IntroductionIt is unknown whether alteplase is effective and safe in patients with mild acute ischemic stroke (AIS). Determining whether symptoms are “disabling” or not is a crucial factor in the management of these patients. This study aimed to investigate the efficacy and safety of alteplase in patients with mild, non-disabling AIS.MethodsWe included all consecutive patients admitted for AIS at our institution from January 2015 to May 2022 who presented a baseline NIHSS score of 0–5 and fit the criteria to receive intravenous thrombolysis. In order to select only subjects with non-disabling AIS, we excluded patients who scored more than 1 point in the following NIHSS single items: vision, language, neglect, and single limb. Patients who scored at least 1 point in the NIHSS consciousness item were excluded as well. This study is a retrospective analysis of a prospectively collected database.ResultsAfter the application of the exclusion criteria, we included 319 patients, stratified into patients receiving and not receiving alteplase based on non-disabling symptoms. The two groups were comparable regarding demographic and clinical data. Rates of a 3-month favorable outcome, defined as a 3-month mRS score of 0–1, were similar, being 82.3% and 86.1% in the treated and untreated patients, respectively. Hemorrhagic complications and mortality occurred infrequently and were not affected by alteplase treatment.DiscussionThis observational study suggests that the use of alteplase, although safe, is not associated with a better outcome in highly selected patients with non-disabling AIS.

Background Despite epidemiologic evidence for increased cardiovascular morbidity and mortality associated with both high dietary and serum phosphate in humans with normal renal function, no controlled phosphate intervention studies of systemic hemodynamics have been reported. Higher serum 25(OH) vitamin D levels are associated with better cardiovascular outcomes, but vitamin D increases intestinal phosphate absorption.Methods We conducted a prospective outpatient study with blinded assessment in 20 young adults with normal renal function randomized to high phosphate (regular diet plus 1 mmol/kg body wt per day of Na as neutral sodium phosphate) or low phosphate (regular diet plus lanthanum, 750 mg thrice/day, plus 0.7 mmol/kg body wt per day of Na as NaCl) for 11 weeks. After 6 weeks, all subjects received vitamin D3 (600,000 U) by intramuscular injection. Outcome parameters were 24-hour ambulatory systolic and diastolic BP (SBP and DBP), pulse rate (PR), biomarkers, and measures of endothelial and arterial function.Results Compared with the low-phosphate diet group, the high-phosphate diet group had a significant increase in mean±SEM fasting plasma phosphate concentration (0.23±0.11 mmol/L); 24-hour SBP and DBP (+4.1; 95% confidence interval [95% CI], 2.1 to 6.1; and +3.2; 95% CI, 1.2 to 5.2 mm Hg, respectively); mean 24-hour PR (+4.0; 95% CI, 2.0 to 6.0 beats/min); and urinary metanephrine and normetanephrine excretion (54; 95% CI, 50 to 70; and 122; 95% CI, 85 to 159 µg/24 hr, respectively). Vitamin D had no effect on any of these parameters. Neither high- nor low-phosphate diet nor vitamin D affected endothelial function or arterial elasticity.Conclusions Increased phosphate intake (controlled for sodium) significantly increases SBP, DBP, and PR in humans with normal renal function, in part, by increasing sympathoadrenergic activity.

The purpose of this study is to examine the effects on education in Italy of the unexpected shift from proximate to distance learning after the COVID-19 outbreak and consequent closure of schools across the country. The study is grounded on the premise that online learning can make education more accessible all over the world: in fact, an increasing number of universities and institutions are turning to blended and distance learning in order to increase participation. Moreover, distance education and technology may help to promote collaboration across many institutions and offer a wider choice of classes. On these premises, the following study intends to trace an overview of the current situation in Italy and outline limits and solutions to a new type of education which could lead to innovative and effective teaching practices. Specifically, the study will explore how teachers and schools have managed the unexpected circumstances, how quickly they have switched to online teaching and with what consequences and limits, and to what extent distance education’s success or failure correlates with the sudden COVID-19 emergency (meaning that there was insufficient time to reorganize the teaching and learning environment), or whether it simply depends on a lack of other objective requisites (teachers’ or students’ digital competences, online resources, and so forth). The project will contribute to discussions of the impact that this exceptional time may have in terms of eliciting new educational practices, and how it can stimulate self-reflection on future teaching strategies, as well as produce educational literature. As this is the first study on distance education during COVID-19, and because of the timing, no empirical or case study research has been done yet.

Until the first decade of the 21th century, scholars and reporters have identified contemporary populism as an element of anti-systemic revolt; furthermore, they have also recognized an incompatibility between populist phenomenon and government function. However, some recent cases of populist parties in power seem to be able to put into crisis more than one certainty regarding the nature and scope of the populist phenomenon. This observation raises the questions of this work: what harmful effects does populism in government produce on liberal institutions, pluralism, and representation in constitutional democracies? Do these effects merely erode the liberal component, or do they extend to produce a degeneration of democracy as a whole? And finally: what are the risks for democracy? The article corroborates the diarchic theories of democracy and aims to demonstrate the lack of compatibility between the principles of liberal democracy and populist principles, which have a negative impact not only on the liberal component, but also on the quality of democracy in its entirety.

Background: To confirm the suspicion of scabies, dermatologists have one pathognomonic sign, “the tunnel” through which Sarcoptes scabiei digs into the epidermis. Light microscopy is considered the most reliable procedure, but it is time-consuming and operator-dependent. Recently, dermoscopy has greatly improved the chances of recognizing mite in situ, but it is still linked to the examiner’s experience and to the magnification capability of the device used. Methods: This article, based on a case-series study, describes a novel diagnostic path, which uses an ultraviolet LED source at 365 nm and a digital camera for the evaluation of lesions that raise the suspicion of scabies. Results: The gallery emits a naked-eye-visible wavy bluish-white linear luminescence, better than that of any standard lighting. UVA light is also able to identify Sarcoptes scabiei as a white or green point-shaped area. This sign can only be appreciated by enlarging its picture to full frame on a common PC monitor. Conclusions: Ultraviolet light (365 nm) seems to offer help in the diagnosis of scabies because it saves time compared with light microscopy and because it does not require contact with the patient’s skin, as in dermoscopy. Although examiner experience remains an important factor, it is easily compensated by procedural simplicity, the cost of the devices and, especially, by the clarity of the results, even in non-specific lesions.

In current political analysis, as well as in discourse, the term populism has become an ‘umbrella term’, embracing a large number of concepts and phenomena. One risk underlying this conceptual stretching is that the term falls into the trap of ‘all-nothing’ and becomes so elastic that populism is used to improperly describe a wide and unrelated variety of phenomena. Some political phenomena might share some characteristics with populist movements but are nevertheless characterised by ideological elements and political projects that are very much in line with the traditional left/right split, as in the case of the new radical right. This article proposes a distinction between the conformational populism typical of some populist parties and the contagious populism typical of the radical right. We argue that in the latter, populism becomes another tool in the arsenal of parties with clearly established identities. Using a comparative approach, we evaluate the programmes, ideological foundations, conception of the people and positions of the 5 Star Movement, The League and the Rassemblement National to illustrate this distinction. Scabies has always represented a diagnostic challenge for dermatologists, especially in subclinical cases or in atypical ones due to the coexistence of other diseases. Fortunately, dermatoscopy has enabled easier and faster in situ diagnosis. The aim of this study is to examine old and new dermatoscopic signs that Sarcoptes scabiei produces on the skin during its whole life cycle through entodermoscopy (dermatoscopy with an entomological focus) which, unlike traditional optical microscope examination, allows the local micro-environment to be preserved intact. Patients were enrolled during outbreaks of scabies from hospitals or nursing homes for the elderly in Bari (Italy). The study was performed applying both immersion and polarized dry dermatoscopy. The systematic use of dermatoscopy highlighted the morphological complexity of the Sarcoptes tunnel that had been described previously as a simple unitary structure. On the contrary, it is possible to distinguish three separate segments of the burrow that introduce a new anatomo-functional concept called the Mite-Gallery Unit (MGU). This approach, based on the mite life cycle and local skin turnover (the latter usually being ignored), allows the dermatologist to recognize not only Sarcoptes using the gallery, but also new descriptors including tunnels without Sarcoptes, those with acari alone, and those with associated signs of inflammation. The diagnosis of scabies using optical microscopy until recently has always involved demonstrating the mite and its products outside the human body (on a glass slide) without taking into account exactly what happens within the epidermis. Entodermoscopy is a term used to encapsulate both the presence of the parasite, the usual target of microscopy, and the changes produced in the superficial layers of the epidermis in situ. Thus, the scabies tunnel or burrow can be shown to be composed of three parts, the Head, Body, and Tail, in which different events affecting both mite and host develop. The Mite-Gallery Unit provides a new anatomical and functional explanation of scabies because it provides a more comprehensive in vivo and in situ dermatoscopic diagnosis. In this respect, dermatoscopy takes into account the behavior of the mite in addition to its interaction with its habitat, the human skin.

Background The kidney is the main organ in the pathophysiology of essential hypertension. Although most bicarbonate reabsorption occurs in the proximal tubule, the medullary thick ascending limb (mTAL) of the nephron also maintains acid–base balance by contributing to 25% of bicarbonate reabsorption. A crucial element in this regulation is the sodium-hydrogen exchanger 1 (NHE1), a ubiquitous membrane protein controlling intracellular pH, where proton extrusion is driven by the inward sodium flux. MicroRNA (miRNA) expression of hypertensive patients significantly differs from that of normotensive subjects. The aim of this study was to determine the functional role of miRNA alterations at the mTAL level. Methods By miRNA microarray analysis, we identified miRNA expression profiles in isolated mTALs from high sodium intake–induced hypertensive rats (HSD) versus their normotensive counterparts (NSD). In vitro validation was carried out in rat mTAL cells. Results Five miRNAs involved in the onset of salt-sensitive hypertension were identified, including miR-23a, which was bioinformatically predicted to target NHE1 mRNA. Data demonstrated that miRNA-23a is downregulated in the mTAL of HSD rats while NHE1 is upregulated. Consistently, transfection of an miRNA-23a mimic in an mTAL cell line, using a viral vector, resulted in NHE1 downregulation. Conclusion NHE1, a protein involved in sodium reabsorption at the mTAL level and blood pressure regulation, is upregulated in our model. This was due to a downregulation of miRNA-23a. Expression levels of this miRNA are influenced by high sodium intake in the mTALs of rats. The downregulation of miRNA-23a in humans affected by essential hypertension corroborate our data and point to the potential role of miRNA-23a in the regulation of mTAL function following high salt intake. Although uncommonly diagnosed, clear cell acanthoma represents an original source of speculative interest for dermatologists. Due to its clinical variability, it is often only recognized accidentally after histology. Dermoscopy has improved the reliability of clinical diagnosis of typical clear cell acanthoma thanks to the vascular pinpoint pattern and desquamative, peripheral collarette. Generally, therapy of clear cell acanthoma is oriented towards ablative solutions, such as surgery or cryotherapy. We propose a conservative therapy, based on the application of topical calcipotriol, which has produced complete regression after 2 months and no relapse one year after the end of treatment. A dermatoscope monitored all changes of clear cell acanthoma, showing its utility not only in diagnosis but also in therapeutic follow-up. This new therapeutic approach should support an inflammatory etiology of clear cell acanthoma, although further observations are needed to confirm this.
