Introduction: Usually ipsilateral fractures of the femur and tibia are not compatible with good results and require surgery. The unsatisfactory results are more likely due to complex patterns of fractures, compromised soft tissue, associated ligament injuries, and concomitant vital organ injuries. There are many classifications to describe this type of fracture but none of them is a prognostic classification. The aim of this study is to validate our classification according to prognostic terms., Methods: This retrospective study encloses patients accepted with a diagnosis of floating knee between January 1st 2014 and December 31th 2020. A total of 372 patients met the inclusion criteria, but only 168 patients were selected for the final review. We have reclassified the 168 patients into three classifications: according to our alphanumeric; according to the Fraser classification; according to Letts and Ran. Our classification is divided into 5 macro categories in increasing order of severity, and considering fracture site, and exposure status. The Tau B Kendall and Cohen's Kappa was used to statistically evaluate the prognostic value, reliability and reproducibility of our classification versus Fraser Classification, Letts and Ran Classification in the prognosis of these injuries., Results: The statistical results showed that classifiying patient into macro category and sub-category it is possible to have a prognostic correlation with functional results. Noteworthy, floating knee is a complex injury with poor results., Conclusion: The floating knee is not only the bone lesion but is above all the lesion of the soft tissues and the extensor apparatus that allow the correct functionality of the knee. These lesions do not always have favorable outcome, with respect to the nonseverity of the lesion as in Fraser's classification. Furthermore, on average these patients are subjected to an average of 6 surgical interventions; in some cases we have assisted to 23 surgical procedures. Aim Distal radius/forearm fractures in adolescent patients remain challenging injuries to treat. Distal radius/forearm bony anatomy is not completely restored with intramedullary K wire fixation. The aim of this study was to compare radiographic and functional outcomes obtained using intramedullary K wire fixation and open reduction and internal fixation in the treatment of distal radius/ forearm fracture. Methods A total of 43 patients who presented with distal radius/ forearm fractures were enrolled and divided into two groups: 23 patients treated with K-wire (IMNK) and 20 patients treated with plate and screws (ORIF). The evaluation criteria were: fracture healing time, objective quality of life measured by the Mayo wrist score (MWS) and quick disabilities of the arm, shoulder and hand score (QuickDash), length time of surgery, complications, sport or play return, forearm visual analogic pain (FVAS), bone healing by radius union scoring system (RUSS). Results In both groups the results obtained were comparable in terms of functional, pain and return to play/sport after the third month after surgery. Bone healing was faster in IMNK than ORIF but without significance (p>0.05). There was less complication in ORIF than IMNK (p<0.05). Conclusion The treatment of adolescent distal radius or forearm fractures remains challenging. One challenge facing the physician is the choice of surgical technique and fixation method, which will be influenced by individual experience and preference. Objectives: To determine the efficacy of hybrid external fixation in the treatment of tibial pilon fractures., Design: Retrospective, multicentre study., Patients/participants: Adult patients with tibial pilon fractures treated with hybrid external fixation., Intervention: Fracture reduction with ligamentotaxis and fixation with XCaliber hybrid external fixator., Main Outcome Measurements: Fracture union, complications, functional outcome (Mazur Ankle Score)., Results: Union was obtained in 159 fractures at an average of 125days; there were three delayed unions and three non-unions. The most frequent complication was superficial pin-track infections (48), all of which responded to local wound care and antibiotics. There were no deep infections and no DVT. Only one fracture had loss of reduction that required frame revision. The overall functional scores were 91 (excellent) for AO/OTA type A fractures, 89 (good) for type B fractures, and 75 (satisfactory) for type C fractures., Conclusions: Hybrid external fixation is an effective method of stabilising tibial pilon fractures, particularly those with marked comminution. The Authors describe the case of a 14-year-old girl who developed a multifocal osteonecrosis (ON), after treatment with chemotherapy and corticosteroids for acute lymphoblastic leukemia (ALL). She came to our attention about 6 months after the beginning of treatment complaining of pain in her left knee and later in both hips and shoulders. Radiography and MRI confirmed the presence of avascular ON at all these sites. The patient underwent bilateral total hip arthroplasty, which was still functioning well at the last follow-up visits at 5 years, and at 5 years and 4 months. ON is increasingly recognized as a complication of the treatment of cancer in children and adolescents, and as patients now frequently survive ALL into adulthood, orthopedists will be increasingly called on to manage this complication affecting multiple joints in children and young adults.

A retrospective study was conducted on 60 patients (53 females, seven males with a mean age of 68 years and 5 months) who underwent a total knee replacement using a mobile bearing. The diagnosis was primary osteoarthritis in 57 cases and rheumatoid arthritis in three cases. None of the patients underwent a bilateral procedure, thus 60 implants (33 all cementless, three all cemented, 24 with only the tibial component cemented) were considered. Three different groups were identified: Group 1: first 20 total knee arthroplasties with menisci. Group 2: first 20 total knee arthroplasties with rotating platform. Group 3: first 20 total knee arthroplasties with AP glide platform. Clinical and radiological results at the final follow up, although different in time among the three groups, have shown no revision due to mechanical or septic reasons, and no signs of impending failure.

Gorham's disease is described as a rare form of massive non-familiar idiopathic osteolysis with a self-limiting nature, where the simil-angiomatous tissue destroys the bone, that is substituted by fibrous tissue. One case of Gorham's disease is presented, that is interesting because of the sites in which the pathology is localized and because of its multicentricity. The authors reached a diagnosis by carefully studying clinical and laboratory evidence as well as imaging and anatomo-histopathological findings, confirmed by the data reported in the literature.

Osteoid osteoma is a benign tumor occurring frequently during the growth years. It may be found in any of the skeletal segments, and it is often present as an isolated neoformation. In some rare cases it may involve more than one segment or it may be multicentric. The authors report their experience with a case of double localization of metachronous osteoid osteoma.

The preliminary results of the surgical treatment of hallux valgus using subcapitate chevron-type osteotomy of the 1st metatarsal modified using a dynamic interfragmentary device are reported. The method satisfies the need to restore angular metatarsophalangeal and intermetatarsal values and improves metatarsal pain caused by hallux valgus correction, which influences the cosmetic results.

It is the purpose of this study to compare the prognostic value of the Herring classification (lateral pillar) with that of the Catterall classification. The material is constituted by 45 patients affected with Perthes' disease and reviewed an average of 24 years after onset. At follow-up the hips were divided into three classes based on joint congruency as suggested by Stulberg. The Catterall classification demonstrated overall a better predictive value for the Herring system. Instead, the most satisfactory reproducibility seems to be that of Herring: 80% as compared to 42% for the Catterall classification.

Meyer's femoral cephalic dysplasia (FCD) is a rare affection that is often confused with Perthes' disease from which it differs in terms of both treatment and prognosis. A constant feature is the absence of the nucleus of ossification that does not occur until 15-18 months of age; at times there are multiple nuclei of ossification, so that the epiphysis has a "morulated" aspect. Fusion of the various centers of ossification occurs at about 5 years of age. The epiphysis does not collapse and it preserves its density and its structure. Differential diagnosis involves Perthes' disease and other pathologies such as multiple dysplasia epiphysealis, hypothyroidism, dyschondroplasia.

This work compares the results of 2 different techniques of anterior cruciate reconstruction performed by 2 surgeons with the same rehabilitation program and same criteria of results evaluation. Forty-five patients were operated on using semitendinous tendon autograft, (average followup, 8 years 4 months), and 30 patients using bone patellar tendon bone autograft (average followup, 7 years 5 months). Functional results were evaluated using the Lysholm and Tegner rating systems, and clinical laxity or stability were evaluated using the Lachmann and Jerk tests. Articular stability and function were significantly better in the patients operated on using bone patellar tendon bone autograft.
