Before the introduction of continuous ambulatory peritoneal dialysis (CAPD), hydrothorax due to peritoneal dialysate was considered to be a rare complication in patients on peritoneal dialysis. In 1981, Kuehnel was the first to describe a CAPD patient with this complication,(1) and since his publication more than 20 cases have been reported. Until now there has been no agreement on the pathogenesis of this condition nor on the best therapeutic approach.
