The tourist sector, despite the present severe constraints imposed by the sanitary emergence, can be considered as an important component of almost all countries' economies. In Italy agriturism, in particular, has been experiences a continuously rising trend in recent years. Clearly, such great interest towards these businesses also calls for a deep attention by an energy point of view, in sight of an energy efficiency improvement, hence a lowering of the pressure that such facilities exert to the natural environment. On the other hand, the European Union has been engaged, for a long time, in awarding the tourist accommodations environmental excellence brands, like the EU Ecolabel. Unfortunately, the achievement of such excellence brands requires the capability of managing complex energy and environmental data, which is often not the prerogative of people running such facilities. In order of contributing to overcome this difficulty, and with the aim of helping the addressing of agritourism towards a Nearly Zero Energy path, we propose here a simple approach that does not require the modelling and simulation of the energy behaviour of an agritourism, being essentially based on the application of easy-to-use ARERA (Italian Regulatory Authority for Energy Networks and Environment) datasheets scheme. On purpose, an application of the method, involving two typical Sicilian agritourism, is presented. The obtained results showed the viability of the proposed methodology, although the need of an update and/or replacement of some technical datasheets arose.

Climate change and energy consumption increasingly call for a better use of energy sources. Renewable sources are still growing in the power production of many countries, so leading to the reduction of CO2 emissions. Further restrictions will be applied to the use of fossil fuels with a lot of consequences; for example, some German cities are all contemplating bans related to diesel pollution, and on the other hand some of the biggest car producers are planning to stop the diesel engines production in a few years. At the same time, some restrictions are applied also to many refrigerating fluids, in order of limiting the ODP and GWP values. This restriction leads to research new fluids and/or to enhance the use of natural refrigerants. In this scenario, Energy Designers are called to conjugate energy needs with the environment safety. Main guidelines concern energy clean production basically obtained by means of renewables and energy saving. In this article authors consider a CCHP for a typical Mediterranean end user such as a large hotel, composed by an ORC linked to a refrigerator. Several working fluids, particularly natural fluids, which operate both in the ORC and in the refrigerator, are investigated.

Background: Although diverticular disease is a common condition, its effective treatment is challenging in clinical practice. Objective: The objective of this article is to assess pharmacological management in different clinical settings of diverticular disease and factors associated with treatment using the Italian registry Registro Malattia Diverticolare (REMAD). Methods: At study enrolment, patients were categorised into subgroups: diverticulosis, symptomatic uncomplicated diverticular disease and previous diverticulitis. We registered demographic, clinical and lifestyle factors, quality of life and the use of treatments for diverticular disease in the last year. Logistic regression analysis assessed the association between clinical factors and treatment consumption. Results: A total of 500 of the 1206 individuals included had had at least one treatment for diverticular disease in the last year: 23.6% (166/702) of patients with diverticulosis, 55.9% (165/295) of patients with symptomatic diverticular disease, and 80.9% (169/209) of patients with previous diverticulitis (p < 0.001). In multivariate analysis, the following factors were significantly associated with treatment use: female gender, family history of colonic diverticula, organic digestive comorbidity and impaired physical quality of life components. Conclusion: Individuals with diverticular disease take medications based on the different clinical settings of disease. We identified different features associated with treatment use in the distinct clinical entities of diverticular disease. ClinicalTrial.gov Identifier: NCT03325829.

Background Indoor allergens are risk factors for asthma: Thus, the characterization of indoor air quality is important for studying environment–health relationships in children. In particular, Dermatophagoides pteronyssinus is the dominant allergen for asthma. We cross-sectionally investigated the relationships among respiratory symptoms and function, airway inflammation, allergen sensitization, and indoor allergen concentration. Methods One hundred and thirty-two children aging 10-14 years and living in a Southern Mediterranean area were evaluated by parental questionnaires. Spirometry, exhaled nitric oxide (FeNO), skin prick tests, total, and specific serum IgE analyses were performed along with the evaluation of home dust samples for the content in Der p 1 allergen. Three clusters were created on the basis of the presence/absence of wheeze in the last 12 months (Wh12m) and Der p 1-specific IgE level. Results Cluster 1 (Wh12m+/high Der p 1 IgE) presented higher FeNO and poorer pulmonary function (lower FEV1 and FEF25%-75%), while its symptom score was not different from Cluster 2 (Wh12m+/low Der p 1 IgE). Cluster 3 (Wh12m−/low IgE) showed the lowest FeNO values and pulmonary function similar to Cluster 2. Within Cluster 1, both Der p 1-specific IgE and FeNO were positively correlated with dust Der p 1. Conclusions Similar asthma phenotypes may occur in children despite differences in their atopic state. In atopic children, sensitizing allergens in the indoor environment may increase airway inflammation worsening pulmonary function. Moreover, environmental exposures may contribute to the development of asthma-like symptoms also in the absence of atopic sensitization, thus contributing to asthma overdiagnosis.

The use of sheep wool as reinforcement of cement in order to produce mortar or plaster involves several advantages for environment and energy. Moreover, in several regions it is treated as a waste and, consequently, its employ is characterized by low cost. Aim of this paper is to evaluate the influence of wool fibres on thermal conductivity and mechanical properties of cement. The samples were prepared using wool fibres, obtained from a breed of Sicilian sheep, with three different length (i.e. 1 mm, 6 mm and 20 mm). Moreover, in order to evaluate the influence of fibre content, the samples were prepared by varying the fibres weight fraction up to maximum level. The thermal conductivity of samples was analyzed by using a heat flow meter, whereas the mechanical behavior was studied through compressive tests. In order to validate the thermal measurements, two theoretical models were applied. First data about the habitat use in a population of Natrix natrix from Sicily. In the present paper the preliminary results of an investigation carried out on a Sicilian population of the grass snake (Natrix natrix) are reported. The leading aim of this study is to compare habitat use among three different classes of individuals (males, females, immatures), using as input the density of observations in four habitat types. Field studies were conducted beside a reservoir (“Lago di Scanzano”), located about 40 km from Palermo (western Sicily). Our results indicate spatial and temporal differences among groups. Young individuals were observed more frequently between April and May along wet meadows dominated by knotgrass, while adults between August and October along the stone pitching of the dam (mainly females), and in a cork oak wood fragment (mainly males). The Kernel analysis shows a low spatial overlap among groups, especially between adults and young individuals. The preliminary results suggest that shelter and prey availability could affect separation among groups.

In the last years several computer programs for thermal simulation of buildings have been quickly spread; they allow to describe the thermal behavior of buildings and to verify their energy efficiency, in order to suggest eventual improvements as from the design stage. Most of these simulation tools generally need a complete input data set and, in particular, information referring to the climatic conditions of the site where buildings will be built-up. As it is well known, among climatic issues, particularly important for the thermal energy balance is the solar radiation. In this paper authors present a short reference year of the solar radiation, more precisely the Monthly Average Day (MAD) for a town of the Southern Italy (Palermo). Moreover, authors have compared the climatic data of the Monthly Average Day (both for global and diffuse solar radiations) obtained from hourly measures for seven years, with those obtained both from geo-astronomical parameters and from the monthly average of the daily global solar radiation, which is commonly adopted on purposes. The comparison does suggest a particular caution in the choice of the method for generating reduced data sets of the solar radiation for these Mediterranean sites.

Within the assessment of the sustainability of plans and actions concerning the built environment, the transportation sector plays an increasing role, due to its importance in the economic and social life of countries. As that, the analysis of the sustainability concerning the transportation sector is now often embodied into the so called Strategic Environmental Analyses (SEA), that should provide local administrators with easy criteria for ranking the environmental suitability of designing policies, and that would seem to encounter the needed features for a correct evaluation of the urban masterplan. The urban noise forecast is very useful for local administrations, which have presently to cope with the urgent problem of assessing the environmental sustainability of policies in urban contexts. Several methods and indicators have been introduced, in order of singling out the environmental impact of the transport sector but, nevertheless, local administrators are called to manage complex set of data. In this work a software based on neural networks is proposed with the aim to forecast the levels of noise pollution. The proposed method essentially allows to link some typical urban planning data with the traffic noise, and to assess the influence of the program choices regarding the noise levels. So, it is possible to forecast new scenarios of acoustic pollution in urban contexts, and to compare the obtained data with the limits of law. To overcome the problem due to the different types of parameters which influence the acoustic pollution phenomena, a neural network has been used.

Brickmaking process is a highly energy-intensive operation, where a considerable amount of energy is spent in the drying and firing processes of bricks. Moreover, most of the Sicilian brick industries utilise electricity, fuel oil and gas oil. On the other hand, the conservation of energy becomes important in the aim of limiting greenhouse gas emission into the atmosphere and reducing costs of materials. In this work, an experimental analysis of the energy utilised in a Sicilian brickwork for the realization of some typical bricks has been carried out. From this study, the embodied energy of the unity of product has been deduced. Finally, by means of an indirect method, the pollutants released in the atmosphere have been evaluated and the pressure exerted on the main environmental ambits has also been computed.

Following a previous work, concerning the energy audit of a nursery school belonging to the public building park of the town of Palermo, some further analyses have been conducted. These mainly refer to the environmental performances of the school that are assessed with regards to different interventions involving the envelope (the glazed part, particularly) and the surrounding spaces. The interventions are hierarchised by means of suitable indicators concerning energy, environment and economy issues. Finally a simple graph is proposed that visually summarises these indicators for each proposed action.

A levofloxacin-based triple therapy and a rifabutin-based regimen are advised as second-line and rescue therapies in the current Italian guidelines for H. pylori eradication. However, no data are available for the efficacy of these treatments in clinical practice.A total of 86 consecutive patients who failed a standard, first-line, triple therapy for H. pylori infection were treated with a 10-day triple therapy including omeprazole 20 mg, amoxycillin 1 g, and levofloxacin 250 mg or 500 mg, each given twice daily. Eradication failure patients received a 10-day rescue therapy with omeprazole 20 mg, amoxycillin 1 g, and rifabutin 150 mg, each given twice daily. A further therapeutic attempt was performed with a 14-day, high-dose dual therapy (esomeprazole 40 mg and amoxicillin 1 g, each thrice daily).Following the second-line therapy, H. pylori infection was cured in 76.4% (95% CI = 67.8-85.0) and 79.5% (95% CI = 70.8-88.2) at intention-to-treat (ITT) and per-protocol (PP) analysis, respectively. After the rescue therapy, bacterial eradication was achieved in 84.6% (95% CI = 65-100). Two patients with persistent infection were successfully cured with the high-dose dual therapy.The efficacy of levofloxacin-based second-line therapy seems to be decreasing, whilst rescue therapy with rifabutin would appear a valid third-line therapy, and a high-dose dual therapy may be used as a further rescue therapy.
