The integration of AI tools like ChatGPT into education has generated significant interest due to their potential to transform learning environments by providing personalized learning, automating tasks, and improving student engagement. However, gaps remain in the literature, particularly in comparing AI-supported education methods with traditional approaches and understanding ChatGPT's specific role in academic writing, literature reviews, and teacher development. This study addresses these gaps through a systematic literature review (SLR), evaluating the effectiveness of AI tools versus traditional teaching approaches. It focuses on ChatGPT's application in areas such as academic writing, lesson planning, student assessment, and the professional development of educators. Additionally, the study explores institutional strategies for balancing the benefits of AI with the need to maintain academic integrity and educational quality. The methodology involved a systematic search across academic databases with a structured analysis of key studies in AI-supported education. The main contributions include identifying the comparative advantages and limitations of AI in enhancing student learning, offering best practices for incorporating AI tools into teaching, and examining prompt engineering as a crucial factor in optimizing AI usage. The study reveals that ChatGPT and other AI tools significantly enhance educational efficiency and engagement, but they require careful management to prevent over-reliance. The study advocates for a balanced approach, integrating both AI and traditional methods to achieve optimal educational outcomes while maintaining academic integrity.

Background: Emergency medical services (EMS) play a critical role in the response to road traffic accidents (RTAs) by providing critical prehospital care to RTA victims, and thus helping to reduce the severity of injuries and casualties from RTAs. Analysing the efficiency of such systems and optimising the service (often with insufficient resources) would be highly beneficial to inform operating guidelines. In this study, we aimed to assess the ambulance utilisation rate for RTAs and provide insights about the system's efficiency and utilisation metrics in Lagos State, Nigeria. Methods: We received hospital capacity information (type of hospital [eg, tertiary] and number of beds, nurses, operating rooms, and so on) from hospital survey data in Lagos. Additionally, we received RTA-specific intervention forms from the Lagos State Ambulance Service. With the limited amount of historical data, we highlighted methods to estimate the system parameters (eg, arrival rate, service rate, and so on) of RTA victims at various hospitals in Lagos. We also developed a parametric model and applied approximate dynamic programming approach for ambulance dispatch and location optimisation with a fixed number of base stations. Furthermore, our routing approach was accompanied by directions from OpenRouteService and queue analysis to determine the most appropriate hospital near an RTA for the EMS systems in Lagos. Findings: Using patient-queue based wait-time analysis, we found that in over 80% of cases, one of the nearest three hospitals was the most appropriate recommendation for patient delivery. This exponentially decays with the next closest hospitals. Additionally, we found that optimising the ambulance relocation method, where the ambulance returns to a base selected by our model as opposed to its dedicated base or a random base, can reduce the average response time of the EMS system by 8%, or 3·5 min (from 41 min to 38 min). Interpretation: Appropriate placement of mobile resources (ie, ambulances, health-care booths, and so on) can help to lower the response time of EMS systems in addressing RTAs. Similarly, optimal routing of patients to the most well prepared and nearby hospitals would significantly increase the system's efficacy and improve health outcomes. Our findings can provide valuable insight into this and further inform resource allocation and guidelines for health systems, by identifying those hospitals that have the highest burden of RTA victims and those that provide the most efficient care in a resource-limited setting. Network Intrusion Detection Systems (NIDS) are designed to safeguard the security needs of enterprise networks against cyber-attacks. However, NIDS networks suffer from several limitations, such as generating a high volume of low-quality alerts. Moreover, 99% of the alerts produced by NIDSs are false positives. As well, the prediction of future actions of an attacker is one of the most important goals here. The study has reviewed the state-of-the-art cyber-attack prediction based on NIDS Intrusion Alert, its models, and limitations. The taxonomy of intrusion alert correlation (AC) is introduced, which includes similarity-based, statistical-based, knowledge-based, and hybrid-based approaches. Moreover, the classification of alert correlation components was also introduced. Alert Correlation Datasets and future research directions are highlighted. The AC receives raw alerts to identify the association between different alerts, linking each alert to its related contextual information and predicting a forthcoming alert/attack. It provides a timely, concise, and high-level view of the network security situation. This review can serve as a benchmark for researchers and industries for Network Intrusion Detection Systems’ future progress and development.

Cloud ERP is a type of enterprise resource planning (ERP) system that runs on the vendor’s cloud platform instead of an on-premises network, enabling companies to connect through the Internet. The goal of this study was to rank and prioritise the factors driving cloud ERP adoption by organisations and to identify the critical issues in terms of security, usability, and vendors that impact adoption of cloud ERP systems. The assessment of critical success factors (CSFs) in on-premises ERP adoption and implementation has been well documented; however, no previous research has been carried out on CSFs in cloud ERP adoption. Therefore, the contribution of this research is to provide research and practice with the identification and analysis of 16 CSFs through a systematic literature review, where 73 publications on cloud ERP adoption were assessed from a range of different conferences and journals, using inclusion and exclusion criteria. Drawing from the literature, we found security, usability, and vendors were the top three most widely cited critical issues for the adoption of cloud-based ERP; hence, the second contribution of this study was an integrative model constructed with 12 drivers based on the security, usability, and vendor characteristics that may have greater influence as the top critical issues in the adoption of cloud ERP systems. We also identified critical gaps in current research, such as the inconclusiveness of findings related to security critical issues, usability critical issues, and vendor critical issues, by highlighting the most important drivers influencing those issues in cloud ERP adoption and the lack of discussion on the nature of the criticality of those CSFs. This research will aid in the development of new strategies or the revision of existing strategies and polices aimed at effectively integrating cloud ERP into cloud computing infrastructure. It will also allow cloud ERP suppliers to determine organisations’ and business owners’ expectations and implement appropriate tactics. A better understanding of the CSFs will narrow the field of failure and assist practitioners and managers in increasing their chances of success.

Decisions to provide life-sustaining medical care for marginally viable newborns present a unique set of morally complex challenges for providers and parents in the United States. This article examines recent legal trends that restrict discretionary decision-making, and critiques commonly employed ethical justifications offered to support permitting such discretion. Abstract Natural gas comprises about a quarter of the United States’ energy use. It is more environmentally friendly than oil and coal due to lower carbon dioxide (CO2) emissions per unit, less costly per unit of energy and more readily available domestically in abundant supply. However, due to a number of barriers in the political, infrastructural, pricing and other arenas, the use of natural gas as a significant energy source in the United States has been limited. In our paper, we highlight the favorable qualities of natural gas and its benefits for the consumer, producer, and environment, having compared the costs of the various components of the natural gas business such as drilling and transport to that of coal and oil. Moreover, we touch upon the major issues that have prevented a more prevalent use of the gas, such as the fact that the infrastructure of natural gas is more costly since it is transported though pipelines whereas other energy sources such as oil and coal have flexible systems that use trains, trucks and ships. In addition, the powerful lobbies of the coal and oil businesses, along with the inertia in the congress to pass a national climate change bill further dampens incentives for these industries to invest in natural gas, despite its various attractive qualities. We also include discussions of policy proposals to incentive greater use of natural gas in the future.

OBJECTIVE: We sought to characterize practices and attitudes of maternal-fetal medicine (MFM) and fetal care pediatric (FCP) specialists regarding fetal abnormalities. STUDY DESIGN: This was a self-administered survey of 434 MFMs and FCPs (response rate: MFM 60.9%; FCP 54.2%). RESULTS: For Down syndrome (DS), congenital diaphragmatic hernia (CDH), spina bifida: MFMs were more likely than FCPs to support termination (DS 52% vs 35%, P < .001; CDH 49% vs 36%, P < .001; spina bifida 54% vs 35%, P < .001), and consider offering termination options as highly important (DS 90% vs 70%, P < .001; CDH 88% vs 69%, P < .001; spina bifida 88% vs 70%, P < .001). For DS only, MFMs were less likely than FCPs to think that pediatric specialist consultation should be offered prior to a decision regarding termination (54% vs 75%, P < .001). MFMs reported report higher termination rates among patients only for DS (DS 51% vs 21%, P < .001). CONCLUSION: MFM and FCP specialists' counseling attitudes differ for fetal abnormalities. The Born-Alive Infants Protection Act (BAIPA), passed by Congress in 2002, has attracted little publicity. Its purposes were, in part, 'to repudiate the flawed notion that a child's entitlement to the protections of the law is dependent on whether that child's mother or others want him or her.' Understood as antiabortion rhetoric, the bill raised little concern among physicians at the time of legislative hearings and passed in both Houses by overwhelming majorities, hardly suggesting contentious legislation. After its signing into law, the Neonatal Resuscitation Program (NRP) Steering Committee issued an opinion stating that '[BAIPA] should not in any way affect the approach that physicians currently follow with respect to the extremely premature infant.' This interpretation of the law, however, may have been short sighted. In April 2005, the US Department of Health and Human Services (DHHS) brought life to the BAIPA, announcing: 'As a matter of law and policy, [DHHS] will investigate all circumstances where individuals and entities are reported to be withholding medical care from an infant born alive in potential violation of federal statutes.' The agency issued instructions to state officials on how the definitional provision within the BAIPA interacts with the Emergency Medical Treatment and Labor Act (EMTALA) and the Child Abuse Prevention and Treatment Act (CAPTA). These interagency memoranda potentially resurrect dormant governmental oversight of newborn-treatment decisions and thus may have influence over normative neonatal practice. Under the BAIPA, the DHHS interprets EMTALA to protect all 'born-alive' infants; hospitals and physicians violating regulatory requirements face agency-sanctioned monetary penalties or a 'private right of action by any individual harmed as a direct result.' According to its memorandum, the DHHS will investigate allegations of EMTALA violations whenever it finds evidence that a newborn was not provided with at least a medical screening examination under circumstances in which a 'prudent layperson observer' could conclude from the infant's 'appearance or behavior' that it was 'suffering from an emergency medical condition.' The memorandum fails to clarify which observers qualify as prudent, what infant appearance or behavior is relevant, or what defines an emergency medical condition. Because these evaluative criteria are not constrained by reference to relevant standards of medical care, the agency arguably substitutes a nonprofessional's presumed sagacious assessment of survivability for reasonable medical judgment. Indeed, under a straightforward reading of the instruction, a family member could conceivably trigger an investigation after observing a relative deliver a 20-week fetus who maintains a heartbeat for an hour before its death. Most physicians would not consider this an emergency medical condition and, rather than perform a screening examination, would provide comfort for the newborn and support for the family. The guideline, however, does not state that professional acumen trumps the layperson's observations in these instances; thus, physicians are left unclear about whether screening examinations are required for all newborns regardless of a priori, reasoned considerations of survivability. In this context, the NRP Steering Committee opinion states that 'at the time of delivery... the medical condition and prognosis of the newly born infant should be assessed. At that point decisions about withholding or discontinuing medical treatment that is considered futile may be considered by... providers in conjunction with the parents acting in the best interest of their child.' However, most pediatricians skilled in screening and resuscitation are not currently called on to perform this function when the gestational age of a nonviable fetus is reasonably certain before delivery. If under the law screening is now required at any gestational age, professional procedure immediately after previable births may need modification. More worrisome, threatened aggressive investigations of alleged EMTALA violations at the soft edges of viability, where futility remains a matter of debate, jeopardize the normative ethical practice of offering discretionary palliative care. The DHHS sent its other instruction to state child protective services agencies responsible for implementing CAPTA regulations; it reiterates the limited situations in which physicians may withhold medical treatment from infants and reemphasizes the local role of 'individuals within health care facilities' to notify authorities of suspected infractions. Its real import, however, is insistence on local execution of legal remedies to prevent nontreatment decisions deemed impermissible by the 1984 Baby Doe rules. Because this new directive encourages governmental oversight of treatment decisions involving imperiled newborns, a period of benign regulatory neglect seems to be over. The federal CAPTA rules arguably remove quality-of-life considerations from the decision-making calculus and therefore may conflict with the best-interests paradigm advocated by the American Academy of Pediatrics and NRP. How courts will respond to the DHHS interpretation of EMTALA and CAPTA under the BAIPA remains unclear. Federal courts have yet to authoritatively examine alleged EMTALA violations involving newborn treatment decisions at the limits of viability. The Wisconsin Supreme Court has permitted an EMTALA claim to go to trial where physicians allegedly did not screen or resuscitate a 22-week newborn delivered in an emergency department, and a lower appellate court has relied on CAPTA to hold that parents do not possess the right to choose against resuscitating an extremely premature newborn. The Texas Supreme Court recently granted physicians the paternalistic prerogative to resuscitate imperiled newborns without attention to parental preference under a common law doctrine of 'emergent circumstances.' These judicial decisions undermine the ethical discretion parents are typically afforded in decision-making before and after delivery in these morally complex situations. The DHHS interpretation of the BAIPA may encourage jurisdictional creep of these kinds of pronouncements as the agency seeks to expand legal protections for born-alive infants. The US Supreme Court has stated that 'courts must presume that a legislature says in a statute what it means and means in a statute what it says there'; thus, judges interpret law by analyzing 'concrete statutory language, not by reference to abstract notions of generalized legislative intent.' The BAIPA indiscriminately defines 'born alive' to include an infant 'at any stage of development... regardless of whether the expulsion or extraction occurs as a result of natural or induced labor, cesarean section, or induced abortion,' and it makes no reference to standards of care or best interests, nor does it specifically protect a parent's decision-making authority. Under the law's strict logic, an 18-week miscarried fetus with a detectable heart beat after delivery is entitled to the full protections of the law as determined by 'any Act of Congress, or any ruling, regulation, or interpretation of the various administrative bureaus and agencies.' Before concluding that the BAIPA would not affect normative neonatal practice, the NRP Steering Committee should have analyzed the act's actual statutory language and avoided relying heavily on imprecise legislative intent. The BAIPA's congressional sponsors did claim that the law 'will not mandate medical treatment where none is currently indicated,' but such political rhetoric is often not sufficient to render law innocuous years after separation from its legislative history. Besides, nowhere in the House record does the majority explicitly acknowledge that discretion to decide the fate of imperiled newborns invests in parents, in consultation with physicians; indeed, the bill's stated purpose was to repudiate that notion. At best, legislators recognized that physicians disagree about the efficacy of resuscitating at the limits of viability, and therefore the current standard of care permits doctors to deem resuscitation a futile endeavor. However, judges may resist characterizing resuscitation as futile, given its poor analytical fit, and substantial public-policy concerns regarding discrimination against future disabled individuals could easily tip a court to preserve incipient, at least, physiologic life under the BAIPA's all-encompassing definition of born alive. Several Sustainable Development Goals (SDGs) (3, 16, 17) point to the need to systematically address massive shortages of human resources for health (HRH), build capacity and leverage partnerships to reduce the burden of global illness. Addressing these complex needs remain challenging, as simple increases in absolute numbers of healthcare providers trained is insufficient; substantial investment into long-term high-quality training programs is needed, as are incentives to retain qualified professionals within local systems of care delivery. We describe a novel HRH initiative, the Global Health Service Partnership (GHSP), involving collaboration between the US government (President's Emergency Plan for AIDS Relief [PEPFAR], Peace Corps), 5 African countries, and a US-based non-profit, Seed Global Health. GHSP was formed to enlist US health professionals to assist in strengthening teaching and training capacity and focused on pre-and in-service medical and nursing education in Malawi, Tanzania, Uganda, Eswatini and Liberia. From 2013-2018, GHSP sent 186 US health professionals to 27 institutions in 5 countries, helping to train 16 280 unique trainees of all levels. Qualitative impacts included cultivating a supportive classroom learning environment, providing a pedagogical bridge to clinical service, and fostering a supportive clinical learning and practice environment through role modeling, mentorship and personalized learning at the bedside. GHSP represented a novel, multilateral, public-private collaboration to help address HRH needs in Africa. Objective: This study was designed to determine the antioxidant activity of the extracts of beetroot by using beetroot energy drink and flavored milk (products). Material & Methods . This experimental trial was conducted at Jinnah University for Women, Pakistan, under the approval of local institutional review board number JUW/DFST/RCB010/2020. All the materials such as beetroot, carrot, cucumber, and lemon were obtained commercially from which two products were formulated: beetroot energy drink (sample1) and flavored milk (sample 2). These formulated products were evaluated for quality analysis (pH and brix), phytochemical screening using the Keller-Kiliani test, Salkowski's test, Alkaline reagent test, lead acetate test, ferric chloride test, protein test, quantitative test of phenol, antioxidant activity, sensory analysis, and shelf life study. The paired t -test was applied to detect significant differences between two samples., Results: The phytochemical analysis revealed that cardiac glycosides, phytosterol, flavonoids, and terpenoids were found in both energy booster drink (EBD) and flavored milk (FM) except phenolic compounds that were found only in EBD. The antioxidant capacity of beetroot juice was far greater than FM. The statistical sensorial analysis of FM and EBD reported a significant mean difference between most of the groups with p < 0.0001., Conclusion: This study concludes that energy drinks having beetroot indicated higher antioxidant capacity than flavored milk. Men who have sex with women are understudied in HIV research despite the extent to which they experience HIV-related mortality and contribute to the epidemic among women. During our experience of developing and piloting an HIV prevention intervention for men living with HIV in South Africa, and planning to have a child with an HIV-negative woman, ethical questions were posed regarding implementation of a male-centered intervention that did not require female partner participation. Two overarching ethical issues were the potential for (1) compromising women's reproductive and sexual autonomy and (2) increasing HIV-acquisition risks for the woman because the intervention efficacy was unknown. We describe here how these concerns were addressed to facilitate development of a male-centered HIV-prevention intervention. We hope this process manuscript will support researchers, clinicians, and reviewers to engage men who have sex with women in HIV prevention and care., (© 2021. Background: Global health field assignments for medical and nursing professionals include a wide variety of opportunities. Many placements often involve individuals practicing in settings very different from their home environments, relying on their professional experience to help bridge cultural and clinical divides., Objectives: There is limited information about the individual factors that might lead to successful longer-term global health experiences in non-disaster settings. In this paper, we report on one cohort of health professionals' experiences of culture shock, stress, and resiliency as volunteers within the Global Health Service Partnership (GHSP), a public-private collaboration between Seed Global Health, the US Peace Corps, and the US Presidents Plan for Emergency Aids Relief (PEPFAR) that placed American medical and nursing educators in five African countries facing a shortage of health professionals., Methods: Using the tools of Project PRIME (Psychosocial Response to International Medical Electives) as a basis, we created the GHSP Educator Support Survey to measure resiliency, stress, and culture shock levels in a cohort of GHSP volunteers during their year of service., Findings: In our sample, participants were likely to experience lower levels of resiliency during initial quarters of global health placements compared to later timepoints. However, they were likely to experience similar stress and culture shock levels across quarters. Levels of preparedness and resources available, and medical needs in the community where the volunteer was placed played a role in the levels of resiliency, stress, and culture shock reported throughout the year., Conclusion: The GHSP Educator Support Survey represented a novel attempt to evaluate the longitudinal mental well-being of medical and nursing volunteers engaged in intense, long-term global health placements in high acuity, low resource clinical and teaching settings. Background: A growing body of published research on tranexamic acid (TXA) suggests that it is effective in reducing blood loss and the risk for transfusion in total knee arthroplasty (TKA). The purpose of this network meta-analysis was to evaluate TXA in primary TKA as the basis for the efficacy recommendations of the combined clinical practice guidelines of the American Association of Hip and Knee Surgeons, American Academy of Orthopaedic Surgeons, Hip Society, Knee Society, and American Society of Regional Anesthesia and Pain Medicine on the use of TXA in primary total joint arthroplasty., Methods: We searched Ovid MEDLINE, Embase, Cochrane Reviews, Scopus, and Web of Science databases for publications before July 2017 on TXA in primary total joint arthroplasty. All included studies underwent qualitative and quantitative homogeneity testing. Direct and indirect comparisons were performed as a network meta-analysis, and results were tested for consistency., Results: After critical appraisal of the available 2113 publications, 67 articles were identified as representing the best available evidence. Topical, intravenous (IV), and oral TXA formulations were all superior to placebo in terms of decreasing blood loss and risk of transfusion, while no formulation was clearly superior. Use of repeat IV and oral TXA dosing and higher doses of IV and topical TXA did not significantly reduce blood loss or risk of transfusion. Preincision administration of IV TXA had inconsistent findings with a reduced risk of transfusion but no effect on volume of blood loss., Conclusions: Strong evidence supports the efficacy of TXA to decrease blood loss and the risk of transfusion after primary TKA. No TXA formulation, dosage, or number of doses provided clearly improved blood-sparing properties for TKA. Background: Tranexamic acid (TXA) is effective in reducing blood loss in total joint arthroplasty (TJA), but concerns still remain regarding the drug's safety. The purpose of this direct meta-analysis was to evaluate and establish a basis for the safety recommendations of the combined clinical practice guidelines on the use of TXA in primary TJA., Methods: A search was completed for studies published before July 2017 on TXA in primary TJA. We performed qualitative and quantitative homogeneity testing and a direct comparison meta-analysis. We used the American Society of Anesthesiologists (ASA) score of 3 or greater as a proxy for patients at higher risk for complications in general and performed a meta-regression analysis to investigate the influence of comorbidity burden on the risk of arterial thromboembolic event and venous thromboembolic event (VTE)., Results: Topical, intravenous, and oral TXA were not associated with an increased risk of VTE after TJA. In addition, meta-regression demonstrated that TXA use in patients with an ASA status of 3 or greater was not associated with an increased risk of VTE after total knee arthroplasty., Conclusion: Although most studies included in our analysis excluded patients with a history of prior thromboembolic events, our findings support the lack of evidence of harm from TXA administration in patients undergoing TJA. Moderate evidence supports the safety of TXA in patients undergoing total knee arthroplasty with an ASA score of 3 or greater. The purpose of our study was to evaluate the efficacy of TXA in primary THA to support the combined clinical practice guidelines of the American Association of Hip and Knee Surgeons, American Academy of Orthopaedic Surgeons, Hip Society, Knee Society, and American Society of Regional Anesthesia and Pain Medicine on the use of TXA in primary total joint arthroplasty., Methods: A search was performed using Ovid-MEDLINE, Embase, Cochrane Reviews, Scopus, and Web of Science databases to identify all publications before July 2017 on TXA in primary THA. We completed qualitative and quantitative homogeneity testing of all included studies. Direct and indirect comparisons were analyzed using a network meta-analysis followed by consistency testing of the results., Results: Two thousand one hundred thirteen publications underwent critical appraisal with 34 publications identified as representing the best available evidence for inclusion in the analysis. Topical, intravenous, and oral TXA formulations provided reduced blood loss and risk of transfusion compared to placebo, but no formulation was clearly superior. Use of repeat doses, higher doses, or variation in timing of administration did not significantly reduce blood loss or risk of transfusion., Conclusions: Strong evidence supports the use of TXA to reduce blood loss and risk of transfusion after primary THA. Introduction: Liposomal bupivacaine has a paucity of data regarding narcotic requirements and hospital length of stay in comparison to other peri-articular injections, specifically in the total hip arthroplasty (THA) population., Methods: 69 patients who underwent THA by a single surgeon were divided into two cohorts over a 3 year period in this retrospective study comparing narcotic requirements, hospital length of stay and cost. The study group (n = 29) received liposomal bupivacaine whereas a matched control group (n = 40) received a pharmacy-mixed cocktail in peri-articular structures. Statistical and clinical differences were reported in this unfunded study., Results: No difference was found in hospital length of stay [2.9 days in the study group (range 1-14) versus 3.1 days (range 1-11) in the control group, p = 0.101], however, the study group required less narcotics per day [22.6 mg (range 5-53.3) versus 29 mg (range 6.7-80.8) in the control group, p = 0.045]. The clinical difference between cohorts averaged less than one pill per day of hospitalization. The cost per patient of the local injection was more than 11 times greater in the liposomal bupivacaine group., Conclusion: Liposomal bupivacaine demonstrated a statistical improvement in narcotic requirements but not in hospital length of stay in comparison to a control group. The effects of liposomal bupivacaine on narcotic requirements and hospital length of stay may not justify its use in total hip arthroplasty patients given the substantial cost of these injections and the minimal clinical difference in outcomes compared to a more cost-effective injection.

Background: Knee osteoarthritis is a disabling disease that costs billions of dollars to treat. Corticosteroid gives varying pain relief and costs $12 per injection, whereas ketorolac costs $2 per injection, per institutional costs. The aim of this study was to compare ketorolac with corticosteroid based on pain relief using patient outcome measures and cost data., Methods: A total of 35 patients were randomized to ketorolac or corticosteroid intra-articular knee injection in a double-blind, prospective study. Follow-up was 24 weeks. Osteoarthritis was evaluated using Kellgren-Lawrence grading. Visual analog scale (VAS) was the primary outcome measure. A query of the institutional database was performed for International Classification of Diseases, Ninth Revision codes 715.16 and 719.46, and procedure code 20610 over a 3-year period. Two-way, repeated measures analysis of variance and Spearman rank correlation were used for statistical analysis., Results: Mean VAS for ketorolac and corticosteroid decreased significantly from baseline at 2 weeks, 6.3-4.6 and 5.2-3.6, respectively and remained decreased for 24 weeks. There was no correlation between VAS and demographics within treatments. There were 220, 602, and 405 injections performed on patients with the International Classification of Diseases, Ninth Revision codes 715.16 and 719.46 during 2013, 2014, and 2015, respectively. The cost savings per year using ketorolac instead of corticosteroid would be $2259.40, $6182.54, and $4159.35 for 2013, 2014, and 2015, respectively, with a total savings of $12,601.29 over this period., Conclusion: Pain relief was similar between ketorolac and corticosteroid injections. Objective: Explore associations between neonatal providers' perspectives on survival, quality of life (QOL) and treatment recommendations., Methods: Providers attending a workshop on neonatal viability were surveyed about survival, perceived QOL and treatment recommendations for marginally viable infants. We assessed associations between estimated survival and perceived QOL and treatment recommendations., Results: In the 44 included surveys, estimates of survival and QOL varied widely. Maximum care was recommended 80% of the time when anticipated QOL was high, versus 20% when anticipated QOL was low (p < 0.001). Adjusted for confounders, odds of recommending maximum intervention were 4.4 times higher when anticipated QOL was high (95% CI 1.9 - 10.2, p = 0.001)., Conclusions: The perspectives of practitioners who provide care to critically ill neonates regarding potential survival and QOL vary dramatically and are associated with the treatments those practitioners recommend. Practitioners should take care to avoid basing treatment recommendations on their own perspectives if they are not well aligned with those of the parents.

The routine use of highly cross-linked ultra-high molecular weight polyethylene (UHMWPE) has remained controversial secondary to the possibility of decreased material properties when compared to conventional UHMWPE. The aim of the present study was to evaluate if thin, sequentially-irradiated, and annealed highly cross-linked UHMWPE tibial inserts would have improved wear properties, while maintaining mechanical integrity, compared to conventional UHMWPE during biomechanical testing under aligned and malaligned conditions. Polyethylene inserts (4.27 and 6.27 mm) manufactured from GUR 1020-UHMWPE were cyclically loaded to analyze for wear. All wear scars were visually examined after loading using scanning electron microscopy (SEM). Volume loss was plotted versus cycle count with linear regression analysis yielding wear rates. There was no statistical difference in wear between both thicknesses for all testing conditions. During aligned condition testing, the volumetric wear rate for sequentially-irradiated and annealed polyethylene thicknesses of 4.27 and 6.27 mm was 4.0 and 4.4 mm3/million cycles; and during malaligned conditions, it was 13.9 and 15.1 mm3/million cycles. For conventional polyethylene during aligned conditions, the volumetric wear rate was 33.0 and 22.8 mm3/million cycles; and during malaligned conditions it was 50.0 and 50.8 mm3/million cycles. By SEM evaluation, condylar wear surfaces for conventional and sequentially-irradiated and annealed polyethylene displayed surface ripples typical of adhesive wear. There were no observed visible differences between the wear scars for conventional compared to sequentially-irradiated and annealed polyethylene with no evidence of fatigue failure. This study demonstrated no differences between polyethylenes with thicknesses of 4.27 and 6.27 mm. This strengthens the conclusion that sequentially-irradiated and annealed highly cross-linked UHMWPE can be utilized in total knee arthroplasty. The successful wear properties of 4.27 mm liners could mean that smaller tibial resections leading to bone stock preservation could be utilized in patients undergoing total knee arthroplasty, although further in-vivo studies are needed.

Peri-viable birth raises an array of complex moral and legal concerns. Background: To determine (1) whether fetal care paediatric (FCP) and maternal-fetal medicine (MFM) specialists harbour differing attitudes about pregnancy termination for congenital fetal conditions, their perceived responsibilities to pregnant women and fetuses, and the fetus as a patient and (2) whether self-perceived primary responsibilities to fetuses and women and views about the fetus as a patient are associated with attitudes about clinical care., Methods: Mail survey of 434 MFM and FCP specialists (response rates 60.9% and 54.2%, respectively)., Results: MFMs were more likely than FCPs to disagree with these statements (all p values<0.005): (1) 'the presence of a fetal abnormality is not an appropriate reason for a couple to consider pregnancy termination' (MFM : FCP-78.4% vs 63.5%); (2) 'the effects that a child born with disabilities might have on marital and family relationships is not an appropriate reason for a couple to consider pregnancy termination' (MFM : FCP-80.5% vs 70.2%); and (3) 'the cost of healthcare for the future child is not an appropriate reason for a couple to consider pregnancy termination' (MFM : FCP-73.5% vs 55.9%). 65% MFMs versus 47% FCPs disagreed that their professional responsibility is to focus primarily on fetal well-being (p<0.01). Specialists did not differ regarding the fetus as a separate patient. Responses about self-perceived responsibility to focus on fetal well-being were associated with clinical practice attitudes., Conclusions: Independent of demographic and sociopolitical characteristics, FCPs and MFMs possess divergent ethical sensitivities regarding pregnancy termination, pregnant women and fetuses, which may influence clinical care.

There are several periprosthetic complications associated with total knee arthroplasty, with femoral fracture as the most common and patellar fractures as the second most common. Patellar fractures are challenging complications that occur almost exclusively on the resurfaced patellae, although unresurfaced patellar fractures have been reported in literature. The purpose of this study is to describe the anatomy of the patella, the etiology of patellar fractures, and strategies to treat and manage these fractures following knee arthroplasty. The vascular supply to the patella may be compromised during total knee arthroplasty and special care must be taken to preserve it. Vessel injury may result in further complications, most notably avascular necrosis with subsequent fracture. Other patient-, surgical-, and prosthetic-related factors can contribute to increased risk of patellar fracture. Patellar fractures are classified into three types. Type I fractures have an intact extensor mechanism with a stable implant. Type II fractures have a complete disruption of the extensor mechanism with or without a stable implant. Type III fractures, which are further subclassified into types IIIa and IIIb, have an intact extensor mechanism but a loose patellar component. While type IIIa fractures have reasonable remaining bone stock, type IIIb fractures have poor bone stock. Type I patellar fractures may be best managed nonoperatively, but types II and III patellar fractures often necessitate surgical intervention. Patellectomy should be reserved for comminuted fractures, as well as fractures in patients with poor bone stock. Larger prospective randomized studies are necessary to better evaluate the treatment algorithm for patellar fractures following total knee arthroplasty.

Religion and its practices have been duly implicated in treating not only problems related to medical health, rather, intervening and preventing such problems as well. In the present article, the authors have reviewed significance of the Islamic prayers (Salah/Namaaz) in healthcare in general and mental health in particular. The nature, procedures, practices and the benefits of Salah have been comprehensively described and discussed. In addition, an attempt to combine yoga and its practices with Salah has been made for intervening and preventing the problems of mental health as an expeditious tool. In upshot, the clinicians in the field of mental health care have been suggested to incorporate these two viewpoints in their intervention program, at least, for the Muslim patients for a more desirable outcome.

Objectives: The expansion of pediatric-based fetal care raises questions regarding pediatric specialists' involvement in pregnancies when maternal conditions may affect pediatric outcomes. For several such conditions, we compared pediatric and obstetric specialists' attitudes regarding whether and when pediatrics consultation should be offered and their views about seeking court authorization to override maternal refusal of physician recommendations., Methods: We used a mail survey of 434 maternal-fetal medicine specialists (MFMs) and fetal care pediatric specialists (FCPs) (response rate: MFM, 60.9%; FCP, 54.2%)., Results: FCPs were more likely than MFMs to indicate that pediatric counseling should occur before decisions regarding continuing or interrupting pregnancies complicated by maternal alcohol abuse (FCP versus MFM: 63% vs 36%), cocaine abuse (FCP versus MFM: 60% vs 32%), use of seizure medications (FCP versus MFM: 62% vs 33%), and diabetes (FCP versus MFM: 56% vs 27%) (all P < .001). For all conditions, MFMs were more than twice as likely as FCPs to think that no pediatric specialist consultation was ever necessary. FCPs were more likely to agree that seeking court interventions was appropriate for maternal refusal to enter a program to discontinue cocaine use (FCP versus MFM: 72% vs 33%), refusal of azidothymidine to prevent perinatal HIV transmission (80% vs 41%), and refusal of percutaneous transfusion for fetal anemia (62% vs 28%) (all P < .001)., Conclusions: Pediatric and obstetric specialists differ considerably regarding pediatric specialists' role in prenatal care for maternal conditions, and regarding whether to seek judicial intervention for maternal refusal of recommended treatment.

Total knee arthroplasty (TKA) is a procedure with excellent clinical results in older patients with a primary diagnosis of osteoarthritis. Many younger patients undergo unicompartmental or high tibial osteotomy rather than TKA, but are not always good candidates for these joint-preserving procedures. The purpose of this study was to review the outcomes of patients 40 years of age and under who underwent TKA. We identified 33 patients (38 knees) who were 40 years of age or less at the time of surgery. These patients had a mean age of 36 years (range, 23 to 40 years), and were followed for a mean of 49 months (range, 16 to 101 months). The survival rate in the study cohort was 97%. For the young patient who is not a candidate for other types of joint preserving procedures, in the senior authors' experience total knee arthroplasties have performed well.

Background: Currently more than 200,000 THAs are performed annually in the United States. In patients with bilateral disease, the chance of subsequent contralateral THA reportedly ranges from 16% to 85%. Factors influencing contralateral THA are not completely understood., Questions/purposes: We therefore determined (1) the 10-year probability free of progression to contralateral THA after index THA, (2) whether demographics differed between those who did and did not ultimately undergo contralateral THA, and (3) whether initial clinical symptoms and/or degree of radiographic osteoarthritis affects progression., Patients and Methods: We retrospectively identified 332 patients with minimum 24-month followup and primary osteoarthritis who underwent unilateral THA between 2001 and 2008. There were 150 men and 182 women with a mean age of 61 years (range, 27-93 years) and a mean BMI of 29.6 kg/m(2) (range, 17.6-49.2 kg/m(2)). We reviewed clinical, radiographic, and demographic data at index THA and last followup and classified patients as low, indeterminate, or high risk of undergoing contralateral THA., Results: Seventy-four of the 332 patients (22%) underwent contralateral THA, resulting in an 83% 10-year probability free of progression to the contralateral hip. Low-risk patients had a less than 1% chance of progression, indeterminate-risk patients had a 16% to 24% chance of progression, and high-risk patients had a 97% chance of progression., Conclusions: Indeterminate-risk patients may be managed nonoperatively and deserve further study with a larger multicenter analysis. Background: Deep infections are devastating complications of TKA often treated with component explantation, intravenous antibiotics, and antibiotic-impregnated cement spacers. Historically, the spacers have been static, which may limit patients' ROM and ability to walk. Several recent reports describe dynamic spacers, which may allow for improved ROM and make later reimplantation easier. However, because of several dynamic spacer problems noted at our institution, we wanted to assess their associated failures, reinfection rates, and functionality., Questions/purposes: We therefore asked whether there were differences between static and dynamic spacers in (1) reinfection rates, (2) complications directly related to the spacer, and (3) final patient functionality as measured by Knee Society objective scores and ROM., Patients and Methods: We retrospectively identified 111 patients (115 knees) with 34 dynamic spacers (30%) and 81 static spacers (70%). Reinfection rates, complications requiring additional surgery, and final Knee Society scores and ROM were collected for all patients., Results: Reinfection rates were comparable between groups. In the dynamic spacer cohort, there were four complications; however, these could all be explained by surgical technical errors or patient weightbearing compliance. All patients with failed results eventually underwent successful two-stage exchange arthroplasty. Final Knee Society scores and ROM were also similar between groups., Conclusions: Reinfection rates, Knee Society scores, and ROM were comparable between the static and dynamic spacer groups. Meticulous surgical technique and proper patient selection should be used to avoid any complications with any spacers.

Breast cancer is an estrogen-driven disease. Consequently, hormone replacement therapy correlates with disease incidence. However, increasing male breast cancer rates over the past three decades implicate additional sources of estrogenic exposure including wide spread estrogen-mimicking chemicals or xenoestrogens (XEs), such as bisphenol-A (BPA). By exposing renewable, human, high-risk donor breast epithelial cells (HRBECs) to BPA at concentrations that are detectable in human blood, placenta and milk, we previously identified gene expression profile changes associated with activation of mammalian target of rapamycin (mTOR) pathway genesets likely to trigger prosurvival changes in human breast cells. We now provide functional validation of mTOR activation using pairwise comparisons of 16 independent HRBEC samples with and without BPA exposure. We demonstrate induction of key genes and proteins in the PI3K-mTOR pathway--AKT1, RPS6 and 4EBP1 and a concurrent reduction in the tumor suppressor, phosphatase and tensin homolog gene protein. Altered regulation of mTOR pathway proteins in BPA-treated HRBECs led to marked resistance to rapamycin, the defining mTOR inhibitor. Moreover, HRBECs pretreated with BPA, or the XE, methylparaben (MP), surmounted antiestrogenic effects of tamoxifen showing dose-dependent apoptosis evasion and induction of cell cycling. Overall, XEs, when tested in benign breast cells from multiple human subjects, consistently initiated specific functional changes of the kind that are attributed to malignant onset in breast tissue. Our observations demonstrate the feasibility of studying renewable human samples as surrogates and reinforce the concern that BPA and MP, at low concentrations detected in humans, can have adverse health consequences.

The purposes of this study were to determine the probabilities of subsequent lower extremity arthroplasty after index knee arthroplasty for osteoarthritis and to evaluate the demographic as well as radiographic factors that may predict progression to arthroplasty in the contralateral knee. Between 1984 and 1994, 646 patients, aged 40 to 75 years, with a primary cruciate-retaining knee were identified. The 10-year probability of having a contralateral knee after index knee was 36%. When grade 4 radiographic changes were present, the probability increased to 70%. Demographic factors played no role in the risk of future contralateral knee. Background: Improvements in prosthetic materials, designs, and implant fixation for THA have led to bearing surface wear being the limitation of this technology. Hard-on-hard bearings promise decreased wear rates and increased survival. However, there may be different survival rates based on bearing materials, manufacturing technologies, and femoral component designs. Additionally, survival rate variability may be based on study design., Questions/purposes: We determined survival rates and study levels of evidence and quality for the following bearings: stemmed metal-on-metal THA, metal-on-metal hip resurfacing, ceramic-on-ceramic THA, and ceramic-on-metal THA., Methods: We performed a systematic review of the peer-reviewed literature addressing THA hard-on-hard bearings. Quality for Level I and II studies was assessed., Results: The four Level I or II second-generation stemmed metal-on-metal THA studies reported between 96% and 100% mean survival at 38 to 60 months. The two Level I hip resurfacing studies reported 94% and 98% mean survival at 56 and 33 months. The four Level I studies of ceramic-on-ceramic THA reported survival from 100% at mean 51 months to 96% at 8 years., Conclusions: While hard-on-hard bearing survival rates have generally been variable with earlier designs, contemporary implants have demonstrated survival of 95% or greater at followup of between 3 and 10 years. Some variability in survival may be due to differences in surgical technique, component positioning, and implant designs. As bearing designs continue to improve with modified materials and manufacturing techniques, use will increase, especially in young and active patients, though concerns remain about the increased reports of adverse events after metal-on-metal bearings.

Background: Young patients with osteonecrosis (ON) treated with THA often have suboptimal function and radiographic failure with eventual revision. Resurfacing may be an option because of potentially increased functionality and decreased radiographic failure, although neither has been confirmed in the literature., Questions/purposes: We therefore compared the clinical scores and radiographic failures of resurfacing patients 25 years or younger with osteonecrosis with those for patients with ON treated with THA., Methods: We identified 17 patients (20 resurfaced hips) who were 25 years or younger and treated by resurfacing for ON. Mean followup was 62 months (range, 32-103 months). This group was matched by gender, age, BMI, and followup to 16 patients (20 hips) treated with THA with a mean 61 month followup (range, 34-88 months). Additionally, the group was compared to 78 patients (87 hips) with ON over age 25 who underwent resurfacing during the same time with a mean followup of 61 months (range, 27-112 months)., Results: The mean Harris score improved from 48 points preoperatively to 93 points with a 100% 7.5 year survivorship in the young resurfacing group. In the THA cohort, the mean score improved from 44 points preoperatively to 94 points postoperatively, with a 93.3% 7.5 year survivorship; similar to the results for all resurfacing patients. Radiographs did not reveal malalignment or progressive radiolucencies in any cohort., Conclusion: At approximately 5 year followup, young resurfacing patients were similar to other resurfacing patients and standard THA patients regarding clinical ratings and radiographic failures.

One section of the Orthopaedic In-Training Examination (OITE) assesses knowledge about hip reconstruction. In the investigation reported here, we examined OITE hip reconstruction questions and sought to identify which literature can be recommended as study aids for this section of the test. All hip reconstruction questions on the OITE from 2002 to 2006 were characterized according to diagnosis and treatment. Journals cited most often in this section were identified from the OITE key. This content domain was compared with the literature in terms of overall proportion of questions/articles related to hip reconstruction and in terms of diagnoses and treatments. Of the 1375 OITE questions asked over the 5 years, 79 were related to hip reconstruction. More than half of these hip reconstruction questions were related to primary total hip arthroplasty, with complications being the diagnosis tested most often. The results of this study suggest that residents may benefit from using general orthopedic journals when preparing for the OITE hip reconstruction section. When preparing an educational program, however, one should be aware that clinical journals may not reflect the OITE in terms of proportion of basic science and biomechanics articles.

Total hip arthroplasty has shown excellent results in decreasing pain and improving function in patients with degenerative disease of the hip. Improvements in prosthetic materials, designs and implant fixation have now resulted in wear of the bearing surface being the limitation of this technology, and a number of hard-on-hard couples have been introduced to address this concern. The purpose of this article is to review the origins, development, survival rates and potential advantages and disadvantages of the following hard-on-hard bearings for total hip arthroplasty: metal-on-metal standard total hip arthroplasty; metal-on-metal hip resurfacing arthroplasty, ceramic-on-ceramic total hip arthroplasty; and ceramic-on-metal bearings. Improvements in the manufacturing of metal-on-metal bearings over the past 50 years have resulted in implants that provide low wear rates and allow for the use of large femoral heads. However, concerns remain regarding elevated serum metal ion levels, potential teratogenic effects and potentially devastating adverse local tissue reactions, whose incidence and pathogenesis remains unclear. Modern total hip resurfacing has shown excellent outcomes over 10 years in the hands of experienced surgeons. Current ceramic-on-ceramic bearings have demonstrated excellent survival with exceptionally low wear rates and virtually no local adverse effects. Concerns remain for insertional chipping, in vivo fracture and the variable incidence of squeaking. Contemporary ceramic-on-metal interfaces are in the early stages of clinical use, with little data reported to date. Hard-on-hard bearings for total hip arthroplasty have improved dramatically over the past 50 years. As bearing designs continue to improve with new and modified materials and improved manufacturing techniques, it is likely that the use of hard-on-hard bearings will continue to increase, especially in young and active patients.

Unlabelled: Two important issues affecting the outcome of total hip arthroplasty have been dislocation and wear, despite excellent clinical results. Larger femoral heads have had success in decreasing dislocation rates; however, there are concerns regarding the subsequent use of thinner polyethylene liners, and their effects on wear rates. Historically, high stresses on thin polyethylene bearings have caused concerns, including rim cracking and catastrophic implant failure with polyethylene thicknesses less than 5 millimeters. Recently, sequentially cross-linked and annealed polyethylene has been shown to reduce the wear rate significantly, compared to conventional ultra-high-molecular-weight polyethylene (UHMWPE) in vitro. The purpose of this study was to analyze the clinical and radiographic outcomes in a cohort of patients treated with sequentially cross-linked and annealed polyethylene bearings with a nominal thickness of 3.8 millimeters. Outcomes were compared to a similar cohort of patients who were treated with total hip arthroplasty during the same time period and utilizing the same polyethylene thickness of 5.8 millimeters or greater., Materials and Methods: We identified 50 patients (53 hips) who had a minimum 2-year clinical and radiographic follow-up after a standard total hip arthroplasty, performed with a thin, sequentially cross-linked and annealed polyethylene bearing surface. There were 15 males and 35 females, with a a mean age of 60 years (range, 16 to 93 years) and a mean body mass index of 28.6 kg/m² (range, 17.2 to 47.5 kg/m²). Overall survivorship was compared to a cohort of 50 patients (53 hips) treated with total hip arthroplasty during the same time period, utilizing the same polyethylene of 5.8 millimeters or greater thickness. Radiographic analysis of polyethylene wear was performed on a subset of 26 hips, using a previously validated two-dimensional computer-aided technique. Volumetric wear was calculated and subsequent annual volumetric and linear wear rates were derived for each patient in the study cohort. Additionally, radiographic analysis was performed to assess for any progressive radio-lucencies or malalignment., Results: The overall survivorship of the study cohort was 100%, compared to a 96% survivorship in the comparison group (two failures due to infection). The mean Harris hip scores in the thin polyethylene cohort improved from 43 points (range, 10 to 67 points) pre-operatively to 91 points (range, 69 to 100 points) postoperatively. Upon radiographic review, no malalignment, radiolucencies, or polyethylene fracture was noted in the study cohort. The mean volumetric wear rate was 0.4122 mm³/year (range, 0.2311 to 0.7310 mm³/year), and the mean linear wear rate was 0.0004 mm/year (range, 0.0002 to 0.0007 mm/year) for the thin polyethylene group. The mean volumetric wear was 0.8839 mm³ (range, 0.4621 to 1.5839 mm³) for this cohort. Excellent clinical and radiographic outcomes were found for patients treated with thin, sequentially cross-linked and annealed polyethylene bearings utilized in total hip arthroplasty, with a nominal thickness of 3.8 millimeters. We have not seen any failures with thin polyethylene liners that have undergone this manufacturing process, which is in contradistinction to results of previously reported thin polyethylene liners. Wear rates were lower than other bearing surfaces at similar periods.

Unlabelled: The Orthopaedic In-Training Examination (OITE) provides a standardized measure to assess the knowledge of orthopaedic residents regarding knee reconstruction surgery. However, there currently are limited resources for residents who are preparing for the knee reconstruction questions on the OITE. The present study assessed the character of the knee reconstruction questions tested and which literature resources may be recommended for residents preparing for this examination., Materials and Methods: All knee reconstruction-related questions found during a 5-year period (2002 to 2006) on the OITE were characterized by the diagnosis and treatment discussed. The most frequently referenced journals were identified from the OITE exam key. The character of the OITE questions was compared to the literature in terms of overall proportion of articles and questions that were related to knee reconstruction, as well as to categories of diagnosis and treatment modality., Results: There were 59 out of 1375 questions (4%) on the OITE over the 5 years that were related to knee reconstruction. Over half of the questions (54%) were related to primary total knee arthroplasty, with osteoarthritis being the most frequently tested diagnosis (30%). The top three referenced orthopaedic journals were The Journal of Bone and Joint Surgery-American, Clinical Orthopaedics and Related Research, and The Journal of Arthroplasty. Compared to the OITE, these journals covered higher percentages of overall knee reconstruction-related questions (18% versus 4%). In addition, the journal literature had a greater focus on treatment modalities (65% versus 41%) and less emphasis on biomechanics, materials, and basic science (18% versus 34%) than the OITE, respectively. The two most frequently cited textbooks represented approximately 78% of the total number of provided textbook references: Orthopaedic Knowledge Update (39%) and Instructional Course Lectures (39%)., Discussion: The results of this study suggest that residents may benefit from using general orthopaedic journals such as The Journal of Bone and Joint Surgery-American in preparation for the OITE. However, residents and residency directors who are preparing their educational programs should be aware that clinical journals may not refect the OITE in terms of the proportion of basic science and biomechanics articles and additional study resources may be necessary.

Pigmented villonodular synovitis (PVNS) is a proliferative disorder that may lead to joint destruction and activity limitation. We conducted a retrospective study to determine the long-term results of localized PVNS (LPVNS) treated with arthroscopic excision, specifically with respect to postoperative activity level and symptom resolution. We reviewed the cases of 11 patients who had been treated with arthroscopic excision and partial synovectomy of LPVNS and been followed up for a mean of 112 months. Preoperative and postoperative Ogilvie-Harris scores, Tegner activity level scores, and UCLA activity level scores were calculated to determine disease-specific and general functional outcomes, respectively. We noted 2 cases in which posteromedial lesions recurred, moderate resolution of preoperative symptoms in most cases, and 2 cases in which the patient developed secondary osteoarthritis requiring surgical intervention. Arthroscopic excision of LPVNS can improve symptoms with a return to preoperative activity levels, but patients may develop secondary osteoarthritis after treatment, as noted in long-term follow-up.

Introduction: Hundreds of thousands of premature neonates born in low-income countries are implicitly denied treatment each year. Studies from India show that treatment is rationed even for neonates born at 32 gestational age weeks (GAW), and multiple external factors influence treatment decisions. Is withholding of life-saving treatment for children born between 28 and 32 GAW acceptable from an ethical perspective?, Method: A seven-step impartial ethical analysis, including outcome analysis of four accepted priority criteria: severity of disease, treatment effect, cost effectiveness and evidence for neonates born at 28 and 32 GAW., Results: The ethical analysis sketches out two possibilities: (a) It is not ethically permissible to limit treatment to neonates below 32 GAW when assigning high weight to health maximisation and overall health equality. Neonates below 32 GAW score high on severity of disease and efficiency and cost-effectiveness of treatment if one gives full weight to early years of a newborn life. It is in the child's best interest to be treated. (b) It can be considered ethically permissible if high weight is assigned to reducing inequality of welfare and maximising overall welfare and/or not granting full weight to early years of newborns is considered acceptable. From an equity-motivated health and welfare perspective, we would not accept (b), as it relies on accepting the lack of proper welfare policies for the poor and disabled in India., Conclusion: Explicit priority processes in India for financing neonatal care are needed. If premature neonates are perceived as worth less than other patient groups, the reasons should be explored among a broad range of stakeholders.

Objective: The aim of this article was to describe how providers in an Indian NICU reach life-or-death treatment decisions., Methods: Qualitative in-depth interviews, field observations, and document analysis were conducted at an Indian nonprofit private tertiary institution that provided advanced neonatal care under conditions of resource scarcity., Results: Compared with American and European units with similar technical capabilities, the unit studied maintained a much higher threshold for treatment initiation and continuation (range: 28-32 completed gestational weeks). We observed that complex, interrelated socioeconomic reasons influenced specific treatment decisions. Providers desired to protect families and avoid a broad range of perceived harms: they were reluctant to risk outcomes with chronic disability; they openly factored scarcity of institutional resources; they were sensitive to local, culturally entrenched intrafamilial dynamics; they placed higher regard for "precious" infants; and they felt relatively powerless to prevent gender discrimination. Formal or regulatory guidelines were either lacking or not controlling., Conclusions: In a tertiary-level academic Indian NICU, multiple factors external to predicted clinical survival of a preterm newborn influence treatment decisions. Providers adjust their decisions about withdrawing or withholding treatment on the basis of pragmatic considerations. Numerous issues related to resource scarcity are relevant, and providers prioritize outcomes that affect stakeholders other than the newborn. These findings may have implications for initiatives that seek to improve global neonatal health.

To evaluate the need for joint arthroplasty within 10 years after index primary Charnley total hip arthroplasty (THA) performed for osteoarthritis, we retrospectively reviewed the cases of 2,547 patients, 50 to 75 years old, from 1969 to 1984, with a minimum potential 20-year follow-up. In this article, we report the age, sex, and time data from this study. For the entire patient population, the 10-year rate of undergoing contralateral THA was 35.0%; ipsilateral hip revision, 6.2%; ipsilateral total knee arthroplasty (TKA), 0.6%; contralateral TKA, 1.9%; and bilateral TKA, 0.2%. The 10-year death rate was 21.8%. With more than 200,000 THAs being performed in the United States each year, these numbers can guide orthopedic surgeons in their discussions about subsequent arthroplasty procedures on other joints.

Anatomic variations in the insertion of the long head of the biceps tendon and superior labral complex have been described (Erickson et al. [1992] AJR Am. J. Roentgenol. 158:1091-1096; Kreitner et al. [1998] AJR Am. J. Roentgenol. 170:599-605; Mariani et al. [1997] Arthroscopy 13:499-501; Vangsness Jr. et al. [1994] J. Bone Joint Surg. Br. 76:951-954). To the authors' knowledge, there have been only five reported cases of congenital absence of the long head of the biceps tendon. Three of these cases were associated with anterior shoulder instability, one with a superior labral anterior posterior lesion and one simply with shoulder pain. Foot and ankle complaints are commonly encountered in orthopedic practice. Midfoot arthritis has the potential to cause a significant amount of pain and disability. A variety of conditions can cause or lead to midfoot arthritis. Treatment consists of either conservative management or surgical arthrodesis of the painful joints. In this article, we review the midfoot (its basic anatomy and biomechanics) and midfoot arthritis (its clinical presentation and etiology, radiographic evaluation, and treatment options).

We identified 1630 unrevised patients who underwent primary total knee arthroplasty (TKA) between 1995 and 2000. Patients were surveyed regarding clinical outcome and activity level, and were queried about actual participation in 40 different athletic activities. One thousand two hundred six patients (74%) responded at a mean of 5.7 years after arthroplasty. Average age at TKA was 67 years. Average University of California at Los Angeles (UCLA) activity level rating was 7.1. Satisfaction with activity level was 91%. Six hundred forty-three patients (53%) responded that their activities were limited by other joints. Patients older than 70 years at arthroplasty had lower UCLA ratings and Knee Society function scores (P < .0001) but higher self-assessment of activity vs peers (P = .001) than those younger than 70 years. Men had higher UCLA scores (P < .0001), Knee Society function scores (P < .0001), and higher self-assessment of activity level vs peers (P < .0001) than women. One hundred eighty-seven patients (16%) reported participating in heavy manual labor or sports deemed "not recommended" in a published Knee Society survey.
