Die Behandlung seltener Erkrankungen in der neurologischen Intensivmedizin wird maßgeblich erschwert durch: a) fehlendes Wissen über der zugrundeliegende Pathomechanismen und b) das Therapiekonzepte entweder fehlen, aus medizinischen Gründen nicht anwendbar oder bei besonders schwer betroffenen Patienten nicht oder nur unzureichend wirksam sind. In dieser Habilitationsschrift werden Arbeiten vorgestellt, die für verschiedene neurointensivmedizinische Krankheitsbilder und Komplikationen (1. Antikörper-vermittelte Autoimmunenzephalitiden, 2. Myasthenia gravis, 3. chronisch inflammatorisch demyelinisierende Polyneuropathie (CIDP), 4. posthypoxische Bewegungsstörungen nach Herz-Kreislauf Stillstand, 5. refraktärer und superrefraktärer Status epilepticus (RSE, SRSE)) neue Therapieansätze auf erste Hinweise hinsichtlich Sicherheit und Wirksamkeit untersucht haben und dabei basierend auf bisherigen Literaturdaten, z.T. auch in Analogieschluss zu anderen Erkrankungen, Mechanismus-basiert in pathophysiologische Prozesse der jeweiligen Erkrankung eingriffen. Dabei konnte für lebensbedrohliche, therapierefraktäre Verläufe verschiedener neuroimmunologischer Erkrankungen, wie Antikörper-vermittelte Autoimmunenzephalitiden, der CIDP und der Myasthenia gravis gezeigt werden, dass über den Therapieansatz der gezielten Plasmazell-Depletion mit dem Proteasominhibitor Bortezomib oder mit dem humanisierten, monoklonalen anti-CD38 Antikörper Daratumumab, der zugrundeliegende Autoimmunprozess deutlich abgemildert bzw. sogar durchbrochen werden konnte. Passend zur klinischen Remission konnte das Verschwinden bzw. ein deutlicher Abfall der autoreaktiven Antikörper, als auch ein Abfall der protektiven Immunglobulinspiegel und langlebigen Vakzin-Titer beobachtet werden. Diese drei Parameter erwiesen sich als geeignete indirekte laborchemische Biomarker für die Therapie-induzierte Plasmazelldepletion. Für die posthypoxischen Bewegungsstörungen nach hypoxisch-ischämischer Enzephalopathie infolge Reanimation konnten Levomepromazin und intrathekales Baclofen als neue Therapieansätze identifiziert werden, um diese sonst sehr schwer behandelbaren, und häufig als fatal eingeschätzten, posthypoxischen motorischen Phänomene besser zu kontrollieren. Die Therapie mit Isofluran erwies sich als gut geeignetes Verfahren, um den RSE und SRSE zu durchbrechen, auch wenn anhaltende Anfallsremission schwierig blieben. Zusammenfassend bilden die in dieser Habilitationsschrift retrospektiv und in Form von kleineren und größeren Fallserien erhobenen Daten den Grundstein für die medizinische Weiterentwicklung der genannten Therapieansätze, um diese zukünftig in multizentrischen, randomisiert-kontrollierten Studien weiterführend zu untersuchen. Auf diesem Weg können innovative neue Therapiekonzepte entwickelt und die Versorgung, Prognose und das Überleben der betroffenen kritisch kranken Patienten verbessert werden., The treatment of rare diseases in neurological intensive care medicine is significantly hampered by: a) a lack of knowledge about the underlying pathomechanisms and b) that therapeutic concepts are either lacking, not applicable for medical reasons or that they are not or only insufficiently effective in particularly severely affected patients. In this habilitation thesis, work is presented that has been carried out for various neurointensive care diseases and complications (1. antibody-mediated autoimmune encephalitides, 2. myasthenia gravis, 3. chronic inflammatory demyelinating polyneuropathy (CIDP), 4. posthypoxic movement disorders after cardiovascular arrest, 5. refractory and superrefractory status epilepticus (RSE, SRSE)) with the aim to investigate new therapeutic approaches for initial indications with regard to safety and efficacy and, based on previous literature data, have intervened in the pathophysiological processes of the respective disease, partly also by analogy with other diseases. It could be shown for life-threatening, therapy-refractory courses of various neuroimmunological diseases, such as antibody-mediated autoimmune encephalitis, CIDP and myasthenia gravis, that the underlying autoimmune process could be significantly attenuated or even interrupted by the therapeutic approach of targeted plasma cell depletion with the proteasome inhibitor bortezomib or with the humanised, monoclonal anti-CD38 antibody daratumumab. In line with the clinical remission, the disappearance or a significant reduction in autoreactive antibodies as well as a drop in protective immunoglobulin levels and long-lived vaccine titres could be observed. These three parameters proved to be suitable indirect laboratory chemical biomarkers for therapy-induced plasma cell depletion. For posthypoxic movement disorders following hypoxic-ischaemic encephalopathy due to resuscitation, levomepromazine and intrathecal baclofen could be identified as new therapeutic approaches to better control these posthypoxic motor phenomena, which are otherwise very difficult to treat and often considered fatal. Therapy with isoflurane proved to be a well-suited method to break through RSE and SRSE, although sustained seizure remission remained difficult. In summary, the data collected in this habilitation thesis retrospectively and in form of smaller and larger case series form the cornerstone for the further medical development of the aforementioned therapeutic approaches in order to investigate them further in multicentre, randomised-controlled studies. In this way, innovative new therapy concepts can be developed and the care, prognosis and survival of such critically ill patients can be improved.

Acute hepatic porphyrias (AHP) can cause severe neurological symptoms involving the central, autonomic, and peripheral nervous system. Due to their relative rarity and their chameleon‐like presentation, delayed diagnosis and misdiagnosis are common. AHPs are genetically inherited disorders that result from heme biosynthesis enzyme deficiencies and comprise four forms: acute intermittent porphyria (AIP), variegate porphyria (VP), hereditary coproporphyria (HCP), and ALA‐dehydratase porphyria (ALADP). Depending on the clinical presentation, the main differential diagnoses are Guillain‐Barré syndrome and autoimmune encephalitis. Red flags that could raise the suspicion of acute porphyria are neurological symptoms starting after severe (abdominal) pain, in association with reddish urine, hyponatremia or photodermatitis, and the presence of encephalopathy and/or axonal neuropathy. We highlight the diagnostic difficulties by presenting three cases from our neurological intensive care unit and give a comprehensive overview about the diagnostic findings in imaging, electrophysiology, and neuropathology., Acute hepatic porphyrias (AHP) can cause severe neurological symptoms involving the central, autonomic and peripheral nervous system. Ziel dieser Arbeit war es – nach Etablierung eines Zellkulturmodells für murine Mesenchymale Stromazellen (mMSC) des Knochenmarks – das regenerative Potential der Zellen im Schlaganfallmodell in vitro und in vivo zu untersuchen. Im Zellkulturschlaganfallmodell des kombinierten Sauerstoff – Glukose – Entzugs (OGD) konnten MSC in signifikantem Ausmaß Neurone vor dem Zelluntergang durch Apoptose schützen. Für diesen Effekt war kein direkter Zell- Zellkontakt zwischen Neuronenkulturen und MSC notwendig. Durch Kokultivierung der MSC mit Neuronen in einem Transwell-Kokultursystem, konnten MSC über einen parakrinen Mechanismus, d.h. durch Produktion trophischer Faktoren, das Überleben der Neurone nach ischämischer Schädigung verbessern. Die Effekte waren an eine Zeit- und Dosisabhängigkeit gekoppelt. In weiterführenden proteinbiochemischen Analysen gilt es prospektiv Kandidaten zu identifizieren, die die neuroprotektiven Eigenschaften der MSC vermitteln. Bisherige Daten ergeben, dass bei der Reduktion des apoptotisch-neuronalen Zelltodes durch MSC die intrazelluläre Phosphorylierung von STAT3 und AKT eine Rolle spielen. Zur Validierung dieser Ergebnisse sind weitere Experimente mit Inhibitoren der PI3K-AKT-Signalkaskade notwendig. Die neuroprotektiven Eigenschaften der MSC in vitro haben sich im tierexperimentellen Schlaganfallmodell der Maus nicht bestätigen lassen. Nach systemischer Applikation der MSC in den MCAo-Tieren konnte weder eine Reduktion der Schlaganfallvolumina, noch eine verbesserte funktionelle Regeneration der Mäuse objektiviert werden. Gründe hierfür sind zum einen, dass die MSC trotz offener Blut-Hirn-Schranke nach dem Schlaganfall nicht in das Hirnparenchym – also den eigentlichen Läsionsort – einwanderten. Die über einen parakrinen Wirkmechanismus vermittelten neuroprotektiven Eigenschaften der MSC können bei fehlender lokalisatorischer Nähe zwischen MSC und Schädigungsort nicht wirksam werden. Die direkte Applikation von MSC an den Schädigungsort, z.B. durch intrazerebrale Injektion der Zellen, würde eine mögliche Alternative bilden, um parakrine Effekte der MSC im Schlaganfallkontext weiterführend untersuchen zu können. Im zweiten Teil dieser Dissertation wurden nonvirale Transfektionsmethoden auf ihre Fähigkeit zur stabilen Transgenexpression in MSC in vitro und in vivo geprüft. Im Vergleich zur viralen Transduktion zeigten nonvirale Methoden eine deutlich geringere Effizienz bei der Generierung stabil transfizierter MSC. Dennoch konnten durch antibiotischen Selektionsdruck und FACS-Sortierung – unabhängig von der transienten Transfektionsmethode – zuverlässig stabil transfizierte MSC-Klone generiert werden. Die stabile Transgenexpression hatte keinen Einfluß auf das Proliferations- und Differenzierungspotential der MSC. Die genetisch modifizierten Zellen zeigten eine dauerhafte Expression und biologische Aktivität ihrer Transgene (EGFP/EPO) in vitro. Nach intraperitonealer Injektion von stabil EPO/EGFP-exprimierenden MSC in C57BL/6 Mäuse wurde in vivo die Transgenexpression durch pharmakologisch relevante Anstiege von Hämatokrit, Hämoglobin- und EPO-Serumspiegel nachgewiesen. Die langfristige Expression von EGFP wurde in immunhistochemischen Untersuchungen durch Persistenz von grün fluoreszierenden Zellen in verschiedenen Organen, 5 Wochen nach Transplantation bestätigt. Die weitgehende Normalisierung von Hämatokrit-, Hämoglobin- und EPO-Serumspiegel im Zeitverlauf ist vermutlich auf das Absterben der MSC in vivo und den damit verbundenen Verlust von EPO- produzierenden Zellen zurückzuführen. Um das Zellüberleben der MSC in vivo zu erhöhen und die potentielle Einsetzbarkeit nonviral stabil transfizierter MSC für gentherapeutische Applikationen nutzen zu können, gilt es Strategien zu entwickeln, die das langfristige Überleben der Zellen in vivo, z.B. in einer Matrix, ermöglichen., In this study, the regenerative potential of bone marrow-derived mesenchymal stromal cells (MSCs) was investigated in an in vitro and in vivo model of stroke. MSCs protected cortical neurons in a significant extent from apoptotic cell death in an in vitro model of cerebral ischemia - oxygen glucose deprivation (OGD). Coculture of MSCs with cortical neurons in a transwell coculture system, but also addition of MSC conditioned media, resulted in improved neuronal survival after ischemic injury in a time and dose dependent manner. This effect was mediated by paracrine trophic factor support by MSCs, whereas direct cell-to-cell contact was not required. Further proteomic approaches are required to identify putative candidates that facilitate neuroprotective capacities of MSCs. Moreover, intracellular signalling cascades like activation of STAT3 and AKT by phosphoylation seem to be involved in reduction of apoptotic neuronal cell death by MSCs after OGD. Neuroprotective effects of MSCs could not be confirmed in a murine in vivo model of stroke (middle cerebral artery occlusion; MCAo). Systemic delivery of MSCs by intravenous injection did not result in reduced infarct volumes and did not improve functional outcome of affected animals. Different reasons have to be considered for these findings: MSCs did not enter the brain instead of an open blood-brain-barrier. Paracrine functions of MSCs could not become effective by persisting spatial separation between MSCs and site of lesion. To enable better investigation of paracrine MSC effects in context of stroke, direct application of MSCs by intracerebral injection might offer an appropriate alternative. In the second part of this dissertation, nonviral transfection methods were investigated for their potential of stable transgene expression in vitro and in vivo. As example of application, the EPO and EGFP gene were introduced into MSCs. In contrast to viral techniques, nonviral approaches showed a much lower efficiency to generate stably transfected MSCs. However, by antibiotic selection with Geneticin and by use of a single-cell colony forming unit (sc-CFU) assay stably transfected MSC clones were reliably generated independent from the applied transient transfection method. Stable transgene expression did not influence proliferation or differentiation potential of MSCs. Genetically modified cells exhibited a persisting expression and biologic activity of their transgenes (EPO/EGFP) in vitro. In vivo transgene expression was proven by intraperitoneal injection of stably EPO/EGFP expressing MSCs in C57BL/6 mice that resulted in sufficient increase of pharmacological relevant parameters like hematocrit, haemoglobin and serum EPO level. Long-term EGFP expression in vivo was confirmed by detection of green fluorescent cells in different abdominal organs five weeks after transplantation. But hematocrit dropped over time, and hematocrit and serum EPO levels had normalized five weeks after initial injection. That might be dued to partial death of cell transplant and thereby loss of EPO-producing cells. To increase MSC survival in vivo, introduction of cells in a collagen or polymermatrix might be feasible to exploit nonvirally transfected MSCs for potentially gene therapeutical approaches in the future.
