ObjectiveAfter a previous survey on the incidence of diabetic ketoacidosis (DKA) at onset of type 1 diabetes in children in 2013–2014 in Italy, we aimed to verify a possible decline in the incidence of DKA at onset during a national prevention campaign.DesignProspective observational study.SettingMulticentre study throughout Italy.InterventionNational awareness campaign started in November 2015 and held until December 2017.PatientsDuring 2016 and 2017 we collected data on all patients aged 0–18 years with new-onset diabetes.Main outcome measuresDKA (pH ResultsRecords (n=2361) of children with newly diagnosed type 1 diabetes were collected from 58 out of 68 (85.3%) centres of the original survey participants and 100% of the previously surveyed tertiary centres. Overall, DKA was observed in 1124 patients, with an increased rate when compared with the previous survey (47.6% vs 38.5%, p=0.002), and severe DKA in 15.3%. In children below 6 years, DKA was observed in 323 out of 617 (52.5%) and severe DKA in 16.7%; in this age group, occurrence of DKA reduced by 21.3% (p=0.009). DKA treatment according to the ISPED guidelines was adopted in 95% of the centres, with a 27% improvement (p=0.025).ConclusionsDuring a 2-year awareness campaign, DKA at onset of diabetes in children and adolescents 0–18 years is still common and increased when compared with the 2013–2014 survey.

Background: Continuous subcutaneous insulin infusion (CSII) is increasing worldwide, mostly because of improved technology. The aim of this study was to evaluate the current status of CSII in Italy. Materials and Methods: Physicians from 272 diabetes centers received a questionnaire investigating clinical features, pump technology, and management of patients on CSII. Results: Two hundred seventeen centers (79.8%) joined the study and, by the end of April 2013, gave information about 10,152 patients treated with CSII: 98.2% with type 1 diabetes mellitus, 81.4% adults, 57% female, and 61% with a conventional pump versus 39% with a sensor-augmented pump. CSII advanced functions were used by 68% of patients, and glucose sensors were used 12 days per month on average. Fifty-eight percent of diabetes centers had more than 20 patients on CSII, but there were differences among centers and among regions. The main indication for CSII was poor glucose control. Dropout was mainly due to pump wearability or nonoptimal glycemic control. Twenty-four hour assistance was guaranteed in 81% of centers. A full diabetes team (physician+ nurse + dietician + psychologist) was available in 23% of adult-care diabetes centers and in 53%of pediatric diabetes units. Conclusions: CSII keeps increasing in Italy. More work is needed to ensure uniform treatment strategies throughout the country and to improve pump use.

Aims Insulin pump failure and/or malfunction requiring replacement have not been thoroughly investigated. This study evaluated pump replacement in children and adolescents with Type 1 diabetes using insulin pump therapy. Methods Data were collected for all participants younger than 19 years, starting insulin pump therapy before 31 December 2013. For each child, age, disease duration, date of insulin pump therapy initiation, insulin pump model, failure/malfunction/replacement yes/no and reason were considered for the year 2013. Results Data were returned by 40 of 43 paediatric centres belonging to the Diabetes Study Group of the Italian Society of Paediatric Endocrinology and Diabetology. In total, 1574 of 11 311 (13.9%) children and adolescents with Type 1 diabetes were using an insulin pump: 29.2% Animas VIBE™, 9.4% Medtronic MiniMed 715/515™, 34.3% Medtronic MiniMed VEO™, 24.3% Accu-Check Spirit Combo™ and 2.8% other models. In 2013, 0.165 insulin pump replacements per patient-year (11.8% due to pump failure/malfunction and 4.7% due to accidental damage) were recorded. Animas VIBE™ (22.1%) and Medtronic MiniMed VEO™ (17.7%) were the most replaced. Conclusions In a large cohort of Italian children and adolescents with Type 1 diabetes, insulin pump failure/malfunction and consequent replacement are aligned with rates previously reported and higher in more sophisticated pump models. Background and Aims: The UVA/Padova Type 1 Diabetes Simulator (T1DMS) has proven to be very useful for the preclinical testing of control algorithms in adults, also thanks to the availability of data which allowed to refine and validate the in silico population. This facilitated several successful artificial pancreas inpatient and outpatient studies. Artificial pancreas studies in children are increasing and the T1DMS could play an important role in algorithmic development. The aim of this study is to assess and, if necessary, to update the in silico children included into the T1DMS. Method: The data set consisted of 13 T1DM prepubertals recruited among the centers of Milano, Napoli, Roma and Torino (Italy). Subjects wore a sensor-augmented insulin pump, and received a standardized breakfast (30g of carbohydrates [CHO]). The T1DMS model was identified from subcutaneous glucose sensor and insulin pump data using a Bayesian approach. Results: The model well fitted the data and provided precise estimates of model parameters. Our results suggest that meal glucose absorption is faster and insulin sensitivity lower, on average, than those included so far in the T1DMS (Figure 1). Based on these findings, a new children population was generated. The new T1DMS was then validated by comparing real vs simulated glucose curves in response to 30 g CHO meal: as a result, the new T1DMS well described the glucose variability observed in the data. Conclusion: The new in silico children population will enable a safer and effective preclinical testing of the closed-loop control in pediatric clinical trials.

- Abstract
Background and Aims: To explore the experiences of parents of 5–8-year-old children with type 1 diabetes participating in a clinical trial regarding artificial pancreas (AP), semistructured interviews, based on the Technology Acceptance Model, were conducted after 3 days of children's treatment. Method: Questions focused on evaluating parents' perceived usefulness of, perceived ease of use of, trust in, and intention to use the new system. Interviews were conducted by a psychologist, and the answers, both audiorecorded and transcribed verbatim, were assessed using qualitative research methods. Results: Altogether, 27 (22 mothers) of 30 parents were interviewed (Table 1), and their overall attitude toward AP was positive (96%). Perceived advantages included stable glucose regulation (52%), better quality of life for children (22%), relief of parents' daily concerns (15%), and reduced need for continual parental monitoring of nocturnal blood glucose (11%), while perceived disadvantages included having to constantly wear a bulky, heavy device (37%) and the risk of technical error (33%). Participants were mostly confident in the positive impact of AP on diabetes control (96%) and in children's, especially older ones', capability to use the system (55%). The reactions of teachers and friends were reported to be generally positive, though some parents (28%) expected an initially fearful reaction from teachers. Nearly all participants expressed trust in AP and in the quality of glucose control (96%), as well as the intention to use the new system when available (100%). Conclusion: Results indicate that, thanks to the psychological and physical benefits of AP, parents expressed a strong likelihood of future acceptance.

- Abstract
Background: continuous subcutaneous insulin infusion (CSII) is increasing due to effectiveness and improved technology. Aim: to evaluate the current status of CSII in Italy. Materials and Methods: clinical features, pump technology, patient management and metabolic control of CSII patients were investigated in two questionnaire-based surveys to 272 Italian diabetes centers. Results: 217 centers (79,8%) answered the first questionnaire giving information on 10,152 patients, 98.2% with type 1 diabetes mellitus, 81,4% adults, 57% female, 61% with a conventional pump and 39% with a sensoraugmented pump. CSII advanced functions were used by 68% of patients, and glucose sensors were used 12 days/month on average. Fifty-eight percent of centers had >20 CSII patients. Main indication for CSII was poor glucose control. Dropout was mostly due to pump wearability or bad glucose control. A complete dedicated team was available in 23% of adult centers and in 53% of pediatric units. In the second survey, data were obtained from 114 centers on 6,886 patients with overall characteristics comparable to the previous one. Mean HbA1c was 60.1 ± 10.7 mM/M. Lower HbA1c levels were associated to age 6–13, pregnancy in the last year, use of advanced functions, sensor use ≥10 days/month, availability of a dedicated team. 6,1% of patients had ≥1 severe hypoglycemic event over the previous year. Conclusions: Advanced patient skills, compliance to sensor use and an integrated team approach are associated to better metabolic outcomes in Italian CSII patients.

Objective. In the PEDiatric ARtificial PANcreas (PedArPan) project we tested a children-specific version of the MMPC on DiAs in 5-9 yearold children during a summer camp. This is the first outpatient AP trial in a population of this age. Study design and methods. Thirty 5-9 year-old children with T1D and their parents were recruited from five Italian pediatric centers and completed an open label, randomized, crossover trial in a summer camp (supervised outpatient setting). Glucose control in three days of AP use was compared with three days of parent-managed SAP. Results. Overnight (00:00-07:30). The AP significantly reduced time-in-hypo compared to SAP, 0% [0.00-2.22] vs. 2.19% [0.00- 12.30], p = 0.002, with no significant difference in time-in-target (70-180 mg/dl), 55.97% (22.53) vs. 59.69% (21.24), p = 0.430, or time-in-tight-target (80-140 mg/dl), 31.27% (20.23) vs. 33.02% (19.80), p = 0.694. Overall (00:00-24:00). There was a threefold reduction of time-in-hypo, 2.02% [1.17-4.54] vs. 6.67% [2.26-11.54], p < 0.001, with AP at the expense of some deterioration of time-in-target, 56.79% (13.47) vs. 63.09% (10.98), p = 0.022. Conclusions. This study showed the feasibility and safety of a wearable MMPC in young children. Next steps will include overnight testing at home and re-tuning the algorithm to boost efficacy during daytime.

Background: The incidence of type 1 diabetes in childhood is increasing by 3 % per year, placing growing demands on healthcare professionals and medical expenditures. Aim of this study wars to assess the organization of care to children with diabetes in Italy. Methods: During 2012 a structured questionnaire was sent to all of the members of Italian Society of Paediatric Endocrinology and Diabetology (ISPED). Questions examined organizational structure of Centers, personnel dedicated to the care of children with diabetes, number of subjects followed, local legal legislation supporting centres. Results: A total of 68 centers taking care to 15,563 children and adolescents with diabetes under 18 years of age were identified with a prevalence of 1.4 per 1,000 people. A wide variation in the organizational background was also reported. Fourty-four centers were organized as outpatient departments, 17 as simple units, 5 as complex units and 2 as simple departmental structures. Most centers had a multidisciplinary team. Ten out of twenty Italian regions had introduced supportive regional legislation, but it was fully applied only in six of them. Conclusion: Great differences between regions were found in organizational structures, staffing levels and supportive legislation. The national legislation on diabetes was broadly implemented throughout the country regions. Further efforts are needed to improve standards and consistency of pediatric diabetes care in Italy.

La diffusione della terapia insulinica sottocutanea continua con microinfusore (CSII) è in continuo aumento nel mondo, grazie anche al miglioramento della tecnologia degli strumenti. Scopo di questo lavoro è stato quello di valutare lo stato attuale della CSII in Italia. A tal fine ai responsabili di 272 strutture diabetologiche che seguono pazienti in terapia con microinfusore è stato inviato un questionario che indagava aspetti clinici, tecnici e organizzativi della terapia con microinfusore. Duecentodiciassette strutture (79,8%) hanno partecipato all’indagine. Dai dati raccolti è emerso che a fine aprile 2013 in tali strutture vi erano 10.152 pazienti in terapia con microinfusore, quasi tutti (98,2%) affetti da diabete di tipo 1, prevalentemente adulti (82,4%) e di sesso femminile (57%). La diffusione della CSII risultava molto disomogenea tra le diverse Regioni e anche tra le diverse strutture diabetologiche di una stessa Regione: solo il 59% delle strutture seguiva più di 20 pazienti ciascuna. Motivo principale per iniziare la CSII era la ricerca del buon controllo glicemico. Il drop-out (8,65% dei casi), avveniva soprattutto per problemi legati alla portabilità della pompa o al mancato raggiungimento del target glicemico. La maggior parte dei pazienti (61%) utilizzava un microinfusore tradizionale, il 39% un microinfusore integrato (32%) o associato (7%) a un dispositivo per il monitoraggio in continuo della glicemia. Le funzioni avanzate del microinfusore venivano mediamente utilizzate solo dal 68% dei pazienti e il sensore era utilizzato mediamente solo per 12 giorni al mese. L’81% delle strutture garantiva una reperibilità 24 ore su 24. Solo nel 40% delle strutture per adulti e nel 50% di quelle pediatriche i pazienti venivano seguiti da un team completo. In conclusione, la CSII si sta sempre più diffondendo in Italia, sia tra i pazienti adulti sia tra quelli pediatrici. È auspicabile una maggiore uniformità tra le diverse Regioni e un uso più adeguato della tecnologia stessa.

Ketoacidosis is a potentially life-threatening complication in patients with type 1 diabetes mellitus (T1DM), particularly children. If diabetic ketoacidosis (DKA) is diagnosed late, the child risks cerebral edema, permanent neurological damage or even death. There have been only few studies of DKA in Italy.From January-May 2014 a nation-wide observational, retrospective study of DKA at diabetes onset was done by the Pediatric Diabetology Study Group (PDSG) of the Italian Society of Pediatric Endocrinology and Diabetes (ISPED), involving 76 Italian centers. DKA was defined using ISPAD criteria; 7457 new cases of T1DM were recruited from mainland Italy and the island of Sicily and 770 from Sardinia, in the period 2004-2013. On the mainland and in Sicily, DKA at diabetes onset was about 32.9% (95% CI 31.8-34.0%), and there was 6.6% (95% CI 6.02-7.20%) of the severe form. Mild and severe DKA risk was significantly higher in children aged 0-4 years; no significant temporal trend was found in the study period. Patients living in Sardinia or having a firstdegree relative with T1DM were at significantly lower risk of DKA at diabetes onset. In the ten-year study period three children died of DKA at onset and four suffered permanent neurological lesions. From November 2011-April 2012 the PDSG conducted a retrospective study based on a sample of 2025 patients with T1DM, aged 0-18 years, involving 29 national centers for pediatric diabetes. The incidence of DKA was 2.4% (IC 95% 1.8-3.1), with children older than ten years at significantly higher risk, probably due to shortages of insulin. Multiple analysis showed a higher risk of DKA in those using a rapid-acting insulin analog and in those with high HbA1c. Young mothers and low levels of education were also associated with DKA.In conclusion, although a wide network of specialized home pediatricians and pediatric diabetes centers is spread across the country, the incidence of DKA at diabetes onset is still high. Further social and health-system efforts are needed to boost awareness of this risk and to reduce damages and costs related to the complication.

The first instrument that measured blood sugar “continuously” appeared 15 years ago. Since then, subcutaneous glycemic sensors have made it possible to obtain continuous blood glucose measurements, giving physicians or patients real-time pictures of the levels of sugar in the blood. The quality of the technology has substantially improved – in the measurement and interpretation of results – although much remains to be done, especially as regards the sensors’ accuracy and reliability. Some very important studies (RealTrend, Eurythmics, ONSET, SWITCH, STAR 3, INTERPRET) have confirmed that continuous glucose monitoring contributes added value to insulin therapy, especially when an insulin pump is used, to achieve therapeutic goals. For almost ten years now it has been possible to combine the pump with a blood glucose sensor, giving what is known as sensor-augmented insulin pump therapy. In the last two years, the findings from other studies have been published, with important updates on blood glucose monitoring implemented with automatic suspension of the insulin infusion as soon as a predetermined threshold is achieved (ASPIRE) or even in relation to the expectation that the threshold will soon be reached (PILGRIM).

A multi-centre, observational, cross-sectional study was carried out to determine whether the healthrelated quality of life (HRQOL) of adolescents with type 1 diabetes is affected by different insulin treatment systems, and which features of HRQOL are impacted by the respective insulin treatment. The study regarded 577 adolescents, aged 10–17 years, with type 1 diabetes treated with continuous subcutaneous insulin infusion (CSII) (n = 306) or multiple daily injections (MDI) (n = 271). The Insulin Delivery System Rating Questionnaire was validated in Italian and was self-completed by the subjects during a routine visit to the centres. Subjects were compared following the domains of the questionnaire. Good HRQOL was seen in subjects treated with either MDI or CSII. Significant differences were not found in the domains for general diabetes, including diabetes worries, social burden and psychological well-being. Multiple quantile regression analysis showed that CSII confers significant advantages in terms of HRQOL with improvements in treatment satisfaction, perceived clinical efficacy and reduction in treatment interference with daily activities.This favourable impact was more evident in subjects reporting lower HRQOL scores, suggesting that CSII may be especially useful for individuals perceiving a poor HRQOL. Analysis of the domains indicated that CSII was associated with a higher HRQOL than MDI. Life-course HRQOL evaluation using a standardised questionnaire can ensure better chronic disease management. This is particularly important when providing individualized care for adolescents, as they become increasingly responsible for managing their diabetes.

This document has been developed by a group of Italian diabetologists with extensive experience in continuous subcutaneous insulin infusion (CSII) therapy to provide indications for the clinical management of CSII in diabetic patients (both type 1 and type 2) based on delivery mode operating in Italy. Although the potential benefits of pump therapy in achieving glycemic goals is now accepted, such results cannot be obtained without specific knowledge and skills being conveyed to patients during ad hoc educational training. To ensure that these new technologies reach their full effectiveness, as demonstrated theoretically and clinically, a careful assessment of the overall therapeutic and educational process is required, in both qualitative and quantitative terms. Therefore, to ensure the cost-effectiveness of insulin pump therapy and to justify reimbursement of therapy costs by the National Health System in Italy, in this article we present a model for diabetes and healthcare centers to follow that provides for different levels of expertise in the field of CSII therapy. This model will guarantee the provision of excellent care during insulin pump therapies, thus representing the basis for a successful outcome and expansion of this form of insulin treatment in patients with diabetes while also keeping costs under control.

Background To investigate serum concentrations of high-sensitive C-reactive protein (CRP) and alpha(1)-acid glycoprotein (AGP) in patients with T1DM, at diagnosis and after 12 months of intensive insulin therapy (T12). Methods CRP and AGP were measured in 44 recent onset T1DM patients (26M/18F, mean age 14.9 ± 9.1 years), and 44 age- and sex-matched controls, using a highly sensitive immunonephelometric assay. Results There were no significant differences in the concentrations of high-sensitive C-reactive protein (hs-CRP) and AGP between patients and controls. hs-CRP levels significantly increased in patients at T12 compared to the levels at diagnosis [0.69 (0.14–15.5) versus 0.43 (0.14–7.47) mg/L, p < 0.05; for males: 0.77 (0.14–15.5) versus 0.35 (0.14–7.47) mg/L, p < 0.05; for females the increase was not significant]. AGP levels were not different at T12 compared to diagnosis. No significant correlations were found between hs-CRP and body mass index (BMI), C-peptide, glycosylated haemoglobin, or insulin dose. A strong correlation was found between hs-CRP values at diagnosis and those at T12 (ρ = 0.73, p < 0.001); indeed, patients with hs-CRP levels above the 50th percentile at diagnosis showed significantly increased hs-CRP values at T12 compared to patients with baseline hs-CRP levels under the 50th percentile [1.61 (0.18–15.5) versus 0.16 (0.14–1.92) mg/L, p < 0.0001)], and to controls [0.55 (0.14–6.50), p = 0.001]. Conclusions This is the first report showing that, despite good metabolic control, 1 year of overt T1DM is sufficient to increase hs-CRP levels, especially in males. hs-CRP levels at diagnosis is a predictor for the values observed at 12 months, suggesting the possibility to select a subgroup of patients requiring strict follow-up for cardiovascular complications. - Abstract
Results: Neonatal/Infancy onset diabetes mellitus (NDM) is a genetic form of diabetes with onset within 6 months from birth. Two distinct types of NDM are recognized: permanent (PNDM) and transient (TNDM). In TNDM remission of hyperglycemia usually occurs within 6 months from diagnosis. TNDM is frequently caused by paternal isodisomy of chromosome 6 and PNDM by mutations of insulin gene, but both condition can be associated with gain-of-function mutations in genes encoding the subunits of the KATP channel, KCNJ11 and ABCC8. Incidence of PNDM in Italy is 1:210.000. ABCC8 and KCNJ11 genes were screened in 14 patients with TNDM. Minimal incidence of NDM was calculated counting patients with PNDM and TNDM born in Italy between 2005 and 2009 and referred to our laboratory for genetic screening by members of the Italian study group on diabetes of ISPED. We identified mutations in 10 patients with TNDM: three heterozygous mutations of KCNJ11 (R50Q [x2], E229K, E179K) in 4 subjects and 7 heterozygous mutations of ABCC8 (H105Y, S459R, T540I, R826W, R1380C, R1380H, V1523M) in 6 patients (1 compound het. for H105 and T540I). In these 10 patients, duration of insulin therapy ranged between 17 day and 8 months. Background: Insulin pumps and real-time continuous glucose monitoring devices have recently been combined into the sensor-augmented pump (SAP) system. The objective of this study was the evaluation of the clinical use of SAP in a large series of children with type 1 diabetes using insulin pump therapy. Methods: A questionnaire was administered in all pediatric diabetologic centers in Italy; data were analyzed only regarding patients 18 years old or younger and using SAP for 6 months or more. Results: Among all patients using an insulin pump, 129 (13.5 +/- 3.8 years old, with a disease duration of 6.3 +/- 3.4 years) have been using SAP for 1.4 +/- 0.7 years. Four hundred ninety-three patients (12.9 +/- 3.4 years old, with a disease duration of 6.2 +/- 3.3 years) using conventional insulin pump therapy for 1.7 +/- 0.5 years have been evaluated as the control group. After 0.5-3 years of using SAP or conventional insulin pump therapy, glycosylated hemoglobin significantly improved (8.0 +/- 1.5% vs. 7.4 +/- 0.8% [P = 0.002] and 8.0 +/- 1.6% vs. 7.7 +/- 1.1% [P = 0.006], respectively); the improvement was higher with SAP (P = 0.005). Insulin requirement showed a significant decrease only in SAP patients (0.88 +/- 0.25 vs. 0.7 +/- 0.23 U/kg/day, P = 0.003). Body mass index did not change during the observation period. No diabetic ketoacidosis episodes were observed during the follow-up, and severe hypoglycemia significantly decreased in SAP patients (P = 0.04). Conclusions: The increased availability of continuous glucose sensors is likely to have a significant impact on pediatric diabetes therapy and education in the near future. In daily settings, patients using SAP can achieve a better control than patients using conventional insulin pump.
