- Abstract
The neurobiological bases of the association between development and psychopathology remain poorly understood. Here, we identify a shared spatial pattern of cortical thickness (CT) in normative development and several psychiatric and neurological disorders. Principal component analysis (PCA) was applied to CT of 68 regions in the Desikan-Killiany atlas derived from three large-scale datasets comprising a total of 41,075 neurotypical participants. PCA produced a spatially broad first principal component (PC1) that was reproducible across datasets. Then PC1 derived from healthy adult participants was compared to the pattern of CT differences associated with psychiatric and neurological disorders comprising a total of 14,886 cases and 20,962 controls from seven ENIGMA disease-related working groups, normative maturation and aging comprising a total of 17,697 scans from the ABCD Study® and the IMAGEN developmental study, and 17,075 participants from the ENIGMA Lifespan working group, as well as gene expression maps from the Allen Human Brain Atlas. Results revealed substantial spatial correspondences between PC1 and widespread lower CT observed in numerous psychiatric disorders. Moreover, the PC1 pattern was also correlated with the spatial pattern of normative maturation and aging. The transcriptional analysis identified a set of genes including KCNA2, KCNS1 and KCNS2 with expression patterns closely related to the spatial pattern of PC1. The gene category enrichment analysis indicated that the transcriptional correlations of PC1 were enriched to multiple gene ontology categories and were specifically over-represented starting at late childhood, coinciding with the onset of significant cortical maturation and emergence of psychopathology during the prepubertal-to-pubertal transition. - Abstract
INTRODUCTION: There are growing concerns about commonly inflated effect sizes in small neuroimaging studies, yet no study has addressed recalibrating effect size estimates for small samples. To tackle this issue, we propose a hierarchical Bayesian model to adjust the magnitude of single-study effect sizes while incorporating a tailored estimation of sampling variance. METHODS: We estimated the effect sizes of case-control differences on brain structural features between individuals who were dependent on alcohol, nicotine, cocaine, methamphetamine, or cannabis and non-dependent participants for 21 individual studies (Total cases: 903; Total controls: 996). Then, the study-specific effect sizes were modeled using a hierarchical Bayesian approach in which the parameters of the study-specific effect size distributions were sampled from a higher-order overarching distribution. The posterior distribution of the overarching and study-specific parameters was approximated using the Gibbs sampling method. RESULTS: The results showed shrinkage of the posterior distribution of the study-specific estimates toward the overarching estimates given the original effect sizes observed in individual studies. Differences between the original effect sizes (i.e., Cohen's d) and the point estimate of the posterior distribution ranged from 0 to 0.97. The magnitude of adjustment was negatively correlated with the sample size (r = -0.27, p < 0.001) and positively correlated with empirically estimated sampling variance (r = 0.40, p < 0.001), suggesting studies with smaller samples and larger sampling variance tended to have greater adjustments. DISCUSSION: Our findings demonstrate the utility of the hierarchical Bayesian model in recalibrating single-study effect sizes using information from similar studies. Item does not contain fulltext, Background: Nicotine and illicit stimulants are very addictive substances. Although associations between grey matter and dependence on stimulants have been frequently reported, white matter correlates have received less attention. Methods: Eleven international sites ascribed to the ENIGMA-Addiction consortium contributed data from individuals with dependence on cocaine (n = 147), methamphetamine (n = 132) and nicotine (n = 189), as well as non-dependent controls (n = 333). We compared the fractional anisotropy (FA), axial diffusivity (AD), radial diffusivity (RD) and mean diffusivity (MD) of 20 bilateral tracts. Also, we compared the performance of various machine learning algorithms in deriving brain-based classifications on stimulant dependence. Results: The cocaine and methamphetamine groups had lower regional FA and higher RD in several association, commissural, and projection white matter tracts. The methamphetamine dependent group additionally showed lower regional AD. The nicotine group had lower FA and higher RD limited to the anterior limb of the internal capsule. The best performing machine learning algorithm was the support vector machine (SVM). The SVM successfully classified individuals with dependence on cocaine (AUC = 0.70, p < 0.001) and methamphetamine (AUC = 0.71, p < 0.001) relative to non-dependent controls. Classifications related to nicotine dependence proved modest (AUC = 0.62, p = 0.014). Conclusions: Stimulant dependence was related to FA disturbances within tracts consistent with a role in addiction. The multivariate pattern of white matter differences proved sufficient to identify individuals with stimulant dependence, particularly for cocaine and methamphetamine.

Previous studies have shown that substance dependence is associated with altered brain structure and function. However, it is unknown whether structural brain asymmetries are different in individuals with substance dependence compared with nondependent participants. Here, a mega-analysis was performed using a collection of 22 structural brain MRI datasets from the ENIGMA Addiction Working Group. Structural asymmetries of cortical and subcortical regions were compared between individuals who were dependent on alcohol, nicotine, cocaine, methamphetamine, or cannabis (n = 1,796) and nondependent participants (n = 996). Substance-general and substance-specific effects on structural asymmetry were examined using separate models. We found that substance dependence was significantly associated with differences in volume asymmetry of the nucleus accumbens (NAcc; less rightward; Cohen's d = 0.15). This effect was driven by differences from controls in individuals with alcohol dependence (less rightward; Cohen's d = 0.10) and nicotine dependence (less rightward; Cohen's d = 0.11). These findings suggest that disrupted structural asymmetry in the NAcc may be a characteristic of substance dependence.

We found that substance dependence was significantly associated with differences in volume asymmetry of the nucleus accumbens (NAcc; less rightward; Cohen's d = 0.15). This effect was driven by differences from controls in individuals with alcohol dependence (less rightward; Cohen's d = 0.10) and nicotine dependence (less rightward; Cohen's d = 0.11). These findings suggest that disrupted structural asymmetry in the NAcc may be a characteristic of substance dependence.

Long persisting scaphoid non-unions or scapholunate ligament ruptures can lead to carpal collapse. The resulting clinical symptoms are restrictions in the range of motion, pain, and loss of grip strength. The symptomatic treatment so far offers different options. In our study, the Variable Angle Locking Compression Intercarpal Fusion Plate (VA LCP ICF Plate) by Synthes® was used in 11 cases of advanced carpal collapse for a four-corner fusion of the wrist. The intra- and postoperative as well as follow-up results have been assessed and compared with those of current literature. The results of the Manchester-Modified Disability of the Shoulder, Arm and Hand Score (M²-Dash) showed an average of 41.5 points (MD=44/SD=16.62/MIN=21/MAX=65). One of the re-evaluated patients complained about pain at rest. One patient stated pain after mild strain; 4 patients complained pain after heavy burden (e.g. boxing, weight lifting). Measuring the range of motion, the operated hand showed a maximum in dorsal extension of 78.31% and in flexion of 57.89% compared to the contralateral, non-operated hand. In performance testing the fist clenching sign as well as pinch grip were complete and void of pain in 100%, whereas opposition (dig. man. I to V) was complete in five patients (83.33%), with moderate pain in one patient (16.67%) and a persisting gap of 0.2 cm in n=1 (16.67%). In comparison with the current literature regarding healing rates, complications, and follow-up results, we recommend the Synthes® VA LCP ICF Plate as a good surgical option in patients suffering from advanced carpal collapse., Lang bestehende Skaphoidpseudarthrosen oder skapholunäre Bandrupturen können zu einem karpalen Kollaps führen. Symptome dessen sind unter anderem Bewegungseinschränkung, belastungsabhängige Schmerzen und Kraftreduktion. In der aktuellen Literatur sind diverse unterschiedliche Prozeduren und therapeutische Optionen für dieses Krankheitsbild beschrieben. 20. Hamate’s coronal fracture: diagnostic and therapeutic approaches based on a long-term follow-up

Hamate fractures are commonly divided into hook fractures and body fractures. The coronal fractures as a special form of hamate’s body fracture are very rare injuries. Because of unspecific clinical findings and the mostly inconclusive x-ray imaging, these fractures are frequently overseen or misdiagnosed. This leads to further complications like secondary arthritis, persisting pain, and functional deficits in patient’s wrist mobility. In our study, a collocation of coronal hamate fractures is analyzed and evaluated with respect to functional outcome after operative treatment and compared to the literature. Furthermore, we compare the strategies for diagnosis and treatment in our clinical center with those presented in the literature. Our standard in the initial diagnostic process is to obtain radiographs in an anterior-posterior, lateral, and 30° oblique view of the wrist. For further diagnosis and preoperative planning, a CT scan of the wrist is obligatory. Due to the high occurrence of comorbidities (especially CMC dislocations) all patients in our cohort obtained operative treatment. In long-term post-operative evaluation, we present the following results: The Manchester-Modified Disability of the Shoulder, Arm and Hand Score (M2 DASH) imposed with an average of 26.22 points (MD=22/ SD=11.31/MIN=18/MAX=52). None of the re-evaluated patients sorrowed for severe pain in rest. Four patients stated pain (ranging from 3 to 5 on numeric analogue scale) after heavy burden (e.g. boxing, weight lifting). In exploring the range of motion of the operated hand the following results are obtained: dorsal extension: average 83.33° (MD=85°/SD=3.54°/MIN=75°/MAX=85°), flexion: average 77.78° (MD=80°/SD=4.41°/MIN=70°/MAX=80°). Additionally, a performance testing was conducted: fist clenching sign: complete without pain in 100%, pinch grip: complete in 77.78%, opposition digitus manus I–V complete in 66.67%. The conservative treatment is not recommended (especially shown in the here presented “add” case with a misdiagnosed fracture). The open approach has its advantages compared to a closed operative procedure and should always be intraoperatively considered as an operative expansion., Hamatumfrakturen werden in Hamulus-ossis-hamati- und Corpusfrakturen unterteilt. Die coronare Os hamatum Fraktur ist, als Sonderform der Corpusfraktur, eine sehr seltene Verletzung. Aufgrund der unspezifischen klinischen Symptomatik und unsicheren projektionsradiografischen Diagnostik wird dieser Frakturtyp in der Primärdiagnostik häufig übersehen oder fehldiagnostiziert. Dies führt zu möglichen Folgekomplikationen, wie posttraumatischer Arthrose, persistierender Schmerzzustände und funktioneller Defizite. In der vorliegenden Studie werden die an unserem Zentrum behandelten coronaren Hamatumfrakturen zusammengestellt, anhand des funktionellen Ergebnisses nach operativer Versorgung ausgewertet und mit der aktuellen Literatur verglichen. Weiterhin wird der diagnostische und therapeutische Algorithmus unseres Zentrums dem publizierten Standard gegenübergestellt. Standdarddiagnostikum sind Röntgenbilder des Handgelenks in drei Ebenen (anterior-posterior, streng seitlich, 30° schräg – Aufnahme). Zur weiteren Diagnostik und insbesondere präoperativen Planung ist die Computertomografie des Handgelenks obligat. Aufgrund des häufigen Vorhandenseins von Begleitverletzungen (insbesondere carpometacapale Dislokationen) wurden alle Patienten operativ therapiert. Im Rahmen der klinischen Langzeitevaluation zeigten sich folgende Resultate: Der Manchester-Modified Disability of the Shoulder, Arm and Hand Score (M2 Dash) lag im Durschnitt bei 26,22 Punkten (MD=22 / SD=11,31 / MIN=18 / MAX=52). Keiner der nachuntersuchten Patienten klagte über starke Ruheschmerzen. Vier Patienten gaben Schmerzen von 3–5 auf der numerischen Analogskala nach stärkerer Belastung (beispielsweise Boxen, Anheben von Lasten) an. Bezüglich des Range of Motion zeigten sich folgende Ergebnisse: Dosalextension: Durchschnitt 83,33° (MD=85° / SD=3,54° / MIN=75° / MAX=85°), Flexion: Durchschnitt 77,78° (MD=80° / SD=4,41° / MIN=70° / MAX=80°). In der Funktionsprüfung zeigten sich: Faustschluss vollständig und schmerzfrei in 100%, Pinzettengriff vollständig in 77,78% und Opposition Daumen–Kleinfinger vollständig in 66,67%. Insbesondere aufgrund des hier gezeigten fehldiagnostizierten Falles („add“ case) kann das konservative Vorgehen nicht empfohlen werden. 22. A new variant of scaphoid reconstruction: Treatment of scaphoid non-union with avascular bone interponate and high compression screw (Synthes(®))

Scaphoid fractures as frequently overseen injuries often result in scaphoid non-unions, that need to be treated to prevent carpal collapse and secondary cartilage damage. Vital bone tissue and compression of fracture and bone graft ends seem to be crucial in for ossification and final bone healing. In the present study we compare our results using a high compression screw (HCS Synthes®) to results in the literature using different kinds of internal fixation including compression screws of various types. We present 22 patients with scaphoid non-unions treated with a bone graft and a HCS Synthes®. We evaluated our post-operative results. The Manchester-Modified Disability of the Shoulder, Arm and Hand–Score (M-Dash) imposed with an average of 29.8 points (MD=29 / SD=9.46 / MIN=18 / MAX=48). None of the re-evaluated patients sorrowed for pain in rest. Five patients stated pain (ranging from 4 to 8 on numeric analogue scale) after heavy burden (e.g. boxing, weight lifting).In exploring the range of motion of the operated hand we deliver the following results: dorsal extension: average 72.73° (MD=80° / SD=17.23° / MIN=30° / MAX=85°), flexion: average 73.64° (MD=80° / SD=8.97° / MIN=60° / MAX=80°), ulnar deviation: average 39.09°, (MD=40° / SD=2.02° / MIN=35° / MAX=40°), radial deviation: average 29.09°, (MD=30° / SD=3.01° / MIN=20° / MAX=30°). Additionally a performance testing was conducted: fist clenching sign: complete without pain in 100%, pinch grip: complete in 100%, moderate pain in n=1 (8.33%), opposition digitus manus I–V complete in 100%, moderate pain n=2 (16.67%). Three patients with persisting fracture gap had a scaphoid bone fractured in the proximal third; one patient even with a very small proximal fragment. One persisting non-union was localized in the middle third (period between injury and operation = 5 years). In conclusion, our patients showed better healing rates compared to results presented in the literature. Non-unions localized in the proximal third of the scaphoid did not seem to benefit using this technique., Kahnbeinbrüche werden häufig übersehen und führen zu Kahnbeinpseudarthrosen, die einer Therapie bedürfen, um den karpalen Kollaps und sekundären Knorpelschaden zu verhindern. In der vorliegenden Studie vergleichen wir unsere Ergebnisse in der Pseudarthrosenversorgung mittels Knochenspan und der Hochkompressionschraube der Firma Synthes® (HCS Synthes®) mit anderen Studien aus der Literatur. Wir stellen die Nachuntersuchungsergebnisse von 22 Patienten vor. Der Manchester-Modified Disability of the Shoulder, Arm and Hand–Score (M-Dash) lieferte durchschnittlich 29,8 Punkte (MD=29 / SD=9,46 / MIN=18 / MAX=48). Keiner der Patienten gab Ruheschmerzen an. Fünf Patienten beklagten Schmerzen in der Höhe von 4 bis 8 auf der numerischen Schmerzskala nach starker Belastung wie z.B. Boxen und Gewichtheben. Der Bewegungsumfang betrug an der betroffenen Hand: Extension: Mittelwert 72,73° (MD=80° / SD=17,23° / MIN=30° / MAX=85°), Flexion: Mittelwert 73,64° (MD=80° / SD=8,97° / MIN=60° / MAX=80°), Ulnarduktion: Mittelwert 39,09°, (MD=40° / SD=2,02° / MIN=35° / MAX=40°), Radialduktion: Mittelwert 29,09°, (MD=30° / SD=3,01° / MIN=20° / MAX=30°). Weitere Testergebnisse: Fist clenching sign: komplett ohne Schmerzen in 100%, Pinch grip: komplett in 100%, mäßige Schmerzen in n=1 (8,33%), Opposition digitus manus I–V komplett in 100%, mäßige Schmerzen n=2 (16,67%). Drei Patienten mit persistierendem Pseudarthrosenspalt hatten eine originäre Fraktur im proximalen Drittel, eine persistierende Pseudarthrose lag im mittleren Drittel bei einem Alter der initialen Pseudarthrose von 5 Jahren. Zusammenfassend zeigten unsere Patienten eine bessere knöcherne Heilungsrate als die Patienten der in der Literatur beschriebenen Studien. 23. Erweiterte Ergebnisanalyse der endonasalen Nasennebenhöhlenchirurgie

Hintergrund: Die endonasale Nasennebenhohlenchirurgie hat sich zur chirurgischen Behandlung der chronischen Sinusitis durchgesetzt. Eine Effizienz-Analyse der Eingriffe ist nur eingeschrankt moglich, hilfsweise werden oft psychometrische Verfahren zur Messung der Lebensqualitat herangezogen. Deren Stellenwert ist nicht gesichert. Methode: Wir haben prospektiv 103 Patienten vor und nach einem Nebenhohleneingriff ausfuhrlich zur Lebensqualitat befragt. Es kamen unterschiedliche Fragebogen mit zusammengenommen 124 Fragen zur Anwendung. Ergebnisse: Die psychometrischen Veranderungen in den Einzelfragen waren stets konkordant. Im Bereich der Lebensqualitat (SF-36-Bogen) liesen sich insbesondere die „korperlichen Schmerzen” und „Vitalitat” postoperativ signifikant bessern. Die Ergebnisse der aufwandigen psychometrischen Befragung lagen stets in Ubereinstimmung mit einer traditionellen, globalen Einschatzung zum Operationserfolg. Schlussfolgerung: Eine Verwendung der Fragebogensysteme fur die Qualitatssicherung oder fur Fehlbelegungsanalysen ist unter Kosten-Nutzen-Gesichtspunkten nicht anzuraten. Background: Endonasal optically guided surgery has gained wide acceptance for treatment of chronic sinusitis. However, irrespective to the advent of minimally-invasive techniques, standard outcome assessment has not always reflected the increased technical and conceptual efforts. „Psychometric” instruments offer additional information on patients' perceptions of health. We analyzed the practical value of an extended outcome assessment. Methods: Standardized interviews were carried out with 103 patients pre- and postoperatively applying several psychometrical instruments („SF-36” and other valid general health-status instruments besides disease-specific questionnaires, providing 124 items/questions altogether). Results: The diverse changes of subtle health status subscores as a rule paralleled each other and were concordant with traditional self-estimated global ratings of the surgical success by the patient. Significant postoperative changes of the „SF-36” questionnaire were especially seen in the subscale „bodily pain” and „level of energy”. With few exceptions only, items reflecting any indirect burden of chronic sinusitis revealed less pronounced improvements postoperatively. Conclusion: It appears legitimate to question the special benefits of extended psychometric outcome research in comparison to traditional global ratings for daily surgical practice.

Розглянута розроблена авторами апаратура, що використовує чутливий елемент на анізотропних напівпровідникових кристалах. Вимірювання здійснюються дистанційно, безконтактним способом для оцінки радіаційних теплових потоків біологічних та промислових об’єктів у статичному та (або) динамічному режимах, в залежності від специфіки об’єкта та умов досліджень і можливої при обстеженні пацієнтів у післяопераційній палаті а також інфікованих та запалених поверхонь тіла методики вимірювань. Запропонована методика є практично єдино можливою при дослідженні післяопераційних полів, інфікованих та запалених поверхонь тіла людини. Точність вимірювань температури складає 0,05°С. Developed by the authors equipment using the sensor on the anisotropic semiconductor crystals is examined. The measurements are performed remotely, in a contactless manner to assess the radiative heat fluxes of biological and industrial objects in static and (or) dynamic modes, depending on the specifics of the object, the conditions of research and possible methods of measurement. The proposed method of measurement is only possible in the study of post-operative fields, infected and inflamed surfaces of the body of an organism. Temperature accuracy was 0,05° C. Developed a special computer program automatically processes the measured signals and identifies the information received with standard graphs corresponding to the normal functioning of the body parameters. The causal interrelation of CNS degeneration and adaptive immunityis assumed as a critical pathogenic element influencing the clinicalphenotype in neuroinflammatory as well as in neurodegenerativedisorders. We have recently shown that primary oligodendrocyticdamage (by overexpression of PLP) leads to a low-grade inflammatoryreaction of high pathological impact in the CNS, potentially reflectingpathogenic principles in some forms of multiple sclerosis. Asa completely novel finding, these reactions were mediated by CD8+T cells whereas CD4+ cells were not necessary for the pathologicaleffects of the immune system. In order to yield further insights intopathogenic relevance and nature of the adaptive immune responsein myelin mutated mice (PLP transgenic mice), we here apply a detailedimmunological characterization of systemic and CNS-specificCD8+ T cell responses in PLP transgenic mice and aged wild typemice. We provide evidence that T effector cells are surveying the CNSof PLP transgenic and wild-type mice. These cells accumulate withage and show a higher level of activation in PLP transgenic mice.Exclusively in PLP transgenic mice, T cells show clonal expansions,as demonstrated by T cell receptor CDR3-spectratype analysis. Althoughwe could not find specific responses of CD8+ T cells againstmyelin-antigen-derived peptides, V-beta/J-beta similarities stronglysuggest specificity against a common antigen. Our data suggest thata primary degenerative CNS disorder, exemplified by PLP-transgenicmice, can be associated with secondary expansions of pathogenicallyrelevant CD8+ T cells in the CNS. These findings have implicationsfor our understanding of the role of secondary adaptive immunereactions in the context of neurodegenerative and neuroinflammatorydiseases.

A.B. is a 32-year-old Caucasian man with a significant, 12-year medical history of diabetes mellitus type I as well as a history of hypertension and hypercholesterolemia. He presented to his primary care doctor with blurred vision and was referred to a retinal specialist, who diagnosed hemorhagic and leukemic retinopathy resulting in limited vision. A complete blood count was drawn during the visit with his retinal specialist. The results were abnormal, revealing a white blood cell count of 30,000/mm3, platelet count of 70,000/mm3, and a hemoglobin of 12.2 gm/dl. The peripheral smear showed peripheral blasts. A.B. was referred to a hematologist, who performed a bone marrow aspiration, confirming the diagnosis of pre-B-cell acute lymphocytic leukemia (ALL). Cytogenetic studies revealed positivity for the Philadelphia chromosome and a translocation of genes 4 and 11. [ABSTRACT FROM AUTHOR]
