Introduction: Neuroleptic malignant syndrome (NMS) is a rare, potentially life‐threatening antipsychotic‐associated disorder that requires an efficient and timely therapy. The aim of the study was to compare the effectiveness of different NMS therapies and to analyze its outcome depending on NMS severity. Method: Systematic search for NMS cases in biomedical databases. The focus of the analysis was on therapy with dantrolene, bromocriptine, and electroconvulsive therapy (ECT) when each was compared with symptomatic therapy. Primary outcomes were the survival rate and the duration of treatment. Result: 405 case reports were included. Overall, no statistically significant differences regarding mortality rate or duration of treatment were found between dantrolene, bromocriptine, or ECT compared to supportive care. A subgroup analysis regarding NMS severity showed that the mortality under specific NMS pharmacotherapy (dantrolene, bromocriptine) and under ECT was significantly lower than under purely symptomatic therapy in severe NMS (P = 0.018). The difference was not significant in mild and moderate cases. Discussion: An overall superiority of the specific NMS therapy (dantrolene, bromocriptine, and ECT) was not found in this study. When regarding severity classification, specific therapies were superior but only in severe cases, and ECT showed the lowest mortality rate. In previous case series, an effect on survival or the duration of the disease could only be observed in part for specific therapies, but the evidence available is inconsistent. The results of this study support our hypothesis that NMS treatment with dantrolene, bromocriptine, and ECT is advantageous over purely symptomatic therapy in severe NMS cases. Objective: Repetitive transcranial magnetic stimulation (rTMS) is an approved treatment for depression. The clinical relevance of its efficacy is unclear. The clinical relevance of findings in the rTMS literature was assessed by translating Hamilton Depression Rating Scale (HAMD) data into Clinical Global Impression-Improvement scale (CGI-I) scores. Method: We performed electronic searches of MEDLINE, Embase, PsycINFO, PubMed and Cochrane Central Register of Controlled Trials for RCTs and non-RCT trials on rTMS using Hamilton Depression Rating Scale (HAMD). Articles were included if published in English before January 2014. We translated HAMD scores into nominal CGI-I scores for rTMS for depression and for treatment-resistant depression (TRD). Results: About 960 abstracts were retrieved. Sixty-three studies were included, yielding 130 study arms. For depression, the mean percentage change in HAMD scores in all sham-controlled rTMS treatment arms was 35.63 (SD 16.35) and for sham-rTMS 23.33 (SD 16.51). For TRD, active rTMS in sham-controlled studies showed a mean HAMD percentage reduction of 45.21 (SD 10.94) versus 25.04 (SD 17.55) for sham-rTMS. When aggregated scores were translated into notional CGI-I scores, for the treatment of depression, the notional CGI-I score difference between rTMS and sham-rTMS was 0.5 in favour of rTMS; for TRD, it was 0.75 in favour of rTMS. Differences between rTMS and sham-rTMS were bigger when all study arms were combined. Conclusion: Whilst rTMS appears to be efficacious for both non-refractory and treatment-resistant depression, the clinical relevance of its efficacy is doubtful. [ABSTRACT FROM AUTHOR]

Tranylcypromine (TCP) is an effective antidepressant with a complex pharmacological profile and a relevant risk of abuse and dependence. Withdrawal phenomena (WP, in the case of TCP-abuse/dependence) or discontinuation phenomena (DP, in the case of absent TCP-abuse/ dependence) subsequent to abrupt termination of TCP are a potentially severe clinical syndrome. We conducted a systematic review of all previously published WP/DP cases following abrupt termination of TCP in order to identify typical clinical presentations and risk factors of WP/ DP and frequency of TCP abuse or dependence within these patients. By searching the Medline and Scopus databases we identified n = 25 cases (cohort WP: n = 18, cohort DP: n = 7). Delirium was found in n = 13 patients (cohort WP: 10/55.6 %; cohort DP: 3/42.9 %), n = 6 demonstrated WP/DP without delirium (WP: 6/33.3 %; DP: 0/0 %) and n = 5 rapid relapse in depression (WP: 1/5.6 %; DP: 4/57.1 %). Mean time until development of WP/DP was 1.9 (WP) and 2.2 (DP) days. Mean duration of WP/DP was 5.7 (WP) and 11.3 (DP) days. All patients of cohort WP were described to feature TCP-abuse/dependence. Patients with delirium were on average older (41.8 years vs. 37.8 years) and featured higher mean prescribed (71.0 mg vs. 38.3 mg) and actually taken daily TCP dosages (285.8 mg vs. 187.7 mg). In conclusion, even termination of lower daily dosages of TCP may result in delirium. Thrombocytopenia features diagnostic value in patients with deliria of unknown etiology. Looking at novelties and advances in medicine in particular in the treatment of major depressive disorder no principally new antidepressant treatment strategy has been established in clinical routine in the last fifty years. However, regarding the considerable issue of treatment resistance in depression, new therapeutic strategies are urgently required. In this context, repetitive transcranial magnetic stimulation above the dorsolateral prefrontal cortex has been proposed as a potential new treatment option for depression; furthermore, in October 2008 a first rTMS-device (NeuroStar TMS Therapy System™) was approved by the FDA for the treatment of treatment resistant major refractory depression in adults. Yet, despite now nearly two decades of research in this field, no final answer concerning its validity for antidepressant treatment in the clinical practice is given. Numerous studies with small sample sizes and heterogeneous designs have been performed in this field yielding to different results. These were subjected to meta-analyses, assessing the antidepressant effect of rTMS, which are briefly summarized in this article. Further, multicentre-trials with larger numbers of patients were performed, which are presented and critically discussed here in more detail. This short review shall thus provide an overview of the current status of knowledge concerning rTMS in depression and it also provides some recommendations for future research in this field. Summary: Repetitive transcranial magnetic stimulation (rTMS) has been increasingly evaluated as a therapeutic tool for the treatment of depression, using various stimulation parameters and protocols. Heterogeneous results have been reported with regard to clinical outcome, at least partly due to the variety of procedures for coil placement above the desired site of stimulation. This article reviews the strategies for coil positioning in the treatment of depression. Considering preliminary clinical evidence, neuronavigated rTMS appears desirable to treat depression, compared to the standard targeting procedure (5cm anterior to the motor cortex). Coil positioning strategy might improve in the future by taking into consideration the individual abnormalities revealed by functional neuroimaging data. [Copyright &y& Elsevier]

Background: Depression, as major depressive disorder (MDD) or as bipolar depression, is frequent among pregnant women (Evans et al., 2001) and an efficient treatment is fundamental. If left untreated, there are specific risks such as preterm delivery, low birth weight, and potentially death due to acute suicidality. Treatment of depression during pregnancy supposes a great challenge for pregnant women, their partner and for physicians involved. Despite psychopharmacotherapy and psychotherapy, electroconvulsive therapy (ECT) and other brain stimulation techniques such as repetitive transcranial magnetic stimulation (rTMS) or vagus nerve stimulation (VNS) are current available treatment strategies to treat depressive symptoms. ECT is recommended by the American Psychiatric Association Task Force on ECT as a safe and effective treatment of depression throughout pregnancy. However, ECT during pregnancy often involves concerns by both the patient and the physician due to unsolved questions regarding safety and tolerability as well as efficacy. Method: In order to retrieve insights into the frequency of reports, safety and efficacy of ECT for the treatment of MDD and bipolar depression during pregnancy we performed a systematic review of the existing literature. All publications between 1942 and the 1th December 2012 in the Medline, Embase and Scopus databases, that deal with ECT in MDD and bipolar depression during pregnancy were considered. Results: N=87 cases of ECT during pregnancy (MDD n =78; bipolar depression n =9) published in n =41 articles were detected. Of these 30 were case reports and 11 case series (with reported cases between n =2 and n =16). Furthermore, we evaluated the number of reported cases between the first report in 1942 and 2012 and divided the results in 4 time periods (1942–1959 n =46, 1960–1977 n =0, 1978–1995 n =14, and 1996–2012 n =18) in order to compare reported frequency. The results of our preliminary analysis suggest that ECT is efficacious and well tolerated in mother and offspring in the majority of analysed cases. In terms of safety, the comparison of the 4 time periods revealed that in the last analyzed time period a lower complication rate was observed. Most reported adverse effects were transient confusion, memory loss, headache and very rare premature contractions (mother) and fetal bradyarrhythmias. Further parameters (e.g. outcome of follow-up investigations, applied, narcotics, gestation age at the time of ECT, mean number of ECT sessions, detailed efficacy parameters as measured by psychometric scales, changes of the frequencies of reported ECTs during pregnancy) will be presented at the congress. Conclusions: Considering the results of our systematic review, ECT seems feasible in pregnant patients with MDD or bipolar depression. Our data support ECT to be more safe in the last decades for mother and offspring; however, the registered reported frequency does not support an increasingly use of this method with regard to the mentioned indication. Introduction: Weight gain is a common side effect of antipsychotic and antidepressive drug therapy, difficult both to avoid and to treat. In several randomized controlled studies, add-on treatment with metformin has been demonstrated to reduce olanzapine-induced weight gain. In our department of psychiatry and psychotherapy, metformin has been used against antihistaminic-drug-induced weight gain since 2008. Objectives: Primary: To demonstrate the efficacy of metformin for prophylaxis of antihistaminic-drug-induced weight gain in a cohort of psychiatric in-patients under naturalistic conditions. Secondary: To identify side-effects of add-on metformin therapy in non-diabetic psychiatric in-patients. Methods: Retrospective case-control analysis of in-patient charts from 3400 psychiatric therapies between 2008 and 2010 at Ulm University Department of Psychiatry and Psychotherapy III. Cases were identified on the basis of metformin prescription without a diagnosis of diabetes. Controls were matched for gender, age, BMI, diagnosis, and drug treatment. Evaluation will be performed with descriptive statistics, covariance analyses and mixed models. Results: No serious side effects could be attributed to metformin therapy on the basis of the charts reviewed. Further results will be presented at the meeting. The metalloprotein ceruloplasmin has been of interest to psychiatrists for decades following early observations of elevated serum copper, and oxidase activity, in patients with schizophrenia. While immunological methods did not confirm elevated serum ceruloplasmin concentrations in schizophrenia, low serum concentrations of ceruloplasmin are typical of Wilson''s disease, Menkes’ disease, and aceruloplasminemia making low ceruloplasmin an important marker of disease. Unfortunately, available normative values are of limited usefulness, the majority being derived from oxidase studies; very few immunoturbidimetric or nephelometric studies were performed on small samples, or did not account for known influencing factors. Therefore, the present study was planned to determine nephelometric ceruloplasmin serum concentrations in a large randomized population sample. 2,445 subjects between 12 and 65 years of age were randomly selected from a southern German town of 22,000 inhabitants. The mean ceruloplasmin serum concentration was 331±104mg/l. Ceruloplasmin serum concentration significantly correlated with gender and age and was higher by 23 mg/l on average in women than in men. There also were significant correlations with lipoprotein concentrations, but not with several other liver-associated features, parameters of metabolism, or markers of inflammation. Different experimental methodologies have been used to investigate pain perception in patients with borderline personality disorder (BPD) overall showing elevated pain thresholds (PT). We assessed PT, sensorial and affective components of pain processing in BPD patients and healthy controls using repetitive peripheral magnetic stimulation (rPMS) as a novel tool for provoking controlled aversive cutaneous sensation. In 10 BPD patients and 8 healthy women we assessed PT, cutaneous sensation, emotional valence and arousal level during rPMS at different intensities on two consecutive days. Additionally, inner tension level was assessed before and after each session. We found significantly higher PT in BPD patients (91% of maximal output of stimulator, vs. 56% in controls); these measures were consistent among both days of assessment, showing a high intra-individual repeat-reliability. In BPD, PT correlated positively with motivational factors of non-suicidal self-injury (to avoid feeling of emptiness r=0.823, p=0.023; to punish oneself r=0.774, p=0.041). All stimulation intensities used were discriminated similarly in both groups. However, emotional valence and arousal level did not vary with stimulus intensity in BPD patients. Furthermore, BPD patients were found to have higher baseline levels of inner tension than controls and, as opposed to controls, they experienced subjective relief after stimulation. Besides demonstrating a distinctive pattern of affective components of pain in BPD, the present study demonstrates that rPMS is a suitable and well-tolerated method for the assessment of pain sensation. Valproic acid (VPA, 2-propylvaleric acid) is originally an antiepileptic drug, which has been in use for more than 30 years in over 100 countries. The clinical application of VPA has expanded in the last years. Approval has been granted by the FDA for treatment of migraine and cluster headache in 1996, and for treatment of mania and long-term prophylaxis of bipolar affective disorder in 1995. In ongoing studies, VPA has been reported to inhibit growth of several types of cancer cells; in addition, effects on neurodegeneration, and on virus replication in HIV infection have been demonstrated potentially expanding the application of VPA in the future. Despite a good tolerability of the drug, reports of hepatotoxicty even in patients without risk factors become more frequent. We analysed all cases of VPA induced severe hepatic side effects reported to the german Federal Institute for Pharmaceuticals & Medical Products (BfArM) between 1993 and 2009. A special intention was to detect correlations with present co-medication as a crucial factor in the break-down of hepatic function. As frequent co-medications in VPA-induced hepatic side effects benzodiazepines, and antiepileptics, especially carbamazepine, lamotrigine and topiramate were found. In addition, propofol as a co-medication was found in 4 lethal cases. Different pathomechanisms of VPA hepatotoxicity and a therapeutic approach with carnitine are discussed. Current international guidelines for prevention of VPA-induced liver failure are contrasted. Weekly control of liver enzymes in the first treatment weeks might help to detect VPA-induced hepatic side effects earlier. [ABSTRACT FROM AUTHOR]

The article presents a case study of a 23-year-old male with bulimia nervosa and major depressive disorder who developed hypogonadism and gynecomastia under duloxetine treatment. The patient complained of swelling and tenderness in the breast after four months of duloxetine medication and bilateral mastectomy. The swellings and tenderness of the breast have subsided after discontinuation of the duloxetine medication.

Acamprosate and naltrexone are established strategies for pharmacologic relapse prevention in patients with alcohol dependence. Regarding pharmacodynamic and pharmacokinetic considerations the combination of both agents for this indication is a reasonable treatment option that has been described to be safe and effective in clinical studies. However, this combination is uncommon in clinical practice. We report the case of a patient with severe alcohol and benzodiazepine dependence who achieved the longest interval of abstinence under combined treatment with acamprosate and naltrexone since the development of addiction. In addition, the currently available evidence regarding efficacy, safety and tolerability of both agents is discussed. In summary the combined treatment with both agents should be considered in patients who did not achieve abstinence under monotherapy unless contraindications are present. [ABSTRACT FROM AUTHOR]

Cognitive deficits are among the core symptoms of patients with Huntington's disease (HD). While impaired attention, visuospatial processing, and memory can be observed during early stages of the illness, HD patients exhibit deficits in executive function on tests requiring planning, problem solving, and cognitive flexibility with progression of the disease. Cognitive dysfunction is already present in individuals who carry the HD gene mutation but remain presymptomatic for motor and cognitive disturbances. This review provides an overview and a discussion of functional neuroimaging findings on cognitive dysfunction in patients with HD and presymptomatic HD gene mutation carriers. In HD patients, currently available evidence suggests a functional deficit of multiple cortical and subcortical regions extending beyond volumetric abnormalities. Early dysfunction of lateral prefrontal and cingulate regions has been shown in individuals with presymptomatic HD, while compensatory responses of posterior brain regions may occur closer to the onset of manifest clinical symptoms. While functional neuroimaging techniques may substantially contribute to defining neurodegenerative disease phenotypes and to identifying neural biomarkers in presymptomatic individuals, the extant data on cognitive function in HD patients and HD gene carriers however is sparse and has to be expanded through further studies. Phonological rehearsal helps to keep selected information consciously in mind for further processing. This part of short-term storage takes place during the delay period of verbal working memory tasks and involves a frontoparietal network as functional magnetic resonance imaging (fMRI) studies have shown. The involved cortical areas can be further investigated by interfering with the local information processing using transcranial magnetic stimulation (TMS). On a single subject level, we identified predominantly left-sided premotor, prefrontal, and parietal areas active during the delay period of a verbal working memory task using event-related fMRI. In a pilot approach, TMS was neuronavigated to the individually active areas by using a stereotaxic device. Then, TMS was applied during the delay period of similar tasks as in fMRI. Error rates increased significantly upon stimulating left premotor cortex, but not upon parietal or prefrontal stimulation. The contribution of the premotor cortex to storage and rehearsal is discussed as an active top-down storage process within the frontoparietal network. [Copyright &y& Elsevier]

Background: Many patients with Alzheimer's disease (AD) are physically frail or have substantial functional impairments. There is growing evidence that such patients are at higher risk for medication-induced adverse events. Furthermore, frailty seems to be more predictive of poor clinical outcomes than chronological age alone. To our knowledge, no systematic review of clinical trials examining drug therapy of AD or behavioural and psychological symptoms of dementia (BPSD) has specifically focused on the topic of physical frailty. Our objective was to evaluate the efficacy and safety of pharmacotherapy in AD patients with frailty or significant functional impairments. Methods: We performed a systematic literature search in MEDLINE, Embase and the Cochrane Central Register of Controlled Trials (CENTRAL) for randomized controlled trials (RCTs) of drug therapy of AD and BPSD in patients with significant functional impairments according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement and Cochrane research criteria. Significant functionally impaired patient populations were identified using the recommendations of the Medication and Quality of Life in frail older persons (MedQoL) Research Group. Screening, selection of studies, data extraction and risk of bias assessment were performed independently by two reviewers. Outcomes including functional status, cognitive function, changes in BPSD symptoms, clinical global impression and quality of life were analysed. For assessing harm, we assessed adverse events, drop-outs as a proxy for treatment tolerability and death. Results were analysed according to Cochrane standards and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Results: Of 45,045 search results, 38,447 abstracts and 187 full texts were screened, and finally, 10 RCTs were included in the systematic review. Selected articles evaluated pharmacotherapy with acetylcholinesterase-inhibitors (AChEI), anticonvulsants, antidepressants and antipsychotics. Studies of AChEIs suggested that patients with significant functional impairments had slight but significant improvements in cognition and that AChEIs were generally well tolerated. Studies of antidepressants did not show significant improvements in depressive symptoms. Antipsychotics and anticonvulsants showed small effects on some BPSD items but also higher rates of adverse events. However, due to the very small number of identified trials, the quality of evidence for all outcomes was low to very low. Overall, the small number of eligible studies demonstrates that significantly functional impaired older patients have not been adequately taken into consideration in most clinical trials investigating drug therapy of AD and BPSD. Conclusion: Due to lack of evidence, it is not possible to give specific recommendations for drug therapy of AD and BSPD in frail older patients or older patients with significant functional impairments. We report our experience with a 34-year-old patient with schizophrenia, paranoid subtype, who demonstrated an elevation of clozapine serum levels subsequent to pregabalin comedication used for the treatment of schizophrenic anxiety. This observation turned to be dose-dependent. Although the pharmacokinetic profile of pregabalin suggests an exclusive renal elimination, our report supports the presumption of a possible direct or indirect hepatic "effect" of pregabalin. Introduction: Drug safety surveillance strongly depends on the spontaneous reporting of adverse drug reactions (ADRs). A major limiting factor of spontaneous reporting systems is underreporting (UR) which describes incorrectly low reporting rates of ADRs. Factors contributing to UR are numerous and feature country-dependent differences. Understanding causes of and factors associated with UR is necessary to facilitate targeted interventions to improve ADR reporting and pharmacovigilance. Methods: A cross-sectional questionnaire-based telephone survey was performed among physicians in outpatient care in a federal state of Germany. Results: From n = 316 eligible physicians n = 176 completed the questionnaire (response rate = 55.7%). Most of the physicians (n = 137/77.8%) stated that they report ADRs which they have observed to the competent authority rarely (n = 59/33.5%), very rarely (n = 59/33.5%) or never (n = 19/10.8%); the majority (n = 123/69.9%) had not reported any ADRs in 2014. Frequent subjective reasons for non-reporting of ADR were (specified response options): lack of time (n = 52/29.5%), the subjective evaluation that the required process of reporting is complicated (n = 47/26.7%) or requires too much time (n = 25/14.2%) or the assessment that reporting of an ADR is needless (n = 22/12.5%); within open answers the participants frequently stated that they do not report ADRs that are already known (n = 72/40.9%) and they only report severe ADRs (n = 46/26.1%). Discussion: Our results suggest a need to inform physicians about pharmacovigilance and to modify the required procedure of ADR reporting or to offer other reporting options. [ABSTRACT FROM AUTHOR]

Introduction: Considering the antidepressant agomelatine (AGM) there is a discrepancy between the widespread knowledge of the potential of AGM to cause hepatotoxic adverse drug reactions (ADR) and the availability of corresponding published data. This impedes an adequate assessment of the hepatotoxicity profile of AGM. Methods: We conducted a query of the database of a German Medical Regulatory Body (BfArM) and analyzed spontaneous reports of hepatotoxic ADR. Results: We identified n=58 cases of AGMrelated hepatotoxic ADR. Most frequent ADR was asymptomatic increase of liver enzymes (79%); n=6 patients (10%) with AGM-related toxic hepatitis were reported. Characteristics of patients: female sex (69%), age > 50 years (mean 54 years), polypharmacy (57%), and presence of cardiovascular risk factors (58.5%). Most of the hepatotoxic ADR (90%) were reported to have improved/ recovered after discontinuation of AGM. Discussion: Our evaluation suggests that AGM features a potential to cause severe forms of hepatotoxicity and emphasizes that a pre-existing liver disease is a contraindication for treatment with AGM. Introduction: Little is known about hepatotoxicity associated with valproic acid (VPA), a widely used substance in neuropsychiatry. Methods: All reported cases to the German Federal Institute for Drugs and Medical Devices between 1993 and 2009 of VPA-induced serious hepatic side effects were evaluated. Results: A total of 132 cases of serious VPAassociated liver failure were identifi ed. Approximately one third (34.8 %) occurred under VPA monotherapy, while the majority was seen with VPA plus co-medication, most frequently antiepileptics (34.8 %) and benzodiazepines (16.7 %). A subgroup of 34 cases (25.8 %) had a fatal outcome, the largest number reported to date. Of these,32.4 % were under VPA monotherapy and 67.6 % under VPA plus concomitant medication. Within the study period a signifi cant increase in the total number of reported cases and the subgroup of fatal cases was found. Discussion: This first pharmacovigilance study of VPA-associated liver failure indicates a higher rate of non-fatal and fatal liver failure when VPA is given with co-medication as compared to monotherapy. Introduction/objectives/aims: Gynecomastia and hypogonadism can occur during treatment with a range of antidopaminergic drugs and is most likely to a medication induced increase in serum prolactin levels. However there is little evidence for an association between antidepressants and hypogonadism. Methods: We report the case of a 23 year old male with a 3 year history of bulimia nervosa and recurring depression who had been treated with duloxetine for a period of 6 months during which he developed pronounced bilateral gynocomastia. Since serum prolactine and serum testosterone levels were normal and duloxetin was initially not considered causative, the drug was continued and bilateral gynecomastia was performed. 4 months later the patient again experienced tension and swelling in the breast. Results: The endocrinologic exams revealed reduced serum testosterone levels and reduced testicular volume. Body weight was 63kg, height 176cm and a neurological exam, including olfactory testing and MR brain imaging was normal. Hypogonadotropic hypogonadism was diagnosed. Discussion: Increased prolactin levels under antipsychotic medications are not uncommon. Some rare instances of gynecomastia have been reported under fluoxetine and venlafaxine, always associated with increased serum prolactin levels. In the present case the close tempral relationship between initiation of duloxetine and the development of gynecomastia and the recurrence under continued medication and resolution after termination of the medication suggests a possible causal relationship. Allogeneous hematogenic stem cell transplantation (HSCT) is a curative therapeutic strategy for numerous benign and malignant hematologic diseases. The transplantation procedure is characterized by significant physical, social and psychological stressors, especially for the graft recipient. This can lead to depression and anxiety. Every HSCT represents a potential risk for transmission of known and unknown diseases. Hematologically transmissible diseases of the donor therefore are considered a contraindication. Recent animal experiments have demonstrated that psychiatric diseases can transmitted via HSCT from donor to recipient. So far there are no such observations in human patients. Here we report the case of a 33 year old male patient who, because of a rare chronic hypoplastic congenital aenemia, underwent HSCT. The BM donor was the brother of the patient and suffered from recurrent depressive disorder. Because of the short stature resulting from his underlying hematologic disease the patient had been in psychotherapy form several years. Depressive episodes had not been observed during this time. 10 weeks post HSCT the patient experienced a depressed mood and recurring anxiety attacs. The initiation of antidpressant medication was considered but not started due to the mild symptomatology. While depressive symptoms after HSCT are not infrequent because of the multiple stressors outlined above, we argue that in the light of the recent animals studies on the transmission of psychiatric disease via hematopoeitic stem cells in the present constellation a hematologic transmission of the mood disorder can also be condsidered. Background and purpose: Functional neuroimaging studies have suggested a dysfunction of prefrontal regions in clinically pre-symptomatic individuals with the Huntington’s disease (HD) gene mutation (pre-HD) during cognitive processing. The objective of this study was to test the impact of cognitive demand on prefrontal connectivity in pre-HD individuals. Methods: Sixteen healthy controls and sixteen pre-HD subjects were studied using functional MRI and a verbal working memory task with increasing cognitive load. Load-dependent functional connectivity of the left dorsolateral prefrontal cortex (DLPFC) was investigated by means of psychophysiological interactions. Results: In pre-HD subjects, aberrant functional connectivity of the left DLPFC was found at high working memory load levels only. Compared with healthy controls, pre-HD individuals exhibited lower connectivity strength in the left putamen, the right anterior cingulate and the left medial prefrontal cortex. Pre-HD individuals close to the onset of motor symptoms additionally exhibited lower connectivity strength in the right putamen and the left superior frontal cortex. The connectivity strength in the left putamen was associated with several clinical measures including CAG repeat length, Unified Huntington's Disease Rating Scale motor score and predicted years to manifest symptom onset. Conclusion: These findings suggest that early prefrontal connectivity abnormalities in pre-HD individuals are modulated by cognitive demand. [ABSTRACT FROM AUTHOR]

Second-generation antipsychotics (SGA) are increasingly used in primary and secondary delusional parasitosis (DP) because of their better overall tolerability compared with first-generation antipsychotics (FGA) such as pimozide. Controlled clinical trials with antipsychotics in DP are lacking, owing to difficulties in obtaining informed consent and in securing adherence to a study protocol by patients with DP. We present the case of an 88-year-old man with a 12-year history of DP secondary to leucoencephalopathy. After 9 days of an age-adapted dose of paliperidone, the patient no longer experienced the presence of vermin on his skin and stopped showering at night to get rid off of them. Paliperidone was well tolerated. At follow-up after 2 weeks, the DP was still remitted. Paliperidone appears to expand the therapeutic arsenal in treating DP with modern SGAs; however, this finding needs to be replicated. [ABSTRACT FROM AUTHOR]

Apart from these typical adverse events the agent has attracted attention for several rare phenomena (priapism, cholestasis, rhabdomyolysis) that indeed feature anecdotal character, but are nevertheless indispensable for a comprehensive understanding of the factual risk profile of quetiapine. The treatment of psychiatric disorders during pregnancy poses a challenge particularly regarding limited pharmacological possibilities. Repetitive transcranial magnetic stimulation (rTMS) and electroconvulsive therapy (ECT) have been described to be successful in single cases, however, guidelines are lacking. In order to contribute to the pool of empirical data that is necessary to create evidenced-based recommendations we present a case of successful treatment of major depression with ECT in a pregnant patient with previous nonresponse to rTMS. Introduction: Chronic tinnitus is a frequent disease accompanied in many cases by high morbidity and reduction in quality of life. Despite its high incidence, treatment remains elusive. Aggregated research over the last years has contributed to a deeper understanding of the neurobiological basis of chronic tinnitus and point to the involvement of auditory and non-auditory brain areas. Repetitive transcranial magnetic stimulation (rTMS) is able to non-invasively modulate brain activity. Pilot studies indicate a potential therapeutic effect of rTMS in tinnitus patients, however, sample sizes were small so far. The aim of this study was to evaluate the efficacy and safety of rTMS in tinnitus in a large patient sample. Methods: 7 study centers participated in this randomized, placebo-controlled trial. From February 2008 until May 2011 a total of 154 patients were enrolled and received 10 sessions of either verum or sham 1Hz-rTMS over the left primary auditory cortex. Effects on tinnitus and accompanying psychiatric symptoms were evaluated using a variety of standardized rating scales (tinnitus questionnaire (TQ), tinnitus handicap inventory (THI), beck depression inventory, etc.). Results and conclusions: rTMS treatment was well tolerated in all patients. In the primary outcome (change of the TQ-score baseline vs. day 12) no significant difference between both study arms was observed. During the follow-up period, a trend to superiority of verum rTMS could be observed, which did not reach statistical significance. This study shows, that 1Hz-rTMS over the left primary auditory cortex is not superior to placebo treatment. Therefore, more effective treatment protocols are needed and combined treatment regimes focusing on auditory and non-auditory brain regions may be better as recent studies have demonstrated. Selfmutilation (SM) is a common phenomenon among patients with psychiatric disorders and has been reported frequently in patients with schizophrenia, personality disorders and occasionally with transsexuality. Several psychotropic agents, as alocohol or amphetamines, are able to induce intoxications during which SM is performed. Though SM related to psychotropic agents is probably a frequent phenomenon, literature lacks of systematic approaches and just provides single case reports. In this context we performed a systematic literature search and identified published cases of psychotropic substance induced selfmutilating behaviour (PSISM). Assessment of these cases shows that the typical patient that is on risk to develop PSISM is male, in the middle age and with a previous history of multiple substance abuse.
