Abstract

Aim: To develop age-, sex-, and ethnic-Appropriate normative reference ranges for standard echocardiographic measurements of the left heart by combining echocardiographic measurements obtained from adult volunteers without clinical cardiovascular disease or significant cardiovascular risk factors, from multiple studies around the world.Methods and results: The Echocardiographic Normal Ranges Meta-Analysis of the Left heart (EchoNoRMAL) collaboration was established and population-based data sets of echocardiographic measurements combined to perform an individual person data meta-Analysis. Data from 43 studies were received, representing 51 222 subjects, of which 22 404 adults aged 18-80 years were without clinical cardiovascular or renal disease, hypertension or diabetes. Quantile regression or an appropriate parametric regression method will be used to derive reference values at the 5th and 95th centile of each measurement against age. Conclusion: This unique data set represents a large, multi-ethnic cohort of subjects resident in a wide range of countries. The resultant reference ranges will have wide applicability for normative data based on age, sex, and ethnicity. © The Author 2013.

Abstract

Russia has one of the highest rates of cardiovascular disease in the world. The International Project on Cardiovascular Disease in Russia (IPCDR) was set up to understand the reasons for this. A substantial component of this study was the Know Your Heart Study devoted to characterising the nature and causes of cardiovascular disease in Russia by conducting large cross-sectional surveys in two Russian cities Novosibirsk and Arkhangelsk. The study population was 4542 men and women aged 35-69 years recruited from the general population. Fieldwork took place between 2015-18. There were two study components: 1) a baseline interview to collect information on socio-demographic characteristics and cardiovascular risk factors, usually conducted at home, and 2) a comprehensive health check at a primary care clinic which included detailed examination of the cardiovascular system. In this paper we describe in detail the rationale for, design and conduct of these studies.

Abstract

Prevalence of Candida species from samples of women population from Maputo, Mozambique. Objective: Vulvovaginal candidiasis (VVC) represents the most frequent mucocutaneous mycoses caused by yeast of the genus Candida. It is the third cause for all cases of vulvovaginitis in reproductive-aged women and, about 8% of women suffer recurrent candida vulvovaginitis. Besides the pathogenesis of symptomatic VVC is not clear and defined, some studies have been shown the increasing prevalence of azole-resistant strain of Candida albicans and, isolation of species with reduced sensibility to fluconazole as C.glabrata and C.krusei. To establish some politics to prevention and control of diseases, is necessary to identify which pathogens are circulating in that specific population. So, the presented study was performed to assess for the first time the prevalence of Candida species isolated from vaginal secretion from symptomatic women from Maputo - Mozambique.Methods: The samples were collected of women with symptoms of vulvovaginitis, attend in the Reference Center for Sexually Transmitted Infections of the Health Center May 1st to the National Institute of Health, in Maputo u2013 Mozambique. The patients are referred from the various health services as: Service of Assistance and Support to Young People (SAAJ), Prenatal Consultation, Chronic Disease Consultation (CDC), Children's Consultation at Risk (CCR), Family Planning and Screening. However, most of the patients are from the SAAJ. It is a part of National Surveillance of Sexual Transmission Infection, from Mozambique. The samples were obtained during May 2016 to September 2017 and when suspicious to VVC were incubated in Sabouraud agar plates to 37u00baC. The Candida-like growths the DNA was extracted to confirm the species by multiplex real-time PCR. To this technique was used a specific anti-sense primer based on variability in the internal transcribed spacer (ITS) region of ribosomal DNA to differentiate species. The reaction consisted a PCR mix containing SYBR green as fluorescent and running on LightCycler instrument (Roche) where Candida identification could be obtained by analyzing the different melting points of the different species. Results: In this period a total of 91 samples showed a Candida-like grothws. When Multiplex PCR was done, 68 (74,72%) were identified as C.albicans, 7 (7,69%) C.glabrata and 1 (1,09%) C.krusei. Seventeen samples showed negative results after PCR analyzes. One hypothesis is that isolates are of other genera of yeast and need to be confirmed using other primers. The average age of women was 22, ranging 15 to 39 years. Conclusion: The present work has provided recent data on the prevalence Candida species in a specific population of Maputo, and it are the first dates about this specific population. Such data is critical for establishment of therapeutic strategies and measures for prevention and control. Besides C.albicans was the most prevalent, we detected two other species that show reduced sensibility to fluconazole and usually reported in recurrence cases, which show the importance of good tools to diagnose and correct species identification.

Abstract

Prevalence of Malassezia sp isolation in body skinObjective: Pityriasis versicolor is a very common superficial mycoses caused by lipophilic yeasts of the genus Malassezia sp. This infection has been reported worldwide but is predominant in tropical climates and frequently in young adults. There are two clinical variaties, hypochromic or hyperpigmented macules, that usually develop in upper members as face, trunk, neck, and arms. In the last years, we have been isolated the yeasts in sites of the body with low density of sebaceous glands. So, the presented study was performed to asses the distribution of Malassezia sp in skin infection from 2014 to 2017. Methods: It was analyzed samples collected from January 2014 to December 2017, in a private Clinical Analysis Laboratory in the South of Brazil. The samples were collected from patients clinically suspected of skin infections and sent to the mycology sector. To Malassezia research, was prepared to direct exam with KOH 20% with Parker Quink Permanent ink, and confirm when the analyzer saw short hiphae and blastoconidia. Results: It were obtained samples from 4958 patients, where sometimes one patient had more than one site to collect. There were 214 positive samples to Malassezia sp., being 121 (56,6%) from young female. The age ranged from 1 to 84 years, being most positive samples collected from the patients with 20 years old or more, both in female and male. Fourteen patients had positive to Malassezia in two or more sites of the body. The prevalence of Malassezia sp identification did not range so much between the years, varying 3,5 to 4,7% samples per year. The most of positive samples were collected to upper members, being back (25,70%) and thorax (12,15%) more prevalent. Besides of this, it was identified yeast in other sites rich in sebaceous gland and, it was curious, some positive samples harvested in lower members as thigh (2,8%) and groin (3,74%). Conclusion: The present work provided the ocurrence of ptiriasys versicolor in young adult population from a tropical country. Besides being more prevalent in young adults, it is important to attend that can attack newborn and elderly people. As, it is important to highlight to health professional the ocurrence of yeast in no common sites of body. This ocurrence propably is related to the increase and use of oil body lotion that predisposes the yeast growth in areas with low sebaceous gland.

Abstract

The causal nature of the association between hypovitaminosis D and poor cognitive function in mid- to later-life is uncertain. Using a Mendelian randomisation(MR) approach, we examined the causal relationship between 25(OH)D and cognitive function. Data came from 172,349 participants from 17 cohorts. DHCR7(rs12785878), CYP2R1 rs12794714) and their combined synthesis score were chosen to proxy 25(OH)D. Cognitive tests were standardised into global and memory scores. Analyses were stratified by 25(OH)D tertiles, sex and age. Random effects meta-analyses assessed associations between 25(OH)D and cognitive function. Associations of serum 25(OH)D with global and memory-related cognitive function were non-linear (lower cognitive scores for both low and high 25(OH)D, pcurvature ≤ 0.006), with much of the curvature attributed to a single study. DHCR7, CYP2R1, and the synthesis score were associated with small reductions in 25(OH)D per vitamin D-decreasing allele. However, coefficients for associations with global or memory-related cognitive function were non-significant and in opposing directions for DHCR7 and CYP2R1, with no overall association observed for the synthesis score. Coefficients for the synthesis score and global and memory cognition were similar when stratified by 25(OH)D tertiles, sex and age. We found no evidence for serum 25(OH)D concentration as a causal factor for cognitive performance in mid- to later life.
