Background: Sarcoidosis is a granulomatous disease of unknown origin. Conventional laboratory and imaging modalities may lead to equivocal conclusions for sarcoidosis diagnosis. 68Ga-citrate PET/CT has been utilized in the diagnosis of inflammatory and infectious diseases due to its beter  performance.  Aims: This study was designed to determine the clinical trenchancy of 68Ga-citrate in sarcoidosis patients as an alternative tracer that binds to the somtastotin receptors of the inflammatory cells in sarcoid granulomas because current laboratory modalities may occasionally lack accuracy for sarcoidosis diagnosis. Conventional criteria including clinical, laboratory, radiologic modalities, and 18F-FDG PET/CT were compared with the 68Ga-citrate PET/CT to evaluate the diagnostic yield in sarcoidosis., Methods: Forty-four sarcoidosis patients were include in the study. Conventional laboratory and imaging  modalities were done in 44 while  68Ga-citrate PET/CT and 18F-FDG-PET/CT  were    performed in 22 patients. The three modalities were compared with in regard to granulomatous activity evaluation, extrapulmonary organ involvement, and identification of coexistent malignant disease., Results: 68Ga-citrate PET/CT revealed a significantly higher diagnostic yield for sarcoidosis compared to conventional laboratory and 18F-FDG-PET/CT imaging modalties. 68Ga-citrate PET/CT revealed 72.7% sensitivity and 68.4% specificity for the detection of granulomatous inflammation in sarcoidosis., Conclusions: 68Ga-citrate PET/CT was conclusive for activity assessment in sarcoidosis for diagnosis, activity assessment, and identification of extrapulmonary organ involvement. As an innovative novel tracer, 68Ga-citrate identified disease activity, extrapulmonary organ involvement, biopsy sites, and coexistent malignancy due to its chemical properties with a higher statistical diagnostic yield compared to conventional laboratory findings and the  PET/CT imaging manifestations in sarcoidosis.

Abstract: Amyloidosis is a protein misfolding disorder characterized by the extracellular deposition of insoluble amyloid fibrils, derived from abnormally folded proteins. These fibrils disrupt tissue structure and function, leading to organ dysfunction. The condition encompasses various subtypes, each associated with distinct precursor proteins and clinical manifestations. 99mTc-PYP scintigraphy is used widely and holds significant importance for diagnosis. 68Ga-FAPI is also a promising radiotracer for various diseases. Purpose: The main purpose is to evaluate the safety, and efficacy of 177 Lutetium labeled macroaggregated albumin (LUTMA) ablation of thyroid nodules., Materials and Methods: Patients with confirmed benign nodules who were not candidate or did not accept surgery were enrolled. Under ultrasonography (USG) guidance, LUTMA which was produced in our department, was administered into the nodules. Nodule volumes were assessed via USG before the injection and at 1-week, 1-month, and 3-months post-treatment. We calculated the volume reduction rates (VRRs) for these intervals. To detect extranodular activity leakage, patients underwent SPECT/CT imaging at one hour, 24 h, and one week post-injection., Results: Fifteen patients (male: 12, female: 3) with benign thyroid nodules were eligible to join this study. These nodules were categorized as cystic (n = 9), solid (n = 3), or mixed (n = 3). Median nodules volume was 6.59 ml (range: 0.56-55 ml). Predicted absorbed dosee to the nodules varied between 10-1036 Gy. The VRRs at 3 months was 85% for all nodule types with gradual increases over time: 0%-92%, 20%-97%, and 28%-98% at 1 week, 1 month, and 3-months, respectively. The median VRR of cystic nodules was 89% (range: 81%-98%) at 3-months. It is significantly higher than solid ones (P = .009). None of the patients experienced adverse reactions or discomfort during the injection or follow-up., Conclusion: LUTMA treatment significantly reduces the volume of benign thyroid nodules, offering relief from disease-associated symptoms and cosmetic concerns. Background: The non-invasive imaging of leukocyte trafficking to assess inflammatory areas and monitor immunotherapy is currently generating great interest. There is a need to develop more robust cell labelling and imaging approaches to track living cells. Positron emission tomography (PET), a highly sensitive molecular imaging technique, allows precise signals to be produced from radiolabelled moieties. Here, we developed a novel leukocyte labelling approach with the PET radioisotope zirconium-89 ( 89 Zr, half-life of 78.4 h). Experiments were carried out using human leukocytes, freshly isolated from whole human blood., Results: The 89 Zr-leukocyte labelling efficiency ranged from 46 to 87% after 30-60 min. Radioactivity concentrations of labelled cells were up to 0.28 MBq/1 million cells. Systemically administered 89 Zr-labelled leukocytes produced high-contrast murine PET images at 1 h-5 days post injection. Murine biodistribution data showed that cells primarily distributed to the lung, liver, and spleen at 1 h post injection, and are then gradually trafficked to liver and spleen over 5 days. Histological analysis demonstrated that exogenously 89 Zr-labelled human leukocytes were present in the lung, liver, and spleen at 1 h post injection. However, intravenously injected free [ 89 Zr]Zr 4+ ion showed retention only in the bone with no radioactivity in the lung at 5 days post injection, which implied good stability of radiolabelled leukocytes in vivo., Conclusions: Our study presents a stable and generic radiolabelling technique to track leukocytes with PET imaging and shows great potential for further applications in inflammatory cell and other types of cell trafficking studies., (© 2023. Purpose: Bone metastasis is essential in patients with prostate cancer (PCa) as it determines prognosis and survival. Hybrid PET/MRI allows simultaneous acquisition of PET and MRI data, thus combining the strength of both technologies allows the detection of bone marrow metastases that are missed by PET/CT. In this retrospective study, we aimed to evaluate the diagnostic efficiency of hybrid PET/MRI with Ga-68 prostate-specific membrane antigen (PSMA) in detecting skeletal metastases in newly diagnosed PCa patients and compared the effectiveness of stand-alone PSMA PET reviewing versus stand-alone whole-body (WB) MRI evaluation. We also investigated the effect of the interpretation of all PET/MR data together on clinical management., Methods: We studied 74 newly diagnosed PCa patients who underwent PSMA PET/MRI for staging purposes. At first, PET and MRI were evaluated separately for bone lesions on a patient-and-lesion basis and then a further joint PSMA PET/MRI interpretation was made., Results: Patient-based sensitivity, specificity, positive predictive value, negative predictive value and accuracy analysis for bone metastasis was, respectively, 1.0, 0.83, 0.54, 1.0, 0.86 for PET; 0.75, 0.96, 0.81, 0.95, 0.93 for WB MRI and 0.91, 0.95, 0.78, 0,98, 0.94 for PET/MRI. The combined PET/MRI evaluation changed the clinical impact in 13.5% of patients (eight correct and two wrong decisions) compared to PET stand-alone interpretation., Conclusion: PSMA PET imaging showed superior sensitivity to WB MRI in detecting bone metastases in newly diagnosed PCa patients, whereas WB MRI has superior specificity and PPV. Furthermore, the specificity and PPV of joint PET/MRI evaluation are better than PSMA PET alone. Objective: To investigate the contribution of 68 Gallium ( 68 Ga)-PSMA (prostate-specific membrane antigen) positron emission tomography (PET) in defining radiotherapy (RT) target volume for glioblastoma and to compare the target volumes defined by Magnetic Resonance Imaging (MRI)., Methods: RT planning Computed Tomography (CT) images were fused separately with pre-operative MRI and PET/MRI images of 10 glioblastoma patients, retrospectively. The contrast-enhanced area in T 1 weighted MRI was contoured as gross tumor volume (GTV) and clinical target volume (CTV1) was obtained by including the cavity and T2/FLAIR hyperintense areas after giving a margin of 2 cm to the GTV. 68 Ga-PSMA uptake area was contoured as biological tumor volume (BTV) and CTV2 was obtained with a margin of 2 cm to BTV. Planning target volumes (PTVs) were created with the 3 mm added to the CTVs. Conformity index (CI), dice similarity coefficient (DSC) and overlap volume (OV) were calculated by obtaining the intersection and union volumes. Volumetric comparison, similarity and overlap analyzes were performed statistically by Wilcoxon signed rank and One sample t -test., Results: The median GTV was 21,96 cc (1,04 - 82,04) and BTV was 25,58 cc (2,43 - 99,47). BTV was on average 47% larger than GTV which was statistically significant ( p = 0.03). For GTV-BTV, CTV1-CTV2 and PTV1-PTV2; mean values of CI were 0,56, 0,76 and 0,76; DSC were 0,70, 0,86 and 0,86; OV were 0,88, 0,94 and 0,94, respectively. There was no significant difference on size and spatial similarity between CTV1 and CTV2, PTV1 and PTV2., Conclusion: Altough BTV was larger than GTV, this significance was lost while we gave the same CTV margin including the peripheral edema. It seems that it may help to improve defining non-enhancing tumor part and also recurrent tumor volume., Advances in Knowledge: Recent studies have focused on the role of 68 Ga-PSMA PET in imaging of glial tumors. It has been observed that 68 Ga-PSMA PET can clearly define the tumor borders and it can be beneficial in target volume delineation, especially in reirradiation of recurrent tumors.

We present the first 99mTc-Vitamin C single-photon emission computed tomography/computed tomography (SPECT/CT) images obtained in patients with SARS-CoV-2 (COVID-19) infection. The CT portion of SPECT/CT images showed mostly peripheral patchy and ground-glass opacities in both lungs, which are consistent with a diagnosis of SARS-CoV-2-associated pneumonia in both patients. 99mTc-Vitamin C SPECT images showed increased tracer uptake corresponding to abnormal lung findings seen on CT in patient 1 who was newly diagnosed and treatment naïve. However, no abnormal uptake corresponding to lung CT findings was seen in patient 2 who received anti-SARS-CoV-2 treatment.

Prostate-specific membrane antigen (PSMA) - based radiopharmaceuticals are promising for the evaluation of PSMA-positive non-prostate cancers. In this case study, 18F-BF3-Cy3-ACUPA and 68Ga-PSMA positron emission tomography/magnetic resonance imaging (PET/MRI) were compared in a patient with metastatic colon cancer. Both 18F-BF3-Cy3-ACUPA and 68Ga-PSMA PET/MRI showed biopsy-proven metastatic left external iliac adenopathy, highlighting the feasibility of PSMA uptake in PET/MRI of metastatic nodal disease from colon cancer. Along with imaging evaluation, PSMA-based radiopharmaceuticals may also be used as a surrogate imaging tracer for potential theranostic applications using alpha or beta emitters in the context of PSMA-directed radiopharmaceutical therapy in advanced and progressive colorectal cancer.

Background: A first-in-human study of [ 18 F]-BF3-Cy3-ACUPA, a small-molecule imaging agent that can be unimolecularly both positron emitting and fluorescent, is conducted to determine its safety, biodistribution, radiation dosimetry, feasibility in tumor detection by preoperative positron emission tomography (PET), as well as its intraoperative fluorescence imaging utility in patients with prostate-specific membrane antigen positive (PSMA + ) tumors., Methods: Ten patients aged 66 ± 7 years received a 6.5 ± 3.2 mCi intravenous injection of [ 18 F]-BF3-Cy3-ACUPA and underwent PET/computed tomography (CT) imaging. Radiation dosimetry of [ 18 F]-BF3-Cy3-ACUPA, normal organ biodistribution, and tumor uptakes were examined. Two patients were prescheduled for radical prostatectomy (RP) with extended pelvic lymphadenectomy approximately 24 hours following [ 18 F]-BF3-Cy3-ACUPA injection and imaging. Without reinjection, intraoperative fluorescence imaging was performed on freshly excised tissue during RP. Frozen sections of excised tissue during RP were submitted for confirmatory histopathology and multiphoton fluorescence and brightfield microscopy., Results: Absorbed doses by organs including the kidneys and salivary glands were similar to 68Ga-PSMA-11 imaging. [ 18 F]-BF3-Cy3-ACUPA physiologic radiotracer accumulation and urinary/biliary excretion closely resembled the distribution of other published PSMA tracers including [ 18 F]-JK-PSMA-7, [ 18 F]-PSMA-1007, [ 18 F]-DCFPyL, and [ 18 F]-DCFBC. 19 F-BF3-Cy3-ACUPA was retained in PSMA + cancer tissues in patients for at least 24 hours, allowing for intraoperative fluorescence assessment of the prostate and of the embedded prostate cancer without contrast reinjection. After 24 hours, the imaging agent mostly decayed or cleared from the blood pool. Preoperative PET and fluorescence imaging findings were confirmed with final histopathology and multiphoton microscopy., Conclusion: Our first-in-human results demonstrate that [ 18 F]-BF3-Cy3-ACUPA is safe and feasible in humans. Larger trials with this PET tracer are expected to further define its capabilities and its clinical role in the management of PSMA + tumors, especially in prostate cancer., (Copyright © 2021. Background/aim: The criteria for surgical management of ureteropelvic junction obstruction are not well-defined, and there is a risk for loss of renal function before the operation. In this context, certain changes in contralateral kidney had been investigated in order to increase the sensitivity of diagnosis. In this study, we aimed to investigate whether contralateral transient minimal hydronephrosis (CTMH) can be considered as an “early alarm” sign for worsening of the affected kidney in infants with hydronephrosis., Materials and Methods: A total of 182 infants (92 surgically treated and 90 conservatively followed-up) with unilateral hydronephrosis were retrospectively analyzed. Ultrasonography and renal scan findings were evaluated. Correlation between the appearance of CTMH, contralateral compensatory hypertrophy (CCH) on ultrasonography, and prognosis of the affected kidney were evaluated., Results: Among the surgically treated patients, 18 (19.6%) patients developed CTMH on average 7 months (0–13 months) before surgery. Among these 18 patients with CTMH, 12 patients (66.6%) had loss of renal function preoperatively, while this ratio was 29.7% on their counterparts (p = 0049). CCH was observed in 31 (33.7%) individuals in surgically treated patient group including all 18 patients with CTMH, while none of the conservatively followed-up patients developed CCH and/or CTMH. In the multiple logistic regression analysis, among the variables investigated, CTMH was found as an independent predictor of the deterioration in the affected kidney and of the poor prognosis (p = 0.011 and p = 0.0004, respectively)., Conclusion: In our study, among the variables investigated, CTMH was found as an independent predictor of the deterioration in the affected kidney and poor prognosis in infants followed-up with isolated unilateral hydronephrosis. Objective: PET imaging with F-18 DOPA (FDOPA) and Ga-68 DOTATATE (TATE) shows the most promising results to detect medullary thyroid cancer (MTC) recurrence. We performed this comparative study to detect the site of recurrent or metastatic disease in MTC patients with elevated serum calcitonin (Ctn) and/or carcinoembryonic antigen (CEA) levels., Methods: We studied 46 MTC patients (25 women, 21 men) with elevated Ctn and/or CEA levels during follow-up who had both FDOPA and TATE PET/CT scans for re-staging purposes., Results: FDOPA PET imaging yielded an overall sensitivity of 86.8%, specificity of 100%, PPV of 100%, NPV of 61.5%, and accuracy of 89.1%, while TATE PET scan had the same values as 84.2%, 87.5%, 96.9%, 53.8%, and 84.6%, respectively, and there was no statistically significant difference between the two modalities with the exception of the specificity value that was higher for FDOPA imaging. In a subgroup of patients with overt Ctn or CEA elevation, sensitivity of FDOPA increased significantly, whereas TATE sensitivity did not change. FDOPA PET imaging was significantly superior in detecting liver and regional lymph node (LN) metastases, while TATE PET scan was significantly better in the skeletal metastases. Early FDOPA demonstrated 11 invisible lesions on late FDOPA., Conclusion: Both FDOPA and TATE PET/CT imaging are useful to localize recurrences in MTC patients. While TATE imaging is superior to reveal skeletal disease, FDOPA seems better in liver and regional LN metastases; therefore, the two modalities appear complementary in monitoring MTC patients with elevated serum Ctn and/or CEA levels., (© 2021. Background: Magnetic resonance imaging (MRI) is the most important imaging modality in the diagnosis and follow-up of glial brain tumors., Objective: The aim of our study is to determine the correlation between tumor grade, determined with postoperative pathological examination, and standard uptake value (SUV), a semi-quantitative parameter, in patients who underwent imaging 68 Ga-PSMA with using PET/MR., Material-Method: Thirty-five out of 38 patients' images whose pathology was consistent with glial tumor, 42 lesions from separate anatomic localizations or with higher activity uptake than the rest of the tumor were evaluated. SUV values measured on PET images and grade relationship were evaluated based on each lesion while mitosis, Ki-67 were evaluated for each patient., Results: Grade, Ki-67, mitosis, necrosis and SUVmax/mean/peak were found statistically significant with moderate/high correlation. The parameter with the highest correlation coefficient was mitosis. (For SUVmax r = 0.64, p = 0). When Grade II and III were considered as the first group and IV as the second group, the cutoff values were found to be 2.3 for SUVmax, 0.21 for SUVmean and 0.63 for SUVpeak. In the diagnosis of HGG, PET's sensitivity is higher than MRI but no statistically difference was found between specificities., Conclusion: 68 Ga PSMA PET imaging is found to be particularly useful in differentiating Grade IV glial tumors from other grades. Objectives: Gallium-68 (Ga-68) prostate specific membrane antigen (PSMA) positron emission tomography (PET) has been shown to be more accurate than multiparametric prostate magnetic resonance imaging (MRI) in detection of primary prostate lesions. Using hybrid PET/MRI we aim to detect the correlation between SUV max and apparent diffusion coefficient (ADC) in primary prostate lesions and to assess their prognostic value in detection of lymph node (LN) metastasis., Methods: Twenty-six patients, who were diagnosed as having prostate cancer with biopsy and underwent Ga-68 PSMA PET/MRI together with biparametric prostate MRI (bpMRI) were included. SUV max , SUV mean and ADC were recorded for index lesions drawing a region of interest (ROI) of 1 cm 2 around the pixel with the highest SUV max (ROI-1) and another ROI following borders of prostate tumor detected by bpMRI (ROI-2). Presence of LN metastasis was recorded according to PSMA PET/MRI, Results: SUV max was inversely correlated with ADC (ROI-1: p=0.010; ROI-2: p=0.017 for b=800). SUV max and SUV means were both higher in patients with LN metastasis and ADC was lower in patients with LN metastasis for ROI-1. SUV max cut-off value of 19.8 for ROI-1 and 20.9 for ROI-2 had sensitivity and specificity of 77.8% and 76.5%, respectively for detection of LN metastasis, whereas ADC (b=800) cut-off value of 0.92x10 -3 mm 2 /s had sensitivity and specificity of 87.5% and 76.5%, respectively. SUV max /ADC (b=800) ratio increased the sensitivity and specificity to 100% and 82.4%, respectively., Conclusion: SUV and ADC values are inversely correlated in primary prostate lesions and the combined use of both values increases the diagnostic accuracy of hybrid PET/MRI in the detection of primary prostate lesions.

Clinical trials involving genome-edited cells are growing in popularity, where CAR-T immunotherapy and CRISPR/Cas9 editing are more recognized strategies. Genetic reporters are needed to localize the molecular events inside these cells in patients. Specifically, a nonimmunogenic genetic reporter is urgently needed as current reporters are immunogenic due to derivation from nonhuman sources. Prostate-specific membrane antigen (PSMA) is potentially nonimmunogenic due to its natural, low-level expression in select tissues (self-MHC display). PSMA overexpression on human prostate adenocarcinoma is also visible with excellent contrast. We exploit these properties in a transduced, two-component, H uman- D erived, G enetic, P ositron-emitting, and F luorescent (HD-GPF) reporter system. Mechanistically analogous to the luciferase and luciferin reporter, PSMA is genetically encoded into non-PSMA expressing 8505C cells and tracked with ACUPA-Cy3-BF3, a single, systemically injected small molecule that delivers positron emitting fluoride ( 18 F) and a fluorophore (Cy3) to report on cells expressing PSMA. PSMA-lentivirus transduced tissues become visible by Cy3 fluorescence, [ 18 F]-positron emission tomography (PET), and γ-scintillated biodistribution. HD-GPF fluorescence is visible at subcellular resolution, while a reduced PET background is achieved in vivo , due to rapid ACUPA-Cy3-BF3 renal excretion. Co-transduction with luciferase and GFP show specific advantages over popular genetic reporters in advanced murine models including, a "mosaic" model of solid-tumor intratumoral heterogeneity and a survival model for observing postsurgical recurrence. We report an advanced genetic reporter that tracks genetically modified cells in entire animals and with subcellular resolution with PET and fluorescence, respectively. This reporter system is potentially nonimmunogenic and will therefore be useful in human studies. PSMA is a biomarker of prostate adenocarcinoma and ACUPA-Cy3-BF3 potential in radical prostatectomy is demonstrated.

Objective: We investigated the utility of PET/CT in cutaneous melanoma (CM) patients with pathological negative sentinel lymph nodes (SLN), within the first year., Methods: The results of PET/CTs and SLN biopsy (SLNB) in 65 patients (39 male and 26 female, mean age 53.8) with a PET/CT in the first postoperative year were evaluated. Within this cohort, the utility of early PET/CT imaging was assessed in patients with negative SLNB. McNemar test was used to compare PET/CT findings with SLNB results., Results: Out of 43 patients with pathologically positive SLNs, 23 patients (53.5%) had positive and 20 patients (46.5%) had negative findings on PET/CT within the first postoperative year. On the other hand, PET/CT results of 22 patients with negative SLNBs were found to be negative in 19 patients (86.4%) and positive in 3 patients (13.6%). For the patients with negative SLNB results, the detection rate of distant metastasis with PET/CT was significantly lower (p<0.001) than that in patients with positive SLNBs., Conclusion: Our results showed that 18 F-FDG PET/CT will most likely (86.4%) be negative during the first postoperative year in patients with a negative SLNB. Therefore, it is concluded that this modality would not provide any significant clinical contribution within this time frame.

[ 18/19 F]-4, an anionic GCPII/PSMA inhibitor for image-guided intervention in prostate cancer, is described. [ 19 F]-4 is radiolabeled with a radiochemical yield that is ≥27% and a molar activity of 190 ± 50 mCi/μmol in a <1 h, one-step, aqueous isotopic exchange reaction. [ 19 F]-4 allows PSMA expression to be imaged by fluorescence (FL) and [ 18 F]-PET. PC3-PIP (PSMA-positive, EC 50 = 6.74 ± 1.33 nM) cancers are specifically delineated in mice that bear 3 million (18 mg) PC3-PIP and PC3 (control, PSMA-negative) cells. Colocalization of [ 18/19 F]-4 PET, fluorescence, scintillated biodistribution, and PSMA expression are observed.

Objective: There is still no consensus about whether to perform PET/CT to detect carcinoma in paraneoplastic neurological syndromes (PNS) in patients with or without antibodies. The aim of this study is to determine the diagnostic accuracy of PET/CT and antibodies in patients with PNS., Material and Methods: A retrospective study was conducted on patients with clinically suspected PNS between 2008 and 2013. The association between histopathological findings, paraneoplastic antibodies, and PET/CT findings were evaluated. Sensitivity and specificity for the detection of underlying malignancy were calculated for PET/CT and paraneoplastic antibodies., Results: A total of 42 patients were analyzed. Of these 42 patients, 32 (75%) had a classical PNS, 6 (14%) had positive PET/CT findings, and 34 were tested for the presence of antibodies (anti-Hu Ab, anti-Yo Ab, and anti-Ri Ab). Twenty one of 34 patients had positive antibodies. Of the 6 patients with positive PET/CT findings, 6 had positive histopathological results. Among 21 patients with positive biomarkers, carcinoma was confirmed only in 5 patients. One patient with negative antibodies, but positive PET/CT findings, was diagnosed with a tumor. Gastric carcinoma was detected in 1 patient with negative PET/CT findings and antibodies during follow-up. Based on the results, PET/CT was found to have 85.71% sensitivity, 100% specificity, 100% positive and 97.22% negative predictive values in the detection of tumors., Conclusion: PET/CT has a certain diagnostic accuracy for detecting underlying malignancy in patients with PNS, regardless of the presence of paraneoplastic antibodies., (Copyright © 2015 Elsevier España, S.L.U. and SEMNIM. Introduction: Single-photon emission computed tomography (SPECT) myocardial perfusion imaging is an accepted method for reflecting the pathophysiological significance of lesions detected by coronary angiography. However, it has an inherent drawback in terms of false-positive perfusion defects for the inferior myocardial wall. To overcome this problem, different acquisition techniques have been proposed, including the computed tomographic-based attenuation correction method. In this respect, a new imaging technique, left supine lateral position SPECT myocardial perfusion imaging with technetium-99m methoxyisobutylisonitrile (Tc-99m MIBI), has been proposed to eliminate this problem and its value has been investigated in this report., Materials and Methods: Sixty-two patients were prospectively and randomly enrolled in this study. They underwent Tc-99m MIBI SPECT in the supine, prone, left lateral, and sitting positions after an adequate stress test on the same day.The presence and extent of defects on stress images were noted in the supine image data set for the 11 myocardial segments, which were then labeled as 1 or 0 if a defect was present or absent, respectively. This evaluation sequence was repeated in all other image data sets. When defects persisted in other scan positions it was regarded as true positive, and when they were resolved they were regarded as false positive. By this means, the percentages of resolving perfusion defects by that imaging position were calculated for each observer per positional pair under comparison., Results: From six interpretations carried out by the nuclear medicine physicians, 6×11×3=198 four-fold tables in 11 segments were analyzed for discrepancies between position pairs. In 31 of 33 discrepant interpretations, defects observed in any of the other positions were resolved in the lateral position. Only in two evaluations of one observer were the discrepancies against lateral positioning for the anterior wall. If the inferior wall was considered alone, it was clearly obvious that lateral positioning was more accurate than the other positions.Intraobserver evaluation showed the methodology to be highly reproducible.The SPECT findings were concordant with coronary angiography results in selected patients., Conclusion: Visual and quantitative evaluations of the variation in inferior wall activity lead us to suggest that SPECT imaging with Tc-99m MIBI be performed in the left lateral position to allow better visualization of the inferior and septal walls in those departments not able to utilize computed tomographic attenuation correction.

Objective: We observed an unusually increased scrotal uptake as an interesting finding in some patients with prostate cancer who were scanned for any possible metastatic disease. This study was designed to investigate the significance of this incidental finding in the technetium-99m methylene diphosphonate scintigraphies., Methods: The study population consisted of 104 patients with biopsy-proven prostate cancer (group I), 55 male patients with other cancers (group II), 30 male patients with nonmalignant diseases (group III) and finally 15 patients with benign prostate hypertrophy (group IV). The square-shaped regions of interest are placed centrally on the scrotum and then on the lateral femoral soft tissue. Then the simple ratios of the scrotal and femoral soft tissue mean counts (S/Bg) were calculated. The statistical significance of differences among the groups in terms of scrotal uptake was determined., Results: Group I showed increased scrotal uptake relative to the other groups. The mean uptake ratios (S/Bg±standard deviation) were 3.49±1.42 in group I, 2.89±0.70 in group II, 2.87±0.75 in group III, and 2.91±0.60 in group IV. This ratio was significantly higher in patients with prostate cancer than the normal group (P=0.024), the group with benign prostate hypertrophy (P=0.004), and the patients with other cancers (P=0.004)., Conclusion: Our results showed that technetium-99m methylene diphosphonate bone scintigraphy, as a routine for detecting metastatic disease or performed for other purposes, could give clues for a hidden prostate cancer. Then, in elderly male patients, we strongly recommend that it is wise to keep one's eye on scrotal activity when bone scans are read and where there is any doubt, take appropriate regions of interest to make quantitative evaluations.

Objective: The aim of this study was to evaluate the feasibility and safety of intravascular radiation therapy (IVRT) using Re-188 filled balloon system in patients with in-stent stenosis., Methods: A total of 39 patients with in-stent restenosis were enrolled as the IVRT (22 patients) and control groups (17 patients) of this study after a successful coronary angioplasty. For irradiation the angioplasty balloon was replaced by a noncompliant balloon of the same diameter but 10 mm longer in length with a proximal and distal radio-opaque marker to deliver the dose of 18 Gy at 0.5 mm depth from the surface of the balloon into the vessel wall. Angiographic follow-up was performed after 6 months., Results: The length of the irradiated segment was between 9.14 and 22 mm and the diameter between 2.5 and 3 mm. In the IVRT group, two patients who did not receive antiplatelet therapy had myocardial infarction. Four patients who presented with stable angina earlier also had angiographically documented in-stent occlusion (two patients) and edge stenosis (two patients) of the target lesion and received angioplasty (18.1%). In the control group, three patients with recurrent angina and four asymptomatic patients had documented in-stent occlusion angiographically at 6 months and these seven patients underwent target lesion revascularization (41.2%). The overall restenosis rate in the IVRT and control groups were 23.91 and 39.86%, respectively (P=0.013). No complications were documented, except anginal pain and ST segment changes., Conclusion: Our results indicated that the Re-188 liquid-filled balloon is feasible, safe, and effective in patients with in-stent restenosis.

Technetium-99m-ethylene-l-l-dicysteine ((99m)Tc-EC) is an agent with a potential for renal imaging. It is reported that EC uses the same tubular transporter system as ortho-hippurate (OIH) and mercaptoacetyltriglycine (MAG3) and that this agent has good imaging properties and higher renal clearance than MAG3. In this study we used (99m)Tc-EC to compare different washout parameters in renal transplanted patients. Sixty nine scans in 55 patients (38 males, 17 females) were performed with (99m)Tc-EC during the follow-up period after kidney transplantation. After bolus injection of 280MBq (99m)Tc-EC, 60x1sec and 29x1min images were taken in anterior position. Perfusions of transplanted kidneys were examined visually and perfusion indices (PI) were calculated according to Hilson's method. The semiquantitative washout parameters such as 20 and 30min to peak activity ratios and 20 and 30min to 3min activity ratios were calculated. The patients were clinically evaluated by nephrologists experienced in renal transplantation cases and followed-up by serum creatinine and blood urea nitrogen determinants which were checked every postoperative day until stabilized to a optimal level for each patient. Two standard deviations above the mean values derived from all cases with normal functioning transplanted kidneys were calculated for each method. Then, these values served as the threshold to differentiate the pathological cases respectively. Of the 69 total studies performed, we found 34 normal kidneys, 14 rejections, 19 acute tubular necrosis (ATN) and 2 cyclosporin A toxicity cases. The number of abnormal cases detected with 30/3min, 30/max, 20/3min and 20/max indices were 27, 26, 24, and 18, respectively. In conclusion, we strongly recommend in studying renal transplants to consider the activity at the 3(rd) min post injection as a reference point instead of the time to maximum activity for washout index calculation. If, for any reason the time to maximum counts using (99m)Tc-(peak activity) is prefered, then the period of study should not be performed for less than 30min to achieve reliable results.

The aims of this investigation were to evaluate the temporomandibular joints (TMJs) with single-photon emission computerized tomography (SPECT) in subjects treated with a mandibular advancement repositioning splint (MARS), and to compare the results with the total effect on dento-facial morphology. The study was undertaken on 17 Class II division 1 malocclusion subjects (nine males, eight females) with mandibular retrusion. Ten patients (five males, five females) formed the treatment group and seven (four males, three females) were used as the control. SPECT was performed only in the treatment group. Cephalometric evaluation showed significant increases in NAPog (P < 0.001) and SNB (P < 0.05) angles. Increased bone formation in theTMJs was analysed with the aid of pre- and post-treatment scintigraphic studies. The results indicate that new bone formation in the mandibular condyles seems to contribute to the increase in mandibular prognathism resulting from functional jaw orthopaedics.

Unlabelled: Both (201)TI and (99m)Tc-methoxyisobutyl isonitrile (MIBI) have been used in the visualization of suppressed thyroid tissue in patients with autonomously functioning thyroid nodules (AFTNs). It has been suggested that thyroid-stimulating hormone (TSH) control is not a major determinant of both tracers. However, the mechanism of thyroid uptake of these agents is controversial. In this study, we compared (201)TI and MIBI in the visualization of suppressed thyroid tissue in patients with a solitary toxic AFTN., Methods: Thirty-two patients (13 triiodothyronine [T3] and 19 T3 + levorotatory thyroxine [T4] hyperthyroid patients) with toxic AFTNs visualized on (99m)Tc-pertechnetate scanning were included in the study. All patients underwent MIBI and (201)TI thyroid scintigraphy within a 3-d interval. The scintigrams were analyzed both visually and semiquantitatively. For the semiquantitative analysis, regions of interest (ROIs) were generated over the nodule (N) and contralateral normal lobe (E), and the mean counts in each ROI were calculated., Results: The N/E uptakes (mean +/- SD) for pertechnetate, MIBI, and (201)TI were 11.37 +/- 4.53, 4.76 +/- 1.38, and 1.63 +/- 0.15, respectively, in T3 + T4 hyperthyroid patients and 9.46 +/- 3.64, 2.73 +/- 0.63, and 1.57 +/- 0.23, respectively, in T3 hyperthyroid patients. Our results showed that (201)TI uptake of suppressed thyroid tissue compared with AFTN was more prominent and significantly higher than that of MIBI for both groups of patients (P = 1.08E-05 for T3 and 6.15E-09 for T3 + T4 hyperthyroidism). There was no significant difference for either pertechnetate or (201)TI (P > 0.05) when the N/E uptakes of both groups of patients were compared. However, the N/E uptake of MIBI in T3 + T4 hyperthyroid patients was significantly higher than that in T3 hyperthyroid patients (P = 6.69E-06)., Conclusion: Clear visualization of suppressed thyroid tissue with both (201)TI and MIBI in patients with low serum concentrations of TSH suggests that TSH is not a major factor in the thyroid uptake of either agent. (201)TI is superior to MIBI in the visualization of suppressed thyroid tissue in patients with a toxic thyroid nodule. An increased rate of metabolism in the follicular cells of AFTNs in T3 + T4 hyperthyroid patients compared with that in T3 hyperthyroid patients might be responsible for the higher N/E for MIBI compared with that for (201)TI.

Various radionuclides, including 67Ga, 201Tl and 99Tcm-sestamibi, have been used to differentiate benign from malignant thyroid nodules. 99Tcm-tetrofosmin, a lipophilic cationic radiotracer, and 99Tcm-sestamibi have also been reported to accumulate in thyroid tumours. In this study, we evaluated the role of 99Tcm-tetrofosmin in the differentiation of malignant from benign thyroid nodules. Seventy-nine patients with solitary non-functioning thyroid nodules were included in the study. Fine-needle aspiration biopsy was performed in all patients. Sixty patients were subsequently operated on and 19 patients refused surgery. After the injection of 370 MBq 99Tcm-tetrofosmin, static images at 5, 30, 60, 120 and 180 min were acquired. Both visual and semi-quantitative analysis was performed. On visual interpretation, the nodules with late retention were classified as positive for malignancy and nodules without late retention were classified as negative for malignancy. In the semi-quantitative analysis, regions of interests were drawn over the nodule and contralateral normal thyroid tissue. The average number of counts was recorded and tumour-to-normal thyroid tissue ratios calculated. Post-operative histology revealed 19 malignant and 41 benign nodules. Of the benign nodules, adenomas behaved similarly to the malignant nodules with late retention of tracer, while adenomatous nodules revealed no late retention on delayed images and could be differentiated from malignant tumours. In the semi-quantitative analysis, there was a significant difference in tumour-to-normal tissue ratios for adenomatous nodules and malignant tumours as well as adenomas. We conclude that it is not possible to differentiate between malignant and benign thyroid nodules with 99Tcm-tetrofosmin. However, 99Tcm-tetrofosmin scintigraphy is helpful in selecting nodules that can be cured by surgical intervention.

Unlabelled: 99mTc-L,L-ethylene, L, dicysteine (EC) clearance shows strong correlation with orthoiodohippurate clearance, and it is possible to estimate effective renal plasma flow from 99mTc-EC clearance. In routine clinical studies, it is practical to use the one or two plasma sample method instead of multiple plasma samples for clearance determination. A single-sample technique was developed for 99mTc-EC, and a regression formula was generated. A prospective study tested the validity of this regression formula., Methods: The study population was composed of 26 patients with a wide range of renal function. Multiple plasma sample 99mTc-EC clearances were calculated from all patients using the open two-compartment model. Single plasma sample clearances were also determined from the 54-min plasma sample using the regression formula published previously., Results: The multiple-sample plasma clearance of 99mTc-EC ranged from 46 to 668 mL/min with a mean of 300.76 +/- 150.73 mL/min. The clearances obtained from the 54-min plasma sample ranged from 49 to 699 mL/min, with a mean of 297.39 +/- 152.23 mL/min. There was an excellent correlation between the clearances obtained by the two techniques (r = 0.99, slope = 0.9911). The standard error of estimation was found to be 25.9 mL/min., Conclusion: This study suggests that 99mTc-EC clearance can be estimated from 54-min plasma samples with an acceptable error of estimation for most routine clinical studies.

Background: In cases of thermal injuries it is always difficult to predict the extent of necrosis to the peripheral ischemic zone. Practically, full-thickness skin burn also affects the underlying muscle, panniculus carnosus, which adheres tightly to the skin. In this proposed model, the muscle which was always partly damaged also covers the ischemic zone of full-thickness burn injury. To evaluate the deeper levels of injury, the status of the micro circulation of thermally affected muscle was evaluated by counting the accumulated radioactive agent, 99mTc methoxyisobutylisonitril (MIBI) in the muscle cells., Methods: 370 MBq/kg (10 mCi/kg) MIBI was administered intravenously to the animals having burn injuries by a comb device as described in previous literature. Then, 20 minutes after injection, whole burned areas were excised and placed under a gamma camera. Each thermally injured area showed four rectangular defects with lower tracer uptakes than the three adjacent interspaces. The tracer uptake of the burned sites and interspaces was analyzed from the acquired images on a computer, and the degree of injury could be assessed quantitatively. Additionally, specimen counts were obtained from selected burned stripes, interspaces and normal tissue for comparison., Results: Burned sites were evident with lower tracer uptakes whereas interspaces with higher uptakes. Captured activity in interspaces indicated that capillary patency mostly maintained and it permitted the arrival of the radiopharmaceutical to the muscle cells., Conclusions: Muscle layer, where just below the burn area and tightly attached to the skin, could be assessed as a representative of the extension of the injury.

Technetium-99m diaminocyclohexane (DACH) is a new tubular agent excreted via a cationic transport mechanism, like cyclosporine-A (CsA). It is expected that 99mTc-DACH will permit effective assessment of tubular function in CsA-treated patients. To establish the pharmacokinetic characteristics of 99mTc-DACH and to ascertain whether this new agent is useful in CsA-treated patients, 11 healthy volunteers and 15 CsA-treated patients underwent renal imaging and clearance studies using 99mTc-DACH and chromium-51 ethylene diamine tetra-acetic acid (EDTA). 99mTc-DACH yielded satisfactory dynamic renal images in all participants. The mean plasma clearance of 99mTc-DACH was significantly greater than that of 51Cr-EDTA in volunteers (109.4¿19.7 ml/min versus 86.6 +/- 13.7 ml/min, P<0.05). However, the urinary excretion of 99mTc-DACH at 90 min was significantly lower than that of 51Cr-EDTA (46.1% +/- 9.3% versus 53.1% +/- 8.6%, P<0.05), most probably due to its partial parenchymal retention. The elimination half-life of 99mTc-DACH was significantly increased in CsA-treated patients in comparison to volunteers, and consequently the plasma clearance values were significantly suppressed in these patients, in contrast to 51Cr-EDTA and endogenous creatinine clearance values. In conclusion, our findings indicate that 99mTc-DACH, as a sensitive marker of cationic tubular function, could be used to monitor renal haemodynamics in patients receiving CsA treatment.

This experiment was designed to study the reactions in the surrounding area of burn injury that may lead to further necrosis in 24 h. To prevent extension of burn size into the tissue adjacent to burn injury, it was attempted to reduce progressive microvascular damage by different drugs (ibuprofen, allopurinol or cyclosporin A (CyA)) in a rat model. The burn model consisted of a row of four 10 x 20 mm burn areas separated by three unburned 5 x 20 mm skin bridges (interspaces). To evaluate microcirculation and perfusion of panniculus carnosus muscle which is beneath the burned area of skin, the radioactive agent, technetium-99m methoxyisobutylisonitrile (99Tc01-MIBI) was used 24 h after the burn. Capillary permeability of injured tissue was assessed by the wet and dry weight technique. In all study groups, interspaces showed higher uptakes of 99Tc01-MIBI between 40 and 95 per cent, in comparison with burn sites in the first 24 h following burn. Among the treated rats better results were obtained by allopurinol and CyA treatment that commenced before burn than ibuprofen. Wet and dry ratios were found to be significantly lower in interspaces in rats pretreated with allopurinol and CyA. Results of this experiment showed that neutrophils and free radical-mediated injury may be involved in the pathogenesis of local response to thermal injury, and allopurinol and CyA have some effects to prevent progressive ischemia, capillary compromise and oedema.

Unlabelled: We investigated the usefulness of 99mTc-methoxyisobutylisonitrile scintigraphy in patients with known or suspected pulmonary tuberculosis (PTB) in comparison with radiological and bacteriological findings., Methods: Thirty-six patients aged 13-59 yr were scanned 15 and 60 min after intravenous injection of 370 MBq (10 mCi) 99mTc-methoxyisobutylisonitrile. Twenty-four patients had active PTB proven by chest radiograph and sputum examinations, two had miliary tuberculosis and ten were suspected of having relapsed PTB with negative sputum examinations and indeterminate chest radiographs. In 12 patients 99mTc-MIBI imaging was repeated 1-3 mo after chemotherapy., Results: Of 24 patients with active localized PTB, 22 (92%) showed increased focal uptake of 99mTc-MIBI, but two patients with minimal infiltration on chest radiographs had no accumulation of 99mTc-MIBI. Both patients with miliary PTB showed diffuse 99mTc-MIBI uptake in the lungs. Among 10 patients with suspicion of relapse, 99mTc-MIBI scans were true-positive in 4 of 5 patients (80%) with culture-proven tuberculosis and false-positive in 2 of 5 (40%) patients with negative sputum cultures. For repeat imaging, 6 of 10 patients with active localized PTB showed reduced MIBI uptake, which correlated with chest radiograph findings, and one patient had increased MIBI uptake again concordant with clinical and radiological findings which were suggestive of resistance to first line chemotherapy of tuberculosis. The other three patients showed no significant scintigraphic changes despite clinical and partial radiological regression., Conclusion: Active PTB granulomas generally present considerable 99mTc-MIBI uptake that is most probably related to disease activity. Therefore, 99mTc-MIBI scanning could be used in the detection and follow-up of active PTB as a complement to routine techniques.

A 43-yr-old man was referred for any possible parathyroid abnormality that could explain his hypercalcemia and slightly increased parathormone levels. The thallium-technetium scan showed a diffuse abnormal thallium uptake incidentally in the bone marrow, otherwise parathyroid scan appearance was normal. He had an essentially normal bone scan, although subsequent nanocolloid scintigraphy demonstrated bone marrow expansion. Further investigations, including a bone marrow aspiration biopsy, confirmed the diagnosis of nonsecretory myeloma. This finding suggests that 201Tl imaging can be a useful tool to investigate those patients suffering from similar myeloid disorders causing bone marrow hyperplasia.

Unlabelled: Technetium-99m-labeled N,N-ethylenedicysteine (99mTc-EC) is a new renal imaging agent introduced as an alternative for 99mTc-labeled mercaptoacetyltriglycine (99mTc-MAG3), with similar renal excretion characteristics. To evaluate the diagnostic characteristics of this agent, a gamma camera study was performed., Methods: Sixteen patients with obstructive renal disease and six normal controls were injected with 90 to 110 MBq of 99mTc-EC and 7.4 MBq 131I-labeled orthoiodohippurate (OIH). Serial images were obtained during 20 min for 99mTc-EC and 30 min for OIH. The study was repeated using 90 to 110 MBq 99mTc-MAG3 during the same week., Results: Renograms, functional ratios and urinary excretion patterns of all three agents were similar. The mean time to peak activity values for OIH, 99mTc-EC and 99mTc-MAG3 were 4.25 +/- 0.37 min, 4.39 +/- 0.32 min and 4.00 +/- 0.24 min, respectively. The time from peak to 50% activity values for OIH, 99mTc-EC and 99mTc-MAG3 were 5.48 +/- 0.80 min, 6.93 +/- 0.69 min and 7.33 +/- 0.85 min, respectively., Conclusion: It is concluded that 99mTc-EC has excellent imaging characteristics and similar excretion properties to OIH. The advantages of 99mTc-EC over 99mTc-MAG3 are lower hepatobiliary uptake and simplicity of preparation.

A variety of techniques have been employed to assess renal function and its evolution after transplant surgery. Since initial perfusion studies are easy to perform, one may be tempted to rely on 99mTc-diethylenetriaminepentaacetic acid (DTPA) scanning alone. Some discrepancies have been observed between perfusion studies and clinical outcome of the renal transplant patients however. We therefore decided to evaluate both 99mTc-DTPA and 131I-hippuran renal scanning in 28 renal transplant patients. Sixty 99mTc-DTPA perfusion and 131I hippuran scintigraphies were performed using a same-day protocol. The examinations were repeated in 14 patients at weekly or monthly intervals to monitor the clinical trend of these patients. The results of this study indicate that, for early detection of the kidney problems, 131I-hippuran scintigraphy was superior to 99mTc-DTPA perfusion scans, but for the proper clinical management of renal transplant patients, both procedures should be utilized.

In this preliminary study, Tc-99m MIBI, a muscle perfusion agent, was used to assess the viability of tissues in two patients with extensive high-tension electrical burns. This proved to be an easy and definitive diagnostic procedure and a practical solution to determining the level of amputation before surgery.

There have been several methods utilized in the diagnosis or assessment of medical or surgical treatments of peripheral arterial diseases. In this study, the diagnostic value of a new tracer: 99mTc-methoxy-isobutyl-isonitrile (MIBI) has been demonstrated in patients with leg claudication. We successively performed muscle perfusion scans in 6 normals and 18 patients with claudication pain during rest and after exercise on the same day. Muscle perfusion abnormalities in all patients were consistent with the defects in the arteries of the affected limbs. The results show that this is a simple and accurate diagnostic procedure and superior to those that have been previously used.

A system has been developed for the registration and combined display of the X-ray image and isotope bone scan. This system has been evaluated by prospectively studying 23 patients who were referred for suspected injury of the wrist. The registration system has an inherent precision for registration of about 1 mm. When patient positioning errors are included, this increases to about 4 mm. Two observers evaluated the sets of images prior to registration and after registration and combined display. They judged that in 16 out of 17 cases judged abnormal (observer 1) and 12 out of 18 cases judged abnormal (observer 2), the registration technique improved localization of a lesion. One case was rejected as registration was not possible due to incorrect positioning of the hand.
