contact area occurring after meniscectomy. We used laser scans combined to surface texturing for measuring the 3D position and area of the femoral and tibial surfaces involved in the joint. In particular, natural condyles (porcine model) were analysed and the reverse engineering approach was used for the interpretation of the results from compression tests and local force measurements in conjunction with staining techniques. The results suggested that laser scans combined to surface texturing may be considered as a powerful tool to investigate the stained contours of the contact area. Beside the largely documented reduction of contact area and local pressure increase, a shift of the centroid of the contact area toward the intercondylar notch was measured after meniscectomy. As a consequence of the contact area shift and pressure increase, cartilage degeneration close to the intercondylar notch may occur. - Abstract
Italians to America is the first indexed reference work devoted to Italian immigrants to the United States. This series contains passenger list information in chronological order on the first major wave of Italian migration during the last two decades of the nineteenth century. As with the highly regarded companion series on German immigrants, Italians to America presents the passenger lists in chronological order, including information on each person's age, sex, occupation, village of origin, and destination, plus the name of the ship, the port of embarkation and the date of arrival. Each volume also contains an introduction on the history of Italian migration to the U.S. and a full name index, greatly simplifying the researcher's job.
