Abstract

Results are reported from laboratory continuous-flow plasma-discharge experiments designed to simulate the formation of hydrocarbons and nitriles from N2 and CH4 in the atmosphere of Titan. Gas-chromatography and mass-spectrometry data were obtained in experiments lasting up to 100 h at temperature 295 K and pressure 17 or 0.24 mbar, modeling (1) cosmic-ray-induced processes in the Titan troposphere and (2) processes related to stratospheric aurorae excited by energetic electrons and ions from the Saturn magnetosphere, respectively. The results are presented in extensive tables and graphs, and the 0.24-mbar yields are incorporated into an eddy-mixing model to give stratospheric column abundances and mole fractions in good agreement with Voyager IRIS observations.

16. Bullous Eruption and Elevated Leukocyte Alkaline Phosphatase in the Course of Busulfan-Treated Chronic Granulocytic Leukemia

Abstract

A 35-year-old man presented with a bullous eruption similar to erythema multiforme following the administration of high doses of busulfan therapy for chronic granulocytic leukemia. There was pancytopenia, hypoplasia of the bone marrow, elevation of serum vitamin B12 and B12 binding capacity and a paradoxical elevation of the leukocyte alkaline phosphatase level. Discontinuation of the busulfan and the administration of prednisone were followed by gradual healing of the skin lesions, reversal of the pancytopenia and reduction of the leukocyte alkaline phosphatase activity to zero. This case illustrates an unusual and previously unreported untoward response to busulfan and is another example of the capacity of the leukocytes in some patients with chronic granulocytic leukemia to develop high alkaline phosphatase activity in response to inflammation.

17. Appendectomy in Hemophilia With the Use of Cryoprecipitated Human Factor VIII to Control Bleeding

Abstract

Appendectomy was safely performed in a 9-year-old boy with classic hemophilia, and hemostasis was achieved by the use of cryoprecipitated human antihemophilic globulin therapy begun preoperatively and continued for 17 days. Except for urticaria, the use of cryoprecipitated factor VIII was entirely free of side effects. Cryoprecipitated factor VIII is unique among all factor VIII concentrates prepared heretofore in that it is a by-product of plasma fractionation. It may be simply and economically prepared in any blood bank geared to plasma fractionation procedures and may be stored for months without loss of potency. As such cryoprecipitated factor VIII is soon expected to become generally available. The broad clinical use of a potent human factor VIII concentrate holds promise of further reducing the risk and morbidity of bleeding in hemophilia and of permitting safe elective as well as life-saving surgery.

26. Nasal Septal Abscess.

Abstract

The article discusses the examination of a man diagnosed with nasal septal abscess. He was initially has myelodysplasia presented with oral thrush, intranasal dryness and congestion. In his direct examination, he was diagnosed with nasal septum and treated with flocunazole. However, after ten days, the postnatal drainage continued and nasal septum has enlarged. The treatment with levofloxacin was initiated and the nasal abscess was surgically drained.

Abstract

Background and Purpose: Intracranial lipomas are uncommon congenital malformations that most often are asymptomatic. A rare subset of intracranial lipomas arises within the vestibule of the inner ear, which can cause sensorineural hearing loss. We present the CT and MR imaging characteristics of lipomas within the vestibule of the inner ear and propose a pathogenetic mechanism for this malformation., Methods: We retrospectively reviewed five cases of vestibular lipoma. T1-weighted axial conventional spin-echo and T2-weighted axial fast spin-echo sequences were performed in all five cases. Four patients underwent T1-weighted fat-saturated imaging, and one underwent CT scanning with Hounsfield units measured., Results: Four female patients ranging in age from 10 to 40 years and one 26-year-old male patient had sensorineural hearing loss. Two patients had progressive high-frequency loss; one, sudden-onset high-frequency loss; and two, congenital high-frequency loss. All had lesions within the vestibule that displayed hyperintensity on T1-weighted images, isointensity to slight hypointensity on T2-weighted fast spin-echo images, hypointensity with fat saturation, and fat attenuation on CT scans. Three of the cases were associated with lipoma of the cerebellopontine angle. In none of these cases were the cerebellopontine angle and labyrinthine lipomas contiguous., Conclusion: Intravestibular lipoma is considered when a focal hyperintense lesion is identified within the vestibule of the inner ear on T1-weighted MR images. Lesion hypointensity on fat-saturated T1-weighted MR images verifies its lipomatous nature and separates it from intralabyrinthine hemorrhage or highly proteinaceous fluid.
