Patients, parents and survivors of childhood cancer are those who represent the critical link between the clinical and technical research spheres. Thus, UNICA4EU worked towards a patient- centric approach to integrate AI in the care pathways for childhood cancer, with evidence-based patient advocacy at its core to build trust while protecting and guaranteeing patients’ fundamental rights [1].The task “Increase Knowledge and Transparency about AI among patients, parents and survivors” was led by CCI Europe as the biggest pan-European childhood cancer parents’ and survivors’ organization which represents childhood cancer parents´ and survivors´ groups and other childhood cancer organizations and reunites 63 member-organizations in 34 countries [2].To investigate the knowledge base of AI application in pediatric oncology among those who are affected, a survey was conducted. The survey translated into nine European languages, gathered responses from 332 individuals. To delve deeper into the survey findings, discussions were held with a diverse focus group, including four parents of childhood cancer former patients (survivors), three childhood cancer survivors, and one bereaved parent, each representing different backgrounds, age groups, and countries.Insights and outcomes of this study produced a report for guiding the multi-stakeholder board of the project when defining the governance structures reg. data sharing, ownership, protection, access and usage.Perspective of parents, patients and survivors of pediatric cancer regarding AI applications in Pediatric Oncology focused in six areas of interest including: data anonymization and data protection, data ownership, data withdrawal, ethical concerns of use of data, data types and, additionally, informed consents. This paper summarizes the respective results, along concluding policy recommendations.

Background: Healthy behaviors, that is, engaging in regular physical activities, maintaining a healthy diet, limiting alcohol consumption, and avoiding tobacco and drug use, decrease the risk of developing late adverse health conditions in childhood cancer survivors. However, childhood cancer survivors may experience barriers to adopting and maintaining healthy behaviors. This study aimed to assess these barriers and facilitators to health behavior adoption and maintenance in childhood cancer survivors. Methods: A focus group (n = 12) and semi-structured telephone interviews (n = 20) were conducted with a selected sample of European and Dutch childhood cancer survivors, respectively. The Theoretical Domains Framework (TDF) was used to inform the topic guide and analysis. Inductive thematic analysis was applied to identify categories relating to barriers and facilitators of health behavior adoption and maintenance, after which they were deductively mapped onto the TDF. Results: Ten TDF domains were identified in the data of which “Knowledge,” “Beliefs about consequences,” “Environmental context and resources,” and “Social influences” were most commonly reported. Childhood cancer survivors expressed a need for knowledge on the importance of healthy behaviors, possibly provided by healthcare professionals. They indicated physical and long-term benefits of healthy behaviors, available professional support, and a supporting and health-consciously minded work and social environment to be facilitators. Barriers were mostly related to a lack of available time and an unhealthy environment. Lastly, (social) media was perceived as both a barrier and a facilitator to healthy behaviors. Conclusion: This study has identified education and available professional support in health behaviors and the relevance of healthy behaviors for childhood cancer survivors as key opportunities for stimulating health behavior adoption in childhood cancer survivors. Incorporating health behavior support and interventions for this population should therefore be a high priority.

Background Physical inactivity and unhealthy dietary habits are known to be disadvantageous for the development of late adverse effects in survivors of childhood, adolescent, and young adult cancer. To make interventions, aimed at improving lifestyle, fit into the daily life of survivors, interventions should be designed and delivered in a person-centred way with a limited time burden. As part of the European PanCareFollowUp project, an eHealth intervention was developed to support sustainable changes to physical activity levels and/or diet of childhood, adolescent, and young adult cancer survivors. This feasibility study aims to gain insight into the feasibility and potential effect sizes of the PanCareFollowUp lifestyle intervention. Methods The PanCareFollowUp lifestyle intervention consists of person-centred 3–6 screen-to-screen sessions with a certified lifestyle coach. The intervention will be evaluated with a single-arm pre-post feasibility study conducted at two survivorship care clinics in the Netherlands. A total of 60 participants who are (i) diagnosed with cancer Discussion Data of this study will be gathered to assess the feasibility and potential effect sizes. This will allow for further intervention refinement as needed as well as to inform a future large-scale intervention study and a manual for implementation at other centres. Evidence suggests that poor health behaviors further increase health risks. Healthcare professionals (HCPs) involved in survivorship care have a key role in providing health behavior support (HBS) but can feel limited in their ability to do so. This study aims to explore European HCPs perceived facilitators and barriers to providing HBS to CCSs. METHODS: Five focus groups with 30 HCPs from survivorship care clinics across Europe were conducted. Topic guides were informed by the Theoretical Domains Framework (TDF) to capture domains that may influence provision of HBS. Focus groups were analyzed with thematic analysis. Transcripts were inductively coded, after which axial coding was applied to organize codes into categories. Finally, categories were mapped onto the TDF domains. RESULTS: Nine TDF domains were identified in the data. The most commonly reported TDF domains were "Knowledge", "Skills", and "Environmental context and resources". HCPs indicated that their lack of knowledge of the association between late effects and health behaviors, besides time restrictions, were barriers to HBS. Facilitators for HBS included possession of skills needed to pass on health behavior information, good clinic organization, and an established network of HCPs. CONCLUSIONS: This study identified education and training of HCPs as key opportunities to improve HBS. Survivorship care clinics should work towards establishing well-integrated structured care with internal and external networks including HBS being part of routine care. Proper understanding of facilitators and barriers should lead to better survivorship care for CCSs.

Background: Healthy behaviors, that is, engaging in regular physical activities, maintaining a healthy diet, limiting alcohol consumption, and avoiding tobacco and drug use, decrease the risk of developing late adverse health conditions in childhood cancer survivors. However, childhood cancer survivors may experience barriers to adopting and maintaining healthy behaviors. This study aimed to assess these barriers and facilitators to health behavior adoption and maintenance in childhood cancer survivors. Methods: A focus group (n = 12) and semi‐structured telephone interviews (n = 20) were conducted with a selected sample of European and Dutch childhood cancer survivors, respectively. The Theoretical Domains Framework (TDF) was used to inform the topic guide and analysis. Inductive thematic analysis was applied to identify categories relating to barriers and facilitators of health behavior adoption and maintenance, after which they were deductively mapped onto the TDF. Results: Ten TDF domains were identified in the data of which "Knowledge," "Beliefs about consequences," "Environmental context and resources," and "Social influences" were most commonly reported. Childhood cancer survivors expressed a need for knowledge on the importance of healthy behaviors, possibly provided by healthcare professionals. They indicated physical and long‐term benefits of healthy behaviors, available professional support, and a supporting and health‐consciously minded work and social environment to be facilitators. Barriers were mostly related to a lack of available time and an unhealthy environment. Lastly, (social) media was perceived as both a barrier and a facilitator to healthy behaviors. Conclusion: This study has identified education and available professional support in health behaviors and the relevance of healthy behaviors for childhood cancer survivors as key opportunities for stimulating health behavior adoption in childhood cancer survivors. - Abstract
Despite the potential of forward osmosis (FO) and membrane distillation (MD) to treat difficult waste streams, these technologies have not yet been applied on a large scale in industry. Also, economic analyses which are crucial in a commercialization of these technologies are not presently available. In this work a techno-economic study of water recovery from anaerobic digested (AD) effluents by a FO-MD hybrid process was performed. OPEX and CAPEX calculations were used to estimate a final water treatment cost based on experimental laboratory results which demonstrated that the FO-MD process can yield a high water quality for AD effluents. However the obtained water fluxes (< 2 Lm-2h-1) were too low for industrial application in the present form of the FO-MD process. The energy consumption of the FO-MD system was 0.52 kWh m-3 which is 100 times lower than the electricity produced from 1 m3 of AD effluents based on methane energy yields. The cost of water produced by the FO-MD system was 1.29 $ m-3, assuming water flux of 10 Lm-2h-1 and membrane cost of 36 $ m-2. Sensitivity analysis shows that the cost of membrane module has a high influence on water cost at fluxes < 8 Lm-2h-1. Analysis shows that for the FO-MD system to be cost efficient, the water flux must exceed 14 Lm-2h-1. Thus our techno-economic assessment shows directions where the technology should be improved in order to be technically feasible and industrially applicable.

Die vorliegende Arbeit beschäftigt sich Carfilzomib, einem Proteasominhibitor der zweiten Generation. Carfilzomib bindet irreversibel an das Proteasom, hemmt dadurch seine Funktion des Proteinabbaus und löst dadurch eine Reihe von bisher noch nicht vollständig verstandenen zellulären Reaktionen aus, die zu einer Induktion von Apoptose in MM-Zellen führen. Einer der Signaltransduktionswege, die durch Carfilzomib induziert werden, ist der JNK-Signaltransduktionsweg, der unter anderem bei zellulärem Stress in die Entscheidung zwischen Zellprotektion und Apoptose eingebunden ist. Welche Umstände welche Wege triggern, und wie genau dieser Prozess reguliert wird, ist bisher noch weitgehend unklar. Da Carfilzomib zu einer Aktivierung des JNK-Signaltransduktionsweg führt und Apoptose induziert, lag die Frage nach einem möglichen Zusammenhang zwischen beiden Mechanismen nahe. Die vorliegende Arbeit überprüft die Hypothese, dass der JNK-Signaltransduktionsweg entscheidend zur Vermittlung der apoptotischen Effekte von Carfilzomib beiträgt. Die Ergebnisse einer ersten Versuchsreihe schienen die Hypothese zu bestätigen, da wir bei einer artifiziellen, sehr starken Aktivierung des JNK-Signaltransduktionsweges durch Anisomycin eine Verstärkung des durch Carfilzomib-induzierten apoptotischen Effektes beobachten konnten. Weitere Versuche, die sich auf die durch Carfilzomib induzierte Aktivierung des JNK-Signaltransduktionsweges fokussierten, konnten die Annahme in unserer Hypothese klar widerlegen, indem sie zeigten, dass die JNK-vermittelte Phosphorylierung von c-jun an Serin 63 und Serin 73 für die Carfilzomib-induzierte Apoptose nicht notwendig ist. Im Gegenteil, bei Inhibition des JNK-Signaltransduktionsweges waren die apoptotischen Effekte durch Carfilzomib sogar stärker ausgeprägt als durch Carfilzomib-Behandlung allein. Die diesem Effekt zugrundeliegenden molekularen Mechanismen ließen sich im Rahmen der Arbeit nicht aufklären. Eine mögliche Erklärung wäre, dass die reaktive Aktivierung des JNK-Signaltransduktionsweges unter dem zellulären Stresszustand, den die Proteasominhibition auslöst, eher eine zellprotektive Wirkung zeigt. Der Wegfall dieser protektiven Wirkung vermag die erhöhte Apoptoserate bei JNK-Inhibition zu erklären. Meine Arbeit unterstreicht dabei den weiterzuverfolgenden Ansatz, die zellprotektiven Mechanismen unter Carfilzomib-Behandlung besser zu verstehen., The present work analyzes the second generation proteasome inhibitor Carfilzomib, a substance that irreversibly binds to the proteasome, inhibiting its protein degradation function and thereby triggering a series of, as yet not fully understood, cellular responses leading to induction of apoptosis in MM cells. One of the signal transduction pathways induced by Carfilzomib is the JNK signal transduction pathway, which has been implicated in the decision pathway between cell protection and apoptosis during cellular stress. The specific circumstances that trigger these pathways and the exact processes for regulation remain largely unclear. As Carfilzomib leads to activation of the JNK signal transduction pathway and induces apoptosis, a correlation between these mechanisms has been postulated. Therefore, the present work tests the hypothesis of whether the JNK signal transduction pathway is crucial in mediating the apoptotic effects of carfilzomib. The results of an initial series of experiments confirmed the hypothesis, as the apoptotic effect induced by carfilzomib was enhanced when the JNK signal transduction pathway was artificially activated to a very high degree by Anisomycin. However, further experiments that focused on Carfilzomib-induced activation of the JNK signal transduction pathway did not support the hypothesis and showed that JNK-mediated phosphorylation of c-jun at serine 63 and serine 73 was not required for carfilzomib-induced apoptosis. In contrast, when the JNK signal transduction pathway was inhibited, the apoptotic effects were even more pronounced by Carfilzomib than by Carfilzomib treatment alone. The molecular mechanisms underlying this effect could not be elucidated within the scope of this work. However, one possible explanation for these results could be that reactive activation of the JNK signal transduction pathway tends to exhibit a cell-protective effect under the cellular stress state induced by proteasome inhibition. The removal of this protective effect may explain the increased rate of apoptosis on JNK inhibition. In this regard, this work underscores the ongoing efforts to understand the cell-protective mechanisms that are impacted by Carfilzomib treatment.

Background: Childhood cancer survivors (CCSs) have an increased risk of developing chronic health conditions. Evidence suggests that poor health behaviors further increase health risks. Healthcare professionals (HCPs) involved in survivorship care have a key role in providing health behavior support (HBS) but can feel limited in their ability to do so. This study aims to explore European HCPs perceived facilitators and barriers to providing HBS to CCSs. Methods: Five focus groups with 30 HCPs from survivorship care clinics across Europe were conducted. Topic guides were informed by the Theoretical Domains Framework (TDF) to capture domains that may influence provision of HBS. Focus groups were analyzed with thematic analysis. Transcripts were inductively coded, after which axial coding was applied to organize codes into categories. Finally, categories were mapped onto the TDF domains. Results: Nine TDF domains were identified in the data. The most commonly reported TDF domains were "Knowledge", "Skills", and "Environmental context and resources". HCPs indicated that their lack of knowledge of the association between late effects and health behaviors, besides time restrictions, were barriers to HBS. Facilitators for HBS included possession of skills needed to pass on health behavior information, good clinic organization, and an established network of HCPs. Conclusions: This study identified education and training of HCPs as key opportunities to improve HBS. Survivorship care clinics should work towards establishing well‐integrated structured care with internal and external networks including HBS being part of routine care. Proper understanding of facilitators and barriers should lead to better survivorship care for CCSs. [ABSTRACT FROM AUTHOR]

- Abstract
Since most of the water used for brewing becomes wastewater, the reduction of wastewater streams is of great importance for the brewing industry. Anaerobic membrane bioreactors incorporating forward osmosis membranes (FO-AnMBRs) could provide an energy-efficient solution for the treatment of brewery wastewater, while recovering water and simultaneously producing biogas. The start-up period of two lab-scale FO-AnMBRs fed with real (Rreal) and synthetic (Rsynth) brewery wastewater was assessed and compared to continuous stirred tank reactor operation without membranes. During reactor start-up, the hydraulic retention time (HRT) of the reactors was reduced from 20 to 2.5 days while increasing the organic loading rate. The CH4 production of Rsynth and Rreal increased up to nine-fold when the membranes were integrated, demonstrating the principle that FO-AnMBR can be used for brewery wastewater treatment. However, process stability decreased with decreasing HRT, possibly due to shear stress and the inadequate C/N ratio of the wastewater.

49. Novel biomimetic membrane technologies for the application in water treatment and energy production

- Abstract
I lyset af globalt vandmangel og øgede vand- og energibehov, er der behov for at finde nye metoder til at levere rent og sikkert vand og vedvarende energikilder. Dette behov påvirker ikke kun udviklingslande, men også industrialiserede lande.Hvor spildevand førhen er blevet opfattet som et affaldsprodukt, har opfattelsen ændret sig til at se spildevand som en kilde til vand, energi og værdifulde stoffer.Membranteknologi kan benyttes til at genindvinde vand fra kommunalt og industrielt spildevand. Osmose, herefter benævnt forward osmosis (FO), har i de seneste år oplevet øget interesse som en ny membranteknologi, takket være dens evne til effektivt at tilbageholde patogener og næsten alle opløste stoffer i spildevandet. Når FO integreres i hybride systemer, for eksempel i kombination med omvendt osmose, herefter benævnt reverse osmosis (RO), kan rent vand af høj kvalitet udvindes. FO kan også kombineres med anaerobe membranbioreaktorer, for at kombinere rensning af spildevand med produktion af biogas, som er en vedvarende energikilde.Det overordnede formål med denne afhandling er at vurdere biomimetiske FO-membraners anvendelse til genindvinding af vand og produktion af biogas. Målene er at 1) undersøge FO-membraners ydeevne i form af vandgennemstrømning og tilbageholdelse af ammoniumkvælstof, orthofosfat og COD (chemical oxygen demand), både i FO alene og i FO kombineret med RO, 2) at analysere tilstopningsmekanismer og rensningseffektivitet, og 3) undersøge processens ydeevne i form af vandgennemstrømning og biogasproduktion, når FO-membranen er integreret i en anaerob membranbioreaktor.Resultaterne viser at et bredt udvalg af spildevand, fra gødning til mikrobisk biomasse kan behandles med FO, samt at høj tilbageholdelse af ammoniumkvælstof, orthofosfat og COD kan opnås. De vigtigste tilstopningsmekanismer var organisk tilstopning og mikrobiologisk tilstopning. Anvendelse af FO kombineret med RO resulterede i endnu højere tilbageholdelse af næringsstoffer. Kemisk rensning af membranen genetablerede 75% vandgennemstrømning, sammenlignet med en ubrugt membran.Ved integration af FO i en anaerob membranbioreaktor, øgedes produktionen af metan sammenlignet med opstartsfasen uden brug af FO-membranen, hvilket indikerer at en øget organisk ladningsrate kan opnås ved hjælp af FO-membranen. Bioreaktorens ydeevne forblev imidlertid ustabil, hvilket kan indikere celleskader som følge af turbulens eller utilstrækkeligt C/N-forhold i bryggerispildevandet.Tilsammen viser disse resultater muligheden for samtidig genindvinding af vand fra spildevand og produktion af biogas, gennem anvendelse af biomimetiske FO-membraner. Imidlertid er der behov for yderligere arbejde for at optimere processerne og opnå en dybere forståelse omkring tilstopning og rengøring af membraner, samt forbehandling af spildevand og optimering af driftsbetingelser i osmotiske anaerobe membranbioreaktorer. In the light of global water scarcity and increasing water and energy demand, new ways to provide safe and clean water and renewable energy sources have to be found. This topic does not only affect developing countries, but affects industrialized countries as well.The perception of wastewater has changed in recent years, from being seen as waste to being seen as a resource for water, energy and valuable compounds.Membrane technology can be used to reclaim water from municipal and industrial wastewater. In recent years, forward osmosis (FO) has raised increasing interest as an emerging membrane technology, because of its ability to efficiently reject pathogens and almost all solutes in the wastewater. When forward osmosis is integrated into hybrid systems, for example in combination with reverse osmosis (RO), high-quality water can be recovered. FO can also be combined with anaerobic membrane bioreactors (FO-AnMBR) in order to simultaneously treat wastewater and produce biogas, which is a renewable energy source.The overall aim of this thesis is to evaluate biomimetic FO membranes for water recovery and biogas production. The goals are 1) to investigate the FO membrane performance in terms of water flux and nutrient rejection, both in stand-alone FO as well as in combination with RO 2) to analyse the fouling mechanisms and cleaning efficiency and 3) investigate process performance in terms of water flux and biogas production in FO-AnMBRs.The results show that a wide variety of wastewaters, ranging from anaerobically digested manure to microbial biomass, can be treated with FO, and high rejection of nutrients (ammonia, orthophosphate, chemical oxygen demand (COD)) can be achieved. The predominant fouling mechanisms in these cases were organic fouling, as well as biofouling. When FO was combined with RO, the double-barrier mechanisms achieved very high nutrient removal. Chemical cleaning was able to restore 75% of the initial water flux of the pristine membrane.When FO was integrated into an anaerobic membrane bioreactor, it was found that the methane production rate increased in comparison to the membrane-less start-up phase, indicating that a higher organic loading rate can be achieved due to the membrane. However, bioreactor performance in terms of methane production yield and rate remained unstable, possibly indicating shear stress or inadequate C/N ratio of the brewery wastewater.Taken together, these results show the feasibility of water recovery from wastewater and simultaneous biogas production, using biomimetic FO membranes. However, further work will be required in the future to optimise the processes and gain a deeper understanding with regards to fouling, cleaning and pretreatment strategies, as well as the optimisation of operational conditions in osmotic anaerobic membrane bioreactors.

50. Design of a water reuse network in an industrial site in Kenya

- Abstract
Sustainable growth of industries in fast developing but water-scarce countries like Kenya can only be achieved by optimizing water usage and reducing wastewater discharge. Besides internal recycling, industries can greatly reduce their freshwater intake by exchanging wastewater streams with neighboring industries and creating symbiosis relations. Previous studies have attempted to design an optimal network of water exchange streams in industrial parks by applying a global multi-objective optimization approach (e.g., [1]). However, only a few have considered the complexity of used water streams in terms of flow rate variation [2] and number of significant pollutants [3]. In this study, we present the outcome of the optimization analysis, covering the above-mentioned knowledge gaps, for water exchange between 10 companies in an industrial park in Ruaraka, Nairobi. A combination of clarification, physico-chemical, membrane-based (Inside® by Aquaporin A/S, Denmark) and disinfection treatments was considered in the optimization exercise as decentralized fit-for-purpose treatment units. Information on volumes and quality of water usage and wastewater discharge was gathered through on-site visits and interviews. The analysis forms part of a research project, Gecko (Green and Circular Innovation for Kenyan Companies), investigating circular solutions within water, materials and energy by specifically focusing on wastewater reuse and targeting industrial symbiosis potentials. The formulation of the water reuse network is shown in the superstructure (an example for three processes) in Figure 1 using a series of multiple treatment units prior to reuse.
